Quality has an important role in a company. A defect product can The customer complaint of PT XYZ keep increasing in November 2017 in amount of 13.178 pcs. The highest defect occurred on frame X and visual problem. After analyzing using fishbone diagram, the cause of highest defect is caused by method and material.

Remanufacturing has gained attention from industry, but the literature lacks the scientific comprehension to realize efficient remanufacturing. This hinders a company from commencing or improving remanufacturing efficiently. To fill this gap, the paper proposes a set of practical success factors for remanufacturing. To do so, it analyzes remanufacturing practices in industry through interviews with staff from remanufacturing companies with long experience. The practical success factors are found to be (1) addressing product and component value, (2) having a customer-oriented operation, (3) having an efficient core acquisition, (4) obtaining the correct information, and (5) having the right staff competence. Next, the paper further analyzes remanufacturing processes theoretically with both cause and effect analysis and means-ends analysis. Since the factors show that, among other things, the product/service system (PSS) is highly relevant to remanufacturing in multiple ways, theories on the PSS are partly utilized. As a result, the distinctive nature of remanufacturing underlying in the processes is found to have high variability, high uncertainty and, thus, also complexity. The obtained insights from practice and theory are found to support each other. In addition, a fishbone diagram for remanufacturing is proposed based on the analysis, including seven m's, adding two new m's (marketing and maintenance) on top of the traditional five m's (measurement, material, human, method, and machine) in order to improve customer value. The major contribution of the paper lies in its insights, which are grounded in both theory and practice.

<p>Gading Indah Agency (GIA) is a company that has over than 25 years serving the<br />needs of print media distribution for its customers. By comparing customer complaint data in<br />June and December 2010, it is known that an increase in the number of customer complaints<br />against the service company by 0.73% which may indirectly indicate the declining performance<br />of the company. Therefore GIA requires a performance management that can measure the<br />performance of the company as a step to determine the position of the company and then used<br />as feedback for evaluation. Performance measurement in accordance with the GIA CRM<br />Scorecard is a method that has the perspective of customer-focused orientation. Stages of<br />designing a model of CRM Scorecard in the company's vision begins with the translation<br />company's mission to the CRM Scorecard in four perspectives, namely Customer Value,<br />Customer Satisfaction, Customer Interaction, Customer Knowledge and GIA have 10 strategic<br />objectives from the perspective of translation CRM Scorecard.</p>

Abstract - PT.I is a company that produces synthetic rattan furniture on an export scale. The various products at PT.I are an attraction for consumers, such as classic and modern weaving models. However, the number of defects produced is higher than the tolerance limit company. Based on data from the weaving section, it shows that there are 5 types of defects that are not in accordance with the standards and quality that have been determined by the company. The purpose of this study is to identify the types of defects that often occur in PT. I and identify the main factors causing product defects using the SPC method. The seven tools consist of a check sheet, histogram, stratification, scatter diagram, p control chart, Pareto diagram, and fishbone diagram. The results showed that the number of defects in October and November exceeded the limit set by the company. The highest level of disability occurred in October. The higher the number of production, the higher the number of product defects. Based on the results of the p control chart, it can be seen that the product is outside the control limits that it should have. The process is in a state of uncontrollability or is still experiencing deviations. To suppress or reduce the number of product defects that occur in production, 3 types of dominant defects can be applied, namely the woven model (254 units), loose woven (122 units), and nail-looking woven (119 units). Factors causing defects in production are derived from human factors/workers, methods, materials/raw materials and work environment.

PT Pertamina EP Field Sangasanga is committed to completing the work program that is launched every year. It is expected to support the 2020 production target of 5380 BOPD as written in the 2020 Work Program. One of the efforts to increase the production rate of Field Sangasanga is to do Well Service Maintenance (WSM). Data of different failures during performing WSM work are collected. The frequency of breakdowns and size of loss were identified and analyzed employing quality improvement tools are; Pareto and Fishbone Diagram. Results indicate that tubing leaks cause 51.6% of the potential loss. Tubing often leaks because the wall thickness is less than the minimum. Root-cause-analysis highlights that most failure in the tubing is caused by the existing inspection tool cannot check the entire thickness of the tubing surface. Leaky tubing leads to many problems, such as increased work time, operational costs, and loss of production. We developed an inspection robot named Thicktube, which uses simple Arduino programming, equipped with a VL53L0X proximity sensor for measurement, SMS feature, and micro-SD feature. The Thicktube is fitted with a motor speed of 0.36 m/s driven by a battery remote control. With three sensor angles, Thicktube is designed to measure the thickness of the tubing as much as 27 points on one joint dimension along 9 meters. The test to compare the result is measured using Thicktube and Ultrasonic Thickness. Thicktube has a measurement accuracy of 82% with a measurement resolution of 1 mm. Thicktube as a preliminary tubing inspection can reduce the incidence of downtime due to leaky tubing. By applying Thicktube tools, a company can save up to Rp. 3.1 billion. In addition, the potential hazards of pulling out the tubing can be avoided.

This automotive company is a company engaged in the automotive components of motorcycles. Its main product is Disk Brake. Automotive companies produced an average defective product in Disk Brake production for the January 2019 – December 2019 period of 1.72%, while the tolerance limit of the company was 1%, where there was a difference of 0.72%. This study aims to reduce the number of defects by providing recommendations for improvement proposals. The method used is DMAIC (Define, Measure, Analyze, Improve, Control) by calculating the company’s sigma value and using FMEA (Failure Mode and Effect Analysis) tools to improve the problem. The Define stage creates SIPOC and CTQ (Critical to Quality) diagrams. At the Measure stage, the results of the calculation of the sigma value obtained a DPMO of 3446.41 with a sigma value of 4.20. The most dominant type of defect in disk brakes is thin material based on the analysis results using the Pareto diagram at the Analyze stage. The causes of disk brake defects are obtained using a fishbone diagram. At the Improve stage, the FMEA calculation found the most potential problem-causing factors, namely the availability of components with an RPN (Risk Priority Number) value of 448 and 5W + 1H analysis, an improvement proposal was obtained to schedule maintenance and replacement of components regularly.

Nowadays, the development of industry is increasing very rapidly. It can be known as the high competition between companies. Not only large and international scaled companies, small and medium-sized companies are also face the same challenges in the global competition. Therefore, the companies need to conduct a quality control to maintain the quality of products produced and to achiece the conformance to specifications of product that has standardized by the company policies in order to improve the customer satisfaction. PT Amanah Insanillahia is a company involved in the bottled water industry. Based on the preliminary study, it shown the quality control only conducted to check the number of defective products produced without further evaluation and analysis. Therefore, there is a need to analyze the data of defective products produced and evaluate the results. The purpose of this study is to analyze the nonconformity product of the bottled water 600 ml brand PRIM-A. The p-chart is used in this research to analyze the number of defective products. Then, the fishbone diagram is used to analyze the causes of defective products. The results show that there is no data out of the control limits and mostly the data near in the central line of the p-chart. Using the fishbone diagram, it concluded that factors of man, machines, materials, methods, and environment are the causes of nonconformity products of the bottled water 600 ml brand PRIM-A. It is hoped this study will beneficial for the company to improve the product quality as well as to increase the customer satisfaction.Key Words:Bottled water, nonconformity, p-chart, quality, fishbone diagram AbstrakDewasa ini perkembangan dunia industri bergerak sangat pesat. PT Amanah Insanillahia merupakan perusahaan yang bergerak di industri Air Minum Dalam Kemasan (AMDK). Berdasarkan studi pendahuluan yang dilakukan diketahui bahwa pengendalian kualitas hanya dilakukan dalam batas pengecekan jumlah produk cacat yang dihasilkan tanpa dievaluasi maupun dianalisis lebih lanjut. Oleh karena itu perlu untuk menganalisis data produk cacat yang dihasilkan dan mengevaluasi hasilnya. Tujuan penelitian ini adalah untuk menganalisis ketidaksesuaian produk air minum kemasan botol 600 ml merek PRIM-A. Peta p digunakan dalam penelitian ini untuk menganalisis jumlah produk yang cacat. Kemudian diagram fishbone digunakan untuk menganalisis penyebab ketidaksesuaian produk. Hasil penelitian menunjukkan tidak terdapat data yang keluar batas kontrol dan sebagian besar produk berada didekat garis tengah dari Peta kendali p. Dari diagram fishbone didapatkan bahwa faktor manusia, mesin, bahan baku, metode, dan lingkungan merupakan penyebab ketidaksesuaian produk air minum kemasan botol 600 ml merek PRIM-A. Home Industry Putih Jaya is a company engaged in the paving blocks manufacturing which in producing paving certainly wanst to minimize any product defects. Known types of paving product  that often occured is an easily cracked paving. An effort in the first stage to reduce the level of disability in this company is to identify the factors that affect product quality paving using fishbone diagram followed by the Taguchi method which is to determine the most influential factor and improve product quality paving so the defect rate can be  decreased. From the data calculation with the fishbone diagram, there are six factors that influential. Among the six factors, those will be identified factor has  the greatest influence resulting product quality deviates toward standards quality. Conclusion of the results showed that percentage of defect that occurs in the company initially around 4%, while by using the proposed improvement by Taguchi method decreases to 2%.  The decreasing of defect percentage of defect means that the improvement of product quality is successfully. The factors the most influential factors to  the paving is a drying process with the largest value of percent contribution in the amount of 34.5%.

This journal presents research on improving product quality in a company. This study aims to identify the factors that cause disability in a product and propose follow-up solutions in order to improve the quality of a product. The method used in the study is the fishbone diagram and the Kipling method (5W + 1H) to improve product quality. The product under study is a yarn product manufactured in the second building of the company. There are 8 types of product defects that cause product quality to decline and are not suitable for sale, which can result in losses to the company, which are not good spools, dirty spools, yarn spools of different diameters, spools of cross yarn, spools of loose yarn, defective cones, incorrect cone types and the weight of the yarn is distorted. From all types of defects, the result shows that the most defects are in the category of bad roll defects of 31%, with the number of defective products of 215 spools of the total defective products of 716 spools. After conducting research, it was concluded that product defects occur due to lack of machine maintenance, mistakes from operators when installing the machine. If the company can reduce the occurrence of defects or damage to the yarn product, it will benefit a lot.

PT. Selamat Sempurna, Tbk is a company that manufactures  automotive parts, namely car filters. This filter product has many part numbers. In this company has one of the production lines, namely Line Seat Press Shop, this line produces components from filters, namely seat components. Seat is the main part of the filter component that functions as a filter holder to the engine. This Seat component consists of several part numbers (shapes or variations) following the filter product, and some part numbers handled by the manual engine B070 & L224 will affect the changes in the production engine settings. This production machine includes machines that require time in a slow setup process for switching dies when switching from one part number to another part number compared to other machines, because the element of the process setting is long. With a fishbone diagram, the root cause of the Change Over Part long problem on the machine is from the method category (there is no preparation desk dies and there is not separation of external setup and internal setup). Previously the time needed for Change Over Part was 1961 seconds. After knowing the root cause of the problem and the improvement of the time needed is only 866 seconds. The time difference is 1067 seconds, so save on average time 1095 seconds or 18 minutes / setup. Keywords: Filter product, Change Over part (COP), Fishbone Diagram, Setup, Process Elements.
