Routine Vestibular Tests May Point Out Vestibular Subtype of Seasickness Only

BACKGROUND: The vestibular system is important in the pathogenesis of seasickness. Our objective is to investigate whether routine vestibular tests detect seasickness.METHODS: Included were 17 professional naval personnel (mean age of 29.76 4.73 yr) diagnosed as having seasickness and 29 healthy age- and gender-matched controls. Cervical (c) vestibular evoked myogenic potentials (VEMP) and ocular (o) VEMP and bithermal caloric tests were performed after ear, nose, and throat examination, pure tone audiometry, and magnetic resonance imaging. Severity of seasickness was evaluated based on the Graybiel scale. P1 latency, N1 latency, P1N1 amplitude, and interaural asymmetry ratios (IAR) of cVEMP and oVEMP were compared between the patients and control groups. Abnormal findings in the caloric test were noted. Presence of an abnormality in any of the three vestibular tests (cVEMP, oVEMP, or caloric test) was accepted as a positive vestibular finding.RESULTS: According to the Graybiel Scale, severe malaise and frank sickness were observed in 3 patients (18.7%) and 13 patients (81.3%), respectively. Graybiel scoring could not be performed in one patient due to general discomfort and bad general condition. In the caloric test, each of three patients (17.65%) showed canal paresis, an incomplete test because of severe nausea, and vomiting and hyperactive response. There were no significant differences in P1 latency, N1 latency, P1N1 amplitude, or IAR of cVEMP and oVEMP (P > 0.05). There were three patients (17.65%) and two patients (11.76%) who had abnormal IAR for cVEMP and oVEMP, respectively.CONCLUSION: Routine vestibular tests may detect some findings in only a minority of patients with seasickness.Satar B, Akin Ocal FC, Karacayli C, Coban VK. AbstractIntroduction:Tinnitus is usually associated with hearing loss, and patients with tinnitus and normal hearing are unusual. Neuro-otological findings have not previously been described in tinnitus patients with normal hearing.Aim:To analyse neuro-otological examination results from a group of tinnitus patients with normal hearing.Materials and methods:Seventeen normal-hearing tinnitus patients seen over a 10-year period were retrospectively evaluated. Their results were compared with those of a control group of 17 normal subjects without tinnitus.Results:The main neuro-otological finding in the tinnitus patients was caloric test abnormality: a unilateral canal paresis was present in 15 of the 17 patients. Caloric tests were normal in 15 of the 17 control subjects.Conclusion:We may infer from these results that tinnitus could be the only clinical manifestation of a cochlear – and presumably cochleo-vestibular – lesion, and that unilateral canal paresis may be the only abnormal finding on neuro-otological examination.

Electrophysiologic testsests can be used to evaluate the function of each of the five vestibular sensory receptors in the labyrinth, in patients with dizziness and imbalance. Tests of semicircular canal function concentrate on vestibular ocular reflex behavior and include the bilateral, bi-thermal caloric test, video head impulse tests (vHIT), and rotary chair tests. Otolith function tests focus on changes in muscle tone and include the cervical and ocular vestibular evoked myogenic potentials (cVEMP and oVEMP). Interpreting vestibular tests requires an understanding of the underlying anatomy and physiology, and an appreciation of central compensation processes and their effects on each test. This chapter reviews the techniques and interpretation of vestibular testing to assess semicircular canal and otolith function and postural control.

Background: To investigate the relationship with prognosis, various neurotological examinations evaluating all four nerves within the internal auditory canal were performed in patients with Bell's palsy. Methods: A total of 69 consecutive patients with Bell's palsy were included. They were treated uniformly with steroid and an antiviral agent and underwent neurotological examinations consisting of electronystagmography, pure-tone audiometry (PTA), electroneurography (ENoG), caloric test, rotatory chair test and cervical vestibular evoked myogenic potentials (cVEMP). According to the final recovery state, patients were divided into two groups: a complete recovery group and an incomplete recovery group. The incidence of abnormal findings in each test was compared between the two groups. Results: Fifty-six patients recovered completely and 13 patients recovered incompletely. No association was observed between the rate of ipsilesional PTA threshold and the rate of abnormal caloric test, function tests and recovery state. However, the initial state of facial palsy, ENoG and the rate of abnormal cVEMP were significantly correlated with the rate of recovery. Conclusion: The results indicate that Bell's palsy may be more comparable to mononeuritis multiplex and that cVEMP could be a useful tool for predicting the prognosis of Bell's palsy.

Objectives: To evaluate the relationship between the loss of the cochleovestibular functions in Ménière’s disease (MD). Methods: Forty-three patients with definite MD underwent pure-tone audiometry (PTA) and caloric test. Canal paresis (CP%), dPTA (interaural difference), and average PTA results were contrasted. IBM SPSS V24 was used for statistical analysis. Results: According to PTA, most patients were in stage C, and caloric weakness was found in 29 patients. Linear ( R 2 = 0.06) and nonlinear correlation tests (rho = 0.245, P = .113) between canal paresis (CP%) and dPTA showed no correlation, as well as between CP% and PTA analysis ( R 2 = 0.007, rho = 0.11, P = .481). As per the categorial analysis, no correlation was detected between the groups either (κ = 0.174, 95% CI: 0.0883 - 0.431). Based on the results of the analysis, it was concluded that a more advanced stage determined by audiometry does not indicate increasing values in the CP% parameter. Conclusions: Audiometric changes do not directly correspond with the vestibular ones; therefore, no specific correlation exists between them. Thus, for therapy planning and diagnosis, both tests are necessary.

This study was conducted with the aim of evaluating the role of information and information resources in the awareness, control, and prevention of COVID-19. This study was a descriptive-analytical survey in which 450 participants were selected for the study. The data collection instrument was a researcher-made questionnaire. Descriptive and inferential statistics were used to analyze the data through SPSS. The findings show that a wide range of mass media has become well known as information resources for COVID-19. Other findings indicate a significant statistical difference in the rate of using information resources during COVID-19 based on age and gender; however, this difference is not significant regarding the reliability of information resources with regard to age and gender. Health information has an undisputable role in the prevention and control of pandemic diseases such as COVID-19. Providing accurate, reliable, and evidence-based information in a timely manner for the use of resources and information channels related to COVID-19 can be a fast and low-cost strategic approach in confronting this disease.

BACKGROUND: Susac syndrome (retino-cochleo-cerebral vasculopathy, SuS) is an autoimmune endotheliopathy characterized by the clinical triad of encephalopathy, branch retinal artery occlusions and sensorineural hearing loss. In contrast to data regarding auditory function, data measuring vestibular function is sparse and the cervical vestibular-evoked myogenic potentials (cVEMPs). OBJECTIVE: To determine whether the video head impulse test (vHIT) can serve as a confirmatory assessment of vestibulocochlear dysfunction in cases of suspected SuS. METHODS: Seven patients diagnosed with SuS underwent pure tone audiometry, a word recognition test, cVEMPs and the vHIT. RESULTS: Five patients were diagnosed with definite SuS, and two with probable SuS. Two patients were asymptomatic for hearing loss or tinnitus, and no sensorineural hearing loss was detected by audiograms. Four patients complained of tinnitus, and three patients reported experiencing vertigo. Three patients had abnormal cVEMPs results. All seven patients’ vHIT results were normal, except for patient #2, who was one of the three who complained of vertigo. The calculated gain of her left anterior semicircular canal was 0.5, without saccades. CONCLUSIONS: This is the first study to describe the results of the vHIT and cVEMPs among a group of patients with SuS. The results suggest that the vHIT should not be the only exam used to assess the function of the vestibular system of SuS patients.

Ethnic-racial identity (ERI) is associated with adaptive outcomes in emerging adulthood, but more research is needed on factors that may inform ERI, such as receiving one’s genetic ancestry results. The current study examined changes in ERI using a pre-test post-test design in which 116 emerging adults 18–25 years were randomly assigned to either receiving their genetic ancestry results before the post-test (the testing condition) or after post-test (the control condition). We also tested whether ethnicity/race and gender moderated these associations. Findings indicated that male students of color (SOC) in the testing condition experienced an increase in ERI affirmation from pre-test to post-test, and male SOC in the control condition experienced a decrease in ERI affirmation from pre-test to post-test. There were no significant differences in ERI affirmation change between students in the testing condition and control condition for female SOC, White males, or White females.

Film in Australia, as with many other nations, is often seen as an important cultural medium where national stories about belonging and identity can be (re)produced in pleasurable and, at times, complicated ways. One such film is Ray Lawrence’s Lantana. Although striking a chord in Australia as a good film about ‘ basically good people’, people that rang ‘brilliantly’ true (Lantana DVD 2002), this paper argues that, at the same time as it produces a fantasy of a ‘good’ Australia, the film also conducts a regulation of what constitutes Australianness. In many ways the imaginary of Australia offered in this film, to its contemporary, urban, professional and intellectual elite audience, still draws on and (re)produces a vision of an Australian community that uses the same narrative frameworks of protection and control as the cruder discourses of ‘white Australia’ offered to an earlier generation of cinema-goers. This film’s central motif of the lantana bush, the out of control weed, that is known as both foreign and local is here emblematic of tensions about belonging, place and otherness. Yet while, within the film’s knowingly reflexive purview any remaining potential for racism is understood and itself under control – we know how to be good mutliculturalists –it is the trope of sexuality in Lantana that provides the real sense of edginess and anxiety about belonging. It is in this arena that the film sets up an idea of danger and –less self-consciously, and in the end more aggressively – marks out who is and who is not part of the community. In this context the motif of lantana signals an ambivalence about difference and the exotic. Lantana is both desirable because of the difference in its attractive Latin looks and repulsive or feared because of other qualities inherent within its difference: a refusal to behave and a propensity to get out-of control, spread and potentially take over. The film here explores desire for a taste of the other (a gay man, a newly separated woman, a Latin dance teacher). However, these fantasies are in the end emphatically shut down as the film ends by producing a vision of subtly normalised hetero, mono, familial (though not necessarily happy) forms of desiring, loving and reproducing in contemporary Australia.

Objective(s). The purpose of this study was to explore the effectiveness of wideband acoustic immittance (WAI) in the diagnosis of otosclerosis by comparing the differences in the energy reflectance (ER) of WAI between patients with otosclerosis and age- and gender-matched normal hearing controls in the Chinese population. Methods. Twenty surgically confirmed otosclerotic ears were included in the otosclerotic group. The ER of WAI at ambient and peak pressures, resonance frequency, and 226-Hz tympanogram were collected prior to surgery using a Titan hearing test platform (Interacoustics A/S, Middelfart, Denmark). All diagnoses of otosclerosis in the tested ear were confirmed by surgery after the measurements. Thirteen normal adults (26 ears) who were age- and gender-matched with the otosclerotic patients were included as the control group. Results. At peak pressure, the ERs of otosclerotic patients were higher than those of the control group for frequencies less than 4,000Hz and were lower for frequencies greater than 4,000Hz. In addition, within the analyzed frequencies, the differences observed at 2,520Hz was statistically significant (p<0.05/16=0.003, Bonferroni corrected). At ambient pressure, the differences observed at 1,260 and 6,350Hz were statistically significant (p<0.05/16=0.003, Bonferroni corrected). Although the differences between the otosclerotic and control groups exhibited similar trends to those in studies implemented in Caucasian populations, the norms in the present study in the control group were different from those in the Caucasian populations, suggesting racial differences in WAI test results. Regarding the middle ear resonance frequency, no significant difference was observed between the two groups (P>0.05). Conclusion. WAI can provide valuable information for the diagnosis of otosclerosis in the Chinese population. Norms and diagnostic criteria corresponding to the patient’s racial group are necessary to improve the efficiency of WAI in the diagnosis of otosclerosis.

Abstract Objectives Idiopathic sudden sensorineural hearing loss (SSNHL) represents a frequently encountered otological entity, of various types and severity, with an array of associated symptoms including vertigo. This is a devastating life-changing condition with a blurry prognosis. The objective of this study was to determine the clinical association of vestibular impairment by electronystagmography (ENG) and caloric tests, and their ability to predict prognosis. Methods An observational, crossectional study was carried out amongst patients admitted with SSNHL. Each consenting patient had an audiometry test performed on admission as well as ENG and caloric tests. Treatment included oral steroids and carbogen with intratympanic steroids used only as salvage treatment. Follow-up was completed after 6 months when hearing gains were evaluated. Finally, an association was sought between the rate of recovery and ENG and caloric test results. Results Of 35 patients included, marked recovery was seen in patients without vertigo when compared to those with vertigo (p=0.003). A statistically significant association was found between the presence of vertigo and hearing deterioration (p=0.008). More so, normal electronystagmography results were associated with marked recovery (p=0.04). Conclusions The vestibular end organs are both subjectively and objectively affected in SSNHL as demonstrated by the abnormal ENG and caloric tests in our study despite the small sample size. Concomitant vestibular involvement carries poorer prognosis and routine identification may help foresee the recovery of patients with SSNHL and as such, aid in patient counseling. ENG and caloric tests are easily available and may be recommended for all patients with SSNHL.
