Evolution of immunotherapy methods for psoriasis and psoriatic arthritis: from total immunosuppression to selective treatment of therapeutic targets

The literature review presents data on existing treatments for psoriasis (P) and psoriatic arthritis (PsA). The relevance of the work is rationale with the difficulties of choosing a particular therapeutic agent by a specialist, depending on the peculiarities of the course of the skin pathological process, damage to the musculoskeletal system, spine and related diseases. Synthetic and targeted drugs are described in a chronological consequences, the review provides accumulated information about their effectiveness in relation to various manifestations of P and PsA, and the safety of their use. In conclusion, current recommendations on the differentiated use of various agents for target therapy based on a comprehensive assessment of the clinical characteristics of patients as well as mechanism of action and side effects of the drugs, are discussed.

Natural compounds can be used as a complementary or alternative medicine for many diseases, such as osteoporosis. Curcumin, a polyphenolic compound and the major active component of turmeric, is reported to play important roles in bone health and osteoporosis. By affecting proliferation, differentiation, lifespan, and activity of osteoblasts and osteoclasts, curcumin can directly modulate bone tissue hemostasis. Due to its insignificant side effects and several therapeutic properties, such as antioxidant, anticancer, antibacterial, antifungal, anti-inflammatory, and antirheumatic, it could be a potential therapeutic agent to prevent and treat osteoporosis. This review aimed to summarize the most important findings of in vitro, animal, and human studies in an effort to clarify the possible effects of curcumin on osteoporosis and to explain the exact molecular mechanism by which curcumin exerts its action.[GMJ.2021;10:e2129] DOI:10.31661/gmj.v10i0.2129

The treatment of pathologies of posterior segment of eye, such as chorioretinitis, uveitis, various neuroretinopathies, vitreous diseases, intraocular hemorrhage, is a rather serious problem due to the difficulties of the adequate delivery of preparations into area of pathological process. One of the most effective ways to deliver drugs to eye tissue in posterior segment are intravitreal injections. The introduction of drugs is carried out directly into vitreous body through a puncture of sclera. This article presents the indications for intravitreal injections, a detailed description of properties of numerous therapeutic agents that can be delivered through intravitreal injections, potential complications of this technique and recommendations for prevention of side effects.

BACKGROUND COVID-19 pandemic prompts the study of coronavirus biology and search of putative therapeutic strategies. OBJECTIVE To compare SARS-CoV-2 genome-wide structure and proteins with other coronaviruses, focusing on putative coronavirus-specific or SARS-CoV-2 specific therapeutic designs. METHODS The genome-wide structure of SARS-CoV-2 was compared to that of SARS and other coronaviruses in order to gain insights, doing a literature review through Google searches. RESULTS There are promising therapeutic alternatives. Host cell targets could be modulated to hamper viral replication, but targeting viral proteins directly would be a better therapeutic design, since fewer adverse side effects would be expected. CONCLUSIONS Therapeutic strategies (Figure 1) could include the modulation of host targets (PARPs, kinases) , competition with G-quadruplexes or nucleoside analogs to hamper RDRP. The nicest anti-CoV options include inhibitors of the conserved essential viral proteases and drugs that interfere ribosome slippage at the -1 PRF site.

As of today, little data is available on COVID-19 in African countries, where the case management relied mainly on a treatment by association between hydroxychloroquine (HCQ) and azithromycin (AZM). This study aimed to understand the main clinical outcomes of COVID-19 hospitalized patients in Senegal from March to October 20202. We described the clinical characteristics of patients and analysed clinical status (alive and discharged versus hospitalized or died) at 15 days after Isolation and Treatment Centres (ITC) admission among adult patients who received HCQ plus AZM and those who did not receive this combination. A total of 926 patients were included in this analysis. Six hundred seventy-four (674) (72.8%) patients received a combination of HCQ and AZM. Results showed that the proportion of patient discharge at D15 was significantly higher for patients receiving HCQ plus AZM (OR: 1.63, IC 95% (1.09–2.43)). Factors associated with a lower proportion of patients discharged alive were: age ≥ 60 years (OR: 0.55, IC 95% (0.36–0.83)), having of at least one pre-existing disorder (OR: 0.61, IC 95% (0.42–0.90)), and a high clinical risk at admission following NEWS score (OR: 0.49, IC 95% (0.28–0.83)). Few side effects were reported including 2 cases of cardiac rhythmic disorders in the HCQ and AZM group versus 13 in without HCQ + AZM. An improvement of clinical status at 15 days was found for patients exposed to HCQ plus AZM combination.

(1) Background: The epidermal growth factor inhibitors (EGFRIs)/tyrosine kinase inhibitors (TKIs) are effective for cancer target therapy, but acneiform rashes or so-called inflammatory papulopustular exanthemas are common (50% to 90%). The conventional therapy for EGFRIs/TKIs-induced skin toxicity is steroids and antibacterial drugs, but it is still ineffective for some patients, and EGFRIs/TKIs dose reduction/interruption may be needed. In this study, a modified Chinese herbal medicine, Huang-Lian-Jie-Du decoction cream with Yin-Cold (YC) medicine characteristic, was investigated for the effect on patients suffering EGFRIs/TKIs-induced skin toxicity. (2) Methods: The modified Huang-Lian-Jie-Du (mHLJD) decoction cream was made from 10 herbal medicines, including 4 major medicines (Huanglian, Huangqin, Huangbo, and Zhizi) in traditional HLJD decoction. Patients with EGFRIs/TKIs-induced skin toxicity were enrolled. Patients were excluded if they also used other cream for skin toxicity. Skin conditions were monitored by follow up every 2 weeks. The patients’ characteristics, the skin toxicities, treatment response, and adverse events were recorded and analyzed until skin problems resolved or the study ended. (3) Results: The mHLJD decoction cream and its sub-packages were stored at 4 °C before use. Thirty-four patients who had grade 1–3 skin toxicities after receiving EGFRIs/TKIs were enrolled. Seven patients withdrew or were excluded. Finally, data from 27 patients were analyzed. The mean grade of rash acneiform was significantly decreased from 2.19 (ranged 1 to 3) to 0.88 (ranged 0 to 2) after mHLJD decoction cream treatment for 4 weeks and to 0.55 (ranged 0 to 2) after mHLJD decoction cream treatment for 8 weeks. Additionally, the mean grade of dry skin was also significantly decreased from 1.57 (ranged 1 to 2) to 0.71 (ranged 0 to 1) after mHLJD decoction cream treatment for 4 weeks. The changes of skin toxicity were significant, with no obvious adverse events. (4) Conclusions: In summary, the mHLJD decoction cream provides benefits for alleviation of EGFRIs/TKIs-induced skin rash acneiform and dry skin. Additionally, no obvious side effects were found in patients using mHLJD decoction cream.
