Radical alterations in the human microbiota composition are well-known to be associated with many pathological conditions. If these aberrations are established at the time of birth, the risk of developing correlated pathologies throughout life is significantly increased. For this reason, all newborns should begin their lives with a proper microbiota in each body district. The present study aimed at demonstrating a correlation between the mode of delivery and the development of a well-balanced microbiota in the lower airways of newborns. 44 pregnant women were enrolled in this study. Microbiological comparative analysis was carried out on tracheobronchial secretions of babies born through vaginal delivery (VD) or caesarean section (CS). All samples showed the presence of bacterial DNA, regardless of the mode of delivery. No viable cultivable bacteria were isolated from the CS samples. On the contrary, VD allowed colonization of the lower airways by alive cultivable bacteria. The identification of bacterial species revealed that Lactobacillus spp. and Bacteroides vulgatus were the most common microorganisms in the lower airways of vaginally-delivered newborns. Data obtained from quantitative PCRs showed a significantly higher total bacterial load, as well as Firmicutes and Lactobacillus spp. amount, in VD samples than CS ones, while no statistically significant difference was found in Torque Teno Virus (TTV) load between samples. Taken together, our findings confirm the hypothesis that passage through the maternal vaginal canal determines more beneficial colonization of the lower airways in newborns.

Introduction: Hearing loss, since birth, is an important public health issue round the globe with a prevalence ranging from 2-3/1000 live births. Neonates born by Caesarean Delivery (CD) refer for Oto-Acoustic Emissions (OAE) test more frequently compared to Vaginal Delivery (VD) soon after birth. During this study, the effects of the modes of delivery on the OAE test results were investigated. Aim: To study the effects of modes of delivery on the OAE test results. Materials and Methods: During this study 500 neonates, born beyond 37 weeks of gestation, at Dhiraj Hospital, Gujarat, India between January 2019-July 2019 without any associated risk factors were enrolled and divided on basis of their mode of delivery-vaginal or caesarean section. Before their discharge from hospital, OAE test was performed on the patients. Modes of delivery and OAE test results were collected and analysed. Results: Results reviled that there was a significant difference in OAE test failure rates between the two delivery mode groups. CD was significantly related to failed OAE. CD infants had a three-fold higher failure rate as compared to VD infants in OAE test (4.12% vs 1.3%). Conclusion: Neonates born by CD had significantly higher failure rates on first OAE as compared to VD. This shows that the mode of delivery contains a significant effect on the results of the hearing screening tests.

Abstract The effectiveness of azithromycin combined with full-mouth scaling procedures was compared to quadrant-wise scaling combined with the same dosage of azithromycin when treating periodontitis patients over a 6-month period. In this randomized clinical trial study, thirty-four individuals diagnosed with generalized stage III and IV periodontitis underwent baseline, 3-month, and 6-month post-treatment examinations. The study population was randomly assigned to either full-mouth scaling (FMS) or quadrant-wise scaling and root planning (QSRP) in addition to their taking of systemic azithromycin (500 mg/day) for three consecutive days. Periodontal probing depth (PD), clinical attachment level (CAL), gingival index (GI), and plaque index (PI) were monitored along with the quantification of total bacterial load and red complex bacterial species (Porphyromonas gingivalis, Tannerella forsythia, and Treponema denticola) in subgingival samples by real time polymerase chain reaction. The volume of gingival crevicular fluid (GCF) was also monitored over time. The primary outcomes included improvements of PD and CAL. Data was statistically analyzed through a repeated-measures analysis of variance (ANOVA) test, multiple least significant difference (LSD) comparisons, Kruskal-Wallis, Friedman, and paired Student t-tests (p<0.05). FMS and QSRP provided similar PD, CAL, GI, PI, and GCF improvements. After treatment, the FMS group displayed lower mean values of total bacterial load and red complex bacterial species in comparison to the QSRP group. FMS and QSRP in conjunction with systemic azithromycin appeared to be an effective and reliable short-term therapeutic approach for the treatment of generalized stage III and IV periodontitis. However, FMD demonstrated superiority in regard to the 6-month antibacterial effects when compared to QSRP.

Context: Quality of life is a broad multidimensional and dynamic concept. It influences the performance of the individual in physical, social, and spiritual aspects of life. Despite the postpartum period is accompanied by many physical, emotional, and social changes in women's health, it is often a neglected aspect of women's health care. Aim: This study aimed to assess the postpartum quality of life among mothers regardless of their delivery mode and to assess the difference in the quality of life at the various mode of delivery. Methods:  A descriptive exploratory cross-sectional study employed to achieve the aim of this study. A structured interview questionnaire and postpartum quality of life scale that include assessment of four health-related domains that were child care, physical, psychological, and social functioning were used to achieve the aims of this study. Results: The study revealed a fair and a good level of quality of life among most of the studied women regardless of their mode of delivery. The quality of life subscales' assessment revealed a non-statistically significant difference between women in various delivery modes in the domains of child care and physical functioning. While statistically significant differences revealed between all groups regarding their physical and social functioning. Conclusion: The study did not show a clear-cut benefit in favor of any modes of delivery in respect of postpartum life quality, and the research hypothesis is not fully supported. The study recommended further a longitudinal study to understand the magnitude, trajectory and underpinning mechanisms of health-related quality of life outcomes following different modes of delivery.

Background: The microorganisms can impend the life of health care professional and particularly the dental practitioners. They can be transmitted by different ways like airborne and droplet transmission. The current study was carried out to identify whether the arch wires that received from the manufactures are free from microbial contamination and to determine the bacterial species attached to the arch wires. Materials and Methods: This study involved eighty samples, consisted of two types of arch wires (nitinol and stainless-steel) from four companies (3M, G&H, Jiscop, OrthoTechnology). These wires inserted in a plane tube that contains 10 -ml of (Tris [tris(hydroxymethyl)aminomethane] and EDTA (ethylenediaminetetraacetic acid) tris-EDTA and brain heart infusion (BHI) broth. A 0.1 ml was withdrawn from the tube and spread on agar plates. The control groups consist of 16 plane tube (8 tubes with tris-EDTA and other 8 tubes with (BHI). Results: Microbial sampling yielded growth from 5 of the 80 arch wires. The predominant bacteria that isolated were Bacillus spp. No growth was recovered from 75 of the samples and from controls. The bacteria were isolated by BHI reagent and no growth was observed by tris-EDTA reagent with statistically significant difference (P<0.05). The Bacillus spp. found only in the G&H and Jiscop companies, however, no statistically significant difference was found among them (P>0.05). With regard to the presence and distribution of bacteria according to the types of wires, the present results clarified that cases of contamination with Bacillus spp. were found in the nitinol arch wires with statistically significant difference (P<0.05). Conclusions: The results of the current study revealed low count of bacterial contamination in the two types of companies (G&H and Jiscop). Not all materials that received from the manufactures are free from contamination and an effective sterilization regimen is needed to avoid cross-contamination.

A multicenter, nonrandomized, prospective, controlled study was conducted to evaluate, as perioperative prophylactic treatment, the anti-infective effectiveness of 0.66% povidone-iodine eye drops (IODIM®) against the bacterial flora of the conjunctival surface of patients who undergo cataract surgery. Eye drops containing 0.66% povidone-iodine were applied to the eye undergoing cataract surgery; the untreated contralateral eye was used as control. One hundred and twenty patients set to receive unilateral cataract surgery were enrolled in 5 Italian Ophthalmology Centers and pretreated for three days with 0.66% povidone-iodine eye drops. The contralateral eye, used as control, was left untreated. Conjunctival swabs of both eyes were collected at the baseline visit and after three days of treatment, just before the cataract surgery. A qualitative and quantitative microbiological analysis of bacterial presence was evaluated by means of bacterial culture, followed by identification. Methicillin resistance determination was also performed on staphylococci isolates. Bacterial load before and after treatment of the eye candidate for cataract surgery was evaluated and compared to the untreated eye. A reduction or no regrowth on the culture media of the bacterial load was observed in 100% of the study subjects. A great heterogenicity of bacterial species was found. The 0.66% povidone-iodine eye drops, used for three days prior to cataract surgery, were effective in reducing the conjunctival bacterial load. The 0.66% povidone-iodine eye drops (IODIM®) might represent a valid perioperative prophylactic antiseptic adjuvant treatment to protect the ocular surface from microbial contamination in preparation of the surgical procedure.

Atypical coeliac disease in young children is frequently missed when it presents atypically as non-gastrointestinal presentations to different specialties. There was a greater delay (54 months) in establishing the diagnosis in those with atypical coeliac disease (p < 0.001). No difference was observed in the mode of delivery or duration of breast feeding, but significant difference was observed between gestational age at birth (p < 0.001). Most cases showed stunted growth and underweight. Irritability, anaemia, rickets, dermatitis herpetiformis, alopecia and intussusception were other common predictors of atypical coeliac disease. Because of a myriad spectrum of non-gastrointestinal symptoms, at any age with diverse presentation, a high index of suspicion is therefore required.

Abstract. Questionnaires are popular and fundamental tools for acquiring information on public knowledge and perception of natural hazards. Questionnaires can provide valuable information to emergency management agencies for developing risk management procedures. Although many natural hazards researchers describe results generated from questionnaires, few explain the techniques used for their development and implementation. Methodological detail should include, as a minimum, response format (open/closed questions), mode of delivery, sampling technique, response rate and access to the questionnaire to allow reproduction of or comparison with similar studies. This article reviews current knowledge and practice for developing and implementing questionnaires. Key features include questionnaire design, delivery mode, sampling techniques and data analysis. In order to illustrate these aspects, a case study examines methods chosen for the development and implementation of questionnaires used to obtain information on knowledge and perception of volcanic hazards in a tourist region in southern Iceland. Face-to-face interviews highlighted certain issues with respect to question structure and sequence. Recommendations are made to overcome these problems before the questionnaires are applied in future research projects. In conclusion, basic steps that should be disclosed in the literature are provided as a checklist to ensure that reliable, replicable and valid results are produced from questionnaire based hazard knowledge and risk perception research.

AbstractInfluenza A is a serious pathogen itself, but often leads to dangerous co-infections in combination with bacterial species such as Streptococcus pyogenes. In comparison to classical biochemical methods, analysis of volatile organic compounds (VOCs) in headspace above cultures can enable destruction free monitoring of metabolic processes in vitro. Thus, volatile biomarkers emitted from biological cell cultures and pathogens could serve for monitoring of infection processes in vitro. In this study we analysed VOCs from headspace above (co)-infected human cells by using a customized sampling system. For investigating the influenza A mono-infection and the viral-bacterial co-infection in vitro, we analysed VOCs from Detroit cells inoculated with influenza A virus and S. pyogenes by means of needle-trap micro-extraction (NTME) and gas chromatography mass spectrometry (GC-MS). Besides the determination of microbiological data such as cell count, cytokines, virus load and bacterial load, emissions from cell medium, uninfected cells and bacteria mono-infected cells were analysed. Significant differences in emitted VOC concentrations were identified between non-infected and infected cells. After inoculation with S. pyogenes, bacterial infection was mirrored by increased emissions of acetaldehyde and propanal. N-propyl acetate was linked to viral infection. Non-destructive monitoring of infections by means of VOC analysis may open a new window for infection research and clinical applications. VOC analysis could enable early recognition of pathogen presence and in-depth understanding of their etiopathology.
