Recent guidelines recommend the umbilical venous catheter (UVC) as the optimal vascular access method during neonatal resuscitation. In emergencies the UVC securement may be challenging and time-consuming. This experimental study was designed to test the feasibility of new concepts for the UVC securement. Umbilical cord remnants were catheterized with peripheral catheters and secured with disposable umbilical clamps. Three different securement techniques were investigated. Secure 1: the disposable umbilical clamp was closed at the level of the inserted catheter. Secure 2: the clamp was closed at the junction of the catheter and plastic wings. Secure 3: the setting of Secure 2 was combined with an umbilical tape. The main outcomes were the feasibility of fluid administration and the maximum force to release the securement. This study shows that inserting peripheral catheters into the umbilical vein and securing them with disposable umbilical clamps is feasible. Rates of lumen obstruction and the effectiveness of the securement were superior with Secure 2 and 3 compared to Secure 1. This new approach may be a rewarding option for umbilical venous catheterization and securement particularly in low-resource settings and for staff with limited experience in neonatal emergencies. However, although promising, these results need to be confirmed in clinical trials before being introduced into clinical practice.

Since colorectal cancer is one of the world’s most common cancers, studies on its prevention and early diagnosis are an emerging area of clinical oncology these days. For this study, a review of randomized controlled, double-blind clinical trials of selected NSAIDs (aspirin, sulindac and celecoxib) in chemoprevention of colorectal cancer was conducted. The main molecular anticancer activity of NSAIDs is thought to be a suppression of prostaglandin E2 synthesis via cyclooxygenase-2 inhibition, which causes a decrease in tumor cell proliferation, angiogenesis, and increases apoptosis. The lower incidence of colorectal cancer in the NSAID patients suggests the long-lasting chemopreventive effect of drugs studied. This new approach to therapy of colorectal cancer may transform the disease from a terminal to a chronic one that can be taken under control.

The aim of this article is to offer a new approach of perception regarding the position of a distant object. It is also a tribute to John Stewart who accompanied the first stages of this research. Having already examined the difficulties surrounding questions of the perception of exteriority within the framework of enactive approaches, we will proceed in two stages. The first stage will consist of an attempt to explain distal perception in terms of individual sensorimotor invariants. This poses the problem but fails to solve it. The second stage will propose a new pathway to account for spatial perception; a pathway that does not deny the initial intuitions of the autopoietic enactive approaches, but one which radically changes the conception of cognition by considering, from the perceptual stage, the need to take into account interindividual interactions. The protocol of an original experimental study will characterize this new approach considering the perceptual experience of objects at a distance, in exteriority, in a space of possibilities without parting from the domain of interaction. To do this, we have to work at the limits of the perceptual crossing, that is, at the moment when the perceptual reciprocity between different subjects begins to disappear.

AbstractObjectives:To evaluate nosocomial infection (NI) surveillance strategies in French ICUs and to identify similar patterns defining subsets within which comparisons can be made.Design:A questionnaire was sent to all French ICUs, and a random sample of nonresponders was interviewed.Participants:Three hundred ninety-five responder ICUs (69%) in France.Results:In 282 ICUs (71%), a dedicated ICU staff member was responsible for infection control activities. The microbiology laboratory was usually in the hospital (90%) and computerized (94%) but issued regular hospital microbiology records in only 48% of cases. Patients receiving mechanical ventilation, central venous catheterization, and urinary catheterization were 90%, 79%, and 60%, respectively. Patients were screened for carriage of mul-tidrug-resistant bacteria on admission and during the stay in 70% and 60% of ICUs, respectively, most often targeting MRSA. Quantitative cultures were used to diagnose ventilator-associated pneumonia (VAP) in 90% of ICUs, including distal specimens in 80% and bronchoscopy specimens in 60%. Quantitative central venous catheter (CVC)-segment cultures were used in 70% of ICUs. All CVCs were cultured routinely in 53% of the ICUs. Despite wide variations in infection control and surveillance strategies, multiple correspondence analysis identified 13 key points (4 structural variables and 9 variables concerning the diagnosis of VAP, the surveillance and diagnosis of catheter-related and urinary tract infections, and the mode of screening of MRSA carriers) that categorize the variability of French ICUs' approaches to NIs.Conclusion:This study revealed profound differences in N1 surveillance strategies across ICUs, indicating a need for caution when using N1 surveillance data for comparisons and benchmarking.

CONTEXT: The use of parenteral nutrition via a central venous catheter is a common practice in the neonatal intensive care setting. Extravasation of the infusate leading to an acute abdomen is a complication that has only rarely been documented. This report describes the case of a premature infant with a femoral catheter placed in the inferior vena cava, who developed an acute abdomen as a result of late retroperitoneal extravasation of parenteral nutrition. CASE REPORT: A pre-term infant receiving total parenteral nutrition via a femoral venous catheter developed an acute abdomen five days after the catheter placement. Extravascular catheter migration to the retroperitoneal space and extravasation of the infusate was diagnosed by contrast injection. Withdrawal of the catheter was followed by prompt cessation of the signs and full recovery from the acute abdomen, without the need for surgery. A review of the literature is presented, emphasizing the clinical and therapeutic aspects of this unusual complication from femoral venous catheterization and parenteral nutrition.

Search engines allow providing the user with data and information according to their interests and specialty. Thus, it is necessary to exploit descriptions of the resources, which take into consideration viewpoints. Generally, the resource descriptions are available in RDF (e.g., DBPedia of Wikipedia content). However, these descriptions do not take into consideration viewpoints. In this paper, we propose a new approach, which allows converting a classic RDF resource description to a resource description that takes into consideration viewpoints. To detect viewpoints in the document, a machine learning technique will be exploited on an instanced ontology. This latter allows representing the viewpoint in a given domain. An experimental study shows that the conversion of the classic RDF resource description to a resource description that takes into consideration viewpoints, allows giving very relevant responses to the user’s requests.

Deforestation is one of the crucial issues in Indonesia. In 2012, deforestation rate in Indonesia reached 0.84 million hectares, exceeding Brazil. According to the 2009 Guinness World Records, Indonesia's deforestation rate was 1.8 million hectares per year between 2000 and 2005. An interesting view is the fact that Indonesia government denied the deforestation rate in those years and said that the rate was only 1.08 million hectares per year in 2000 and 2005. The different problem is on the technique how to deal with the deforestation rate. In this paper, we proposed a new approach for automatically identifying the deforestation area and measuring the deforestation rate. This approach involves differential image processing for detecting Spatio-temporal nature changes of deforestation. It consists series of important features extracted from multiband satellite images which are considered as the dataset of the research. These data are proceeded through the following stages: (1) Automatic clustering for multiband satellite images, (2) Reinforcement Programming to optimize K-Means clustering, (3) Automatic interpretation for deforestation areas, and (4) Deforestation measurement adjusting with elevation of the satellite. For experimental study, we applied our proposed approach to analyze and measure the deforestation in Mendawai, South Borneo. We utilized Landsat 7 to obtain the multiband images for that area from the year 2001 to 2013. Our proposed approach is able to identify the deforestation area and measure the rate. The experiment with our proposed approach made a temporal measurement for the area and showed the increasing deforestation size of the area 1.80 hectares during those years.

We here a case of extravasation of the fluids due to misplacement of the catheter causing hepatic collection of TPN in a very low-birth weight preterm (VLBW) infant.

Angiopoietin-like protein 3 (ANGPTL3) is a key physiological regulator of plasma lipid and lipoprotein metabolism that involves the control of enzymes, lipoprotein and endothelial lipases. Inhibition of ANGPTL3 offers a new approach for correcting the health risks of dyslipidemia, including familial hypercholesterolemia, mixed hyperlipidemia, metabolic syndrome and/or severe hypertriglyceridemia. ANGPTL3 inhibition with nucleic acid-based antisense oligonucleotide and siRNA can correct dyslipidemia chiefly by reducing production and increasing catabolism of triglyceride-rich lipoprotein and LDL particles. Early clinical trials have demonstrated that these agents can safely and effectively lower plasma triglyceride and LDL-cholesterol levels by up to 70 and 50%, respectively. However, the long-term safety and cost–effectiveness of these agents await to be confirmed in an ongoing and future clinical trials.

Abstract An advanced surface for Central Venous Catheterization (CVC) training and evaluation was designed using sensorization techniques, including the use of a hall effect sensor array to measure the insertion depth of a catheter. The sensor array was tested for accuracy in both static and dynamic scenarios, and was found to be sufficiently accurate; measuring position with an accuracy of ±1.1 mm on average. The highest deviations in measured positions were located at the extreme ends of the array where calculations rely on only a single sensor. The maximum deviation in measured position was found to be 3.5 mm. This low-cost system of catheter measurement has the potential to improve feedback and assessment of CVC training.
