21the Wheelchair Project Case Study: Leveraging Collaborative Efforts and Partnerships to Provide Assitive Devices for People Affected by Disability in Thailand and Southeast Asia

Many people affected by disability require assistive devices, particularly mobility aids, in order to live independently and fully integrate into their communities. Throughout the world, numerous methods have been implemented in facilitating the provision of these devices to people affected by disabilities. This case study intends use the Rajanagarindra Institute of Child Development Wheelchair Project, located in Chiang Mai, Thailand, to evaluate the viability and efficacy of mobility-provision programs which adapt wheelchairs and other assistive devices not originally intended for the end user. Programs which must “retro-adapt” equipment, like the Wheelchair Project, rely heavily on medical and mechanical expertise to identify the recipients’ needs and safely modify the device accordingly. By examining the distribution statistics from the Wheelchair Project over its 20 years of operation and the needs of the disability community reported by the National Statistics Office of Thailand alongside a cost-benefit analysis of the declared value provided versus operational cost, this case-study systematically details the scope and financial viability of this approach. While not without constraints, this model succeeds in effectively capitalizing on available resources (both in regard to physical equipment as well as professional expertise) to meet the needs of those affected by disabilities in Thailand and Southeast Asia. Although still limited in size and reach, the Wheelchair Project demonstrably shows its viability and the potential this approach presents given the right circumstances.

Evisceration Secondary to Coughing in a Child Undergoing Ventriculoperitoneal Shunt Surgery

Abdominal complications related to ventriculoperitoneal shunting are an entity described in the literature. Among these is evisceration. In this article we present a case of evisceration in a 5-month-old child, six days after DVP placement. We will discuss the probable etiology and prevention of such a complication.

Waldmann’s Disease Revealed by Chronic Diarrhea and Lymphedema: Case Report

Intestinal lymphangiesctasia (IL) is a rare disease characterized by the dilation of intestinal lymphatics. It can be classified as primary or secondary depending on the underlying etiology. Usually seen in childhood or adolescence, chronic diarrhea and diffuse edema are the main clinical manifestations of the disease. True lymphoedemas can also be present and affect the lower and upper limbs, they are concomitant with the diagnosis or occur during the course. The diagnosis is based on the visualization of duodenal lymphangiesctasia. The primitive nature of the disease being affirmed by the elimination of the diseases causing secondary lymphangiectasias. Treatment is based on a strict hypo lipid diet enriched with medium chain triglycerides (MCTs). We present through this work the observation of a 12-month-old infant who presented with primary intestinal lymphangiesctasia revealed by chronic diarrhea with lymphoedema of the left upper limb.

Have You Seen A Kaleidoscope? A Case of Visual Migraine Aura

This is a case of migraine, presenting with the interesting visual aura of a ‘kaleidoscope’: waves with flickering movements made up of a variety of colors (red, blue, green, yellow), commencing laterally and spreading superiorly in the visual field of the left eye. This description is quite similar to a fortification spectrum, which usually expands and spreads in a C-shape fashion over one side of the visual field. Several different possible pathophysiological explanation are shared as well as the common differential diagnoses. In the work up of a patient with visual aura migraine, a comprehensive and detailed history is important, together with the appropriate investigations, in order not to miss any serious syndromes, including stroke syndromes and epilepsy.

Mobbing in the Case of Primary Health Care Providers

Objectives: Primary healthcare workers are the main drivers of the population-oriented health education programs. In this work, we aim to offer an account of the conditions that lead to mobbing in Primary Health Care Employees, and of the ways to address this problem and its consequences. Methods: This study has been conducted on the medical staff in primary health care units in the province of Antalya, with the permission of the Provincial Health Directorate and the approval of the ethics board of the Antalya Education and Research Hospital. It was planned as a cross-sectional study; survey forms were filled out by 752 employees during an internal training for primary health care in 2017. After informing the subjects regarding the aims of the study, we gave them a survey of 21 questions. The average time for the individuals to answer the questions was 30 minutes. The survey was designed to ask the individuals their age, gender, educational background and occupation, whether they know of any case of mobbing, whether they were subjected to mobbing themselves and for how long, the position and the gender of the perpetrator, and whether and how they addressed the issue. The answers of the participants were analyzed with descriptive statistical analysis, the frequencies were determined and chi-square test was used. Results: 72.2% (543) of the participants declare that they heard the words "mobbing" or "psychological harassment" before. In our study, the rate of exposure to mobbing was found to be 30.4% among primary care providers. We have found that mobbing exposure was significantly higher among females and midwives and nurses. Our study reveals that in every occupational group perpetrators are mostly in management positions; that 36-45 age group was the most victimized group by both genders; that the most common method in order to handle mobbing is the loss of communication which is a new problem between the perpetrator and the victim and that the most common response to mobbing is to share it with friends; and our study finds that the period of mobbing in the midwife-nurse group is mostly 19 months and more, while this period takes to 9-12 months in the case of doctors. Discussion: One of the most important steps in preventing mobbing is to take timely measures in organizations and to prevent the problem from harming the organization and employees. Educating employees about emotional assault within their working environment is proposed as an important solution for them to protect themselves when they are subjected to harassment.
