scholarly journals The Effects of Upper-Body Exoskeletons on Human Metabolic Cost and Thermal Response during Work Tasks—A Systematic Review

Background: New wearable assistive devices (exoskeletons) have been developed for assisting people during work activity or rehabilitation. Although exoskeletons have been introduced into different occupational fields in an attempt to reduce the risk of work-related musculoskeletal disorders, the effectiveness of their use in workplaces still needs to be investigated. This systematic review focused on the effects of upper-body exoskeletons (UBEs) on human metabolic cost and thermophysiological response during upper-body work tasks. Methods: articles published until 22 September 2020 were selected from Scopus, Web of Science, and PubMed for eligibility and the potential risk of bias was assessed. Results: Nine articles resulted in being eligible for the metabolic aspects, and none for the thermal analysis. All the studies were based on comparisons between conditions with and without exoskeletons and considered a total of 94 participants (mainly males) performing tasks involving the trunk or overhead work, 7 back-support exoskeletons, and 1 upper-limb support exoskeleton. Eight studies found a significant reduction in the mean values of the metabolic or cardiorespiratory parameters considered and one found no differences. Conclusions: The reduction found represents a preliminary finding that needs to be confirmed in a wider range of conditions, especially in workplaces, where work tasks show different characteristics and durations compared to those simulated in the laboratory. Future developments should investigate the dependence of metabolic cost on specific UBE design approaches during tasks involving the trunk and the possible statistical correlation between the metabolic cost and the surface ElectroMyoGraphy (sEMG) parameters. Finally, it could be interesting to investigate the effect of exoskeletons on the human thermophysiological response.

Many of those working on construction sites are exposed to demanding work loads; construction workers lift and carry heavy materials and work in awkward postures. Occupational injuries and accidents due to poor ergonomics are more common in the construction industry and many times lead to human tragedies, disrupt construction processes and adversely affect the cost, productivity, and the reputation of the construction industry. In Sweden, it is reported that concrete workers have the highest relative work-related musculoskeletal injury frequency. Therefore, the use of ergonomic production methods to prevent this can have a significant human, social and financial impact. Research introduced here presents a case study of comparative analyses of ergonomic situations for concrete workers performing concrete casting processes. Three different ergonomic risk assessment methods were used to assess the physical strain, hand-arm vibration and noise affects risks involved in concrete casting work tasks. The combination of technical and managerial factors results in a system where workers are as efficient and safe as possible during their work tasks, and thus, makes the construction work environment sustainable. The aim of our research is to find practical methods to evaluate and compare two different concrete casting methods from an ergonomic perspective. The focus is on the production of cast-in-place concrete bridge constructions where the traditional concrete casting method is compared with the SCC (Self-Compacting concrete) casting method. To be able to identify work-related musculoskeletal injury risks due to concrete casting work tasks, QEC (Quick Exposure Check for musculoskeletal risks), PLIBEL (Checklist for identification for Ergonomics Hazards) and ErgoSAM (Ergonomic production technology method) methods were used. Ergonomic risks analysis methods QEC, PLIBEL and ErgoSAM have all shown capabilities to evaluate construction work activities and thus determine whether a construction work activity constitutes a musculoskeletal risk to the worker or not before any ergonomic intervention is introduced. As a result the present ergonomic risks emanating from work methods used in the traditional concrete placing can be significantly reduced with the use of self-compacting concrete (SCC) that eliminates awkward work postures, noise and hand arm vibration, thereby reducing if not eliminating musculoskeletal injuries among concrete workers during their concrete casting work tasks.

Patients who undergo abdominal surgery under general anesthesia develop hypothermia in 80–90% of the cases within an hour after induction of anesthesia. Side effects include shivering, bleeding, and infection at the surgical site. However, the surgical team applies forced air warming to prevent peri-operative hypothermia, but these methods are insufficient. This study aimed to confirm the optimal application method of forced air warming (FAW) intervention for the prevention of peri-operative hypothermia during abdominal surgery. A systematic review and meta-analysis were conducted to provide a synthesized and critical appraisal of the studies included. We used PubMed, EMBASE, CINAHL, and Cochrane Library CENTRAL to systematically search for randomized controlled trials published through March 2020. Twelve studies were systematically reviewed for FAW intervention. FAW intervention effectively prevented peri-operative hypothermia among patients undergoing both open abdominal and laparoscopic surgery. Statistically significant effect size could not be confirmed in cases of only pre- or peri-operative application. The upper body was the primary application area, rather than the lower or full body. These findings could contribute detailed standards and criteria that can be effectively applied in the clinical field performing abdominal surgery.

Abstract Background Several studies show positive effects of new non-medical therapies known as complementary and alternative medicines, such as the discipline of tai chi. As healthcare professions are among the most vulnerable for work-related stress, this systematic review aims to investigate the relationship between tai chi practice and wellness of healthcare workers. Methods Cinahl, Scopus, Web of Science and PubMed were searched in September 2019. Full-text articles, written in English and published after 1995, were recruited if they focused on positive effects of tai chi on the psychophysical wellbeing of healthcare workers, in comparison with alternative techniques (such as yoga or traditional care). Outcomes were reduced work-related stress, better physical and psychological function, improvement in attention and/or productivity; no restrictions about study design were applied. Quality assessment was performed with the Newcastle-Ottawa Scale on cohort/cross-sectional studies, the Jadad scale for randomized clinical trial, AMSTAR for systematic reviews and CASE REPORT scale for case study. Results 6/111 papers were included: 3 clinical trials, 1 observational study, 1 systematic review and 1 case report. The methodological quality was of medium level. 2/3 trials found a significant increase in individuals' wellbeing and improvements in stress levels and nursing staff’s motivation in their work. In the observational study tai chi was a prevalent mind-body practice to reduce stress. The systematic review suggested that tai chi could be a useful tool to reduce stress-related chronic pain. In case report the effectiveness was observed in medical students. Conclusions This study highlights the full potential and possible benefits derived from tai chi but its application to improve health professionals' wellbeing is still limited, and the absence of a standardized intervention impacts on the methodological quality and reduces the robustness of the retrieved evidence. Key messages Tai chi can improve many pathological conditions and reduce work-related stress. Further research is needed to gain robust evidence of its efficacy for wellbeing of healthcare workers.

Abstract Falls in hospitals and care homes are a major issue of international concern. Falls cost the US $34 billion a year, with injurious falls being particularly life-limiting and costly. Shock-absorbing flooring decreases the stiffness of the ground surface to reduce the impact of a fall. There is a growing body of evidence on flooring for fall-related injury prevention, however no systematic review exists to inform practice. We systematically reviewed the evidence on the clinical and cost-effectiveness of shock-absorbing flooring use for fall-related injury prevention in care settings. We searched six databases, clinical trial registries, conference proceedings, theses/dissertations, websites, reference lists, conducted forward citation searches, and liaised with experts in the field. We conducted study selection, data collection, and critical appraisal independently in duplicate. We evaluated the influence of shock-absorbing flooring on fall-related injuries, falls, and staff work-related injuries. We adopted a mixed methods approach considering evidence from randomised, non-randomised, economic, qualitative, and implementation studies. We assessed and reported the quality of outcomes using the GRADE approach and Summary of Findings Tables. This review, conducted over the course of 2019, summarises the certainty of the evidence on whether and which shock-absorbing floors influence injuries from falls, the chance of someone falling over, and work-related injuries in staff (e.g. from manoeuvring equipment across softer floors). Our findings are applicable to health and social care professionals, buildings and facilities managers, carers, older adults, architects, and designers. Purpose – The purpose of this paper is to provide a synthesis and evaluation of literature surrounding the job demands-resources (JD-R) model (Demerouti et al., 2001) in the first decade since its inception, with particular emphasis on establishing an evidence-based universal application towards different national and international work contexts. Design/methodology/approach – The study uses a systematic review approach following the stages suggested by Tranfield et al. (2003). Based on empirical data from 62 studies, the authors systematically analyse the application of the JD-R model and queries whether it is applicable outside merely domestic work contexts. Findings – The authors find convincing support for the JD-R model in different national contexts. However, the authors also found an absence of studies employing the JD-R model in cross-national settings. None of the empirical studies in the sample had explicitly considered the international context of today’s work environment or had clearly associated JD-R research with the IHRM literature. Research limitations/implications – Based on the wide acceptance of the JD-R model in domestic work contexts and the increased interest in work-related outcomes such as burnout and engagement in the IHRM literature, the study identifies a gap and suggests future research applying the JD-R model to international work and global mobility contexts. Originality/value – This study is the first to systematically assess the application of the JD-R model in domestic and international work contexts based on a systematic review of empirical literature in the first decade since the inception of the model. The study identifies a lack of internationally focussed JD-R studies and invites further empirical research and theoretical extensions.

Journal of the Medical Library Association, 108(1), 89–97. https://doi.org/10.5195/jmla.2020.665 Abstract Objective – This study explored reports of burnout among librarians who assist with systematic review preparation. Design – Electronic survey (Copenhagen Burnout Inventory). Setting – The survey was advertised via three email discussion lists based in the United States of America. Subjects – The study surveyed 198 librarians and information specialists who support the systematic review process. Of these, 166 completed the personal burnout scale, 159 completed the work burnout scale, and 151 completed the client burnout scale. Methods – The Copenhagen Burnout Inventory (CBI) is a validated survey that includes three separate scales: personal burnout, work-related burnout, and client-related burnout. The end of the survey addressed demographics, including questions on the respondents’ involvement with systematic reviews. Survey questions use a 0 to 100 rating scale, with 0 indicating Never/To a Low Degree and 100 indicating Always/To a High Degree. The researchers shared the survey to the email discussion lists MEDLIB-L and DOCLINE and advertised it on the Medical Library Association (MLA) News. Survey answers were collected using Qualtrics Survey Software. Once emailed, the survey remained open for one month. Data was coded in Excel and analysis included scoring following the CBI metrics, as well as TukeyHSD and Kruskal-Wallis tests to determine differences in demographic groups. Main Results – Reported burnout levels were significantly lower for those who spend more than 80% of their time helping with systematic reviews compared to those who spend less than 10%. The consistent use of a systematic review support tool was also associated with significantly lower burnout levels. Other comparisons were not significant. The average overall response score for personal burnout was 48.6. The average score for work-related burnout was 46.4 and the average score for client-related burnout was 32.5. Reference librarians reported the highest average total burnout scores (47.1), while research librarians had the lowest (37.7). Conclusion – Consistency, either in time spent dedicated to systematic reviews or in the use of a support tool, was associated with lower levels of burnout among librarians and information specialists. The authors suggest that these results could inform ways of improving burnout among those assisting with systematic reviews.
