The increase of insecurity levels in Mexico, as well as the fact that violence is a frequent experience among health personnel, motivated this study whose purpose was to evaluate the perception of security and violence that social service medical interns (SSMI) had on the institutions and localities where they carried out their social work and make visible the main types of violence to which they were exposed. This was a cross-sectional study, based on a perception survey self-administered to 157 SSMI from Zacatecas, in Mexico. A high proportion of the participants (75.8%) stated that they were victims of violence, describing 134 incidents; however, only 33.6% of SSMI made an official report. The reported incidents were related to organized crime (31.9%), verbal violence (20.6%), violence by the authorities (14.7%) and sexual harassment (11.8%). One hundred percent of the victims of sexual harassment were women (p = 0.039). According to the above, it is a priority to generate strategies to prevent and reduce the risk of exposure to the violence generated in the medical units and communities where SSMI carry out their activities as medical graduates, as well as, to efficiently process formal violence reports to promote a safe environment that favors the fulfillment of the practice of SSMIs in Mexico.

The study of healthcare personnel migration in Ireland reports that most medical graduates plan to leave the country’s health system. It may be possible to address this challenge by understanding and addressing the reasons why young doctors plan to leave. Future studies should contribute to the retention of early career doctors in highincome countries such as Ireland. This will help reduce the migration of doctors from low- and middle-income countries in order to address the global health workforce crisis and its impact on the attainment of universal health coverage in all health systems.

Abstract Background Early recognition and the optimal management of anaphylaxis saves lives but studies from different countries have demonstrated gaps in knowledge and practices between healthcare workers. There is a paucity of such data from Sri Lanka. We assessed knowledge, perception and self-confidence in the diagnosis and management of anaphylaxis amongst pre-intern medical graduates who would soon become first-contact doctors attending emergencies. Methods This cross-sectional study included pre-interns who graduated with Bachelor of Medicine, Bachelor of Surgery (MBBS) degrees in 2019 from three Sri Lankan universities with differing undergraduate curricula. Using consecutive sampling data were collected within four months of the final-MBBS examinations with a self-administered questionnaire and the answers on case diagnosis and management were used as the basis of outcome scores. Results 385 participants responded (response rate: 91.5%). 16.4% correctly identified all anaphylaxis triggers. Only 7.3% correctly diagnosed all ten case scenarios and 34.5% all seven cases of anaphylaxis. 98.2 and 97.9% correctly identified 1:1000 adrenaline as the first-line treatment and the intramuscular route. 9.9% would preferentially but incorrectly use the intravenous route if access was available. Only 79.2 and 55.6% knew the correct adult and paediatric doses of adrenaline and 50% agreed that follow-up care was needed. The mean scores for case diagnosis and management of anaphylaxis were 7.7/10 ± 1.4 and 16.9/20 ± 1.9, respectively. Multiple linear regression indicated that the final MBBS results classification (class of degree or no class indicated) was a positive predictor of case diagnosis score [class vs no class: B = 0.662 (95% CI 0.347–0.978), p < 0.001] and being a graduate of University 2 [B = 1.568 (95% CI 1.182–1.953), p < 0.001] and passing with a class at final MBBS [B = 0.716 (95% CI 0.319–1.113), p < 0.001] were positive predictors of management score. Self confidence in diagnosing and managing anaphylaxis were rated as 79.7 and 62.1% and there was a positive correlation between knowledge and perception scores and self-confidence (case-diagnosis: rpb = 0.111, p = 0.03; management: rpb = 0.164, p = 0.001). Conclusions Knowledge, perception and self confidence in the diagnosis and management of anaphylaxis was sub optimal amongst pre-interns and we identified areas that need improvement. A higher MBBS qualification classification was a predictor for correct diagnosis and management and confidence in diagnosis and management positively correlated with knowledge and perception scores. Further and enhanced educational and training strategies are needed for this life threatening emergency condition.

Background: Periodic students’ feedback ensures and enhances the effectiveness of the curriculum of medical subject. Objective: This study was designed to obtain students appraisal of the undergraduate pharmacology curriculum and teaching methodology. Methodology: This cross-sectional study was conducted in the Department of Pharmacology at Colonel Malek Medical College, Manikganj, Bangladesh from May to June, 2019 for a period of two months. Questionnaire-based data were collected from students of both sexes who had completed 3rd Professional examination. Likert Scale was applied to collect students’ feedback on pharmacology. Results: A total number of 40 students were recruited for this study of which 60.0% students were strongly agreed about organized teaching. About 47.5% students were agreed that the course was appropriate at their level and achieved their goals in Pharmacology. However, 55% of them strongly agreed that teacher provided clear constructive feedback during class. Total 60%, were agreed that the course improved their problem-solving skills. Overall, 55% students were agreed regarding their satisfaction on Pharmacology course delivery and conduction. Conclusion: In conclusion, periodic appraisal from students regarding evaluation of the student-centric undergraduate pharmacology curriculum is vital in developing efficient medical graduates. Abstract Objectives One Health approach to health considers human, animal, and ecosystem health as a whole and advocates cooperation across disciplines to reach the highest level of health. The aim of the study is to determine the attitudes of the medical interns’ toward the One Health approach and to evaluate related factors. Methods This cross-sectional study targeted all interns of the Faculty of Medicine of 2019–2020 academic year (n=356). Sociodemographic characteristics, health status, habits, and attitudes toward One Health were asked in the questionnaire, which was created by the researchers using the literature. The study data were collected based on self-report during occupational health and safety training, before the COVID-19 pandemic. Chi-square and logistic regression analyses were also performed. Results The study group included a fair gender distribution (47.8% female), and the average age was 23.6 ± 1.2. Out of the 316 participants, 40.2% had not heard the One Health concept before. In total, 85.4% of them declared a positive attitude toward the concept. The probability of high attitude was found to be 5.03 times (95% CI 1.10–23.12) higher in those with above-average success status and 4.08 times (95% CI 1.15–14.52) higher in those who had kept animals. In students’ responses to the attitude questionnaire items, the epidemic sub-dimension had the highest level as 86.1%, and the antibiotic resistance sub-dimension had the lowest level as 77.2%. Conclusions The current study revealed that the awareness on One Health concept among medical interns is limited, however, students’ attitudes are positive. Recent SARS-CoV 2 pandemic showed us the urgent need for proper implementation of’ One Health approach in the medical education curriculum. Meanwhile, One Health focused courses should be introduced into the curriculum of final year medicine and related programs.

Background: Sexual harassment on public transport is an everyday occurrence for millions of girls and women around the globe. With the skyrocketing population of Kathmandu valley public transport has been facing enormous pressure. The major victims of these harassments happens to be college going girls and working women as they tend to travel more in public transports. This study sought to determine the prevalence and factors responsible for sexual harassment in public transport among female health science students.Methods: A descriptive cross-sectional study was carried out among 396 female health science students studying in Manmohan Memorial Institute of Health Sciences using self-administered structured questionnaire. Participants were selected through stratified random sampling method.Results: The prevalence of sexual harassment among female health science students was found to be 79.6%. Physical harassment was the most prominent type of harassment (67.1%) followed by verbal (61.2%) and non verbal 34.6%. After the experience of sexual harassment, 44.6% scolded the harasser, 29.1% kept silent, 17.3% dropped at nearest bus station whereas remaining 9% reacted in other different ways. Overcrowding was considered as main reason for sexual harassment by 69.2% of the participants. 36.8% of female students didn’t use any precautionary methods whereas 32.6% avoided going out alone at night.Conclusion: The study reveals that the prevalence of sexual harassment is significantly high. With increasing population and limited vehicles people are left with no option rather than travelling in the overcrowded vehicles where the chances of experiencing various sorts of sexual harassments are very high. In order to address this emerging issue, prompt and appropriate intervention should be taken by government, public and especially the status of women must be raised in society.JMMIHS.2018;4(1):20-32

Introduction: Career is the progress and actions taken by a person throughout lifetime, especially those related to that person’s occupation, and is often composed of the jobs held, titles earned, and work accomplished over a long period of time. It plays a very important role in the success of one’s life. Aim: The aim was to compare the career prospects among medical and dental residents in a private institution in Thodupuzha, Kerala. Materials and Methods: The study included a total of 60 medical residents and 52 dental residents and was carried out using a self-administered questionnaire. Descriptive statistics and Chi-square test were performed for statistical analysis. P < 0.05 is considered to be statistically significant. Results: In medical profession, it was found that majority (71%) of the residents were satisfied with their career. In dental category, 88% wished for medical sector, whereas in medical category, 60% would opt for engineering. It was reported that poor pay scale among 54% of the medical category and 59% of dental residents suggested less job in the market was the reason for dissatisfaction. Majority (73%) in medical residents and a very minimal (23%) of the dental residents would recommend the own profession to the fellow colleagues (P < 0.05). Conclusion: The dental residents were found to be dissatisfied with their career compared with medical graduates. Better job perspectives and better placements need to be created. Endorsement of effective policies is required.

BackgroundLead damages most body organs and its effects are most profound in children. In a study in Beirut in 2003, before banning the leaded gasoline, 79% of the participants showed blood lead levels (BLLs) higher than 5 µg/dL. The prevalence of lead exposure in Lebanon after the ban on leaded gasoline has not been studied. This study assessed the BLL in Lebanese children aged 1–6 years.MethodsThis cross-sectional study was conducted in three hospitals in Beirut. The children’s BLLs were tested, and their caregiver completed a questionnaire to identify subgroups at risk of exposure. Participants were provided with a WHO brochure highlighting the risks of lead.ResultsNinety children with a mean age of 3.5±1.5 years were enrolled in the study and had a mean BLL of 1.1±0.7 µg/dL, with all values being below 5.0 µg/dL, showing a marked decrease in BLL compared with the mean BLL before the ban on leaded gasoline in 2002. Having a father or a mother with a college degree (p=0.01 and p=0.035, respectively) and having a monthly household income greater than $1000 (p=0.021) were associated with significantly lower BLL. Having more rooms at home and residing close to construction sites were associated with a significantly lower BLL (p=0.001 and p=0.026, respectively). Residing in a house aged >40 years and receiving traditional remedies were associated with a significantly higher BLL (p=0.009 and p<0.0001, respectively).ConclusionBLLs have declined among Lebanese children and this could be attributed to multiple factors including the ban of leaded gasoline. It would be beneficial to conduct a larger study with a nationally representative sample to better characterise the BLL.

Background: It is known that medical doctors suffer from increased rates of depression with medical interns being most at risk. Despite this, little is known about the prevalence of depression in interns in South Africa.Objectives: This study aimed to assess the prevalence of depressive symptoms in interns employed at Groote Schuur Hospital, a tertiary hospital in the Western Cape.Method: The study was a cross-sectional study. All 91 interns were invited to participate in the study and consenting interns were required to complete a demographic and related questionnaire and the Beck Depression Inventory 2 (BDI-2).Results: Fifty-four (59.3%) of all invited interns participated in the study. Twenty-two interns (40.7%) reported a BDI-2 score of 14 or greater, indicating at least mild self-reported symptoms of depression. Features associated with a BDI-2 score of 14 or greater, included female gender, a previous diagnosis of depression, seeing a psychotherapist and previously being on antidepressant medication during internship. Other features also significantly associated with higher BDI-2 scores included suicidal ideation, thoughts of emigration, wanting to leave medicine and using substances to cope. The most significant associated feature of high BDI-2 scores was a subjective feeling of being ‘burnt out’.Conclusion: Interns had a higher prevalence of depressive symptoms when compared to the general population. The feeling of being ‘burnt out’ was the most significant factor associated with the severity of depressive symptoms. It is imperative that the mental health of both medical students and newly qualified doctors be prioritised, supported and monitored.

IntroductionMedical graduates should be competent in gynaecological examination as well as associated skills including speculum use and swabbing. Male and female medical students may have different opportunities to practise these skills in clinical environments, potentially impacting on confidence and competence. This study explores this further via reviewing students’ learning experience in genitourinary medicine (GUM) and obstetrics and gynaecology (O&G) clinics.MethodsCross-sectional study of 759 year 4 and year 5 University of Birmingham medical students via an online questionnaire. This explored degree of participation, impact of gender and self-reported confidence.ResultsOverall response rate was 31% (233/759). Students of either gender who observed an examination being performed by a clinician were more likely to perform the same examination. Female students reported more opportunities to practise gynaecological examination and associated skills. Female students were more likely to be granted consent to perform speculum examinations, vaginal swabbing and vaginal bimanual examinations. Sixty-five per cent of male students felt that their gender affected their learning experience with female patients. Despite this, there was no significant difference in self-reported confidence level in performing gynaecological examinations between genders at the end of placement.ConclusionThe majority of male students perceived that their gender impacted their clinical experience in O&G and GUM. Self-reported confidence levels were unaffected, which could reflect varying approaches to competence between genders. The link between observing examinations and subsequent opportunities to practise is key. This could demonstrate students developing rapport and trust with patients, and clinicians’ roles as gatekeepers.
