Background and objectives: To investigate the serum procalcitonin (PCT) levels among patients with rheumatoid arthritis (RA) without active infection compared with healthy controls and to understand the relationship of PCT with RA disease activity, and treatment received by patients. Materials and Methods: Patients aged 20 years and above with clinician-confirmed diagnosis of RA and healthy volunteers were included during regular outpatient visits, and those with active infection symptoms and signs were excluded. RA disease activity was measured using the Disease Activity Score-28 for Rheumatoid Arthritis with erythrocyte sedimentation rate (DAS28-ESR). Medications received by the patients were also recorded. Results: A total of 623 patients with RA and 87 healthy subjects were recruited in this study. The mean PCT were significantly higher in patients with RA (6.90 ± 11.81 × 10−3 ng/mL) compared with healthy controls (1.70 ± 6.12 × 10−3 ng/mL) (p < 0.001) and the difference remained statistically significant after adjusting for age and sex. In addition, multiple linear regression analysis showed that a lower rank-transformed PCT serum level was significantly correlated with the use of biologics (p = 0.017) and a high DAS28-ESR score (p = 0.028) in patients with RA. Conclusion: Patients with RA have a significantly higher serum PCT levels compared with healthy controls. The use of biologics and an active RA disease activity were associated with a lower level of PCT in patients with RA. Further investigation is required to determine the optimal cutoff value of PCT among patients with RA and its association with disease activity and biologic usage.

Background:Rheumatoid arthritis (RA) is the most common inflammatory arthritis, affecting 0.5-1% of the general population worldwide. Patients with RA are often in a relatively immunocompromised status. Previous study have evaluated procalcitonin (PCT) application among autoimmune and autoinflammatory diseases for the diagnosis of systemic bacterial infection (1, 2) To date, the correlation of baseline PCT levels and RA disease activities, different treatment, or the expression of proinflammatory cytokines in patients with RA remained unknown. Objectives:To investigate the baseline PCT levels among patients with RA without active infection compared with healthy controls and to understand the relationship of PCT with RA disease activity, treatment received by patients, and the expression of proinflammatory cytokines. Methods:Patients aged 20 years and above, with clinician-confirmed diagnosis of RA were included during regular outpatient visits. RA disease activity was measured using the DAS28-ESR.Results:A total of 623 patients with RA and 40 healthy subjects were recruited in this study. The mean PCT were significantly higher in patients with RA (6.90 ± 11.81 * 10-3ng/mL) compared with healthy controls (1.14 ± 3.26 * 10-3ng/mL) (p = 0.002). After adjusted for age and sex, the PCT levels remain significantly elevated in patients with RA (p = 0.001). PCT was not significantly correlated with biologic agent, age, sex, disease duration, disease activity, C-reactive protein, conventional synthetic disease-modifying antirheumatic drugs (csDMARD), and comorbidities. Multiple linear regression analysis showed that PCT was inversely associated with the expression of IL-1β (B = −0.883; CI 95% −1.388, −0.378, p = 0.001) and IFN-α (B = −1.197; CI 95% −2.153, −0.242, p = 0.014) after adjusted for age and sex. Conclusion:Patients with RA have significantly higher baseline PCT compared with healthy controls. The treatment received by patients with RA did not affect the level of PCT. Further investigation is required to determine the optimal cutoff value of PCT among patients with RA before applying it in daily clinical practice.References:[1]Scire CA, Cavagna L, Perotti C, Bruschi E, Caporali R, Montecucco C. Objective. To determine serum glucose-6-phosphate isomerase (GPI) concentrations in patients with rheumatoid arthritis (RA), and to test whether they correlate with objective measures of disease activity.Methods.Sera from 116 patients with RA, 69 patients with non-RA rheumatic diseases, and 101 healthy controls were analyzed. Levels of soluble serum GPI were measured by ELISA. Histological disease activity was determined with the synovitis score in synovial needle biopsies from 58 of the 116 patients with RA. Thirty-one of the 58 synovium samples were stained for CD68, CD3, CD20, CD38, CD79a, and CD34 by immunohistochemistry. Demographic data were collected, as well as serological and clinical variables that indicate RA disease activity, for Spearman correlation analysis.Results.Serum GPI level correlated positively with the synovitis score (r = 0.278, p = 0.034). Significantly higher soluble GPI levels were detected in the RA sera compared with sera from healthy controls and the non-RA disease controls (2.25 ± 2.82 vs 0.03 ± 0.05 and 0.19 ± 0.57 μg/ml, respectively; p < 0.0001). The rate of serum GPI positivity was significantly higher in the RA patients than in the non-RA disease controls (64.7% vs 10.1%; p < 0.0001). Spearman analysis showed no significant correlation between serum GPI level and Disease Activity Score in 28 joints at baseline. After initiation of antirheumatic treatments, GPI levels decreased significantly (2.81 ± 3.12 vs 1.44 ± 2.09 μg/ml; p = 0.016), paralleling improvement of the disease activity indices.Conclusion.Elevated serum GPI may be involved in the synovitis of RA and may prove useful as a serum marker for disease activity of RA.

The purpose of this study was to examine the effect of simultaneous and partial marketing mix on the purchase decision of prospective consumers of Grand Cempaka Housing. This research uses quantitative methods, and is a survey research from the types of explanatory research whose focus is placed on relational research: studying the relationship of variables. The object of research is the residents of Pasuruan City and the sample in this study is the Prospective Consumer of Grand Cempaka Housing. Data analysis uses multiple linear regression analysis methods. The results of the analysis show that Products, Prices, Places and Promotions simultaneously have a significant effect on the Decision of Purchasing Prospective Consumers of Grand Cempaka Housing. This shows that the marketing mix is one of the marketing strategies that should be considered so that the implementation of the marketing strategy can run successfully because the elements in the marketing mix influence each other. Variables in the marketing mix which include Products, Prices, Places and Promotions partially have a significant effect on the Decision of Purchasing Prospective Consumers of Grand Cempaka Housing. Keywords: marketing mix; purchase decision; property; housing

This study addresses the relationship of disability with principal lifetime occupation in the elderly. Daily function, cognitive, and physical health variables were assessed in an Italian population of 524 community-dwelling elders aged seventy and over. Farmers had 1.4 (95% C.I.: 0.6 to 2.2) instrumental daily functions lost higher than white-collar workers. Adjustment for age, education, and financial dissatisfaction with multiple linear regression analysis decreased the difference to a still significant figure of 0.9 (95% C.I.: 0.1 to 1.7). On the contrary, adjustment only for cognitive status resulted in complete disappearance of the association [0.2 functions lost (95% C.I.: −0.5 to 0.9)]. The data indicate that greater disability in the most disadvantaged occupational groups may be due to poorer cognition.

This study investigated the financing needs and the financial management practices of banana farmers in Inopacan, Leyte, the relationship between financial management practices to profitability and the relationship of the various socio-demographic variables to the relevant financial management practices. The relevant financial management practices considered in this study were those found to be significantly related to profitability. Results showed that limited capital for labor and the occurrence of pests and diseases particularly bugtok, which is caused by a bacterium called Ralstonia solanacearum, were among the major problems expressed by the banana farmers that need financing. Financial management practices such as monitoring the status of loan and financial planning and budgeting were widely practiced by the farmers. Out of the 25 farmer-respondents who availed of loan, 21 of them monitored the status of their loans. More than half of them also practiced financial planning and budgeting of their income and loan. The multiple linear regression analysis revealed that monitoring financial budgets and comparing farm profitability with that of other farms or benchmarking are significantly and positively related to profitability. The logistic regression analysis showed significant relationships of household size and being able to avail credit with the relevant financial management practices. The results imply that farmers who have more household members and who have availed of credit are more likely to monitor financial budgets and/or compare profitability with other farms.
