A traditional Japanese diet (JD) has been widely regarded as healthy, contributing to longevity. The modern Japanese lifestyle has become markedly westernized, and it is speculated that the number of people who eat JD is decreasing. A simple evaluation of people with low adherence to JD will help improve dietary life. We developed a simple assessment tool that can capture JD, and examined factors associated with low adherence to JD. A total of 1458 subjects aged 18 to 84 years completed a brief self-administered diet history questionnaire. We constructed an empirical Japanese diet score (eJDS) consisting of 12 items from the common characteristics of a JD. In our participants, 47.7% of subjects reported low adherence to JD and only 11.1% demonstrated high adherence. In multivariate logistic regression analysis, younger age persons, physically inactive persons, and heavy drinkers were associated with low adherence to JD. Based on the cutoff values of eJDS, we proposed to create a Japanese diet pyramid that is easy to use visually. In conclusion, the eJDS and the Japanese diet pyramid will be useful tools for nutrition education and dietary guidance.

AbstractObjectiveTo identify dietary patterns in the current Japanese diet and evaluate the associations between these patterns and Na and K intakes.DesignDietary patterns were extracted by factor analysis from the intakes of food groups assessed with a validated self-administrated diet history questionnaire. Na and K intakes and urinary Na:K were assessed by repeated 24 h urine collection.SubjectsHealthy Japanese adults aged 20–69 years (353 men and 349 women).SettingTwenty study areas in twenty-three prefectures in Japan.ResultFour dietary patterns were identified in each sex. After adjustment for several confounding factors, the ‘Fish and vegetable’ pattern was associated with higher urinary Na excretion, but the association was not significant (P=0·37 in men and P=0·06 in women). This pattern was also associated with higher K excretion in both sexes. The ‘Noodle’ pattern tended to be associated with higher urinary Na excretion (P=0·17 in men and P=0·04 in women) and higher Na:K (P=0·02 in men). The ‘Meat, vegetable and oil’ (in men)/‘Meat and oil’ (in women) and ‘Bread and confectioneries’ patterns were not associated with urinary Na excretion (in men) or were negatively associated (in women).ConclusionsContrary to the case in Western countries, the ‘Fish and vegetable’ and ‘Noodle’ patterns contributed to higher Na intake in Japan. Target foods for salt reduction should be set based on careful consideration of the relationships between dietary patterns and Na and K intakes in the target population.

Background and Purpose: Recurrent ischemic strokes increase the risk of disability and mortality. The role of conventional risk factors in recurrent strokes may change due to increased awareness of prevention strategies. The aim of this study was to explore the potential risk factors besides conventional ones which may help to affect the advances in future preventive concepts associated with one-year stroke recurrence (OSR). Methods: We analyzed 6,632 adult patients with ischemic stroke. Differences in clinical characteristics between patients with and without OSR were analyzed using multivariate logistic regression and classification and regression tree (CART) analyses. Results: Among the study population, 525 patients (7.9%) had OSR. Multivariate logistic regression analysis revealed that male sex (OR 1.243, 95% CI 1.025 – 1.506), age (OR 1.015, 95% CI 1.007 - 1.023), and a prior history of ischemic stroke (OR 1.331, 95% CI 1.096 – 1.615) were major factors associated with OSR. CART analysis further identified age and a prior history of ischemic stroke were important factors for OSR when classified the patients into three subgroups (with risks of OSR of 8.8%, 3.8%, and 12.5% for patients aged > 57.5 years, ≤ 57.5 years/with no prior history of ischemic stroke, and ≤ 57.5 years/with a prior history of ischemic stroke, respectively). Conclusions: Male sex, age, and a prior history of ischemic stroke could increase the risk of OSR by multivariate logistic regression analysis, and CART analysis further demonstrated that patients with a younger age (≤ 57.5 years) and a prior history of ischemic stroke had the highest risk of OSR.

The identification of sodium and potassium intake in youths is an important step to preventing the increase of blood pressure in childhood. We examined food intake and estimated mineral intake using a brief-type self-administered diet history questionnaire (BDHQ) to test its validity as a comparison with urinary excretion in Japanese youths. The subjects were 5th and 8th graders (n = 2377), who completed the BDHQ and permitted the use of their overnight urine specimens. Sodium intake was poorly associated with sodium excretion (Rho = 0.048), and the coefficients of dietary potassium and a sodium-to-potassium molar ratio were 0.091–0.130. Higher soybean paste (miso) intake and pickles were significantly associated with higher sodium excretion (p ≤ 0.005). However, these foods were positively associated with potassium excretion (p = 0.002–0.012), and not associated with an excreted sodium-to-potassium ratio. Fruits and dairy products were positively associated (p ≤ 0.048), whereas beverages were negatively associated with potassium excretion (p ≤ 0.004). The association of the sodium-to-potassium ratio was opposite to that of potassium (p ≤ 0.001). The choice of foods, potassium, and the sodium-to-potassium ratio assessed using the BDHQ are available as part of health education for youths, but the assessment of sodium intake in population levels should be carefully conducted.

Ordovician and Siluro-Lower Devonian magmatic rocks in the northern Appalachians south of the Iapetus suture are currently interpreted as distinct belts composed of multiple, small, peri-Gondwanan terranes that amalgamated during the sequential closures of Iapetus (latest Ordovician), the Tetagouche backarc basin (early Silurian), the Acadian seaway (Siluro-Devonian), and the Rheic Ocean (Devono-Carbonferous) (multiple terrane model). Here, the Siluro-Lower Devonian magmatic belts are shown to have slab failure affinities and together with the Ordovician arcs form paired belts parallel to the Iapetus suture, which suggests that they were emplaced along the common, peri-Avalonian margin during pre- and post-collisional processes. The Iapetan suture and the paired belts are inferred to repeat in Atlantic Canada due to dextral, strike-slip processes of mid-Late Devonian or younger age (terrane wreck model). In Newfoundland, the repetition is inferred to be the result of oblique, dextral offset of ca. 250 km. In the Quebec Embayment, the Iapetan paired magmatic belts are repeated twice in the limbs of a Z-shaped orocline related to oblique, dextral offsets of ca. 1200 km of the southern limb. Limited Siluro-Devonian paleomagnetic data indicate no paleolatitudinal differences across the Iapetus suture, however ca. 100° post-mid Silurian clockwise rotation is indicated for the middle fold limb; these data favour the terrane wreck model. The terrane wreck model results in a simple tectonic scenario of southerly subduction of Iapetus beneath a single ribbon continent (Avalonia sensu lato) that was subsequently deformed.

This study examined sociodemographic, diagnostic, psychological, and episode-based variables in a sample of 130 psychiatric patients admitted to treatment at least twice in a 6-yr. period. Short length of initial hospitalization ( r = -.30, p <.01) and younger age on initial admission ( r = -.20, p <.05) were significantly correlated with frequent hospital admissions. Scores on four of the subscales of the WAIS-R were significantly correlated with readmission, confirming that patients who have fewer cognitive resources are at risk of frequent admissions. A multiple regression analysis combining variables to predict readmission accounted for only 12% of the common variance ( r128 = .34, p <.01), however, indicating that a prediction equation with these variables has limited clinical utility.
