AbstractObjectiveTo identify dietary patterns in the current Japanese diet and evaluate the associations between these patterns and Na and K intakes.DesignDietary patterns were extracted by factor analysis from the intakes of food groups assessed with a validated self-administrated diet history questionnaire. Na and K intakes and urinary Na:K were assessed by repeated 24 h urine collection.SubjectsHealthy Japanese adults aged 20–69 years (353 men and 349 women).SettingTwenty study areas in twenty-three prefectures in Japan.ResultFour dietary patterns were identified in each sex. After adjustment for several confounding factors, the ‘Fish and vegetable’ pattern was associated with higher urinary Na excretion, but the association was not significant (P=0·37 in men and P=0·06 in women). This pattern was also associated with higher K excretion in both sexes. The ‘Noodle’ pattern tended to be associated with higher urinary Na excretion (P=0·17 in men and P=0·04 in women) and higher Na:K (P=0·02 in men). The ‘Meat, vegetable and oil’ (in men)/‘Meat and oil’ (in women) and ‘Bread and confectioneries’ patterns were not associated with urinary Na excretion (in men) or were negatively associated (in women).ConclusionsContrary to the case in Western countries, the ‘Fish and vegetable’ and ‘Noodle’ patterns contributed to higher Na intake in Japan. Target foods for salt reduction should be set based on careful consideration of the relationships between dietary patterns and Na and K intakes in the target population.

AbstractObjectiveAlthough dietary pattern approaches derived from dietary assessment questionnaires are widely used, only a few studies in Western countries have reported the validity of this approach. We examined the relative validity of dietary patterns derived from a self-administered diet history questionnaire (DHQ) among Japanese adults.DesignThe DHQ, assessing diet during the preceding month, and 4 d dietary records (DR) were collected in each season over one year. To derive dietary patterns, 145 food items in the DHQ and 1259 in the DR were classified into thirty-three predefined food groups, and entered into a factor analysis.SettingThree areas in Japan; Osaka (urban), Nagano (rural inland) and Tottori (rural coastal).SubjectsA total of ninety-two Japanese women and ninety-two Japanese men aged 31–76 years.ResultsWe identified three dietary patterns (‘healthy’, ‘Western’ and ‘Japanese traditional’) in women and two (‘healthy’ and ‘Western’) in men, which showed a relatively similar direction and magnitude of factor loadings of food groups across the first and mean of four DHQ (DHQ1 and mDHQ, respectively) and 16 d DR. The Pearson correlation coefficients between DHQ1 and 16 d DR for the healthy, Western and Japanese traditional patterns in women were 0·57, 0·36 and 0·44, and for the healthy and Western patterns in men were 0·62 and 0·56, respectively. When mDHQ was examined, the correlation coefficients improved for women (0·45–0·69).ConclusionsDietary patterns derived from the DHQ could be used for epidemiological studies as surrogates of those derived from DR.

Abstract Food frequency questionnaires, the primary method of dietary assessment in large-scale nutritional epidemiologic studies, preclude an informed evaluation of the timing of dietary intake and meal-specific dietary intake. In this study, we developed the Meal-based Diet History Questionnaire (MDHQ), a self-administered questionnaire designed for estimating food and nutrient intakes for each meal type separately. The development was done based on a 16-day dietary record obtained from 242 Japanese adults. The MDHQ consisted of the three different parts, with a total of 196 items. Part 1 of the MDHQ asks about consumption frequency of generic food groups (n 24) for each meal type: breakfast, morning snack, lunch, afternoon snack, dinner, and night snack. Part 2 of the MDHQ asks about relative consumption frequency of sub-food groups within one of the generic food groups which are asked in Part 1. Combining information derived from Parts 1 and 2 enables to increase the number of foods we can estimate efficiently but within a limited number of questions. Part 3 of the MDHQ asks about general eating behaviors, which are intended to use in a variety of ways during dietary intake calculation. A series of calculation algorithms for food groups, energy, and nutrients were also prepared. Given that the MDHQ was empirically developed based on comprehensive information on actual food consumption, this innovative tool may be promising for future epidemiological research on meal patterns and time of day of dietary intake, or chrono-nutrition research. A rigorous evaluation of validity of the MDHQ is warranted.

Although several nutrients and foods are suggested to be preventive against postpartum depression, all previous studies have primarily focused on single nutrients or foods. In contrast, studies on dietary patterns, namely the measurement of overall diet by considering the cumulative effects of nutrient, may provide new insights into the influence of diet on postpartum depression. We prospectively examined the association between dietary patterns during pregnancy and the risk of postpartum depression among 865 Japanese women. Diet was assessed with a validated, self-administered diet history questionnaire. Dietary patterns from thirty-three predefined food groups (energy-adjusted food (g/d)) were extracted by factor analysis. Postpartum depression was defined as present when the subjects had an Edinburgh Postnatal Depression Scale score of ≥ 9 at 2–9 months postpartum. A total of 121 women (14·0 %) were classified as having postpartum depression. Three dietary patterns were identified: ‘Healthy’, ‘Western’ and ‘Japanese’ patterns. After adjustment for potential confounders, neither the ‘Healthy’ nor the ‘Japanese’ pattern was related to the risk of postpartum depression. Compared with the first quartile of the ‘Western’ pattern, only the second quartile was independently related to a decreased risk of postpartum depression (multivariate OR 0·52, 95 % CI 0·30, 0·93), although no evident exposure–response associations were observed (P for trend = 0·36). The present study failed to substantiate clear associations between dietary patterns and the risk of postpartum depression. Further studies with more accurate measurements are warranted to confirm the relationship between dietary patterns and the risk of postpartum depression.

Although many epidemiological studies have examined the association of dietary glycaemic index (GI) and glycaemic load (GL) with health outcomes, information on the reproducibility and relative validity of these variables estimated from dietary questionnaires is extremely limited. We examined the reproducibility and relative validity of dietary GI and GL assessed with a self-administered diet-history questionnaire (DHQ) in adult Japanese. A total of ninety-two Japanese women and ninety-two Japanese men aged 31–76 years completed the DHQ (assessing diet during the preceding month) and 4 d dietary records (DR) in each season over a 1-year period (DHQ1–4 and DR1–4, respectively) and the DHQ at 1 year after completing DHQ1 (DHQ5). We used intraclass correlations between DHQ1 and DHQ5 to assess reproducibility, and Pearson correlations between the mean of DR1–4 and mean of DHQ1–4 and between the mean of DR1–4 and DHQ1 to assess relative validity. Reproducibility correlations for dietary GI and GL were 0·57 and 0·69 among women and 0·65 and 0·58 among men, respectively. Validity correlations for dietary GI and GL assessed by DHQ1–4 were 0·72 and 0·66 among women and 0·65 and 0·71 among men, respectively. Corresponding correlations for DHQ1 were 0·53 and 0·58 among women and 0·57 and 0·60 among men, respectively. White rice was the major contributor to GI and GL in both methods (49–64 %). These data indicate reasonable reproducibility and relative validity of dietary GI and GL assessed by a DHQ for Japanese adults, whose dietary GI and GL are primarily determined by the GI of white rice.

AbstractObjectiveDietary questionnaires for assessing dietary intakes among populations of individuals aged 80 years or older (the very old) are very limited. We examined the relative validity of forty-three nutrients and twenty-seven food groups estimated by a brief-type self-administered diet history questionnaire (BDHQ) targeting very old Japanese, using semi-weighed dietary records (DR) as a reference.DesignBetween June and August 2012 and between June 2015 and February 2016, a three-day non-consecutive DR (at two-week intervals) and a BDHQ were completed.SettingTokyo, the capital prefecture of Japan.SubjectsEighty very old Japanese (thirty-six men and forty-four women) aged 82–94 years.ResultsThe median intakes of 40–70 % of the crude and energy-adjusted nutrients estimated by the BDHQ were significantly different from those estimated by the DR. The median Spearman’s correlation coefficient of nutrient intakes between the BDHQ and the DR was 0·39–0·46. About half (48–56 %) of the food groups were significantly different in terms of the median intakes estimated by the BDHQ and the DR in crude and energy-adjusted values. The median Spearman’s correlation coefficient between the BDHQ and the DR was 0·45–0·48.ConclusionsAcceptable Spearman’s correlations (≥0·3) were obtained for many dietary intakes among the very old Japanese population. The BDHQ is a good candidate for epidemiological studies among very old Japanese, although, for some nutrients and food groups, the difficulty of estimating accurate median intakes is one of the limitations for the tool. Further efforts to enhance the validity of the BDHQ for very old populations are needed.

AbstractObjectiveTo validate a diet history questionnaire (DHQ) using a weighed food record (WFR) as the standard method in the estimation of food consumption and nutrient intake in a group of adults.DesignWFR: all foods consumed by subjects during 7 consecutive days were weighed and recorded by nutrition students. Two DHQ interviews were carried out on days 1 (first diet history questionnaire, DHQ1) and 28 (second diet history questionnaire, DHQ2).SettingCosta Rica.SubjectsSixty adults: 30 men and 30 women; 30 living in urban and 30 in rural areas.ResultsIn comparison to the WFR, the DHQ1 gave statistically significant higher estimates of the mean intake of 19 nutrients for men and of three nutrients for women. The uncorrected correlation coefficients for nutrient intake according to both methods ranged from 0.40 to 0.83 for males and from 0.22 to 0.62 for females. Percentage of subjects classified in the same quartiles of nutrient intake according to each method ranged from 33.3% to 63.3% for males and from 23.3% to 53.3% for females. Misclassification in extreme quartiles ranged from 0% to 13.3% for both sexes. The mean food group consumption, according to the DHQ1, when compared with the WFR, gave statistically significant differences for three of the 18 food groups for men and for two groups in the case of women. The two applications of the DHQ gave similar results.ConclusionValidation of a DHQ using a WFR as the standard method gave results that compare favourably with those reported by other authors. This study found important differences in the response of men and women to the DHQ: among men, the estimates of mean nutrient intake from DHQ1 were significantly greater than those of the WFR, while in the case of women, the mean nutrient intake estimates from both methods were not significantly different. There was a higher degree of correlation between the DHQ1 and the WFR mean nutrient intakes among men than among women. The DHQ showed good reproducibility.

AbstractObjectiveTo evaluate the Canadian Diet History Questionnaire I (C-DHQ I) food list and to adapt the US DHQ II for Canada using Canadian dietary survey data.DesignTwenty-four-hour dietary recalls reported by adults in a national Canadian survey were analysed to create a food list corresponding to C-DHQ I food questions. The percentage contribution of the food list to the total survey intake of seventeen nutrients was used as the criterion to evaluate the suitability of the C-DHQ I to capture food intake in Canadian populations. The data were also analysed to identify foods and to modify portion sizes for the C-DHQ II.SettingThe Canadian Community Health Survey (CCHS) – Cycle 2.2 Nutrition (2004).SubjectsAdults (n20 159) who completed 24 h dietary recalls during in-person interviews.ResultsFour thousand five hundred and thirty-three foods and recipes were grouped into 268 Food Groups, of which 212 corresponded to questions on the C-DHQ I. Nutrient intakes captured by the C-DHQ I ranged from 79 % for fat to 100 % for alcohol. For the new C-DHQ II, some food questions were retained from the original US DHQ II while others were added based on foods reported in CCHS and foods available on the Canadian market since 2004. Of 153 questions, 143 were associated with portion sizes of which fifty-three were modified from US values. Sex-specific nutrient profiles for the C-DHQ II nutrient database were derived using CCHS data.ConclusionsThe C-DHQ I and II are designed to optimize the capture of foods consumed by Canadian populations.

AbstractObjectiveGlobally, the Na consumption of most people exceeds the WHO recommendation. To be effective, salt reduction programmes require assessment of the dietary sources of Na. Due to methodological difficulties however, comprehensive assessments are rare. Here, we identified Na sources in the Japanese diet using a 4 d diet record that was specifically designed for Na source description.DesignA cross-sectional study.SubjectsApparently healthy men (n 196) and women (n 196) aged 20–69 years.SettingThe subjects were recruited from twenty-three of forty-seven prefectures in Japan.ResultsThe proportion of discretionary Na intake in total Na intake was 52·3 % in men and 57·1 % in women, and was significantly lower in younger subjects. The two major food groups contributing to Na intake were seasonings such as salt or soya sauce (61·7 % of total Na intake in men, 62·9 % in women) and fish and shellfish (6·7 % in men, 6·6 % in women). The third major contributor differed between men and women (noodles in men, 4·9 %; bread in women, 5·0 %). Further, the contribution of each food group to total Na intake differed among age groups.ConclusionsWhile individual efforts to decrease Na intake remain important, population approaches to reducing Na content in processed foods are already equally important and will assume greater importance in the future even in Japan, an Asian country facing a rapid Westernization in dietary habits.

A traditional Japanese diet (JD) has been widely regarded as healthy, contributing to longevity. The modern Japanese lifestyle has become markedly westernized, and it is speculated that the number of people who eat JD is decreasing. A simple evaluation of people with low adherence to JD will help improve dietary life. We developed a simple assessment tool that can capture JD, and examined factors associated with low adherence to JD. A total of 1458 subjects aged 18 to 84 years completed a brief self-administered diet history questionnaire. We constructed an empirical Japanese diet score (eJDS) consisting of 12 items from the common characteristics of a JD. In our participants, 47.7% of subjects reported low adherence to JD and only 11.1% demonstrated high adherence. In multivariate logistic regression analysis, younger age persons, physically inactive persons, and heavy drinkers were associated with low adherence to JD. Based on the cutoff values of eJDS, we proposed to create a Japanese diet pyramid that is easy to use visually. In conclusion, the eJDS and the Japanese diet pyramid will be useful tools for nutrition education and dietary guidance.
