Motivating healthcare professionals is a critical function of healthcare management. Without solving the problems of motivation, it is impossible to really improve the quality and culture of providing medical care to the population, as well as to increase the efficiency of the activities of medical institutions and the industry as a whole on the basis of the competent use of all types of resources, material, financial and personnel. The use of motivation also helps to reduce the number of errors in work due to the human factor associated with its low level and with an insufficiently serious attitude to work.

Modern social and cultural reality forces the heads of medical institutions to pay increased attention to the human factor, rethinking such basic concepts as «management» and «organization». It is important to cultivate the most comfortable person-centered work environment in medical teams. The main goal of this article was to demonstrate the feasibility of using robust management to maintain the constant attention of the medical administration on the interpersonal relations of employees. The article contains a brief description of robust management tools, a description of the person-centered approach, as well as examples of the use of robust regulators and socionic profiling estimators. The potential of robust management is not revealed when the management simply declares its merits, but when the entire medical team wants and can use it, actively involved in achieving the desired result.

Abstract Background International studies show that child maltreatment is a widespread but often underestimated problem that causes high individual, social and economic costs. Child maltreatment is an important topic for the medical sector as well. On the one hand, affected persons often seek support and help from healthcare professionals, but on the other hand, assaults can also occur in medical institutions by healthcare professionals. Surprisingly, there is hardly any data on the frequency of child maltreatment by healthcare professionals in general and particularly by nursing staff. Methods Therefore, in a large representative survey of the German population of 2,516 subjects aged between 14 and 91, the experience of child maltreatment in medical institutions by nursing staff was assessed retrospectively. Results Of the 46 subjects who had an inpatient stay in a child and adolescent psychiatry before the age of 18, 33.3% reported to have experienced maltreatment by nursing staff, while 17.3% of the 474 persons who had an inpatient stay in general or pediatric hospitals experienced maltreatment by nursing staff. All forms of maltreatment were significantly more frequent in psychiatric compared to general and pediatric hospitals. Conclusions The results of our representative retrospective survey demonstrate that maltreatment by nursing staff are not rare individual cases, but that medical facilities bear systemic risks for assault. Therefore, it is necessary that all medical institutions, in particular psychiatric hospitals, address this issue. In order to reduce the risk for assaults, it is important not only to implement structural measures but also to develop an attitude that emphasizes zero tolerance for violence against children and adolescents.

The article emphasizes the importance and necessity of establishing a School for Tuberculosis Patients. The results of the questionnaire of medical nurses and patients of the anti-tuberculosis clinic are presented to the readers. The author shares the experience of organizing the School and draws attention to the fact that the best means of safety for the medical worker and the patient in the medical organization is competent actions of all participants of the process.

The aim of the study was to search for effective forms of selection of nursing personnel and their placement in a medical organization. Results: the analysis and study of the reasons for the turnover of nursing staff were carried out, effective forms of selection and placement of personnel in the organization were recommended. Conclusion: a qualitatively new level of development of the health care system cannot be achieved without the effective use of human resources. The study showed the role of personnel management, effective use of «human resources», rational selection of medical personnel.

This article provides an overview of the literature data on the analysis of the role of nutrition in the prevention and treatment of nephrological diseases. Scientists' data on the importance of certain nutrients for human health were also analyzed. The article under consideration is devoted to the problem of organizing medical nutrition in medical institutions of nephrological departments, with the aim of further developing measures to optimize this nutrition.

Повышение эффективности использования государственных ресурсов и качества предоставляемой медицинской помощи в настоящее время остается одной из приоритетных задач науки управления здравоохранением. Зарубежными странами накоплен опыт решения подобных задач. В промышленности и сфере услуг разные методы находят успешное применение. Одним из таких подходов стало внедрение в практику принципов бережливого производства. В октябре 2016 года по инициативе Управления по внутренней политике Администрации Президента Российской Федерации стартовал пилотный проект по совершенствованию системы оказания первичной медико-санитарной помощи «Бережливая поликлиника»: создание медицинской организации, ориентированной на потребности пациента, бережное отношение к временному ресурсу как основной ценности за счет оптимальной логистики реализуемых процессов, организованной с учетом принципов эргономики и соблюдения объема рабочего пространства, создающей позитивный имидж медицинского работника, организация оказания медицинской помощи, которая основана на внедрении принципов бережливого производства. Создание «Бережливой поликлиники» стало возможно с помощью реализации комплекса мероприятий, направленных на соблюдение приоритета интересов пациента, организацию оказания медицинской помощи пациенту с учетом рационального использования его времени, повышение качества и доступности медицинской помощи, обеспечение комфортности условий предоставления медицинских услуг, повышение удовлетворенности уровнем оказанных услуг, сокращение нагрузки на медицинский персонал за счет повышения эффективности деятельности медицинской организации переходом на электронный документооборот, сокращением объема бумажной документации. В работе представлен анализ организационных направлений по созданию новой модели оказания первичной медико-санитарной помощи Increasing the efficiency of the use of state resources and the quality of medical care provided currently remains one of the priority tasks of the science of healthcare management. Foreign countries have accumulated experience in solving such problems. In industry and the service sector, different methods are successfully applied. One of these approaches was the introduction of lean manufacturing principles into practice. In October 2016, at the initiative of the Department for Internal Policy of the Presidential Administration of the Russian Federation, a pilot project to improve the system of providing primary health care "Lean Polyclinic" was launched: the creation of a medical organization focused on the needs of the patient, respect for the time resource as the main value due to optimal logistics of the implemented processes, organized taking into account the principles of ergonomics and compliance with the volume of the working space, creating a positive image of the medical worker, the organization of medical care in which is based on the implementation of the principles of lean production. The creation of the "Lean Polyclinic" became possible through the implementation of a set of measures aimed at observing the priority of the patient's interests, organizing the provision of medical care to the patient, taking into account the rational use of his time, improving the quality and availability of medical care, ensuring the comfort of the conditions for the provision of medical services, increasing satisfaction with the level of services provided. Services, reducing the burden on medical personnel by increasing the efficiency of the medical organization, switching to electronic document management, reducing the volume of paper documentation. The paper presents an analysis of the organizational directions for the creation of a new model of primary health care delivery

Purpose of the study. To demonstrate the difficulties of objective interpretation of providing medical care to patients with critical limb ischemia.Materials and methods. The results of treatment of more than 29 thousand patients of one of the highly specialized medical organizations of St. Petersburg were studied, in which the main activity of the staff is the examination of patients with surgical infection of soft tissues (particularly, the critical limb ischemia).Results. It has been ascertained that, despite the unconditional success of providing medical care to patients with critical limb ischemia, a high frequency of unsatisfactory treatment results for this category of patients remains (in 2017, among 685 patients of the specialized department of the highly specialized medical organization, the proportion of cases of performing high amputations of limbs and mortality, respectively, was 11,7% (80) and 2,6% (18)). It has been demonstrated on clinical examples that the pronounced comorbidity of critical limb ischemia patients and the severity of their general condition (combined with organizational difficulties in providing them with providing medical care in conditions of highly specialized medical organization) contribute to the formation of complications (up to lethal outcomes) of not only the underlying, but also concomitant diseases. Difficulties in providing medical care of adequate quality in life-threatening conditions for patients of highly specialized medical organization in cases of critical limb ischemia necessitate a wider interaction between specialists in narrow and multidisciplinary hospitals as part of the emergency surgery service.Conclusion. When creating specialized medical centers outside of multidisciplinary medical institutions, the possibilities should be provided for the regulated involvement of highly professional consultants with a reasonable algorithm for routing patients in cases of the need for their emergency re-hospitalization to other medical institutions.
