The analysis of existing methods and principles of occupational risk assessment is presented. It is concluded that there is no methodology that takes into account the special features of professional activity of employees of the fuel and energy complex, working conditions factors, as well as the degree of employee fatigue. Recommendations for making adjustments to existing regulatory documents have been developed.

Aims - to analyze the historical aspects of the formation of the national normative framework in the field of primary and periodic medical examinations, to discuss the existing problems in the carrying out of primary and periodic medical examinations of workers and also to make suggestions for quality improvement of primary and periodic medical examinations and for introduction of occupational risk assessment into the practice of primary and periodic medical examinations. Conclusion. For quality improvement of primary and periodic medical examinations, for efficiency improvement of primary and secondary prevention of occupational diseases it is considered to be necessary to take the following suggestions: - fuller coverage of working people by periodic medical examinations; - thorough selection of organizations that perform the special assessment of working conditions; - the use of the results of the special assessment of working conditions for improving working conditions and informing workers about working conditions; - implementation of occupational risk assessment into practice of primary and periodic medical examinations; - the use of modern methods of molecular and genetic screening;

The article reflects methodological approaches to the construction of a priori models for assessing the occupational risk of medical workers in physiotherapeutic departments of sanatoriums under the influence of a complex of negative factors of the production environment. The authors carried out a comprehensive hygienic assessment of the occupational risk of medical personnel with the combined use of physiotherapeutic procedures in one of the sanatoria and resort institutions of the Altai Territory. The main objectives of the study were to measure physical, radiation and chemical factors in the work environment at workplaces of medical personnel and to calculate the degree of the occupational risk of workers in accordance with a priori risk assessment models. Based on the results of measurements of physical factors at workplaces of medical workers of physiotherapeutic departments, it is possible to conclude that complex harmful effects include inadequate levels of the air temperature in the work area, artificial illumination, air ionic composition of air and electromagnetic fields. The calculated risk was 0.0975, which corresponds to a moderate risk. The obtained results testify to the existence of a risk, the level of which cannot be neglected, despite the compliance of the level of impact with normative documents. The results of studies of the radiation factor at workplaces in the radon therapy department indicate the equivalent equilibrium radon volume activity to range from 25-109 Bq/m³,and the ambient dose equivalent of gamma radiation in the range of 0.15-0.18 μSv/h, which corresponds to hygienic standards. Therefore, the use of risk-oriented models seems reasonable even in working conditions that are not harmful or dangerous according to the criteria for a special assessment of working conditions. The leading role of electromagnetic radiation for this category of workers is proved in the course of calculations using risk-oriented models of professional risk assessment. It is proposed to develop measures to reduce the electromagnetic load, taking into account the medium and short-term outlook, with an annual review of the degree of occupational risk.

Objective: to develop and theoretically substantiate such an approach, which should ensure the creation of information and calculation-analytical basis for planning and implementation of effective preventive measures aimed at minimizing occupational risks in the workplace. Various approaches to the calculation of occupational risk are analyzed, primarily those based on the results of the assessment of working conditions in the workplace on the indicators of production factors (certification of jobs). Among the general shortcomings of the methods that use the results of job certification, the main ones should be noted: the probability of violation of the measurement method, insufficient objectivity in the assessment of production factors, the long period between measurements, high cost. It is noted that the main tasks that need to be addressed are: the formation of information databases for calculations, development of assessment methods and the involvement of qualified experts. A method of occupational risk assessment is proposed, the peculiarity of which is taking into account the results of measurements of the levels of production factors and expert assessments. As a result of calculations, the calculated value of occupational risk for a particular profession allows us to draw a conclusion about the extent to which working conditions meet current regulatory requirements. Conclusions: the proposed approach provides incentives for employers to ensure safe and harmless working conditions, as well as active involvement of employees in solving problems of labor protection. As promising areas for improving the assessment of occupational risks noted: the introduction of a flexible mechanism for systematic survey of working conditions in the workplace, justification of levels of gradation of occupational risk, the introduction of modern information systems for accounting and analysis of production factors in dynamics.

Occupational risk assessment was carried out for the employees of 956 different professions of 20 enterprises under study in five sectors of the economy: construction, mining, manufacturing, agriculture, transportation, and warehousing. The study was conducted using the Methodological Recommendation on the implementation of the occupational risk management system at an enterprise. This Methodological Recommendation was developed by the Republican state enterprise on the right of economic management — Republican Scientific Research Institute for Labour Protection of Ministry of Labour and Social Protection of Population of the Republic of Kazakhstan. Occupational risks were assessed in the context of specific professions and structural divisions of the enterprises by five indicators: harmful working conditions, injury risk of the labor process, safety of production equipment, availability of personal protective equipment, and risk of morbidity. Based on the results of the assessment, the degree of individual occupational risk on each assessed indicator is determined by five gradations: I — acceptable risk, II — low, III — medium, IV — high and V — very high. According to the results of the occupational risk assessment, a high risk (4 degree) was established at the enterprises of mining, construction, agricultural industries, as well as transport, and warehousing industries. According to the results of equipment safety assessment, a high risk (grade IV) was identified at the enterprises of transport and mining industries. Due to the low provision with the personal protective equipment and its non-compliance with the certification requirements, a high risk is assigned to the enterprises of mining and construction industries. It is established that a high degree of occupational risk is due to the injury risk of working conditions, insufficient provision of personal protective equipment along with their low quality, the severity and intensity of the labor process accompanied by a high degree of the employees morbidity, as well as the use of the outdated equipment in production.

The article analyzes the international and domestic experience in building a safety and hygiene management system. It is noted that the priorities in the process of ensuring safe and harmless working conditions for many developed countries are: revision and improvement of the legal framework for occupational safety and health, encouraging employers to ensure proper working conditions, and raising awareness of managers and employees on occupational safety and health. Recommendations for planning measures depending on the hazards present in enterprises and the results of risk assessment are contained in the documents of the International Labor Organization. Methods and methods of detection and assessment of risk factors can be chosen according to the characteristics of the workplace. Effective occupational risk reduction activities involve the implementation of a certain sequence of tools and management methods (control hierarchy), as specified in the international standard ISO 45001. It is proposed to use the following priorities for risk reduction: elimination of hazards, replacement of processes, operations, materials or equipment for less dangerous, the use of technical means and methods of management, reorganization of work and training of employees, administrative control of hazards, personal protective equipment. According to the results of the research, in the absence of resources for the implementation of measures of the highest priorities, the most effective approach was the combination of technical means and management methods, reorganization of work and training, as well as administrative control of hazards. To implement occupational health and safety management systems based on a risk-based approach, it is necessary, first of all, to develop and approve a methodology that determines the procedure for calculating occupational risk and ensure that the results of risk assessment are taken into account when planning preventive measures.

The digital transformation of the economy and society prompts the renewal of methods in occupational health as well. There are outlined fundamentals of the draft revision of the “Guidelines for the Assessment of Occupational Risk to Employee Health. Organizational and methodological foundations, principles and criteria for the assessment». R 2.2.1766-03. The draft contains methods of occupational risk (OR) assessment supplied with information materials and software for working on the Internet in real-time. Algorithms and models for calculating the probability of impairment of health according to data of the Special Assessment of Working Conditions (SAWC) as well as causation algorithms - work-relatedness assessment according to data of periodic medical examinations (PME) for the validity of risk assessment from the standpoint of evidence-based medicine are presented. There are set out methods for calculating the probability of the formation of occupational diseases (OD) of the main nosological forms from the exposure of noise, hand-arm and whole-body vibration, heat and cool microclimate, fibrogenic dusts, physical labor (local, regional and general loads), strenuous work, standing work with the likelihood of varicose lower veins limbs, multi-factor exposure and low-back pain. Risk assessments for the reproductive health of workers are also discussed. The models are based on the documents of the WHO, ILO, ISO, domestic and foreign literature data that meet the principles of evidence-based medicine. The problems of work-related diseases (WRD) - the concept of WHO (1987) -basics of etiology, qualitative and quantitative criteria for causation, models of consensus and evidence-based medicine, as well as projects for integrated work on the detection systems for WRD in the European Union countries are considered. Single-digit indices of working conditions and health problems, moral harm caused to an employee suffering from OD or WRD. Hygienic recommendations to employers on OR management, prevention priorities, the role of personal protection equipment, the basics of corporate health promotion programs, and ethical and economic aspects are described. The calculation methods are provided by the software of the electronic online reference book “Occupational Risk” (URL: http://medtrud.com/) in order to ensure evidence-based decisions of hygienists. In conclusion, a turning point in occupational health is noted: a decrease in the detection of traditional ODs without compensation by the number of OD and WRD from new technologies, materials and risk factors; this problem requires urgent action.

The aim of this work is improving the procedure for professional risk assessment of passenger bus drivers, taking into account ergonomic, psychosocial, individual, and hygienic factors. To develop the methodology for assessing the professional risk of drivers the theory of analysis was used, which allows to establish alternative solutions of the problem by determining an integrated criterion that depends on a number of individual indicators characterized by ergonomic, psychosocial, individual, and hygienic factors influencing the working conditions of the passenger bus driver. A checklist has been developed to quickly assess the occupational risk of passenger bus drivers, which takes into account the impact of several harmful production factors that contribute to the creation of unfavorable conditions for the occurrence of occupational diseases.  A feature of this approach is the ability to distinguish the level of risk for each harmful production factor and determine the overall level of occupational risk. It is recommended to reduce the effect of occupational risk on the driver of a passenger bus to improve the system of medical examinations, the system of admission to the vehicles driving in the implementation of passenger traffic and strengthen control over the technical condition of vehicles. The relationship between the integrated criterion of occupational risk of the driver and indicators that are characterized by ergonomic, psychosocial, individual, and hygienic factors, which are formed by the working conditions of the driver in the carriage of passengers. As a result, a new approach (checklist) is proposed, which can be used in trucking companies without any additional equipment after training in testing.
