Background and aims: Osteoporosis is one of the most common metabolic bone diseases with systemic involvement of the body skeleton. The Peganum harmala seed contains high amounts of carboline alkaloids, which have been shown to have positive effects on bone formation in animal studies. In the present study, the effect of an oral capsule of P. harmala seed on bone density was evaluated in menopausal women prone to osteoporosis. Methods: In this randomized controlled clinical trial, 100 women referring to the orthopedic clinic with a diagnosis of osteoporosis were included and divided into the intervention group treated with calcium D (500 mg) twice a day, Osteofos (70 mg) per week, and P. harmala (500 mg) twice‐a‐day, and the control group treated with calcium D and Osteofos. Before and three months after the intervention, patients were evaluated for osteoporosis using bone densitometry. Finally, independent t-test, paired t-test, and repeated measures ANOVA were used for statistical analysis. Results: The mean bone mineral density (BMD) of the femur before and after the intervention showed significant improvements in the intervention and control groups (P<0.001). The mean differences in BMD before and after the intervention were significant in both control and intervention groups with higher improvements in the intervention group (P<0.001). Although the mean BMD of the spine before the intervention was not significantly different between the two groups (P=0.167), it was better in the intervention group after the intervention (P=0.030). Conclusion: The findings of the present study confirmed the beneficial effects of P. harmala on osteoporosis while the lack of any changes in liver enzymes.

Introduction: In the last decades, Hodgkin lymphoma (HL) has become a curable disease with a long-term disease-free survival achieved in about 80% of patients. The two main chemotherapy protocols presently used in HL treatment are: ABVD (adriamycin, bleomycin, vinblastine, dacarbazine) and escalated BEACOPP (EB) (bleomycin, etoposide, adriamycin, cyclophosphamide, vincristine, procarbazine, prednisone). Patients treated with ABVD also receive glucocorticoids to minimize regimen toxicity and as antiemetic prophylaxis. Similarly, prednisolone given for 14 days is an integral part of the EB regimen. However, glucocorticoids are known to interfere with bone formation and remodeling, which results in increased fracture risk. To reduce the rate of complications, such as aseptic necrosis of the femur head, according to the H2 Israeli protocol, patients treated with EB receive prednisone for seven days only. The current study was designed to evaluate the vertebral density (VD) changes associated with HL therapy. Methods: In this retrospective study, data on all newly-diagnosed HL patients treated at the Rambam Health Care Campus between 2008-2016 were retrieved from the institutional computerized database. Patients were grouped based on their treatment protocol and further sub-grouped according to the number of treatment cycles: I ‒ ABVDx2-4, II ‒ ABVDx6, III ‒ EBx2+ABVDx4, IV ‒ EBx4-6+ABVDx2. VD findings of PET/CT scans performed at baseline and post-treatment were compared at L3 vertebral level. The Hounsfield unit (HU) scale was used to evaluate bone mineral density (BMD). The dose of hydrocortisone equivalents (HE) per meter square of the body surface area was calculated for each patient. Results: All the identified newly-diagnosed HL patients (n=213) were included in the analysis. Their median age was 29 (18-59) years. Both baseline and post-treatment scan results were available for all patients. Mean VD at baseline was 198 HU (30.5-320) and post-treatment ‒ 165.5 HU (27.5-277.3). At baseline, 7 patients had osteopenia (<120 HU) and 1 patient had osteoporosis (<90 HU). The corresponding post-treatment numbers were 21 and 9 patients, respectively. Data on mean VD at baseline and post-therapy categorized according to the treatment received are presented in Table 1. The mean VD change from the baseline level was 14.7% in the ABVD group as a whole, 9.3% HU in the ABVDx2-4 subgroup and 17.3% in the ABVDx6 subgroup. The mean VD change from the pre- to post-therapy level was 20.5% for the whole EB group, 24.3% for the EBx2 + ABVDx4 subgroup and 15.4% for the EBx4-6 + ABVDx2 subgroup. The relative VD decreased post-treatment in all the subgroups. Using the ABVDx2-4 group as reference (HR=1), the HR for ABVDx6 group was 6.51 (95% CI 2.8-15.3; P=0.000), with HR=16.11 (95% CI 5.6-46.8; P=0.000) for EBx2 + ABVDx4 group and HR=2.78 (95% CI 0.9-8.2; P=0.065) for EBx4-6 + ABVDx2 group. There was a significant decrease in VD of 116 patients with HE >3.4gm/m2 compared to 24 patients with HE ≤2.1 gm/m2 (HR=6; 95% CI 2.1-17.2; P=0.001). The present study demonstrated a bone density loss of ≥16% in 48% of patients and of ≥26% in 20% of patients. Osteopenia and osteoporosis developed in 7% and 4% of treated patients. Individuals receiving EBx6 had a less significant bone loss compared to those receiving EBx2 +ABVDx4. This could be explained in part by the older age (19% were above 46 years) of the latter patients. A similar VD loss of ≥16% was observed in patients treated with ABVDx6 and those receiving EB containing regimen (one week of steroids only) (56.6 and 59%, respectively). A multivariate analysis for prediction of VD loss showed that age ≥30 (HR=2.19; 95% CI 1.13-4.24; P=0.02) and chemotherapy protocols other than ABVDx2-4 (ABVDx6: HR=6.65; 95% CI 2.80-15.78; P=0.000; BEACOPPx2-6: HR=7.87; 95% CI 3.14-19.71; P=0.000) were significant risk factors (Fig. 1). Conclusions: The current study demonstrates that VD reduction is a significant problem in HL patients above the age of 30 years, treated with 6 cycles of chemotherapy containing HE >3.4gm/m2. As it may lead to osteopenia, a restrictive approach to the use of glucocorticoids should be applied in this patient population. Background: The coverage of 90 Fe tablets in Semarang in 2015 was 26.619 of 29.490 pregnant women (97%) but the incidence of anemia in pregnant women was still quite high. One important factor which influences the levels of hemoglobin is multi micronutrient.Objective: This study aims to analyze the changes in hemoglobin levels with multi micronutrient supplementation in pregnant women with anemia.Methods: A Quasy experimental study with randomized control group pre-post test design. The study samples were 40 pregnant women with gestational age of 20 ± 1 weeks divided into two groups, the intervention group (multi micronutrient supplementation) and the control group (Fe supplementation) for 6 weeks. The study was conducted from November 2016 to January 2017. The levels of hemoglobin were measured using cyanmethohemoglobin. Paired t test and independent t test were used for data analyses.Results: The mean increase in hemoglobin levels after multi micronutrient supplementation was 1.545 (1.292) g/dl, whereas the mean increase in hemoglobin levels after Fe supplementation was 0.757 (0.742) g/dl. The difference in hemoglobin levels before and after multi micronutrient supplementation was significantly different (p=0.049). Cohen’s d effect size test showed value of 0.748.Conclusion: Multi micronutrient supplementation could improve hemoglobin levels. Increase in the levels of hemoglobin with multi micronutrient supplementation was higher than Fe supplementation.

Background: Reduced vaginal mucus to dyspareunia (pain during intercourse) is often experienced by perimenopausal women. This decreases sexual drive and awakening in women who influence sexual quality with their partner. One of the body and mind intervention exercises in premenopausal women is loving yoga. This study aims to find out the effectiveness of loving yoga against the sexual function of perimenopausal women. Methods: This type of research is a quantitative experiment with a one-group pretest-posttest design approach. Research by giving loving yoga intervention for four weeks (with intensity twice a week). Loving yoga movement focuses on kegel gymnastics and pelvic muscle movements, such as plank movement, upward dog, happy baby, downward-facing dog, cat-cow, pigeon, cobbler, leg-up-the-wall, reclining-big-toe, and bridge. Researchers measured the sexual function in 30 women by purposive sampling with inclusion criteria were aged 40-50 and still sexually active before and after loving yoga using the Female Sexual Function Index (FSFI) questionnaire. Analyze data using paired sample T-test. Results: This study result is seen from the mean value of 14.33, which means the FSFI score ≤ 26.5. While after complementary therapy loving yoga, the mean value of respondents is 30.13 or an FSFI score ≥ of 26.5, which means no sexual dysfunction. Based on the calculated Paired T-Test variables, the sexual function is obtained at the value of –28,748 with a significance value of < 0,000 (p < 0,05). Conclusion: Loving yoga has an influence 28,748 times on changes in the sexual function in perimenopausal women.

Background: Academic stress is a major problem that often occurs in nursing students. Islamic spiritual mindfulness (ISM) is an adaptive coping strategy used to mitigate various negative psychological reactions to respond stressors experienced by the body to build self-awareness that any problems experienced today are the scenario of Allah.Purpose: This study aimed to determine the effects of Islamic spiritual mindfulness therapy to reduce stress.Methods: This study used a pre-post quasi-experimental design with a control group. Participants were bachelor students of nursing who are Muslim and experienced stress in working on the thesis (n=36) and were assigned to two groups: the experimental group receiving Islamic spiritual mindfulness and the control group. Data were collected using questionnaires of Depression Anxiety Stress Scale (DASS) and analyzed using paired t-test and unpaired t-test.  Results: The results showed that the mean stress level of students in the intervention group decreased from 20.6±2.97 to 11.4±5.81 after the intervention. Meanwhile, in the control group, the mean student stress level slightly decreased from 19.7±2.82 to 17.8±5.01. The t-test obtained a p-value of 0.001, indicating that there were significant differences in stress levels between the intervention group and the control group. The effect size obtained the value of 0.59, meaning the level of influence was in the medium category.Conclusion: This study concluded that Islamic spiritual mindfulness was effective in reducing stress among nursing students working on the thesis. It is recommended for the nursing profession, especially mental health nurses, to apply Islamic spiritual mindfulness therapy as one of the psychotherapy interventions.

Abstract Objectives Postmenopausal women are at greater risk of depression. Depression may negatively affect the quality of life of women. An emotional freedom technique (EFT) is an evidence-based therapy combining cognitive and exposure components with acupressure. This study aimed to evaluate the effect of EFT on depression in postmenopausal women. Methods This was a randomized controlled trial in which 88 women with mild to moderate depression recruited from a menopausal clinic in Ahvaz, Iran, and randomly assigned into two groups of EFT (n=44) and control for sham therapy (n=44). Women in the EFT group received two sessions of training and asked to continue EFT for 8 weeks, one time per day. The Beck Depression Inventory (BDI2) completed by women before and after the intervention. The control group received training on sham acupressure points similar to the intervention group. Data collected using a demographic and BDI2. Women requested to complete the BDI2 before and after the intervention. The independent t-test, chi-square, and ANCOVA were used to analyze data. Results The mean depression score in the intervention group reduced from 20.93 ± 4.6 to 10.96 ± 4.38 in comparison to the control group that reduced from 19.18 ± 2.79 to 17.01 ± 6.05 after intervention (p=0.001). After the 8 week intervention, the frequency of moderate depression decreased from 56.8 to 9.35% in the intervention and from 50 to 29.5% in the control group. In total, 63.4 and 34.15% in the intervention and control groups were free of depression respectively after the intervention (p<0.001). Conclusions The results of this study showed that using EFT for 8 weeks could significantly reduce the mean score of depression in postmenopausal women. Using this method in public health centers for postmenopausal women is recommended.

The present study aims to identify the effects of systematic walking on exercise energy expenditure (EEE) and blood profiles in middle-aged women. Fifty-two female nurse managers, aged 32 to 57 years (42.0 ± 6.2), were randomly assigned to an intervention group (IG) and a control group (CG) for a 12-week study of the walking program. EEE was measured using a microelectronic device. Blood profiles were assessed before and after the walking program. The mean EEE (kcal/kg/d) in the IG and CG was 4.73 ± 1.02 and 3.88 ± 0.81 ( P = 0.01), indicating an increase of 1.17 ± 0.98 and 0.46 ± 0.68 from baseline ( P = 0.01), respectively. The mean change in high-density lipoprotein cholesterol in the IG and CG was 1.8 ± 8.3 mg/dL and −2.9 ± 7.0 mg/dL ( P = 0.051); that in insulin was −4.5 ± 7.5 μU/dL and −0.6 ± 4.3 μU/dL ( P = 0.046), respectively. These results show that systematic walking increases EEE and improves blood profiles.

Hemodialysis is a routinely performed therapy on chronic kidney patients, leading to psychological problems among subjects who undergo hemodialysis, such as anxiety and depression. One of the efforts to overcome anxiety and depression is with the dhikr intervention. Dhikr presents hearts to remember and be obedient to Allah followed by the words and actions in various conditions.  Discover dhikr's influence on the level of anxiety and depression in patients on hemodialysis in  Purwokerto Islamic Hospital. It was a quantitative study using quasi experiment with a non-quivalent control group design. The number of samples was 12 subjects consisting of 6 subjects in the control group and six subjects in the intervention group with the purposive sampling technique. Instruments were used to measure anxiety and depression are the HADS ( Hospital Anxiety and Depression Scale ) before and after dhikr intervention as much as 12 times. Research is carried out in Hemodialysis Unit of  Purwokerto Islamic Hospital. Test statistics on research are used paired t-test and independent t-tests. This study showed a decrease in the mean level of anxiety from 5, 83 to 1.67 in the experimental group ( p = 0.003). The mean level of depression experienced a decline of 9, 67 becomes 4.67 on a group experiment ( p = 0.003). Dhikr reduces the level of anxiety and depression in hemodialysis patients at the Purwokerto Islamic Hospital.

Background. During hospitalization large number of invasive procedure recived by patient and preceived as threatening and anxiety experience. One of the invasive procedures that commonly done is the venous blood extraction. The children in preschool age preceived venous blood extraction as something that endanger the integrity of the body and lead to anxiety experience. To reduce the anxiety during the venous blood extraction, a nurse could perform a clay theraphy. The aim of this research is to ascertain the effect of clay therapy toward scoreof anxiety in preschool age children that undergoing venous blood extraction in RSUD Al-Ihsan.Methode. The study was quasi-experiment with nonequivalent control group posttest only. A total of 34 children who recieve venous blood extraction was assigned into 2 group, 17 children in control and 17 children in intevention. The children anxiety level measured using anxiety observation sheet before the procedure complete. Data were analyzed used independent t test for bivariate and logistik regresion for multivariate. Result Findings. The results  show a significat difference in anxiety score between control group and intervention group with p-value 0,001 < α 0.05 which means there is impact of clay therapy to level anxiety in preschool age children undergoing invasive procedure in RSUD Al-Ihsan. Conclusion. This research indicate that clay therapy may be used to reduce anxiety in children that undergoing venous blood extraction.

Objectives: Low bone mass is a common disorder in elderly population which predisposes to fracture with minimal trauma. This study was performed to find out the association between the Body Mass Index (BMI) and Bone Mineral Density (BMD) in postmenopausal women.Materials and Methods: This cross sectional study was carried out at Institute of Nuclear Medicine and Allied Sciences Comilla and Mitford, Dhaka over a period of 12 months from January 2013 to December 2013. A total 93 postmenopausal women were enrolled for this study. All postmenopausal women underwent a BMD scan of femoral neck and lumbar vertebrae using a Dual Energy X-ray Absorptiometry (DEXA). Participants were categorized into three groups according to their age and BMI. BMD were expressed base on T-score according to WHO criteria. The relation among BMI, age and BMD were assessed.Results: The results of this study showed that the mean age of the study group was 57.13±7.49 years with range of 46 to 75 years. The most postmenopausal women were in age group 55-65years. The mean BMI of the study subjects were 24.18±5.08 kg/m2 with a range of 15.62 to 36.20 kg/m2. Among 93 subjects osteopenia was greater at lumbar spine (45.2%) with T-score mean±SD-1.83±0.33 and osteoporosis at femoral neck (51.6%) with T-score mean ±SD-3.36±-0.67. Pearson’s correlation coefficient test showed inverse relationship between age and BMD both lumbar spine (r = -0.301, p = 0.003) and femoral neck (r = -0.303, p=0.003) whereas the positive relation between BMI and BMD both at lumbar spine (r=0.338, p=0.001) and femoral neck (r =0.343, p=0.001). These showed that with advancing age, BMD decreases and the risk of osteoporosis increases and with increasing BMI, BMD increases and risk of osteoporosis decreases.Conclusion: The findings of this study portrait that aging and low BMI are risk factors associated with bone loss. So preventive measure should be taken for high risk post menopausal women.Bangladesh J. Nuclear Med. 17(2): 138-141, July 2014

Background : Hypertension is a condition of high blood pressure in the arteries that lasted continuously for the long term. Hypertension can be overcome in two ways: pharmacology and non-pharmacology. Pharmacology treatment usually use medicine while one of the ways for non-pharmacology treatment is to consumes apple juice.Research Objective : The aim of this research is to know the effect of apple juice on blood pressure decrease in elderly people with hypertension at Muara Kaman Community Health Center.Methods : The design of this research used quasi experimental design (quasi experiment) with pretest posttest control group design. The sample of the research was hypertensive respondents at Muara Kaman Community Health Center as many as 30 respondents were divided into 2 groups, 15 intervention groups and 15 control groups. The sampling technique used purposive sampling. The research instruments were stethoscope and sphygmomanometer, data analysis using t test dependent and t test independent.Results : There were a significant influence between pretest and posttest of apple juice in the intervention group (P value = 0,000; P <0.005). The mean difference in systolic blood pressure in the intervention group and the control group also had significant differences (P value = 0.002; P <0.05), where as the mean difference in diastolic blood pressure in the intervention group and the control group also had significant differences (P value = 0,039; P <0,05).The Conclusions : The results of this research showed that consumes apple juice gives an effect on the blood pressure of hypertensive patients by consumes regularly.The Suggestions : Nurses and the public can use apple juice as one of the ways to handling to hypertension sufferers besides antihypertensive drugs.
