AGE FEATURES OF BIOREGULATORY THERAPY OF DENTAL DISEASES

В статье представлены данные по применению пептидных биорегуляторов клеточного гомеостаза и защитных функций организма при различных стоматологических заболеваниях и патологии органов и тканей челюстно-лицевой области. Рассмотрены вопросы, связанные с иммунофармакологическим действием и эффективностью клинического применения природных и синтетических пептидных тимомиметиков (Тималина, Тимогена, Вилона, Эпиталамина, Кортексина) и пептидных биорегуляторов из хрящевой (Сигумир, Хондролюкс) и других тканей при травме, а также воспалительных, дистрофических, онкологических и других патологических процессах тканей полости рта и челюстнолицевой области. Особое внимание уделено их использованию в комплексном лечении стоматологических заболеваний у лиц пожилого и старческого возраста. The article presents data on the use of peptide bioregulators of cell homeostasis and the protective functions of the body in various dental diseases and pathology of organs and tissues of the maxillofacial region. Issues related to the immunopharmacological effect and clinical use of natural and synthetic peptide thymomimetics (thymalin, thymogen, vilon, epithalamin, cortexin) and peptide bioregulators from cartilaginous (sigumir, chondrolux) and other tissues in case of trauma, as well as infl ammatory, inflammatory and other pathological processes of tissues of the oral cavity and maxillofacial region. Particular attention is paid to their use in the complex treatment of dental diseases in the elderly.

The review summarizes studies that used the timalin immunomodulator in many dental diseases. Issues related to its immunopharmacological action and effective clinical use in inflammatory, traumatic, and other pathological processes of the maxillofacial region were considered. Special attention was paid to their use in medical complexes for diseases of tissues and organs of the oral cavity in various age groups.

Teeth and mouth are parts of the body that cannot be separated, where the teeth are in the oral cavity so that if there is interference with the teeth it will affect the mouth. Knowledge to recognize dental and oral diseases from an early age is very much needed to be able to maintain healthy teeth and mouth. Lack of knowledge about these oral and dental diseases will result in severe damage to teeth and mouth. This study aims to measure the severity of oral disease and provide a diagnosis of oral and dental disease so that later it can be used as a reference for consultation with a doctor. The method used in this research is Forward Chaining to represent the rules of 27 symptom facts and 8 diseases described by experts. The results of testing for this method are as many as 10 patient data were diagnosed to get the same result as the doctor's analysis so that the accuracy rate is 80%. The expert system designed with the Codeigniter Framework can provide insight by being able to identify the disease suffered by patients and measure the severity of the oral and dental disease suffered.

Treatment of odontogenic phlegmon of the maxillofacial region remains an urgent problem of surgical dentistry due to the steady increase in their frequency, tendency to generalize the process and adverse effects. In this regard, there is a constant search for ways to improve the effectiveness of treatment of patients with odontogenic phlegmons of maxillofacial region. New technologies of drug treatment of this pathology are developed and put into practice. In recent years the attention of researchers in various conditions accompanied by the development of oxidative stress began to attract drugs that have antioxidant action, as it became known that the excess of reactive oxygen species in the regenerating wounds in the hearth of purulent inflammation affects the course of the course, wound healing. Interest in the treatment of various pathological conditions of the body is caused by the domestic antioxidant multifunctional enzyme "Bioceruline" which is composed of copper. In this regard, the purpose of our study was to evaluate the effectiveness of the use of "Biocerulin" in the complex treatment of patients with odontogenic phlegmon of maxillofacial region. The object of the clinical study was 45 patients with odontogenic phlegmons of the maxillofacial region from 18 to 55 years old, who were treated in the maxillofacial department of PU "Poltava M.V. Sklifosovskyi regional clinical hospital PRC». All patients were without somatic pathology in compensated clinical condition. Immediately after hospitalization, patients underwent an emergency opening of purulent lesion, revision and adequate drainage of the affected cellular spaces, removal of the "causal" tooth. The extent of surgery and the choice of optimal access depended on the location and prevalence of phlegmon. Depending on the treatment, patients were divided into comparison groups (22 patients) and the main group (23 patients). The traditional therapy was used in the comparison group. "Biocerulin"was prescribed for the patients of the main group in addition to the traditional therapy. Analysis of obtained data in the treatment of patients with phlegmon revealed that both groups of patients had clear differences in the clinical course of purulent-inflammatory process. On the 2nd day after the opening of the phlegmon theweakness, headache, and sleep disorders were noted in the most of the patients of the comparison group who received traditional therapy. Pain with varying degrees of intensity was present in all patients. Improvement of the general condition of patients of this group was noted not earlier than 3-5 days after surgery. The improvement of the general condition and reduction of pain, normalization or decrease in body temperature, the beginning of restoration of function of the jaw apparatuswere notedin patients of the main group on the background of the introduction of "Bioceruline" already on the 2-3rd day after surgery. Thus, on the basis of the conducted researches it is possible to conclude that the basic (traditional) conservative therapeutic measures used for patients of the comparison group are not effective enough for the relief of the local and general inflammatory process, which caused the prolonged nature of organ-specific and functional rehabilitation of patients as a whole. The use of "Bioceruline" in complex therapy of patients with odontogenicphlegmons of the maxillofacial regionprovides rapid and intensive recovery of the general condition of patients and accelerates reparative processes in the wound and shortens the treatment time via 1.4 bed-days.

Заболевания полости рта являются одними из наиболее распространенных заболеваний, с которыми сталкивается здравоохранение во всем мире. Эта проблема затрагивает людей всех возрастных групп. Анализ литературных данных позволяет выделить множество факторов риска развития кариеса и его осложнений, которые могут касаться военнослужащих помимо несбалансированного питания, курения, неудовлетворительной гигиены полости рта и нерегулярного посещения стоматолога. Множество международных исследований демонстрируют актуальность проблемы стоматологических заболеваний среди военнослужащих, указывая на отсутствие своевременной диспансеризации, низкую мотивацию к регулярному посещению стоматолога и тщательному проведению самостоятельной гигиены полости рта. Кроме этого, стоит учитывать специфику выполняемых военными задач за весь период службы, во время которых они оказываются под воздействием множества неблагоприятных факторов, которые в свою очередь, влияют на микроэлементный состав организма, что непосредственно влияет на общее состояние здоровья. Необходимость применения в армии РФ более актуальных по своему составу витаминно-минеральных комплексов (ВМК) уже неоднократно описана в современных исследованиях. Но до настоящего времени не проводилось работ по изучению взаимосвязи между микроэлементным статусом организма и состоянием твердых тканей зубов и пародонта Diseases of the oral cavity are one of the most common diseases that public health has worldwide. In addition, it is worth considering the specifics of the military tasks performed for the entire period of service, during which they are influenced by many unfavorable factors, which in turn affect the microelement composition of the body, which directly affects the overall health. The need to use in the army of the Russian Federation more relevant in its composition vitamin-mineral complexes (VMC) has been repeatedly described in modern studies. But so far no work has been done to study the relationship between the microelement status of the body and the state of hard tissues of teeth and periodontium

Oral health among the elderly population is an integral part of their overall health. Good oral health facilitates effective chewing, contributes positively to speech, social confidence and is associated with better cognitive and functional capacity in the elderly. According to the World Health Organization (WHO) it is generally acceptable if older patients have about 20 functional teeth in the oral cavity. Toothlessness is today seen as a physical disability, as the imperative part of the body is lost and this in turn affects patients' personal satisfaction.The main goal of this research was to determinate the prosthetic status and the need for prosthetic rehabilitation at the elderly in the city of Strumica, Macedonia, and as specific goals were set as follows: to assess the overall oral health of patients, including the frequency of visiting a dentist, to determine oral hygiene habits; to determine the number of lost teeth associated with the patients’ age; to determine the type of prosthetic appliance and their current state in the patients' mouth; to indicate negative and positive criticism and the level of satisfaction of patients from prosthetic devices (in terms of aesthetics); to determine the type of diet and general health status of patients; to determine the status of total prostheses in terms of retention, stabilization, extension and occlusion.The study included 40 patients from Strumica aged 50 to 75 years, 18 women and 22 men. The presented data were obtained from the research conducted in the period from 20.06.2018 to 30.08.2018 in which the following procedures were performed: anamnestic data, clinical examination and completing a questionnaire by patients.Based on the data obtained, their analysis, and the results of our research, we can make the following conclusions: Hygiene habits and attitudes may be related to gender distribution, level of education, and they do not always produce positive results; Education is one of the most important factors contributing to patients' awareness of their health and the need for regular check-ups in dental practice and daily oral hygiene; General illnesses often play a major role in the oral cavity, and poor habits such as smoking, alcohol consumption and other harmful products, including diet, are major contributing factors to caries and periodontal disease leading to tooth loss; With increasing age, patients have a growing number of lost teeth and their awareness of replacing them is linked to both aesthetic and functional aspects; The dentist should always take care to preserve the natural teeth in the patient's mouth, to get the most out of the situation and to seek the most appropriate prosthetic rehabilitation and prosthetic compensation.

In the prevention oforal diseases, dental hygienic status, proper oral hygiene, the proper choice ofmeans and methodsfor performing hygienic dental activities are essential. However, even among future doctors, the hygienic state of the oral cavity leaves much to be desired. Basic knowledge of oral hygiene and the principles ofprevention ofdental diseases are necessary for physicians of various specialties, since there is often a relationship between dental diseases and diseases of other organs and systems of the body. The article gives an assessment of the hygienic status of students of different courses and identifies the sources that they use to gain knowledge on oral care.

Despite the rapid development of dentistry and advances in the prevention of diseases of the maxillofacial region, implantology, and the treatment of dental diseases, the number of patients who need prosthetics with removable orthopedic structures of dentures remains high, and only increases with age, due to the increasing rate of aging. society of the planet (M.Yu. Ogorodnikov, 2007; V.I.Shemonaev, 2012; E.S. Kalivradzhiyan, 2013; E.A.Zholudev, 2014; CE. Bragin, 2014; L.D. Veisheim, 2014). Among the reasons for the high demand for orthopedic treatment with removable dentures, the leading position is still held by insufficient sanitation of the oral cavity, untimely access of patients to the dentist. In addition, an important role is played by the problem of imperfection of materials and technologies used for the manufacture of dentures, leading to a deterioration in the condition of the dentition.

Особое место в структуре стоматологической заболеваемости занимает патология пародонтального комплекса, в частности - хронический генерализованный пародонтит, который по данным ВОЗ находится на втором месте по частоте распространения среди всех стоматологических заболеваний, уступая лишь кариесу. Первые признаки воспаления пародонта начинают проявляться уже в молодые годы, а к пожилому периоду - распространенность заболеваний пародонта достигает значений, стремящихся к 100%. Решение проблемы своевременной диагностики, профилактики и лечения патологии пародонта по-прежнему является актуальнейшей задачей современной стоматологии. Генерализованный пародонтит - это не только очаг хронической инфекции и источник сенсибилизации организма, но также ведущая причина потери зубов (особенно в старших возрастных группах), приводящая к серьезным деструктивным последствиям, дезорганизующим зубочелюстной аппарат. Данный факт способен серьезным образом повлиять на качество жизни человека, делая пародонтологическую проблему не только медицинской, но и социальной. Несмотря на очевидную актуальность данного вопроса, а также большой интерес к научным исследованиям в области пародонтологии, стоит признать, что уровень стоматологического здоровья, а также оказания пародонтологической помощи, в том числе пожилому населению - далеки от идеала. In the treatment of 54 patients with severe inflammatory processes of the maxillofacial region, the method of long-term catheterization of the external carotid artery through one of its branches was used to administer antibiotics and anticoagulants. The results obtained make it possible to recommend this technique for wide clinical use in the complex of intensive therapy for purulent processes of the face.
