Особое место в структуре стоматологической заболеваемости занимает патология пародонтального комплекса, в частности - хронический генерализованный пародонтит, который по данным ВОЗ находится на втором месте по частоте распространения среди всех стоматологических заболеваний, уступая лишь кариесу. Первые признаки воспаления пародонта начинают проявляться уже в молодые годы, а к пожилому периоду - распространенность заболеваний пародонта достигает значений, стремящихся к 100%. Решение проблемы своевременной диагностики, профилактики и лечения патологии пародонта по-прежнему является актуальнейшей задачей современной стоматологии. Генерализованный пародонтит - это не только очаг хронической инфекции и источник сенсибилизации организма, но также ведущая причина потери зубов (особенно в старших возрастных группах), приводящая к серьезным деструктивным последствиям, дезорганизующим зубочелюстной аппарат. Данный факт способен серьезным образом повлиять на качество жизни человека, делая пародонтологическую проблему не только медицинской, но и социальной. Несмотря на очевидную актуальность данного вопроса, а также большой интерес к научным исследованиям в области пародонтологии, стоит признать, что уровень стоматологического здоровья, а также оказания пародонтологической помощи, в том числе пожилому населению - далеки от идеала. Во многом это объясняется сложностью этиологии и патогенеза заболеваний пародонта, а также, нередко, отсутствием комплексного понимания данной патологии при ее терапии A special place in the structure of dental morbidity is occupied by the pathology of the periodontal complex, in particular, chronic generalized periodontitis, which, according to the WHO, is in second place in terms of prevalence among all dental diseases, second only to caries. The first signs of periodontal inflammation begin to appear already in the young years, and by the old age the prevalence of periodontal diseases reaches values tending to 100%. Solving the problem of timely diagnosis, prevention and treatment of periodontal pathology is still the most urgent task of modern dentistry. Generalized periodontitis is not only a source of chronic infection and a source of sensitization of the body, but also a leading cause of tooth loss (especially in older age groups), leading to serious destructive consequences that disorganize the dental apparatus. This fact can seriously affect the quality of life, making periodontal problems not only medical, but also social. Despite the obvious relevance of this issue, as well as the great interest in scientific research in the field of periodontology, it is worth recognizing that the level of dental health, as well as the provision of periodontal care, including the elderly population, is far from ideal. This is largely due to the complexity of the etiology and pathogenesis of periodontal diseases, as well as, often, the lack of a comprehensive understanding of this pathology during its therapy

Subject. In 2017, the “Happy Smile” project was developed at the dental faculty of the Ural State Medical University and its active implementation began, during which teachers and students conduct preventive measures in educational institutions, shopping centers, hospitals of the city. The goal is to increase the literacy of the population in relation to the prevention of oral diseases, as well as the quality of life of the population by reducing the level of dental morbidity, the formation of professional and super-professional competencies of a modern graduate dentist. Methodology. According to WHO, the prevalence of dental diseases does not tend to decrease. An urgent task today is the development of programs and projects aimed at the health of the population. Results. In 2017, the staff of the Department of Therapeutic Dentistry and Propaedeutics of Dental Diseases of the Ural State Medical University took the initiative to create a Happy Smile preventive project at the faculty. The main objectives of the project were: the creation of a comprehensive preventive program covering all age groups of the population; carrying out activities aimed at updating the preservation and maintenance of dental health; improving the quality of medical (dental) education by involving students in medical volunteering; expansion of the territory of events to nearby cities of the Sverdlovsk region. Conclusions. Participation in the project forms significant professional and supra-professional competencies of students, which helps to improve the quality of training of future dentists. The involvement of students in the implementation of the project will increase their relevance in the labor market. Implementation of the project is a health-saving technology that reduces dental incidence in Yekaterinburg and the Sverdlovsk Region.

Увеличение доли пожилых и старых людей в структуре населения является особенностью настоящего времени, которая охватывает практически весь мир. Постарение населения изменило область задач врачей разных специальностей, в том числе стоматологов, делая повышение качества оказания стоматологической помощи и жизни людей старших возрастных групп наиболее приоритетными. Важной в поддержании здоровья людей старших возрастных групп является биорегуляция, которая представляет сложный комплекс межклеточных взаимодействий, направленный на поддержание гомеостаза и отвечающий за течение таких важных физиологических процессов, как дифференцировка и деление клеток, регенерация тканей, старение организма, обмен и воспроизведение генетической информации и многое другое. Биорегулирующая терапия направлена на ликвидацию нарушений гомеостаза для нормализации функций поврежденных клеток, тканей и органов. Цель работы - исследовать основные направления использования, достижения и перспективы биорегулирующей терапии в области геронтостоматологии. Представлены основные результаты применения биорегулирующей терапии в геронтостоматологии при лечении заболеваний пародонта, слизистой оболочки полости рта, височно-нижнечелюстного сустава, одонтогенных гнойно-воспалительных заболеваний и очагов стоматогенной хронической инфекции, а также перспективы их дальнейшего применения. The increase in the proportion of elderly and old people in the structure of the population is a feature of the present time, which covers almost the entire world. The aging of the population has changed the scope of tasks of doctors of various specialties, including dentists, making improving the quality of dental care and the life of older people a top priority. Bioregulation is important in maintaining the health of people of older age groups, which is a multiple complex of intercellular interactions aimed at maintaining homeostasis and responsible for the course of such important physiological processes as cell differentiation and division, tissue regeneration, aging of the body, exchange and reproduction of genetic information, and much other. Bioregulatory therapy is aimed at eliminating homeostasis disorders in order to normalize the functions of damaged cells, tissues and organs. The purpose of this work is to investigate the main directions of use, achievements and prospects of bioregulatory therapy in the field of gerontostomatology. The main results of the use of bioregulatory therapy in gerontostomatology in the treatment of periodontal diseases, oral mucosa, temporomandibular joint, odontogenic purulent-inflammatory diseases and foci of stomatogenic chronic infection, as well as the prospects for their further use are presented.

As a result of the continuing changes in the condition of tissues and organs of the dental cavity and the body as a whole, there is a rise in the intensity of tooth decay and periodontal diseases with the onset of pregnancy. The aim of our research was to look into the prevalence and severity of pathology in the hard tissues of teeth and periodontal tissues during pregnancy, as well as to figure out how to better organize therapeutic and prophylactic steps for pregnant women. A comprehensive epidemiological study of 170 pregnant women aged 18 to 40 living in Tbilisi was carried out according to the WHO method. The findings show a high prevalence of dental cavity (from 61 percent to 98.8%) and a high severity of dental caries among pregnant women (from 2.55 1.03 to 14.31 5.44). The intensity of caries indicators increased as the gestational stage lengthened, but the greatest increase was observed in the second trimester of pregnancy. The results indicate a substantial prevalence of periodontal pathology (according to the PMA index) among pregnant women, with the level of pathology increasing with their age and gestational age (from 41.7 percent to 78.8 percent). By the end of the second trimester of pregnancy, the most noticeable improvements in periodontal tissues had been observed. Oral hygiene played a big role in determining the presence and incidence of periodontal disease. Low dental health indicators and worsening dental status during pregnancy point to the need for a more comprehensive diagnosis of all risk factors for major dental diseases, as well as the implementation of all stages of therapeutic and prophylactic interventions during pregnancy, based on the findings of a complete dental examination.

High health indicators, including dental health, are a condition of professional activity of cosmonauts. The assessment of indicators of dental status in members of the squad of astronauts on the basis of the survey on the Map-assessment of dental status of the who in the age groups under 35 years 35 - 44 years old over 45 years old. The values of the intensity of caries and periodontal disease, the quality of light-curing seals and endodontic treatment, the presence of orthopedic structures, including implants are presented. The lower intensity of caries, as well as the prevalence and intensity of periodontal diseases among the members of the cosmonaut team in comparison with the Russian indicators; insufficient quality of previous dental treatment regarding fillings and root canal obturation; low need for treatment of caries and dental prosthetics in connection with timely and quality treatment and replacement of removed teeth in specialized clinics of Moscow after enrollment in the cosmonaut corps, as well as in connection with the real dispensary dental care of cosmonauts.

Since 2009, the National Health Insurance in Korea (NHI) has been implementing a series of policies to expand the scope of dental benefits. This study reviewed the changes in co-payments and dental use patterns before (2008 to 2012) and after (2013 to 2017) the NHI’s dental health insurance reform. The study used Korea Health Panel data of 7681 households (16,493 household members) from a 10-year period (2008–2017). Dental expenditures and equivalent income using square root of household size were analyzed. Dental services were categorized into 13 types and a concentration index and 95% confidence interval using the delta method was calculated to identify income-related inequalities by a dental service. Dental expenditures and the number of dental services used increased significantly, while the proportion of out-of-pocket spending by the elderly decreased. The expenditure ratio for implant services to total dental expenditures increased substantially in all age groups, but the ratio of expenditures for dentures and fixed bridges decreased relatively. The concentration index of implant services was basically in favor of the rich, but there was no longer a significant bias favoring the better-off after the reforms. The dental health insurance reform in Korea appears to contribute not only to lowering the ratio of out-of-pocket to total dental expenses per episode in the elderly but also to improving the inequality of dental expenses.

AbstractIn recent years have seen increasing percentage of the elderly in the overall population. This has driven the attention to the lifestyle factors that influence the health and quality of life of this social group, including their nutrition and physical activity. Universities of the Third Age (U3A) are a valuable platform for the dissemination and broadening of the knowledge related to these topics. The nutritional habits of 61 U3A students in Poznań were evaluated on the basis of a modified KomPAN questionnaire. Their nutritional status was determined using the body mass index (BMI) and waist to hip ratio (WHR) indices. Nearly half of the respondents were overweight and 16% had first degree obesity. The WHR index in women was on average 0.8, while in men it was 1.01. Only 13% of the students declared regular eating, with 60% consuming 4-5 meals a day. Women were found to eat snacks between meals more often than men. It was also found that the majority of the elderly do not add salt to ready meals or sweeten beverages with sugars. Taking into account the observed nutritional problems and the occurrence of improper eating habits of the elderly, it is recommended to continue the education on the prevention of common diet-related diseased.

The purpose of the article is to conduct a study of the main problems of delivery of orthopedic care in dentistry. It is known that dental health determines the overall health of the body. The comfort of life of the population depends on their condition, since damaged teeth negatively affect the state of the digestive system and respiratory organs. As for the aesthetics of the appearance, here teeth have a special role, since they are able to provide both proper speech and the necessary level of human sociability. Thus, improving the quality of delivery of medical care is an important task for the industry today. The problem of the availability of orthopedic dentists exists only in those areas of the region where there is no orthopedic care encounters at all, or orthopedic care encounters are carried out by part-time doctors. Accordingly, it can be said that municipal dental clinics are generally provided with the necessary personnel. In this regard, one can talk about the need to improve the quality of treatment of dental diseases in polyclinics, primarily in terms of orthopedic care for the population. However, today there are frequent cases of return visits for orthopedic care due to its poor-quality during initial treatment. And the doctor’s task during second denture treatment is to avoid mistakes made earlier and to provide competent and highquality orthopedic services. The author concludes that the results of a study of the work of orthopedic units of the region showed a steady growth of most indicators year by year, but a number of economic problems were found during the analysis of the profitability reserves of orthopedic dental care. So, it is necessary to create a unified system for calculating the financial plan for the correct assessment of the status of orthopedic dental care for the population, to analyze the quality indicators for subsidized denture treatment, to introduce the concept of “prosthesis working lifespan”, which will establish the reasons and justify the terms of the second denture treatment.

BACKGROUND: Preservation and strengthening of the health of civil aviation flight personnel is an extremely urgent problem and is associated with ensuring flight safety. AIM: This study aimed to determine the structure and analyze the organs and tissues of the masticatory apparatus among civil aviation flight personnel. MATERIALS AND METHODS: To determine the incidence and intensity of main dental diseases and the morbidity structure of organs and tissues of the masticatory apparatus in civil aviation pilots, an in-depth examination was carried out in 120 men aged 2545 years (study group 1) working on various modern passenger and transport aircrafts. The duration of flight operations ranged from 2 to 17 years, with a total flight duration of 140 to 5580 h. The comparison group consisted of 146 men (aged 2545 years) of non-flying specialties (study group 2). The generally accepted methods for assessing the prevalence and intensity of the course of major dental diseases were used. RESULTS: The incidence and intensity of dental disorders, periodontal disease, temporomandibular joint disorders, and parafunctions of the masticatory muscles were defined. The incidence and intensity of major dental diseases among civil aviation flight personnel were clearly directly proportional to their flight time (r=0.547) and duration of work in civil aviation (r=0.482) but to a lesser extent to their age (r=0.416). CONCLUSIONS: Non-carious lesions of the teeth, inflammatory and dystrophic diseases of the periodontal and oral mucosa, painful dysfunction of the temporomandibular joint, and bruxism are more common among civil aviation flight personnel than among non-flying personnel, which is due to the unfavorable effect of air flight factors on organs and tissues of the mastication apparatus and the body of the pilots as a whole.

The widespread expression saying that "loneliness is the No. 1 enemy of old age" has, in the 21st century, becomes obvious and banal now that suicide rates among the elderly in many countries and in Israel, the case study for this paper, is higher than among other age groups in those countries.Loneliness itself, its ramifications, consequences, and implications on the lives of the elderly have made it a dominant factor in terms of its potential damage.Old age and loneliness have been the subjects of many different definitions, studies and theories, but despite the quantity and quality of these attempts worldwide, and particularly in Israeli research, there is a conspicuous lack of any comprehensive plan that would address the implications and destructive effects of the loneliness experienced by the elderly. Many limited-scale programs exist on a local level, but they fail to address the clear need to eradicate menacing loneliness.The authors of this essay plan to outline basic principles for designing an operational concept for a national plan to combat the consequences and effects of depression among the elderly, using Israel as an example.This paper will begin with a review of the definitions and literature on the phenomenon of depression, and a review of the literature dealing with the unhealthy connection between old age and depression. We will then review various programs worldwide that deal with the phenomenon of loneliness and old age; continue to present data on the effects of loneliness on elderly Israelis; present various Israeli programs and projects that attempt to deal with this phenomenon; and conclude by detailing the proposed principles for a comprehensive operational approach that maps out this painful phenomenon in Israel and proposes a plan of action on how to best deal with it.

Aim. Evaluation of dental diseases prevalence in professional athletes engaged in power sports and of the influence of biologically neutral products based on propolis on the level of their dental health. Methods. The examination of organs and tissues of the oral cavity of professional athletes engaged for a long time in power sports was carried out. With the use of the developed specialized questionnaire dental status of the athletes was determined. Based on the analysis of the results of clinical and laboratory studies anti-inflammatory and anti-bacterial effectiveness of products based on propolis were evaluated. Results. The results of the survey showed that not all athletes are informed about the possibility of aggravation of pathological processes in soft and hard tissues of oral cavity during active training, mainly, in power sports. Lack of awareness of surveyed athletes about the need for timely individual oral hygiene procedures and regular dentist visits was revealed, especially during intense training before and after competitions. On evaluation of dental status of professional combat athletes high prevalence and intensity of inflammatory periodontal disease and caries were found. High anti-inflammatory efficiency of the product «Pomegranate Balm» for correction of dental status of professional athletes was proven. Conclusion. High incidence of inflammatory periodontal diseases of mild to moderate severity was revealed in athletes during prolonged and intense physical training; efficacy of biologically neutral products based on propolis was established.
