FAMILY DIABETES AND MAIN NONCOMMUNICABLE DISEASES: THE DESCRIBED AND POTENTIAL EFFECT OF METFORMIN (remembering Vladimir Dilman)

Работа посвящена двум медико-биологическим проблемам - семейному диабету (имея в виду наличие случаев сахарного диабета 2-го типа в семье, в том числе в разных поколениях), а также особенностям его связей с основными неинфекционными заболеваниями человека и приурочена к юбилею известнейшего отечественного специалиста (теоретика и врача) в области геронтологии и эндокринологии профессора В. М. Дильмана. Широко признанные труды Владимира Михайловича, основанные на оригинальных идеях и породившие важные практические следствия (включая применение антидиабетических бигуанидов в не использовавшихся до него областях, необходимость устранения метаболической иммунодепрессии, учета изменений с возрастом на уровне гипоталамического порога в различных гомеостатических системах и целого ряда других предложений), как представляется, в течение долгого времени будут питать интеллектуальным материалом и стимулировать к дальнейшему поиску его последователей и специалистов, которым еще предстоит окунуться в область, очерченную В. М. Дильманом и интересовавшую его в течение многих лет. The work discusses the two biomedical problems: family diabetes (bearing in mind the presence of cases of type 2 diabetes mellitus in the family, including its different generations) and the features of relationship of family diabetes with major non-communicable human diseases (NCDs). The paper is timed to the anniversary of the famous - in our country and abroad - expert in the field of gerontology and endocrinology, Professor V. M. Dilman. The widely recognized works of V. M. Dilman, based on original ideas and giving rise to important practical consequences (including the use of antidiabetic biguanides in areas not studied before him, the need to eliminate metabolic immunodepression, to take into account the changes with age at the level of the hypothalamic threshold in various homeostatic systems and a whole number of other essential proposals), which for a long time, as it seems, will stimulate the further scientific search of his followers and specialists, who have yet to get acquainted with the area that attracted Prof. Dilman and interested him for many years.

Background: Type 2 diabetes mellitus is a metabolic disease that characterized by high blood sugar levels. This condition is often not noticed immediately and usually patient starting to realize it when complications have been occurred. A long-term complication of type 2 DM that occurred in men is erectile dysfunction (ED). ED is a condition when a person is unable to achieve or maintain an erection for sexual intercourse. One factor that influence the occurrence of ED and its severity in type 2 DM patients is the duration of the disease. Objective: The aim of this study was to determine the relationship of type 2 DM duration and the occurrence of ED. Methods: This study is an observational analytic cross-sectional study conducted at the Puskesmas (Public Health Center) Denpasar Barat I. The research data was obtained through medical record data and fill the International Index of Erectile Function (IIEF-5) questionnaire on 36 type 2 DM patients aged around 40-60 years. The statistical analysis used was Fisher's exact test. Results: The results showed that of the 36 samples, 19 (52.8%) samples had type 2 DM <24 months and 17 (47.2%) samples had type 2 DM >24 months. It was found that 5 (13.9%) samples did not experience ED while the rest experienced ED with different severity. There was a significant relationship between the type 2 DM duration and the occurrence of erectile dysfunction at Puskesmas Denpasar Barat I (p = 0.022). Conclusion: Study has found that type 2 DM patients with the longer duration (>24 months)  have a higher occurrence of ED and tended to be more severe compared to those with shorter duration (<24 months). Further studies should be performed with higher number of patients and more controlled risk factor so it will be more accurate in determining the relationship between the duration of type 2 DM and ED.

Abstract Introduction: The high rate of diabetes mellitus index (DM), along with the increase in cardiovascular compromise that DM favors, and the scarcity of epidemiological data regarding the prevalence of peripheral arterial disease (PAD) in this population, make it important to study risk factors associated with the development of PAD in the population with type 2 diabetes mellitus (DM2). Objective: To estimate the prevalence of PAD together with the associated factors in a sample of patients with DM2, treated in the Family Health Strategies (FHS) program, in the municipality of Pindamonhangaba, SP. Methods: Quantitative research in a cross-sectional study of 38 individuals who were diagnosed with DM2, between 40 and 77 years old, selected by convenience sampling and treated in the family health program in two different districts of the municipality. The method consisted of the evaluation of personal and anthropometric data, anamnesis and physical examination including the ankle-brachial index (ABI). Results: PAD was present in 21.1% (95%CI: 16.9 to 25.8) of the investigated population. Risk factors observed were age range of 51 to 69 years (75%), overweight (50%), systemic arterial hypertension (SAH) (100%), smoking (62.5%) and physical inactivity (87.5%). Conclusion: The prevalence of PAD was more than a fifth of those diagnosed with DM2, and the most prevalent associated risk factors were SAH, physical inactivity, smoking and overweight with and without PAD.

AbstractDiabetes mellitus is a chronic disease that cannot be cured, but can be prevented by using the 5 pillars of diabetes mellitus management, one of which is controlling blood sugar levels. Patients with diabetes mellitus need support from the family because the family can influence a person's behavior and lifestyle so that it has an impact on the quality of life of patients with type 2 diabetes mellitus. To determine the relationship between family support and blood sugar levels in patients with type 2 diabetes mellitus through a literature review. The data collection technique uses a literature review method of 5 articles sourced from an online database with electronic searches on Google Scholar, Garba Garuda, and ProQuest published in 2017-2021. The critical analysis instrument used is using a strobe. The results of a literature review on the family support variable were 2 articles with supporting categories as many as 128 respondents, 2 articles with less categories as many as 63 respondents, and 1 article with good categories as many as 22 respondents. Variable blood sugar levels 2 articles in the controlled category as many as 98 respondents, 1 article in the poor category as many as 45 respondents, 1 article in the controlled category as many as 17 respondents, and 1 article in the diabetes category as many as 29 respondents. Untuk mengetahui hubungan dukungan keluarga dengan kadar gula darah pada pasien diabetes mellitus tipe 2 melalui literature review. Teknik pengumpulan data menggunakan metode literature review 5 artikel yang bersumber database online dengan penelusuran elektronik pada Google Scholar, Garba Garuda, dan ProQuest yang dipublish pada tahun 2017-2021. Instrumen telaah kritis yang digunakan yaitu menggunakan Strobe. Hasil literature review pada variable dukungan keluarga 2 artikel dengan kategori mendukung sebanyak 128 responden, 2 artikel dengan kategori kurang sebanyak 63 responden, dan 1 artikel dengan kategori baik sebanyak 22 responden. Variable kadar gula darah 2 artikel dengan kategori terkendali sebanyak 98 responden, 1 artikel dengan kategori buruk sebanyak 45 responden, 1 artikel dengan kategori terkontrol sebanyak 17 responden, dan 1 artikel dengan kategori diabetes sebanyak 29 responden. Terdapat hubungan antara dukungan keluarga dengan kadar gula darah pada pasien diabetes mellitus tipe 2.Kata kunci: Diabetes mellitus tipe 2;dukungan keluarga;kadar glukosa darah;kadar gula darah
