The Problem of Natural Focal Infectious Diseases in the Volgograd Region: A State-of-the-Art Review

Introduction: The importance of the problem of natural focal infections (NFIs) in the world is associated with their wide spread and a severe clinical course. The Volgograd Region, which is part of the Southern Federal District, is endemic for a whole number of NFIs. Regional epizootic and epidemic manifestations in foci of plague, tularemia, Q fever, ixodes tick-borne borreliosis (Lyme disease), hemorrhagic fever with renal syndrome (HFRS), Crimean – Congo hemorrhagic fever (CCHF), and West Nile fever (WNV) have been of varying degrees of activity over a long period of time. Objective: To study the features of epidemic and epizootic manifestations of natural focal diseases (plague, tularemia, Lyme disease, HFRS, WNV, CCHF, Q fever) in the territory of the Volgograd Region. Materials and methods: We analyzed the results of epizootologic and epidemiologic surveillance carried out in natural foci of contagious diseases in the Volgograd Region over the past decade, including data on the incidence of NFIs and the results of laboratory testing of zoological and entomological assays. We did an epidemiologic analysis of the ten-year rate, structure and changes in the incidence of natural focal diseases in the population of the Volgograd Region. Electronic databases were developed and analyzed in Microsoft Excel 2010 (Microsoft, USA). Results and discussion: We established that over the past decade, the number of endemic areas in the Volgograd Region has risen from 18 to 25 for СCHF and from 5 to 11 for Lyme disease. At present, all regional districts are endemic for tularemia, WNV, HFRS, and Q fever, 25 districts – for CCHF, and 11 – for Lyme disease. We observed a general promising tendency towards a decrease in incidence rates for all nosological forms. At the same time, the incidence of Q fever and Lyme disease remains low while that of HFRS and СCHF is somewhat higher. The West Nile fever, epidemic manifestations of which have been registered since 1999, is the main challenge in terms of NFIs in the Volgograd Region today.

West Nilefever (WNF) is the most important arbovirus infectionfor the Volgograd region. The observed decrease in the WNF incidence from 2001 to 2006, in 2008 and2009, the reducing the number ofsevere forms ofdiseases only give evidence ofa temporary regression of the epidemic process in the territory of the Volgograd region. In the territory of the Volgograd region there are prerequisites and harbingers of the complication of epidemiological situation on West Nile fever. The aim of the performed in the Volgograd region epidemiological surveillance for WNF is to provide the information necessary and sufficient for the analysis of the epidemiological situation, as well as to determine the direction of policy and activities in public health and in making management decisions.

Viral hemorrhagic fevers (VHFs) represent a group of similar clinical entities contagious constitutional diseases, caused by four different types of RNA viruses: Flaviviridae, Bunyaviridae, Arenaviridae i Filoviridae. These diseases proceed with high fever and damage of the circulatory system leading to homeostasis disorders, commonly accompanied by symptoms of hemorrhagic diathesis. VHFs are typically transmitted through infection vectors (mosquito) or through direct physical contact with infectious material. West Nile fever is the disease which is caused by West Nile virus from the Flaviviridae family. It begins flu-like symptoms, then it appears maculopapular rash and lymphadenopathy. At the most cases the symptoms retreat idiopathically. This disease can proceed as West Nile Neurological Disease in 1% of infected. The article presents three diseases, which can be present in tropical climate such as: Ebola hemorrhagic fever, dengue hemorrhagic fever, West Nile fever.

Relevance. Natural and climatic conditions, a variety of species composition of arthropods and vertebrates determine the possibility of circulation of arboviruses in the Volgograd region. The existence of natural foci of some arbovirus infections and the possibility of the formation of others suggests the need for annual monitoring of the causative agents of arbovirus diseases. Аim. Evaluation of the results of monitoring of arbovirus infections in the Volgograd region in 2019.Materials and methods: 806 blood serum samples from donors, 44 blood serum samples from febrile sick people, 300 blood serum samples from horses and 94 pools of blood-sucking mosquitoes were examined by immunofernal analysis. Result of the study of serum samples from donors in the Volgograd region, in 140 (17.4%) of 806 were found to have antibodies to the pathogen of West Nile fever (in 35 (4.3%) – IgM, in 105 (13.0%) – IgG), in 7 (2.2%) of 319 – to the Crimean hemorrhagic fever virus (in 4 (1.3%) – IgM, in 3 (0.9%) –IgG), and in 7 (2.9%) of 240 – IgG to the viruses of the California serogroup. Specific antibodies against viruses of Sindbis, Batai and Uukuniemi in the samples was not detected. The largest number of positive samples with the presence of IgG and IgM to the West Nile virus was found among residents of Volgograd (61 out of 240, 25.4%) and Volzhsky (25 out of 100, 25, 0%). Among 44 blood serums of febrile patients, 1 sample (2.3%) was found to contain an antigen of the Sindbis virus, and 2 samples (4.5%) – antigens California serogroup viruses. Specific immunoglobulins against West Nile virus were detected in 84 (28%) of 300 blood serums of farm animals (horses). In the study of 94 samples of field material (blood-sucking mosquitoes), West Nile virus antigen was detected in 14 (14.9%), Sindbis virus – in one sample (1.0%), Batai virus – in four samples (4.2%). Conclusions: the obtained results, along with the circulation of West Nile virus and Crimean hemorrhagic fever virus virus in the Volgograd region, indicate the presence of Sindbis, Batai and California serogroup viruses and necessitate further study of their role in the infectious pathology of the population.

The epidemic rise in the incidence of West Nile fever (WNF) in the season of 2018 was observed in the countries of the European Union (EU) and bordering states and exceeded the values of all previously recorded epidemic rises of 2010–2012. An increase in the incidence rate was registered in the USA and Canada, however, it did not exceed the indicators of epidemic rises of 2007–2012. In the territory of the Russian Federation, the WNF epidemiological process became more intense mainly in the territory of the Southern and North Caucasian Federal Districts. In general, in Russia, the incidence rates were 2 times lower than the average annual rates, but significantly exceeded those of 2017. The epidemic process had a number of peculiarities in the seasonality, the structure of morbidity and the clinical manifestation of WNF. Genotyping of the isolated WNV RNA fragments from clinical and biological material showed that I, II and IV West Nile virus genotypes were circulating in the European part of Russia. Forecast of epidemic situation development in 2019 reveals further increase in the incidence and does not exclude the possibility of a significant localincrease of WNF incidence in certain regions of Russia.

Aim. Analysis of epidemic manifestations of natural-foci infections (NFI), clarification of spectrum of their causative agents, determination of epizootic activity of natural foci in the Crimea Federal District (KFD). Materials and methods. Epizootologic examination of 10 administrative districts of KDF was carried out. 291 pools (2705 specimens) of ixodes ticks and 283 samples of organs of small mammals were studied by PCR method for the presence of DNA/RNA of causative agents of a number of NFI. Results. Morbidity by NFI in KFD was registered by 6 nosologies: Lyme borreliosis, Marseilles fever, leptospirosis, tularemia, intestine yersiniosis and tick-borne viral encephalitis, wherein, transmissive infections made up 91.6%. Circulation of causative agents of Crimea hemorrhagic fever, Q fever, group of tick-borne spotted fever, Lyme borreliosis, human granulocytic anaplasmosis, human monocytic ehrlichiosis, hemorrhagic fever with renal syndrome, West Nile fever, tularemia and leptospirosis was established. Conclusion. Due to activity of natural foci of NFI further monitoring of epidemiologic and epizootologic manifestations of these infections in the Crimea, including using genetic methods of analysis, is necessary for ensuring sanitary-epidemiologic welfare of KFD population.

Problematic issues on creation and practical introduction of specific immune biologic preparations for therapy and prophylaxis of natural-foci arbovirus infections - West Nile fever (WNF) and Crimean hemorrhagic fever (CHF), that are not available until now, are examined. Persistent natural foci of WNF and CHF with epidemic manifestations have formed in Southern and North Caucasian Federal Districts of Russia. Markers of Wfest Nile virus are being detected in central regions of Russia and Siberia, and the presence of fraction of population immune to this infection is also detected. Analysis of literature sources that has been carried out forms a theoretical basis for creation of novel specific preparations for etiotropic therapy and prophylaxis ofWNF and CHF. Use of blood from healthy donors with sufficiently high titers of class G immunoglobulins residing in certain subjects of Russian Federation in natural foci territories with the most intensive epidemic process is possible as raw material.

West Nile Fever (WNF) is the most common arbovirus infection caused by West Nile Virus (WNV), which has been responsible for numerous epidemic outbreaks of disease among humans, birds and horses on all continents, with the exception of Antarctica, over the past two decades. On the territory of Ukraine, the earliest reports of cases of WNV circulation in humans and birds relate to the 70s of the XX century. In Poltava region WNF was first registered in 2011. Though the epidemiological and clinical patterns of WNF in Ukraine and Poltava region remain understudied, primarily due to the lack of alertness of practitioners to the problem of WNF and to broader access to laboratory diagnosis of the disease. The first clinical case of West Nile Viral encephalitis, registered on the territory of Poltava region, has been given and analyzed.

Objective: To analyze the results of epizootic monitoring of natural foci of particularly dangerous infections common to humans and animals in Rostov Oblast to establish their activity.Materials and Methods: The data of epizootic monitoring conducted by zoologists; the results of laboratory research of field material obtained in natural foci of infectious diseases. Descriptive, zoological, genetic, serological methods, and retrospective epidemiological analysis were used.Results: The results of the conducted epizootic monitoring showed the presence in Rostov Oblast of natural foci of particularly dangerous infectious diseases common to humans and animals. While the natural focus of the plague is in a depressed state and does not show activity, in the foci of tularemia, West Nile fever (WNF), Crimean hemorrhagic fever (CHF), ixodic tick-borne borreliosis (ITB), hemorrhagic fever with renal syndrome (HFRS), Q fever, Batai fever, California serogroups, Syndbis revealed the antigens of these pathogens in biological material obtained from small mammals and blood-sucking insects, which indicates the epizootic activity of these foci. In the natural foci of WNF, CHF, ITB, HFRS, cases of these disease among the population were recorded, which confirms the epidemic activity of these foci.Conclusion: The obtained results of epizootic monitoring over the period from 2008 to 2020 emphasize the relevance and necessity of continuing these studies in natural foci in Rostov Oblast.

The article considers the geography of emerging and re-emerging natural focal diseases in Russia. The data on the distribution and peculiarities of epidemic manifestations of eight key infections (ixodid tick-borne borrelioses, tick-borne encephalitis, hemorrhagic fever with renal syndrome, tularemia, leptospirosis, West Nile fever, Crimean-Congo hemorrhagic fever and Astrakhan rickettsiosis fever) belonging to different groups of emerging and re-emerging diseases are presented. We estimated and mapped the risk of exposure to these infections in the European territory of Russia within natural borders (biomes).
