BIOREGULATORY THERAPY IN GERONTOSTOMATOLOGY: ACHIEVEMENTS AND PROSPECTS

Увеличение доли пожилых и старых людей в структуре населения является особенностью настоящего времени, которая охватывает практически весь мир. Постарение населения изменило область задач врачей разных специальностей, в том числе стоматологов, делая повышение качества оказания стоматологической помощи и жизни людей старших возрастных групп наиболее приоритетными. Важной в поддержании здоровья людей старших возрастных групп является биорегуляция, которая представляет сложный комплекс межклеточных взаимодействий, направленный на поддержание гомеостаза и отвечающий за течение таких важных физиологических процессов, как дифференцировка и деление клеток, регенерация тканей, старение организма, обмен и воспроизведение генетической информации и многое другое. Биорегулирующая терапия направлена на ликвидацию нарушений гомеостаза для нормализации функций поврежденных клеток, тканей и органов. Цель работы - исследовать основные направления использования, достижения и перспективы биорегулирующей терапии в области геронтостоматологии. Представлены основные результаты применения биорегулирующей терапии в геронтостоматологии при лечении заболеваний пародонта, слизистой оболочки полости рта, височно-нижнечелюстного сустава, одонтогенных гнойно-воспалительных заболеваний и очагов стоматогенной хронической инфекции, а также перспективы их дальнейшего применения. The increase in the proportion of elderly and old people in the structure of the population is a feature of the present time, which covers almost the entire world. The aging of the population has changed the scope of tasks of doctors of various specialties, including dentists, making improving the quality of dental care and the life of older people a top priority. Bioregulation is important in maintaining the health of people of older age groups, which is a multiple complex of intercellular interactions aimed at maintaining homeostasis and responsible for the course of such important physiological processes as cell differentiation and division, tissue regeneration, aging of the body, exchange and reproduction of genetic information, and much other. Bioregulatory therapy is aimed at eliminating homeostasis disorders in order to normalize the functions of damaged cells, tissues and organs. The purpose of this work is to investigate the main directions of use, achievements and prospects of bioregulatory therapy in the field of gerontostomatology. The main results of the use of bioregulatory therapy in gerontostomatology in the treatment of periodontal diseases, oral mucosa, temporomandibular joint, odontogenic purulent-inflammatory diseases and foci of stomatogenic chronic infection, as well as the prospects for their further use are presented.

Особое место в структуре стоматологической заболеваемости занимает патология пародонтального комплекса, в частности - хронический генерализованный пародонтит, который по данным ВОЗ находится на втором месте по частоте распространения среди всех стоматологических заболеваний, уступая лишь кариесу. Первые признаки воспаления пародонта начинают проявляться уже в молодые годы, а к пожилому периоду - распространенность заболеваний пародонта достигает значений, стремящихся к 100%. Решение проблемы своевременной диагностики, профилактики и лечения патологии пародонта по-прежнему является актуальнейшей задачей современной стоматологии. Генерализованный пародонтит - это не только очаг хронической инфекции и источник сенсибилизации организма, но также ведущая причина потери зубов (особенно в старших возрастных группах), приводящая к серьезным деструктивным последствиям, дезорганизующим зубочелюстной аппарат. Данный факт способен серьезным образом повлиять на качество жизни человека, делая пародонтологическую проблему не только медицинской, но и социальной. Несмотря на очевидную актуальность данного вопроса, а также большой интерес к научным исследованиям в области пародонтологии, стоит признать, что уровень стоматологического здоровья, а также оказания пародонтологической помощи, в том числе пожилому населению - далеки от идеала. Во многом это объясняется сложностью этиологии и патогенеза заболеваний пародонта, а также, нередко, отсутствием комплексного понимания данной патологии при ее терапии A special place in the structure of dental morbidity is occupied by the pathology of the periodontal complex, in particular, chronic generalized periodontitis, which, according to the WHO, is in second place in terms of prevalence among all dental diseases, second only to caries. The first signs of periodontal inflammation begin to appear already in the young years, and by the old age the prevalence of periodontal diseases reaches values tending to 100%. Solving the problem of timely diagnosis, prevention and treatment of periodontal pathology is still the most urgent task of modern dentistry. Generalized periodontitis is not only a source of chronic infection and a source of sensitization of the body, but also a leading cause of tooth loss (especially in older age groups), leading to serious destructive consequences that disorganize the dental apparatus. This fact can seriously affect the quality of life, making periodontal problems not only medical, but also social. Despite the obvious relevance of this issue, as well as the great interest in scientific research in the field of periodontology, it is worth recognizing that the level of dental health, as well as the provision of periodontal care, including the elderly population, is far from ideal. This is largely due to the complexity of the etiology and pathogenesis of periodontal diseases, as well as, often, the lack of a comprehensive understanding of this pathology during its therapy

Epidemiological studies concerning the new coronavirus disease called COVID-19 show that elderly and old people are more susceptible to symptomatic, severe course of the disease, and also to death as its consequence. These age groups frequently suffer from associated, aging-related, chronic inflammatory diseases, in the case of COVID-19 described as co-morbidities. This paper describes the mechanisms of infection by SARS-CoV-2 virus and the development of acute COVID-19 and of its chronic form called long COVID, as well as the participation of various components of the immune system in the development and course of  this disease in the context of changing properties (aging) of both the innate and adaptive immunity in the elderly. In particular, the role of two key phenomena occurring in the aging immune system and precipitating or at least facilitating the aging-related diseases including COVID-19, namely the immunosenescence and inflammaging, is discussed.

In this article the review of foreign and domestic literary sources, which are devoted to the actual problem of modern dentistry – the treatment of inflammatory diseases of periodontal tissues: gingivitis and periodontitis are presented. The complex approach to their treatment involves the appointment of a significant amount of pharmacotherapeutic drugs. Therapeutic failures and iatrogenic complications have led to the fact that today the interests of doctors and population to medicinal products significantly increased. The purpose of the study is to analyze the data of scientific literature on the use of plant-based medicinal products for the treatment of periodontal inflammatory diseases over the past 10 years. Materials and methods. Comprehensive and systematic analysis of literature. Review and discussion. The analysis of information sources on the use of plant-based medicinal products in dentistry both independently and in the composition of medical and prophylactic means has established that the modern assortment of plant-based preparations in the pharmaceutical market of Ukraine to a certain extent is limited. The emergence of new plant-based species that have been tested in conditions of experimental pathology and require an evidence-based clinical base is noted. The composition of plant-based preparations used for the treatment of inflammatory periodontal diseases include vitamins, biologically active substances, glycosides, alkaloids, in connection with a wide range of action: antiseptic, anti-inflammatory, regenerating, hemostatic, antioxidative. The data on plant-based preparations that are most often used such as chamomile extracts, calendula, hypericum, plantain, kalanchoe, aloe, eucalyptus, milfoil, nettle, calamus and plant-based species are summarized. The medicinal agents considered are mainly recommended for local treatment of periodontal diseases in the form of dental care means, mouth rinse, gel, chewing gum, herbal liquer. It is known that the complex treatment of periodontal diseases includes a general influence on the body. The properties of green tea with its wide range of actions are investigated. With antioxidant properties, it can be a healthy alternative for controlling destructive changes in periodontal diseases. Attention is drawn to the proposed unique natural complex “Resverazin” due to a wide range of pharmacological action, low toxicity and relative safety. The drug produces antioxidant, anti-inflammatory, immune stimulating, vasodilative, neuroprotective action. Conclusion. Based on the literature analysis, it can be concluded that the accumulated experimental and clinical data on the therapeutic properties of plants prove perspective of their use in the complex treatment of inflammatory periodontal diseases. Future studies are mandatory for further confirmation of the effectiveness of these medicinal plants

<p><strong>BACKGROUND:</strong> Periodontal diseases, such as periodontitis, are considered the main cause of tooth loss in the elderly.The present study is aimed to determine the relationship between periodontal condition and quality of life. Quality of life consists of a range of people’s objective needs related to the self-perception of well-being.</p><p><strong>METHODS:</strong> This study was done from January 2014 to June 2015 in a healthcare clinic in Zahedan, southeast Iran. Using the random sampling method, the researchers enrolled 700 individuals over 35 years of age. The participants initially completed a demographic questionnaire consisting of data, such as age, sex, educational level, and smoking habit. Then, the periodontal chart was completed. Moreover, patients, based on the number of their natural teeth, were divided into two groups (≥10 teeth in both maxillary and mandible arches and &lt;10 teeth in at least one arch). The body mass index (BMI) was also measured.To assess the participants’ general health, the WHO’s quality of life questionnaire (WHOQOL-BREF) was used.</p><p><strong>RESULTS: </strong>Of the 700 enrolled individuals, 53.3% were womenand 47.7% were men. Moreover, most of the participants (63.71%) had BMI of less than 25 and 68% did not smoke.We found that as the people’s periodontal status deteriorated, their quality of life also declined and the total mean score in all four health domains decreased (P&lt;0.001)<strong>.</strong>Moreover, people with more than 10 teeth in both arches scored higher with respect to life quality than those with less than 10 teeth in at least one arch (P&lt;0.001).</p><p><strong>CONCLUSION: </strong>This studyindicates a decrease in the general quality of life in patients with periodontal disease.The authors suggest performing studies with larger sample sizes andcohort studies for more reliable results.</p>

Alongside Richard Haydocke’s translation of Giovanni Paolo Lomazzo’s treatise on painting (1598), the article examines concepts of color concerning cosmetics, painting and complexion as they relate to aesthetics, artistic and medical practice in the sixteenth and seventeenth centuries. Beginning with white and red as ideal colors of beauty in Agnolo Firenzuola’s Discourse on the beauty of women (1541), the essay places color in relation to major issues in art, medicine and empiricism by discussing beauty as a quality of humoral theory and its colors as visual results of physiological processes. Challenging the relation of art and nature, gender and production, Lomazzo’s account of complexion and Haydocke’s additions on cosmetic practices and face-painting provide key passages that shed light on the relation of cosmetics colors, art writing and artistic practices at the convergence of the body, art and medicine in the context of the emerging English virtuosi around 1600.

Currently, regardless of the age and social affiliation of the population, periodontal disease is the most common. Despite the modern technologies used in dentistry, the issues of prevention and treatment in dentistry remain unresolved. Dentists are interested in the fact that periodontal diseases are inextricably linked with systemic disorders in the body, which are responsible for its physiological functions. And the effectiveness of treatment for periodontal diseases is affected by all the factors that lead to the pathological process, i.e. diseases of internal organs and periodontal diseases are inextricably linked. In this regard, the metabolism in the body of certain vitamins, in particular vitamin D, forces dentists to re-examine the effect of these vitamins on pathological factors that lead to pathologies of the oral cavity.Diseases that affect the duration and quality of life of the patient have the ability to spread widely, for example, cardiovascular diseases, tuberculosis, HIV infection, etc., and oral diseases, in particular, caries and pathologies, can be attributed to this category. periodontium.

Healthy smile may affect person’s confidence and self-image. Senior dental problem can be common, from tooth decay to periodontal diseases, since oral health directly impacts the health of the rest of the body and mind, these issues need to be taken seriously. Elderly individual may start limiting the kinds of food they eat, prone to experience low self-esteem and other psychological problems, they may find it difficult to socialize to other people, changes of mood because of pain they experience in oral cavity and difficulty in communication due to changes in oral motor function such as phonation and deglutition. Descriptive type of research, 273 individuals were selected through convenience and purposive sampling. The respondents were all partially and completely edentulous individuals, aged sixty and above, residing at Barangay San Juan, Apalit, Pampanga, Philippines. Researcher made questionnaire, validated by licensed dentists, psychologists, and guidance counselor, were utilized as the research instrument to determine the respondent’s psychosocial well-being and quality of life. Based on data gathered, dental caries is the most common cause of the respondents’ tooth loss. Result showed that the effects of toothloss on health, behavioral, and socio-economic factors of the respondents are equal, and social aspect is greatly affected. Psychosocial well-being of the respondents is in between strongly affected and not affected at all, an indication that quality of life is affected by the effects of edentulism. Socialization and health aspect are the most commonly affected by edentulism. Feelings were encountered and coped differently by individual. Awareness and proper education on oral health practices can help prevent tooth decay and tooth loss that may improve the psychosocial well-being and overall quality of life of an individual, especially to future generations.

The purposeof this study was to develop data about growth-weighted indexes of men of youthful and Mature age, ethnic Kyrgyz.Material and methods. 955 men of youthful and Mature age, ethnic Kyrgyz living in Osh and its vicinities, practically healthy were examined. The method of complex anthropometry included estimation of 21 parameters necessary for somatotyping. The length and weight of the body were determined. Morphometric data processing was carried out with the help of statistical programs Microsoft Excel and STATISTICA 6.0 package. The differences between the two compared values were considered reliable according to the student's criterion at p < 0.05.Results. Somatotyping of men showed predominance of abdominal somatotype, with a minimum percentage of breast somatotype. The analysis of growth-weight indicators in different somatotypes showed that regardless of the somatotype, the length of the body increases slightly during the transition from adolescence to the 1st period of adulthood, which is explained by the continuation of growth processes. A slight decrease in body length in the 2nd period of adulthood is due to natural physiological processes and is a proven age-dependent fact.Conclusion. In the result of the study shows the distribution in the relative number of different somatotypes of men in the Kyrgyz population; shows the age and somatotypological characteristics of height and body mass that has significant scientific and practical importance.

The aim of the study was to evaluate the morphology structure of sperm cells and quality of individual male deer obtained in the Lublin Upland during the 2015/2016 hunting season. The comparison included two sepa-rate periods of the hunting season resulting from varying physiological processes related to the estrus period of this species. In the first study with regards to the estrus period, males were characterized by an average of 1.6 grams less testicle weight, while body weight was 1.9 kg higher. Increasing the mass of the testicles in the repro-ductive season proceeded simultaneously with the increase in the share of properly developed sperm cells to the level of 70.6%. During the pre-estrus period, the percentage of spermatozoids with main and secondary defects accounted for almost 60%. Between the compared periods, an approximately 6.8% body weight loss occurred. The highest percentage of properly developed spermatozoids was present in deer aged 4 and 5 years. Regarding the greatest body weight loss in these two age groups, it is indicated that this age is the climax in terms of poten-tial accession to the breeding and effective mating of females and the possibility of fertilization. Analysis of the relationship between body weight and testicle weight showed low and statistically insignificant correlation coef-ficients (rxy=0.331-0.393) between those features.

The search for new ways of early detection and improving the effectiveness of treatinginflammatory periodontal diseases is one of the urgent tasks in modern dentistry. Inflammatory diseases in the periodontal tissues lead to the loss of teeth, the appearance offoci of chronic infection in the oral cavity, a decrease in the body's reactivity, microbial sensitization, and the development of allergic conditions. This pathological process is not a strictly limited pathology but, as a rule, is just one of the manifestations of more serious systemic diseases. The general state of human health, the quality of life, his socio-mental status and even his role in society suffer.Keywords:periodontium, inflammation, platelet autoplasm
