Introduction: It has been proven that the oral hygiene is determined by the quantitative and qualitative composition of the bacterial biofilm, by the local immunity, and the rheological properties of the oral fluid. The aim of this study was to observe the dynamics of these parameters in the short term postoperative period after dental implantation in patients with a different state of periodontal tissues, depending on maintenance therapy. Materials and methods: 124 healthy people, aged 18 to 34, were examined. Depending on the follow-up maintenance therapy after dental implantation, patients were divided into two equal study groups: patients of the main group received the proposed differentiated maintenance therapy, the volume and multiplicity of which were determined by the initial oral hygiene state; patients of the control group underwent traditional treatment. Patients with healthy periodontium comprised a comparison group. Using the digital drop shape, the viscosity coefficient b2 was determined. Identification of the biofilm’s microorganisms was carried out with aerobic and anaerobic cultivation techniques. The presence of immunoglobulin SIgA in the oral liquid was determined by Manchini’s radial immunodiffusion method. Results: In the main group, the viscosity coefficient b2 and the content of SIgA immunoglobulin in the oral fluid were significantly higher than in the control group (p <0.05). The proposed maintenance therapy led to the restoration of the species domination of the probiotic microflora, proportional to the healthy periodontium both in the frequency of selection, and the level of insemination. The indicated dynamics of these parameters leads to a probable decrease of the Green-Vermillion HI (p<0,05). Conclusions: Proposed maintenance therapy leads to a probable improvement of the oral hygiene state based on Green-Vermillion HI, caused by the normalization of the surfactant properties of the oral liquid, biofilm’s ratio of microbial associations, and local oral hygiene.

Generalized periodontitis is the most common form of periodontal pathology, especially in the second half of life. Nowadays, the problem of periodontal disease is relevant due to its high prevalence, tendency to progression, multifaceted influence on the dentoalveolar system and the whole organism as well as uncertain treatment. Therefore, there is a need to find optimal ways of prevention and treatment of this disease. Close relationships between periodontal pathology and systemic diseases, such as hypertension, which affects every 2nd-3rd adult were determined.The objective of the research was to determine the status of oral cavity hygiene in patients with generalized periodontitis of II degree and stage II hypertension.Materials and methods. The study included 30 patients with generalized periodontitis of II degree and stage II hypertension being treated in the Ivano-Frankivsk Regional Clinical Cardiology Dispensary; the average age ranged from 35 to 54 years (the main group). The control group included 10 patients of the same age without generalized periodontitis and somatic pathology .The status of oral cavity hygiene was determined using the Green Vermillion index. The diagnosis of periodontal disease was made on the basis of the classification proposed by M.F. Danilevskyi. The obtained results were subjected to variation-statistical analysis using statistical package “Stat Soft 6.0”; classical methods of variational statistics were applied; mean values and their reliability were evaluated.Results. The results of examination showed poor oral hygiene in almost all patients. The analysis of hygienic indices showed the following results: in patients of the main group, the Green Vermillion index was 1.99 ± 0.13 points (p<0.001) which corresponds to unsatisfactory level of oral hygiene. In patients of the control group, this index was 1.10 ± 0.17 points which corresponds to satisfactory level of oral hygiene. According to the index, 9 (30.0%) patients of the main group had satisfactory oral hygiene, 12 (40.0%) patients had poor oral hygiene, and in 9 (30.0%) patients, poor oral hygiene was observed.In the control group, 2 (20.0%) patients had good oral hygiene, 6 (60.0%) patients had satisfactory oral hygiene and in 2 (20.0%) patients, unsatisfactory oral hygiene was observed. There were no patients with poor oral hygiene. The analysis of the indicators of the Green Vermillion index showed that in case of the pathological process exacerbation the oral hygiene status in patients deteriorated.Conclusions. The level of oral hygiene in patients of both groups was low due to incorrect selection of personal hygiene products or their untimely replacement. In patients with generalized periodontitis of II degree and stage II hypertension, the level of oral hygiene was lower than in somatically healthy persons: the worse status of oral cavity hygiene – the more pronounced changes in the periodontal tissues. We can suppose that high blood pressure affects the status of the oral cavity, creates a higher risk and exacerbates the periodontal diseases.

Vascular disorders play a significant role in the development of dystrophic inflammatory process. There is a direct correlation between the degree of damage to blood vessels in the jaw and a depth of the destructive process in periodontal tissue. A certain role is played by endogenous factors, such as: age, vitamin deficiency, diabetes mellitus, essential hypertension and others. Clinical and epidemiological studies using pathological techniques showed significant changes in vascular wall of the artery in the periodontium, the interdental artery in particular.  Atherosclerosis, essential hypertension and periodontal pathology were proven to occur in individuals older than 40 years.The objective of the research was to determine periodontal tissue status in patients with stage II hypertension and generalized periodontitis of II degree of severity.Materials and methods. The study involved 36 patients with stage II hypertension and generalized periodontitis of II degree of severity (the main group). The patients’ age ranged from 35 to 54 years. The control group included 10 patients of corresponding age without generalized periodontitis and somatic pathology. To assess the status of periodontal tissues, we applied the Papillary-Marginal-Attached Index and the Community Periodontal Index of Treatment Needs. When diagnosing periodontal disease, the classification of M.F. Danilevskyi was used.Results. The analysis of the indicators of the Community Periodontal Index of Treatment Needs index showed the following results: in patients of the main group, the index was 2.38±0.07 points (p<0.001) pointing out a need for a course of professional oral hygiene. In patients of the control group, the index was 0.5±0.17 points indicating that there was no need for treatment, however, there was a need for improving oral hygiene. The indicators of the Papillary-Marginal-Attached Index in the main group were 55.49±1.96 points pointing out a severe degree of gingivitis.Conclusions. According to the analysis of the indicators of the Community Periodontal Index of Treatment Needs index (2.38±0.07), the patients of the main group had low hygiene level requiring a course of professional oral hygiene.

The aim: To study the clinical efficciency of the proposed scheme of gingivitis treatment in women with physiological course of pregnancy. Materials and methods: Clinical approbation of the proposed complex of therapeutic and prophylactic measures was carried out in 32 pregnant women with gingivitis, aged 18 to 35 years. The efficiency of the treatment was assessed by the dynamics of the clinical picture, the index assessment of an oral hygiene and the periodontium condition in each trimester of pregnancy. Results: According to the data of performed observations, the high efficiency of the proposed treatment scheme is shown: 84.4% in the main group versus 46.9% in the control group. It has been clinically proven that the proposed scheme of inflammatory periodontal diseases treatment in terms of therapeutic efficiency exceeds the generally accepted basic treatment. Conclusions:The use of the proposed complex of therapeutic and prophylactic measures in the main group of patients contributed to the elimination of the inflammatory process in the periodontal tissues, suspended the progression of existing diseases, made it possible to prevent the emergence of new nosological forms, improved the condition of the oral hygiene, which makes it possible to recommend it for treatment of periodontal diseases in pregnant women.

The purpose of the research is to improve the efficiency of restorative treatment of patients with chronic generalized periodontitis based on the inclusion in therapeutic schemes of innovative mineral bishophytic complexes ― gel "Bishophytic" and rinser "Polymineral." Materials and methods. The study involved 116 patients at the age of 35 to 73 years old suffering from periodontal pathology (mild and moderate chronic generalized periodontitis). They were distributed by simple randomization into 3 groups: the main group included 43 people who received standard treatment in accordance with Russian clinical recommendations for the treatment of periodontitis, 45 minutes later the patients were prescribed some applications with 10% dental gel "Bishophytic" on the gum, in addition, the patients were recommended to use a rinser "Polymineral" for 6 months; the group of comparison included39 people who received standard treatment and gum applications with 10% "Bishophytic" dental gel; the control group) included 38 people who received only standard treatment. Results. Inclusion of mineral bishophytic complexes in treatment programs of patients with CGP contributes to significant reduction of inflammatory activity of periodontal tissues: 61.3% (p 0.01) had the improvement of hygienic and plaque indices in comparison with initial data; 54.6% (р 0,01) had the improvement after additional use of only gum applications with "Bishophytic" gel; the improvement was 52.2% (р 0.01) after basic drug therapy. Thus, the improvement of enzymatic processes was by 30.0% (p 0.01), 26.9% (p 0.01) and 7.8%; reduction of cytokine activity by 62.8% (p0.01), 61.1% (p 0.01) and 40%; the improvement of dental health by 84.0% (p 0.01), 80.7% (p 0.01) and 74.5% (p 0.01) at рkg-ог 0,05 and рkg-ог0,01. Conclusion. Inclusion of innovative mineral bishophytic complexes (gel "Bishophytic" and rinser "Polymineral") in treatment programs of patients with chronic generalized periodontitis contributes to optimization of therapeutic measures: clinical effectiveness in the main group was 93.0%; after usage of dental gel "Bishophytic" (group of comparison) it was 89.7% and after basic therapy (control group) 81.6%.

The purpose of the study was to investigate the rheological properties of oral fluid and its mineralizing potential in persons with generalized periodontitis on the background of lesions of the gastrointestinal tract. Material and methods. The rheological properties of oral fluid and its mineralizing potential were studied in 100 patients (main group) and 50 patients (comparative group). The overall functional activity of the salivary glands was assessed by the rate of secretion of mixed saliva. Oral fluid was collected for 15 minutes by spitting into graduated test tubes. The rate of salivation (ml / min) was the total volume of saliva collected divided by the time of oral fluid collection. The viscosity of oral fluid was determined using an Oswald viscometer with a capillary length of 10.0 cm and a diameter of 0.4 mm. The surface tension of saliva was determined by the method of T. L. Redinova. In the oral fluid of patients examined the calcium content by the method of A. V. Karakamov and inorganic phosphate by the method of D. F. Boltz [10]. The research was performed in compliance with the basic provisions of the "Rules of ethical principles of scientific medical research with human participation", approved by the Declaration of Helsinki (1964-2013), ICH GCP (1996), EEC Directive № 609 (dated 24.11.1986), orders of the Ministry of Health of Ukraine № 690 dated 23.09.2009, № 944 dated 14.12.2009, № 616 dated 03.08.2012. Each patient signed an informed consent to participate in the study, and all measures were taken to ensure the anonymity of patients. To assess the probability of the obtained results of the study used a variational-statistical method of analysis using Microsoft Excel. Statistical calculation of the results of clinical and laboratory studies was carried out according to conventional methods. Results and discussion. In recent years, a number of researchers have tried to systematize the internal pathology associated with periodontal disease. In foreign literature, a number of priority somatic risk factors for generalized periodontitis, in particular gastrointestinal diseases, have been formulated. In patients with generalized periodontitis on the background of gastrointestinal pathology, an increase in oral viscosity was studied. Oral viscosity along with poor oral hygiene, and decreased local immunity may serve as an additional factor in increasing the formation of plaque, which contributes to the intensification of inflammatory and dystrophic periodontal diseases in this contingent of patients. Increasing the rate of salivation in patients of the main group can be considered as an adaptive-compensatory response of the salivary glands to changes in oral homeostasis. The decrease in the mineralizing potential of the oral fluid according to the surface tension, calcium, phosphorus content and their ratio indicates a decrease in the resistance of the oral fluid and demineralization of the enamel, which leads to deterioration of dental status in patients. It should be noted that in patients with generalized periodontitis on the background of gastrointestinal lesions, the imbalance of the analyzed indicators was more pronounced in almost healthy people with inflammatory-dystrophic diseases of periodontal tissues. In Helicobacter pylori-positive patients of both groups of the study, a decrease in the rheological properties of oral fluid was determined, which indicated more pronounced changes in oral fluid homeostasis compared with the data obtained in Helicobacter pylori-negative patients. Conclusion. The study showed that in Helicobacter pylori-positive patients of the main group the imbalance of physico-biochemical, rheological and remineralizing parameters of oral fluid were more pronounced than in Helicobacter pylori-negative patients of the main group and in Helicobacter pylori-positive patients of the comparison group

Aim: to study the state of local immunity and oral hygiene in patients with GP suffering from AN.Materials and methods: 35 patients with AN were examined at the age of 19-35 years. In the study we used immunological methods, an index assessment of the state of oral hygiene (PMA, API, GI).Results and discussion: in patients of I group poor oral hygiene was established. The amount of lysozyme in the oral fluid in I group was 0.014 ± 0.07 g/l. A decrease SIgA (11SIgA) was established in patients of I group: I, II and control (135.31 ± 23.17, 130.26 ± 24.21 and 300.34 ± 27.38 mg/l, respectively). A tendency to an increase in the level of IgG in the oral fluid in patients of group II compared with I and the control group (5.35 ± 0.36, vs. 4.57 ± 0.04 and 4.98 ± 0.23 g/l, respectively) was established.Conclusions: poor oral hygiene is an objective indicator which reflects low motivation to maintain oral health. The observed immunodeficiency (11SIgA) in patients with GP is a mirror image of changes occurring in the body of patients with AN.

Goal. Improving the scheme of treatment and prevention of periodontal diseases in workers of the woodworking industry of Chernivtsi region, who in the process of professional activity have long-term contact with unfavorable factors of the production environment. Materials and methods. 70 workers of the woodworking industry with diagnosed chronic generalized periodontitis of the I degree were treated (35 people – the main group (A); 35 people – comparison group (B)), age – from 25 to 45 years. To compare the results of laboratory studies, an additional survey of 25 healthy individuals of the same age with an intact periodontium were conducted. They formed the control group (C). Clinical examination of patients was performed according to standart methods: subjective (complaints, medical history, life history) and objective (examination, periodontal indices, determination of the level of gingival attachment). Laboratory methods of research included determination of urease and lysozyme activity in saliva, degree of dysbiosis of oral cavity. As maintenance therapy, patients of the main group were prescribed the proposed composition DEPE. Results. After the treatment, a significant improvement in the hygienic status of patients in both groups was observed, but difference between groups A and B in the indicators of oral hygiene after treatment was no statistically significant (pA2–B2 > 0.05). There was a significant improvement in periodontal indices after treatment in patients of both groups (PMA index according to C. Parma, bleeding index according to Muhlemann in Cowell I. modification, Russell periodontal index, PSR-test), but the indicators in patients of the main group were significantly better – pA2–B2 < 0.05. In patients of the main group (A) the level of attachment loss decreased by 1.83 times, comparison group (B) – by 1.71 times. The difference in levels of attachment loss after treatment between groups A and B is statistically significant (pA2–B2 < 0.05). The activity of the enzyme urease in patients of the main group (A) and comparison group (B) after treatment was significantly reduced (pA1–A2 < 0.001, pB1–B2 < 0.001), but only in the main group it reached the level in the control group (C) (pA2–C > 0.05). The level of lysozyme after treatment was significantly increased in patients of both observation groups (pA1–A2 < 0.001, pB1–B2 < 0.001), and reached the level of lysozyme in patients of the control group (C) (pA2–C > 0.05, pB2–C > 0.05). The degree of oral dysbiosis in patients of the main group (A) was significantly reduced by 5.43 times (pA1–A2 < 0.001), and its difference from the degree of dysbiosis in patients of the control group (C) is statistically insignificant (pA2–C > 0.05 ). The degree of oral dysbiosis in patients of the comparison group (B) was significantly reduced by 3.04 times (pB1–B2 < 0.001), but still remained significantly different from oral dysbiosis in patients of the control group (C) (pB2–C < 0.05). Conclusion. The proposed pharmacological composition DEPE is an effective antiseptic solution and can be used in the phase of maintenance therapy to improve the treatment and prevention of periodontal disease in workers of woodworking industry. The high prevalence of inflammatory processes in the periodontitis of children, the ineffective effectiveness of preventive and curative measures can contribute to the development of generalized periodontitis, therefore it remains urgent to develop pathogenetic effects in the treatment of chronic catarrhal gingivitis.The aim of the study – a comparative assessment of the traditional treatment of chronic catarrhal gingivitis in children aged 6–7 years with therapy using an anti-inflammatory gel with neovitin.Materials and Methods. 61 children aged 6–7 years, suffering from chronic catarrhal gingivitis, were taken under clinical supervision and divided into groups: the main (30 children) and the comparative (31 children). All children underwent conventional therapy in accordance with the protocols for the provision of medical care – basic therapy. In the main group, for a month, pathogenetic agents were used: an anti-inflammatory gel with neovitin, hygiene products were recommended. In the comparative group, 3 % hydrogen peroxide solution was used for local therapy, for rinsing of the mouth (during the first week of treatment) – Rotokan, a therapeutic and prophylactic toothpaste.Examination and control examinations were carried out with the hygienic state of the mouth determined by the hygienic index of Yu. A. Fedorov and V. V. Volodkina, prevalence and intensity of the inflammatory process in the gums with the help of the Schiller-Pisarev test and the PMA index, oral fluid index (lysozyme content, formation of oxydradicals, urease activity, degree of dysbiosis, pH and viscosity).Results and Discussion. After the treatment, elimination of gum inflammation in patients of the main group was accompanied by a decrease in the PMA index by 3.8 times; formation of oxyradicals decreased in children of the main group by 6.35 %; in the comparative group – by 6.15 %. Urease activity decreased in children of the main group by 16.37 % (p<0.05), the viscosity of the oral fluid of children decreased in 2.65 times in the main group and in 2.13 times in the comparative group (p<0.05). The lysozyme content increased in children of the main group by 15.61 % (p<0.05) in the comparison group – by 9.63% (p<0.05). The treatment after 0.5 years caused an increase in the lysozyme content in the oral fluid of the children of the main group to the level of the control group. The degree of dysbiosis decreased in the main group, in the comparison group tended to increase.Conclusions. The use of the proposed therapy with gel with neovitin contributed to the best results of treatment: reducing the degree of inflammation of the gums, the number of visits to the doctor, improving the indices of nonspecific protection in the oral fluid of 6-7 years-old children and, to a greater extent, six months after the treatment.

The Hodgkin’s disease (lymphogranulomatosis, LGM) stands at the top of the list among malignant lymphomas in children. The highest rate of it occurrence coincides with childhood. LGM is most common at 4-6 and 12-14 years. The scope of scientific clinical research on odontoand parodontopathology is limited among pediatric population. The quantitative and qualitative composition of plaque, its thickness and area is primary pathogenic chain. In turn, unsatisfactory oral hygiene contributes to the development of decay and periodontal and oral mucosal disease in children with LGM. Our research aims to study the oral hygienic condition in patients with LGM during different clinical stages of the underlying disease. In order to reduce the toxic effect of LGM treatment, rational oral hygiene preventive measures shall be implemented. Matherials and methods. A total of 45 patients of both sexes in the 5-15 year age group diagnozed with lymphogranulomatosis (LGM) were examined. 243 healthy age-and-sex-matched children were included in the control group. The patients with lymphogranulomatosis (LGM I) were examined after the initial diagnosis. Group II (LGM II) consisted of patients who had completed the first polychemestry treatment cycle. Group III (LGM III) included patients who had a constant remission period (from over 6 months to 5 years). The oral hygienic condition was studied using the Pahomov’s hygienic index, the simplified oral hygiene index (OHI-S by Greene and Vermilion), the PHP hygiene hygiene efficiency index (Podshadley, Haley, 1968), the modified Turesky index (1970), and API surfaces [6]. The data was statistically analyzed using Student's-Fisher's method. The Pahomov's hygienic index (PHI) in children aged 5-15 with LGM was quite different in the main and control groups (p<0.05). The score of PHI was 2.3 ± 0.05 (p<0.05) in patient group with LGM (LGM I). The Pahomov's hygienic index had increased in the children aged 5-15 diagnozed with LGM. The Pahomov’s index values from the second (LGM II) and third (LGM III) examinations were found to be quite different when compared to the control group of children of the same age (p<0.05). This PHI value was 3.39 ± 0.11 (very poor) after the second examination (LGM II) and was at 2.7 ± 0.10 (poor) during the third examination (LGM II). The satisfactory and unsatisfactory oral hygiene conditions according to the Pahomov’s index were observed in 5-15 year old children with LGM. The satisfactory and unsatisfactory oral hygiene conditions have been established using the Pahomov’s index, Green-Vermilion, Tureski, PHP, API in 5-15 year old children with LGM. The children might have completely given up on tooth brushing due to increased trauma and bleeding gums. Soft plaque is often responsible for dental decay in primary and permanent teeth in children and periodontal diseases. The analysis of the individual oral hygiene condition determined poor level of hygiene skill in patients with LGM compared to somatically healthy children. Hygienic condition and care depend on age, clinical stage of the underlying disease, the course of pathological complications in hard dental and soft oral tissues. Conclusion. Our clinical study established a poor oral hygiene condition by using different groups of dental hygiene indicators in patients with LGM. This condition particularly worsens during chemotherapy, which causes significant changes in periodontal, oral mucous and hard dental tissues. Children have low awareness of oral cavity care requirements or the specifics of selecting hygiene supplies and facilities. In those cases, the dental examination was required for raising awareness and providing oral hygiene control which was carried out among patients at different stages of the Hodgkin’s disease and it remission. Therefore, adopting such measures would provide an opportunity to increase the resistance of hard dental and periodontal tissues.

Background. The study of the features of the course and the general links of pathogenesis in patients with chronic generalized periodontitis (CGP) with comorbid pathology is one of the priorities of modern medicine. Objectives ― to assess and analyze the level of apoptosis marker annexin A5 and the value of dental indices in patients with CGP of varying severity without General somatic pathology and against the background of bronchiectatic disease Methods. The study involved 90 patients with present study included varying degrees (light, medium, heavy), which were divided into two groups: patients with no somatic pathology (n=40) and patients with comorbid pathology in the form of bronchoectatic disease (n=50). Somatically healthy individuals with intact periodontal disease (n=40) were examined as a control group. The values of dental indices – PMA, PI, Muhlemann and OHI-s. The level of annexin A5 was determined in the oral fluid by enzyme immunoassay. Results. In patients with CGP revealed a statistically significant higher value of the level of annexin A5 compared with somatically healthy individuals with intact periodontal and statistically significantly higher in patients with more severe periodontitis. At the same time, in patients with comorbid pathology, the level of annexin A5 in patients with CGP with bronchoectatic disease with mild, average and heavy degree of periodontitis is statistically significantly higher than in patients with CGP without somatic pathology. The revealed relationships between the value of dental indices and the level of annexin A5 indicate the influence of the studied marker of apoptosis on the state of periodontal tissues. Moreover, the patients with comorbidity the power value of the identified links more than the present study included patients with no somatic pathology. Conclusion. The obtained data show the influence on the currents present study included the presence of comorbid pathology in the form of bronchoectatic disease and presence of common mutually aggravating link of pathogenesis of apoptosis and the opportunity to use annexin A5 as predictive marker of progression, as the present study included patients with no somatic pathology and in the background bronchoectatic disease.
