Phototherapy is a therapeutic procedure in dermatology. Phototherapy uses certain ultraviolet wavelengths classified into 3 types,broadband UVB (BBUVB), narrowband UVB (NBUVB), and psoralen UVA (PUVA). The most widely used is NBUVB for treating psoriasis, atopic dermatitis and vitiligo. The mechanism of action of phototherapy is to reduce epidermal proliferation, suppress the immune system, reduce the inflammatory process, and induce repigmentation. Before starting phototherapy, the dose adjustment depends on skin type and minimal erythema dose (MED). Frequency of use is recommended based on the severity of the disease, continuously administered needed. Besides of this therapeutic effect, some contraindications and side effects that need to be considered in the use of phototherapy such as drug interactions, photoallergic and risk of skin malignancy.

Atopic dermatitis (AD) is one of the most common inflammatory diseases of childhood, and it is the first one in gradual development of allergic diseases, also known as «atopic march». Sensitization establishment during the AD uncontrolled course is associated with the high risk of developing of serious allergic pathologies, increase in the severity of the disease course, and patients’ quality of life reduction. Thereby, it is crucial to achieve quick jugulation of the inflammatory process in case of severe AD with early onset of disease. This article shows modern therapeutic approaches to disease control in children.

Chitinase-3-like protein 1 (YKL-40) is suggested to be associated with type 2 T helper response and atopy. The aim of the study was the evaluation of serum YKL-40 level in atopic dermatitis. The study was performed on 59 patients: 27 males and 32 females, aged from 18 to 64 years. The severity of the disease was assessed by the SCORAD and objective SCORAD indexes. The severity of pruritus was measured by the visual analogue scale. Blood samples were taken to examine serum level of YKL-40, total IgE level, C-reactive protein level, white blood cell count, and neutrophil count. YKL-40 serum levels were significantly higher in patients with atopic dermatitis compared to the controls. There was a positive correlation between YKL-40 concentration and SCORAD, objective SCORAD, and pruritus. This study has shown that YKL-40 serum level is increased in patients with atopic dermatitis and reflects the severity of symptoms.

Abstract Scuticociliatosis, caused by an opportunistic ciliate protozoan, is responsible for significant economic losses in marine ornamental fish. This study reports the occurrence of Uronema spp., parasitizing ten species of marine reef fish at an ornamental fish wholesaler: Blue green damselfish (Chromis viridis), Vanderbilt’s Chromis (Chromis vanderbilti), Pennant coralfish (Heniochus acuminatus), Threespot angelfish (Apolemichthys trimaculatus), Goldspotted angelfish (Apolemichthys xanthopunctatus), Sea goldie (Pseudanthias squamipinnis), Orchid dottyback (Pseudochromis fridmani), Threadfin butterflyfish (Chaetodon auriga), Vagabond butterflyfish (Chaetodon vagabundus), and Bluecheek butterflyfish (Chaetodon semilarvatus). Diseased fish showed disorders such as hemorrhages and ulcerative lesions on the body surface. Histopathological analysis of the muscle, liver, gut, kidney, spleen, gills, and stomach revealed hemorrhages and degeneration of muscle fiber, vacuolar degeneration of hepatocyte, inflammatory process and granuloma in the liver, atrophy of intestinal villi, inflammatory process and granuloma in the kidney, melanomacrophage centers, as well as inflammatory process in the spleen, epithelial cells hyperplasia and granuloma formation in the gills, and vacuolar degeneration and eosinophils in the stomach. Due to the severity of the disease, it is necessary to implement biosecurity measures with rapid and accurate diagnosis to minimize the risk of economic losses caused by Uronema spp.

The prevalence of atopic dermatitis has significantly increased in developed countries during the past several decades. Surveys performed in Hungary also show a growing number of atopic dermatitis (AD) cases, although, a carefully designed case-controlled studies have not been performed. Therefore, we investigated the prevalence of AD in individuals under 19 years of age within the agricultural area of East-Hungary. Combined data obtained with Schultz-Larsen questionnaire on 1158 children were analyzed, and 25% of the index persons were examined by dermatologist. The mean prevalence of AD determined by questionnaires appeared to be 17.5% in the entire study population. Result of dermatological examination verified the validity and sensitivity of the questionnaire. A negative correlation was found between the severity of the disease and the length of breast feeding period. (Spearman's correlation coefficient = − 0.2247,p= 0.034). The prevalence of AD in an East-Hungarian agricultural area is nearly as high as that reported for populations residing in industrially developed countries, with a higher prevalence during childhood. Data suggest that premature abruption of breast feeding maybe one of the major factors among other environmental factors that is contributing to the development of AD.

The aim of the study was to assess the level of blood neuron specific enolase (NSE) in children with chronic gastroduodenitis (CGD) depending on Hеlicobacter pylori (HP), the severity of the inflammatory process, sex. The study involved 73 children with CGD. Group I– children with CGD associated with HP (HP CGD+); group II – with CGD not associated with HP (CGD, HP–). The control group (GK) – 28 healthy children. In the diagnosis of the disease, the endoscopic and morphological methods were used; the urease, microscopy, molecular biological and serological tests were performed to verify HP. The elevated levels of NSE were revealed in children groups I and II compared with GK (p < 0.01, p < 0.01). High levels of NSE in serum were observed in boys with CGD HP+ compared with girls with CGD HP+ (p < 0.01). Similar changes related to the gender were identified in group II (p < 0.01). In group I the highest values of NSE were identified with erosive gastroduodenitis (p < 0.01). In group II the opposite direction of enzyme changes was revealed and depended on the severity of the disease. Тhe increase of NSE in the blood of children with CGD HP+ and HP-indicates the presence of enzyme in the mechanisms of the inflammatory process in the mucous membrane gastroduodenal area outside the continuum of HP-infection. A multidirectional character of NSE changes depending on the characteristics of lesions mucous membrane gastroduodenal zone in groups I and II does not exclude the NSE involvement into the mechanisms of formation of the severity of the disease. Higher levels of NSE in boys as in CGD, HP+ and HP – do not exclude the relations of NSE and sex hormones.

Concentrations of the immunoglobulin that is associated with reaginic activity, IgE (γE), were measured in sera of 116 children with bronchial asthma. High γE (&gt;700 ng/ml) were found in only 30. High γE levels were found in children with many positive skin tests (P &lt; 0.01) and with active atopic dermatitis (P &lt; 0.01). Long term, systemic corticosteroid therapy was found to be associated with normal γE values (P &lt; .001) even when the characteristics of the disease would otherwise have indicated high values. As no correlation is found to be evident between severity of the disease and γE levels, it may be suggested that corticosteroids may decrease intravascular γE. The serum γE values failed to reveal two or more clinically different groups among our patients.

Fibronectin (Fn) level of blood plasma is studied by the immunofermental method in patients with heavy forms of eczema, psoriasis and atopic dermatitis. An increase of Fn level of blood plasma is revealed in all groups under examination, having no tendency for a decrease in clinical improvement. The results of investigation of the nature and dynamics of Fn characteristics in specific patients allow considering them as the factors, reflecting the integral characteristics of reactivity of the changed connective tissue of derma. The definite types of correlations peculiar to each of dermatoses are determined, relating Fn levels with other characteristics, typical of inflammatory process.

Atopic dermatitis and psoriasis are common chronic dermatosis characterized by prolonged recurrent course. Depending on the severity of the disease topical and systemic treatment are applied. Keratoplastic agents play an important role in treatment of skin diseases. These agents include Naftalan Cream 30%, which significantly improves the skin condition. The results of treatment of 10 patients with atopic dermatitis and 10 patients with psoriasis vulgaris Naftalan cream 30% are presented.
