Summary. Aim. The article is aimed to assess the nonspecific factors of the body’s resistance in patients with erysipelas and in the development of sepsis in these patients. Materials and methods. 114 case histories of patients who underwent inpatient treatment in the center of purulent-septic surgery in the Non-profit municipal enterprise “City Hospital № 3” in Zaporizhzhia for the period 2019-2020 were analyzed. According to the forms of the disease, the patients were distributed as follows: erythematous — 24 (21.0 %), bullous — 28 (24.6 %), phlegmonous — 48 (42.1 %), necrotic — 14 (12.3 %). Recurrent cases of the disease were noted in 21 patients. From them: with erythematous form — 4 (16.7 %), bullous — 5 (17.9 %), phlegmonous — 9 (18.7 %), necrotic — 3 (21.4 %). Among 10 patients, the disease was complicated by sepsis. 6 patients died, mortality was 60 %. Results and their discussion. Depending on the detected disorders, the patients were divided into three groups: with uniformly activated immune status; with a suppressive type of immune response; with a mixed type of immune status, where with a normal or suppressive cellular link of immunity, activation of some indicators of the humoral link against the background of a normal or reduced level of complement is determined, which indicates the sensitization of T-cell populations with an antigen and the development of autoimmune processes. Conclusions. An increase in complement levels is an indicator of active antigen-antibody binding reactions in patients with erysipelas. Weak NBT-reaction in patients with sepsis indicates depletion of the enzymological activity of neutrophilic leukocytes and, to some extent, can serve as a predictor of a lethal outcome. An increase in the rates of phagocytosis is a prognostically favorable sign indicating the effective removal of the antigenic material of immune complexes from the patient’s body.

SUMMARYThe detection of egg-derived antigens in the serum and urine of Schistosoma mansoni-infected individuals and experimental animals would provide an alternative method to assess the tissue egg burden. The detected levels are, however, not only a function of the amounts of antigen produced, but also of the processing or clearance by the host. In the present study the immunolocalization pattern of antigens using 2 recently described monoclonal antibodies to repetitive carbohydrate epitopes of S. mansoni soluble egg antigen (114–5B1–A and 114–4D12–A) in various organs of the host was investigated. In the liver strong immunoreactivity could be detected around the entrapped eggs and in egg-shells, as well as in Kupffer cells accumulating both antigen and schistosomal pigment. In the spleen, immunohistochemistry revealed antigen in the plasma as well as in secondary lysosomes of macrophages. Strong labelling was found in the vesicles of the eosinophilic granulocytes: indirect evidence perhaps for the presence of antigen–antibody complexes. In conclusion, the secreted egg antigens were sequestered in the reticulo-endothelial macrophages of the liver and the spleen as already partly described for worm-derived antigens. The presence of large quantities of antigenic material in the spleen could suggest an important role of this organ in the clearance of antigen and might even provide an additional explanation for the hepatosplenomegaly mainly present in S. mansoni-infected children.

Objective. To determine the tactics of infusion therapy in patients with ischemic stroke (IS) depending on the severity of the violation of energy-structural status (ESST). Materials and methods. A study of 32 patients with severe IS on the National Institutes of Health Stroke Scale (16,7±1,5), who were in the department of anesthesiology with intensive therapy units of the Municipal Non-Profit Enterprise «City Hospital № 9» Zaporizhzhia City Council. Of these, 11 were men (34,4 %; the average age – 68,2±2,5 years), 21 were women (65,6 %; average age – 72,1±1,6). Results and discussion. In patients with IS, disorders ESST were defined as hyperergic damage at values of cardiac index (CI) of 4,45-5,09 L×min-1×m-2 and oxygen consumption index (IVO2) 186-210 ml×min-1×m-2, and at values of CI ≥5,10 L×min-1×m-2 and IVO2 ≥211 ml×min-1×m-2 – as hyperergic insufficiency. While hypoergic damage ESST occurred at values of CI 2,33-1,82 L×min-1×m-2 and IVO2 104-85 ml×min-1×m-2, and at CI ≤1,81 L×min-1×m-2 and IVO2 ≤84 ml×min-1×m-2 hypoergic insufficiency of ESST was observed. The daily fluid requirement of a patient with IS was calculated according to the formula 4+2+1: for the first 10 kg of weight – 4 ml×kg-1×h-1; from 11 to 20 kg – 2 ml×kg-1×h-1; from 21 kg – on 1 ml×kg-1×h-1 (Park G.R., Roe P.G., 2005; Netyazhenko V.Z., Halushko O.A., 2012). Infusion therapy in patients with IS and hyperergic damage ESST was performed with 0,9 % sodium chloride solution according to the formula 4+2+1 on the background of the use of esmolol intravenously bolus 250 mg and subsequent administration of 50 mсg×kg-1×min-1, and in hyperergic insufficiency 500 mg of esmolol intravenously bolus and subsequent administration of 100 mсg×kg-1×min-1. While in hypoergic damage ESST on the background of infusion therapy used dopamine or dobutamine 1-5 mсg×kg-1×min-1, and in hypoergic insufficiency, the dose of dopamine or dobutamine was increased to achieve the desired effect. Conclusions. The personification of infusion therapy depending on the severity of the violation of ESST can improve the results of treatment of patients with IS in the most acute period.

Materials and methods. 114 case histories of patients who were hospitalized in the center of purulent-septic surgery of the Non-profit municipal enterprise «City Hospital № 3» in Zaporozhye for the period 2019–2020 were analyzed retrospectively and prospectively. Results. It was found that most often the focus was localized on the upper and lower extremities. In the complex treatment of patients with surgical forms of erysipelas, the main place belongs to antibiotic therapy (ABT) and early surgical treatment of the area of the pathological process. Determination of the level of procalcitonin in blood serum makes it possible to assess the progression of the inflammatory process and is a sensitive test for the effectiveness of treatment. Conclusions. The incidence of complicated forms of erysipelas requiring surgery in the general population was 78,9%. Patients with circular erysipelas need the most extensive surgery, when it is necessary to perform an autopsy within the area of inflammation with longitudinal and transverse wavy autopsies throughout the pathological focus. Determination of the level of procalcitonin in blood serum makes it possible to assess the progress of the process and is a sensitive test for the effectiveness of treatment. Not all ticks carry dangerous pathogens, but their bites can threaten humans with diseases such as Ixodes tick-borne borreliosis (Lyme disease), Miyamoto disease, tick-borne viral encephalitis and about 150 other nosological entities. The season of life activity of Ixodes ticks begins in March and usually lasts until November and has two pronounced seasonal peaks during the year: over April – May and August – September. The relevance of the investigation is determined by the lack of the information on the incidence of tick bite cases among the population in Ukraine, and there no well elaborated sets of measures in order to prevent this problem. The purpose of this study is to provide a detailed analysis of the incidence of Ixodes tick bite cases among the adult population in the Vinnytsia region and in the city of Vinnytsia, to compare the number of detected cases for the period from 2013 to 2018, and to identify the most reliable and available prevention methods. The research was conducted at the laboratory of the municipal non-profit enterprise "Vinnytsia City Hospital G1" (KNP "VMKL"); statistical data were provided by the State Regional Sanitary and Epidemiological Station. We compared the prevalence of the disease in different districts of the Vinnytsia region and its insidence for the period from 2013 to 2018. During this period, the number of detected cases of tick bites resulted in the occurrence of various dangerous diseases went up from 53 to 317 cases among the entire adult population of the Vinnytsia region. For all years except 2018, the highest incidence rate was consistently observed in the Mogilev-Podilsk district district, even exceeding the level in more densely populated and large areas. Over the period from 2013 to 2018, the number of detected cases of tick bites in the adult population that resulted in various dangerous diseases rapidly and significantly increased. This necessitates to elaborate the set of preventive measures and to educate the population of all ages. Much attention should be paid to promoting vaccination as the only reliable way and the development of sanitary and hygienic recommendations for residents of municipal and rural areas (in the spring and summer).

Summary. The efficiency of using the dynamic blocking intramedullary osteosynthesis (BIOS) technique for tibial fractures with the use of biodegradable implants based on the MS-10 magnesium alloy was determined. Objective: to approve the technique and to study the effectiveness of dynamic intramedullary osteosynthesis of diaphyseal fractures of the tibia using locking screws made of the biodegradable magnesium alloy MS-10. Materials and Methods. The study was carried out in the Department of Traumatology of the Municipal Non-Profit Enterprise “City Hospital of Emergency and Ambulance” of Zaporizhzhia City Council in Zaporizhzhia on 15 patients with diaphyseal fractures of the tibia, who underwent BIOS. Cannulated intramedullary titanium rods were used as a fixator. In the proximal section, a screw made of titanium alloy was introduced into a dynamic hole; a screw made of a biodegradable magnesium alloy MC-10 was inserted into a static hole. In the postoperative period, the patients underwent a course of complex rehabilitation measures. X-ray control was performed on day 1, at week 4, week 8, and week 16 after the surgery. Results. After the start of the dosed load on the injured limb, a fracture of the locking screw made of MC-10 alloy was radiographically recorded in all patients. The dynamic screw moves in the dynamic hole and the fracture is dynamized. The formation of callus in the fracture zone was unremarkable and took an average physiological time. The quality and structure of callus did not show any pathological features. Four months after the surgery, all patients were socially adapted and did not use additional support when moving. Conclusions. Based on the data obtained, it is possible to draw a conclusion about the possibility and advisability of using biodegradable implants made of magnesium alloy MS-10 in a dynamic BIOS of long bones of the human skeleton.

The ideal marker of bacterial infection should not only allow for early diagnosis, but also provide information about the course and prognosis of the disease. Nowadays the most well-studied and widely used in clinical practice is procalcitonin (PCT), but its value in surgical forms of erysipelas is insufficiently defined. The aim of the article is to determine the diagnostic and prognostic significance of procalcitonin in patients with surgical forms of erysipelas.114 case histories of patients who were hospitalized in the center of purulent-septic surgery of the Non-profit municipal enterprise "City Hospital №3" in Zaporozhye for 2019-2020 period were analyzed retrospectively and prospectively. It was found that most often the focus was localized on the lower extremities. In the complex treatment of patients with erysipelas, the main place belongs to antibiotic therapy (ABT) and early surgical treatment of the area of the pathological process. Determination of the level of procalcitonin in the serum makes it possible to assess the progression and generalization of the process and is a sensitive test for the effectiveness of treatment.Radical surgery and timely targeted ABT are key elements of success in the treatment of surgical forms of erysipelas and are not subject to revision. Procalcitonin is a diagnostic marker that can predict the development of sepsis.

Purpose. The purpose is to find out the frequency of surgical forms of erysipelas among the general population of the disease. Materials and methods. Traditionally, there is a widespread point of view about streptococcal etiology of erysipelas. However, recently, staphylococcus or microbial associations are increasingly sown from the focus of inflammation in patients. The relevance of studying the course of erysipelas is determined by the tendency to relapse, a significant increase in complications of the disease in recent years and the final phenomena. The existing clinical situation is largely explained by changes in the spectrum of pathogens of purulent surgical infection of soft tissues. Its leading pathogens are mainly staphylococci and streptococci, as well as gram - negative aerobic bacteria. The nature of the clinical course of the disease, including the nature of the local inflammatory focus, depends on the totality of the pathogenic properties of microbes that form the microbial landscape. It was found that most often the local focus was localized on the lower extremities. 114 case histories of patients who were hospitalized in the center of purulent-septic surgery in the Non-profit municipal enterprise “City Hospital № 3” in Zaporizhzhia for the period 2019-2020 were analyzed retrospectively and prospectively. All patients were diagnosed with erysipelas on admission, according to the forms: erythematous - 24 (21.0%), bullous - 28 (24.6%), phlegmonous form - 48 (42.1%), necrotic - 14 (12.3%). %). Disease relapses were noted in 21 patients. In terms of gender: men - 47 (41.2%), women - 67 (58.8%). The patients' age was 62.1 ± 2.6 years. Automated devices "Vitek-2" or "BaCT ALERT" (France) were used to determine the sensitivity of microorganisms in a purulent focus to antibacterial drugs. Statistical analysis was performed using the “Statgraphics Plus for Windows 7.0” software package. Research results and their analysis. It was found that most often the local focus was localized on the lower extremities. A total of 26 types of bacteria were identified from the wound contents of patients with destructive forms of erysipelas. No bacterial growth was found in 3 (4.8%) patients, and mixed flora was isolated in 27 (45.8%) patients. Gram-positive flora accounted for 61.0%, gram-negative flora - 35.6%, anaerobes - were not found, fungi - 3.4%. Conclusions. The number of destructive forms of erysipelas among the general population of the disease is more than 50%. The study of purulent-necrotic foci in patients with destructive forms of erysipelas makes it possible to determine the qualitative and quantitative composition of microflora, sensitivity to antibiotics, and make adjustments to the conduction of antibiotic therapy.

The aim of our work was to study the characteristics of the immune status based on the analysis of the cellular, humoral links of immunity and dynamics of immunoglobulins in adults with community-acquired pneumonia (CAP), and their importance in the pathogenesis and prognosis of the disease. Materials and methods. The study was carried out in the period 2017-2020 on the basis of the therapeutic department of the Municipal non-profit enterprise “City Clinical Multidisciplinary Hospital № 25” of Kharkiv City Council. The study involved 20 adult patients with CAP aged 18 to 80 years (mean age 36.5±10.3). The control group consisted of 20 apparently healthy individuals (mean age 39.5±12.5). The study of the immune status of patients was carried out by determining the phagocytic index, the content of lymphocytes, CD3+, CD4+, CD8+, CD16+, CD20+ and the level of immunoglobulins of classes A, G, M in the dynamics of the disease: on the first and tenth days after hospitalization. Results. On the first day of hospitalization, there was a significant imbalance in phagocytosis, T- and B-links of immunity. On the tenth day of treatment, the phagocytic index, the number of CD3+, CD4+, CD8+, CD16+ and CD20+ increased. An increase in Ig M, a decrease in Ig A and Ig G were also noted. Thus, standard therapy for patients with CAP leads to its clinical and radiological resolution, but is not accompanied by the normalization of immunity parameters. Conclusions. The features of immune disorders in patients with CAP were revealed: pneumonia increases the imbalance in the cellular link of immunity, the level of CD3+ and CD4+ decreases, there is no dynamics of the relative amount of CD20+. There are no significant changes in the humoral link of the immune status. For the treatment of patients with CAP, in addition to standard pharmacotherapy, it is necessary to include drugs that stimulate the immune system
