High Prevalence of Metabolic Syndrome and of Insulin Resistance in Psoriatic Arthritis is Associated with the Severity of Underlying Disease

Objective.To investigate the prevalence of metabolic syndrome (MetS) and of insulin resistance (IR) in an ethnically homogeneous cohort of established psoriatic arthritis (PsA), and to identify clinical associations of MetS and IR in patients with PsA.Methods.A cohort of 283 patients with PsA all meeting ClASsification for Psoriatic ARthritis (CASPAR) criteria was included. All underwent detailed skin and rheumatologic assessments, along with cardiovascular risk factor evaluation. IR was defined as an elevated homeostasis model assessment (HOMA-IR) value of > 2.5. Severe PsA was defined as the presence of 1 or more of the PsA-related radiographic damage features (peripheral joint erosions, osteolysis, sacroiliitis), and PsA requiring tumor necrosis factor inhibitor therapy.Results.The demographic and clinical characteristics of the cohort were mean age 54.6 ± 12 years, 52% female, mean PsA duration 19 ± 9 years. MetS was present in 44% of the studied patients (n = 283). On multiple regression analysis, a significant association of MetS was noted with more severe PsA (OR 4.47, p < 0.001), higher smoking pack-years (OR 1.03, p = 0.02), and worse EQ-5D scores (OR 1.28, p = 0.02). Data on IR were available for 263 patients, and among them, the mean HOMA-IR was 1.43 ± 1.09. Forty-one patients (16%) had IR. On multiple regression analysis, a significant association of IR was noted with more severe PsA (OR 3.49, p = 0.03), later psoriasis age of onset (OR 1.07, p = 0.001), and higher body mass index (OR 1.22, p < 0.001).Conclusion.Among patients with PsA, MetS and IR are highly prevalent, and are independently associated with the severity of underlying PsA.

Purpose. To clarify whether the incidence of metabolic syndrome (MetS) and the overlap of MetS components are affecting the ocular circulation shown by laser speckle flowgraphy (LSFG). Materials and Methods. We studied 76 consistent patients. Blowout score (BOS) and blowout time (BOT), which are the pulse waveform analysis parameters, and mean blur rate (MBR) using LSFG in the optic nerve head (ONH) and choroid were evaluated. Throughout, the ONH was separated out from the vessels and tissue for analysis and MBRs in the ONH were divided into four sections (superior, temporal, inferior, and nasal). Results. Thirty-two patients were diagnosed having Mets. MBR-Tissue (P=0.003), MBR-All (P=0.01), MBR-Choroid (P=0.04), and BOS-Choroid (P=0.03) were significantly lower in patients with MetS than in the patients without MetS. Multiple-regression analysis revealed the temporal side of MBR-Tissue and BOS-Choroid which were identified as factors contributing independently to the overlap of the MetS components. Multiple-regression analysis also revealed that the MetS components were identified to be factors independently contributing to the BOS-Choroid and temporal side of MBR-Tissue. Conclusion. Our study clarified that the incidence of MetS and the overlap of the MetS components are significantly affecting the ONH and choroidal microcirculation.

Background: Many studies have established the association between overt hypothyroidism and metabolic syndrome (MS). However, the relationship between subclinical hypothyroidism (SCH) and MS varies from study to study. This study aimed to investigate associations between SCH and components of MS and vitamin D status in Korean adults.Methods: Using data from the 6th Korea National Health and Nutrition Examination Survey (2013-2015), chi-squared tests and t-tests were used to explore relationships between thyroid hormones and components of MS and vitamin D status. Multiple regression analysis was undertaken to assess relationships between SCH and components of MS and vitamin D.Results: Systolic blood pressure and fasting glucose were significantly higher in the euthyroid (EUT) group than in the SCH group, and vitamin D status was significantly lower in the SCH group than EUT. However, in multiple regression analysis, SCH presented no significant association with components of MS or vitamin D status even after adjusting for age, sex, drinking, and smoking.Conclusions: In Korean adults, there is no significant association between SCH and MS or vitamin D.

Abstract Background: Glomerular hyperfiltration is a well-recognized early renal alteration in subjects with diabetes mellitus and a strong and independent predictor of cardiovascular events in these patients. Prediabetes has been associated with increased glomerular filtration rate (GFR), however, the association between prediabetes, glomerular hyperfiltration and early markers of cardiovascular damage has not been investigated. The aim of this study was to investigate the association between renal hyperfiltration (RHF) and early markers of cardiovascular disease in subjects with prediabetes.Methods: Arterial stiffness [Augmentation Pressure (Aug), Augmentation Index (AugI)], subendocardial viability ratio (SEVR), pulse wave velocity (PWV), intima-media thickness (IMT), glycated hemoglobin (HbA1c), oral glucose tolerance test and estimated GFR (eGFR) were evaluated in 230 subjects with prediabetes. The eGFR was assessed using the Chronic Kidney Disease Epidemiology Collaboration formula (CKD-EPI). Hyperfiltration was defined as an eGFR above the 75th percentile.The subjects were divided into two groups according to the presence or absence of RHF: 169 subjects with prediabetes without RHF and 61 subjects with prediabetes with RHF.Results: Subjects with RHF showed higher Aug, AugI and lower SEVR compared with prediabetic subjects with lower eGFR (14.1±7.2 vs 10.8±6.2, 32.9±12.7 vs 27.6±11.7, 153.5±27.8 vs 162±30.2, P<0.05). No differences were found in PWV and IMT values between the two groups. We then performed multiple regression analysis to test the relationship between Aug, SEVR and several cardiovascular risk factors. In multiple regression analysis Aug was associated with age, systolic blood pressure (SBP), homeostatic model assessment for insulin resistance (HOMA-IR) and eGFR; the major determinants of SEVR were eGFR, HOMA-IR and SBP.Conclusions: Our data show that subjects with prediabetes and RHF exhibited an increased Aug, AugI and a reduced SEVR. Longitudinal studies are needed to explore whether hyperfiltration increases the possibility of diabetic nephropathy and cardiovascular disease in individuals with prediabetes.

(1) Background: Pediatric obesity is an emerging public health issue, mainly related to western diet. A cross-sectional study was conducted to explore the association between macronutrients intake and cardiometabolic risk factors in obese adolescents. (2) Methods: Ninety-three Italian obese adolescents were recruited; anthropometric parameters, body composition, glucose and lipid metabolism profiles were measured. Macronutrients intake was estimated by a software-assisted analysis of a 120-item frequency questionnaire. The association between macronutrients and cardiometabolic risk factors was assessed by bivariate correlation, and multiple regression analysis was used to adjust for confounders such as age and sex. (3) Results: By multiple regression analysis, we found that higher energy and lower carbohydrate intakes predicted higher body mass index (BMI) z-score, p = 0.005, and higher saturated fats intake and higher age predicted higher HOmeostasis Model Assessment of insulin resistance (HOMA-IR) and lower QUantitative Insulin-sensitivity ChecK (QUICK) index, p = 0.001. In addition, a saturated fats intake <7% was associated with normal HOMA-IR, and a higher total fats intake predicted a higher HOMA of percent β-cell function (HOMA-β), p = 0.011. (4) Conclusions: Higher energy intake and lower carbohydrate dietary intake predicted higher BMI z-score after adjustment for age and sex. Higher total and saturated fats dietary intakes predicted insulin resistance, even after adjustment for confounding factors. A dietary pattern including appropriate high-quality carbohydrate and reduced saturated fat intakes could result in reduced cardiometabolic risk in obese adolescents.

This research aims to investigate how far the influence of branding, product quality, and price toward consumer’s satisfaction in beverage franchise business. This research utilizes 5 likert scale questionnaire which is tested by using multiple regression analysis to reveal whether or not there is partial and simultaneous influence of branding, product quality, and price toward consumer’s satisfaction in beverage franchise business. Sampling method is accidental sampling technique, in which sample of particular population is taken based on the accessibility and availability of the sample during the sampling process. Sample used is 100 samples among consumers or customers of Mang Endy Milkshake. The result shows that branding, product quality, and price influence consumer’s satisfaction in beverage franchise business.

Seaweed-Kappaphycus-Euchema Cottonii and Denticulum species was first cultivated at Sabah side of Sebatik in 2009. By November 2014, sixty one Sabahan seaweed farmers cultivated 122 ha or 3,050 long lines. Thirty Sabahan seaweed farmers in Kampung Pendekar (3.2 m.t dried) and 31 in Burst Point (12.5 m.t dried) produced 16 metric tonnes of dried seaweed contributed 31% to Tawau’s total production (51 m.t). The remaining 69% were from farmers in Cowie Bay that separates Sebatik from municipality of Tawau. Indonesian in Desa Setabu, Sebatik started in 2008. However, the number of Indonesian seaweed farmers, their cultivated areas and production (as well as quality) in Sebatik increased many times higher and faster than the Sabah side of Sebatik. In 2009 more than 1,401 households in Kabupaten Nunukan (including Sebatik) cultivated over 700 ha and have produced 55,098.95 and 116, 73 m.t dried seaweed in 2010 and 2011 respectively. There is a divergence in productions from farming the sea off the same island under similar weather conditions. Which of the eight explanatory factors were affecting production of seaweeds in Sebatik? Using Cobb Douglas production function, Multiple Regression analysis was conducted on 100 samples (50 Sabahan and 50 Indonesian). Results; Variable significant at α = 0.05% are Experience in farming whereas Farm size; Quantity of propagules and Location — Dummy are the variables significant at α 0.01%. Not significant are variables Fuel; Age; Number of family members involved in farming and Education level.

AbstractInternet addiction has become a worrying phenomenon for Indonesian teenagers. This research was conducted to determine whether the psychological factors will influence internet addiction in adolescents. This research uses quantitative with multiple regression analysis method. The winning sample is 200 adolescents. The instrument collects data using a scale internet addiction scale that compiled by Griffiths (2005) and developed by Lemmens (2009), Parenting Authority Questionnaire (PAQ) developed by Buri (1991), Social Skill Inventory (SSI) developed by Riggio (1986), and A Rasch-Type Loneliness Scale compiled by De Jong Gierveld (2006).  The results showed that there were significant differences in the parenting style, social skills, and loneliness on the tendency of internet addiction in adolescents.AbstrakAdiksi Internet menjadi salah satu fenomena yang mengkhawatirkan untuk remaja Indonesia. Penelitian ini dilakukan untuk mengetahui faktor psikologis apakah yang memberikan pengaruh terhadap kecenderungan adiksi internet pada remaja. Sampel pada penelitian ini berjumlah 200 remaja dengan menggunakan metode analisis kuantitatif. Penelitian ini menggunakan alat ukur sebagai berikut, alat ukur adiksi internet yang dikembangkan oleh Lemmens (2009), Parenting Authority Questionnaire (PAQ) yang dikembangkan oleh Buri (1991), Social Skill Inventory (SSI) yang dikembangkan oleh Riggio (1986), dan A Rasch-Type Loneliness Scale yang disusun oleh De Jong Gierveld (2006). Berdasarkan hasil pengujian ditemukan pengaruh signifikan gaya pengasuhan, keterampilan sosial, dan kesepian terhadap kecenderungan adiksi internet pada remaja.
