Background: Non-adherence to the recommended therapy causes patients with heart failure to experience recurrence of the disease. Reminder book on therapy adherence is very useful, because it assists in monitoring adherence to therapy carried out by patients while at home. Therefore, this study aims to determine the effect of the existence of a reminder book on adherence to therapy among patients with heart failure in a private hospital in Malang.Design and method: A pre-experiment design with one-group pre and posttest was used. The respondents were 18 patients that received counseling on the management of heart failure therapy while at home and were given a reminder book. Furthermore, the modified MMAS-8 scale was the study instrument used to measure adherence.Result: The results showed that most respondents were above the age of 65 with a treatment duration of 1 to 5 years. It was discovered that most of the respondents had never received information about heart failure therapy. Data analysis which was carried out using the Wilcoxon test with a p-value of 0.001, showed that there was a significant difference between respondents' compliance before and after being given a reminder book. Conclusion: From this study, it was concluded that providing a reminder book has an effect on therapeutic adherence in patients with heart failure. Researchers recommend that hospitals should make use of this reminder as a tool to control or supervise outpatient therapy.

This paper discusses the effect and evaluation of echocardiography based on lipid nano contrast agent on patients with heart failure and atrial fibrillation in cardiology department, providing reference for clinical diagnosis and treatment. Fifty two patients with diastolic heart failure diagnosed by echocardiography were selected for routine echocardiographic examination after optimizing the drug treatment scheme, and then the patients underwent treadmill exercise test and stress echocardiography evaluation. The results of conventional echocardiography and stress echocardiography after treatment were compared with those before treatment, and the clinical parameters and biochemical indexes before and after treatment were compared. Results after treatment, the clinical symptoms of the patients improved, the level of NT proBNP in the N-terminal forebrain decreased significantly, and the exercise tolerance increased significantly. Compared with the conventional echocardiography before and after treatment, the left ratio and e′ value of stress echocardiography after treatment increased significantly, while E/e′ decreased significantly. There was no significant difference in the indexes of general echocardiography before and after treatment. After treatment, positively correlated with the ratio of peak a to peak E. The results show that the sensitivity of stress echocardiography to evaluate ischemic diastolic heart failure has been improved, and some indexes have clinical significance. Compared with conventional echocardiography, it can effectively evaluate the therapeutic effect of drugs.

Orthodontic treatment may lead to environmental changes in the oral cavity that may occur in a number of users of the orthodontic appliance, such as changes in bacterial concentration, salivary buffering capacity, salivary acidity (pH) and salivary flow rate. Xylitol stimulates salivary flow and helps restore saliva acid/base balance because it cannot be converted to acid by oral bacteria. To identify the difference in salivary pH before and after chewing xylitol gum in the users of fixed orthodontic appliance. This research is quasi-experimental with one group pretest-posttest design. Samples were students of SMA Kolombo Sleman who used a fixed orthodontic appliance. Data were analyzed using a Wilcoxon test. The salivary pH of the users of a fixed orthodontic appliance before chewing xylitol gum mostly belonged to acid criterion (43.8%), and the base criterion (34.3%). After chewing xylitol gum, the salivary pH of the users of fixed orthodontic appliance mostly belonged to a neutral criterion (78.1%). The results of the Wilcoxon test showed that the salivary pH before and after chewing gum xylitol in the users of fixed orthodontic appliance had a P value of 0.027 <α 0.05. There is a significant difference in salivary pH before and after chewing xylitol gum in users of fixed orthodontic appliance.

Fatigue is a widespread clinical complaint among adults with type 2 diabetes. Fluctuating glucose levels can cause fatigue. Several factors are associated with fatigue in diabetic patients, including physiological factors such as hypoglycemia or hyperglycemia, psychological factors such as depression associated with diabetes. Progressive muscle relaxation is a procedure to get relaxation in the muscles through two steps, namely by applying tension to a muscle group and stopping the tension then focusing on how the muscle relaxes, feeling the sensation of relaxation and fatigue is reduced. The aim of this study was to identify the effect of progressive muscle relaxation on fatigue symptoms in type II DM clients in 15 control groups and 15 intervention groups. The research method used a quasi-experimental design with a pretest-posttest control group design approach, consisting of one treatment (in the intervention group) and a control group. Data analysis was performed using univariate and bivariate analysis using t-independent and t-dependent tests. Wilcoxon test results in the intervention group and the control group showed a significant p-value of 0.002 (<0.05), meaning that there was a significant difference in fatigue symptoms in the intervention group before and after the intervention. This shows that there was a decrease in fatigue symptoms in the intervention group before and after combination OHO therapy and progressive muscle relaxation exercises (previously the average respondent in the intervention group experienced symptoms of severe fatigue, after intervention, the average value of fatigue symptoms turned into mild fatigue ). Conclusion: Progressive muscle relaxation combined with OHO therapy can reduce symptoms of fatigue compared to only OHO therapy.

BACKGROUND: Psychological stress is a condition that is experienced by many adolescents which affect the Hypothalamic-Pituitary-Gonadal axis. Testosterone is known as a sex steroid hormone that is susceptible to acute stress and can be measured through saliva. Disruption of the reproductive system can affect the sexual maturation process. AIM: To understand the difference in salivary testosterone levels in puberty children before and after given a stressor. METHODS: A quasi-experimental intervention study was conducted at Antonius Bangun Mulia junior high school, Medan, North Sumatera, in July-October 2017. Subjects were students aged 12-14 years with sexual maturity G2 for boys and M2 for girls. Psychological stress intervention was generated by the Wechsler intelligence scale for children fourth edition (WISC IV). Saliva was collected before and after the intervention. The analysis was done with Wilcoxon test and a P value < 0.05 was considered significant. RESULTS: Forty-two subjects of 24 male students and 18 female students with sexual maturation Tanner II (54.8%) and Tanner III (45.2%). This study obtained that there was a statistically significant difference in salivary testosterone levels before and after the subject was given a stressor (P = 0.015, CI 95%). This difference also was seen within sexual maturation Tanner II (P = 0.045, CI 95%). No difference was observed in testosterone levels based on gender, male students (P = 0.065, CI 95%) and female students (P = 0.112, CI 95%). CONCLUSION: Stress can affect salivary testosterone levels. There was a statistically significant difference in salivary testosterone levels before and after psychological stress in puberty children.
