This study aims to examine the connection between the therapeutic alliance (TA) – from both the patient’s and the therapist’s perspectives – and communicative intentions during the initial phase of adolescent psychotherapy. The sample comprised the first three sessions of 19 adolescents’ individual psychotherapeutic processes. Fifty therapeutic conversation segments were analyzed, representing 2043 adolescent and 3208 therapist speaking turns. The variables studied were Therapeutic Alliance, measured with the Working Alliance Inventory and Communicative Intentions (Exploring, Attuning and Resignifying), assessed by the Therapeutic Activity Coding System. A multilevel analysis was performed (HLM software). The Tasks dimension of the TA perceived by the adolescents increases the likelihood that their discourse will focus on providing and requesting information (Exploring), while also reducing the probability that they will seek to work together with the therapist to achieve mutual understanding (Attuning). The therapists’ discourse shows that a strong TA in the Bonds dimension makes it more probable for their utterances to focus on the construction of new meanings (Resignifying). Also, a stronger TA in the Goals dimension makes it more likely for their discourse to be directed toward seeking Attunement with the adolescent.

Psychotherapy research focusing on adults as well as on children and adolescents has revealed a positive association between the quality of the therapeutic alliance (TA) in the first sessions and therapeutic outcomes. Nevertheless, there is controversy regarding which perspective (of the adolescents, therapists, or parents) and what moment of evaluation (first, second, or third session) is most strongly associated with outcome. This study aims to describe the TA during the initial phase of psychotherapy with adolescents, from several perspectives, and relate it to the intermediate and final outcomes of the therapy. The study is descriptive-correlational and was conducted in naturalistic settings. The sample comprised 20 individual psychotherapeutic processes (15 females, 5 males; average age: 15.8 years; standard deviation=1.04). The variables studied were: TA, measured with the Working Alliance Inventory, applied to adolescents, therapists, and parents, and therapeutic outcomes, measured using Lambert’s Outcome Questionnaire (OQ- 45.2). The results show that the TA improved from session one to session three, both in the adolescents and the therapists. The TA, as perceived by the adolescents in the third session, is positively correlated with final outcomes (r=-.732*, P=.025). The TA evaluated from the therapists’ perspective in the third session, specifically the bond component of the TA, predicts the final success of psychotherapy with adolescent patients [β=0.426, Exp. (B)=1.531, degree of freedom=1, P=0.034]. The importance of the initial construction of the working alliance by both participants is discussed.

Abstract Objective(s): To develop a novel method of examining facial affects in psychotherapeutic settings and interpreting the subliminal microexpressions and emotions they represent. Results: Therapy sessions are videotaped, and verbal content is evaluated with computer assistance from ATLAS.ti. Visual content is screened for facial action units/micro-expressions using the Facial Action Coding System (FACS) and reevaluated using the Emotional Facial Action Coding System (EmFACS) in order to discern the underlying emotions. Working Alliance Inventory scores are compiled to assess their impact on the quality of the therapeutic relationship. Reliability of facial action unit coding is ensured by a training course and an independently evaluated standardized test at Innsbruck University. Interrater reliability was excellent (Cohen’s κ > 0.80). Application of this method on a small study population to measure non-verbal communication and affects demonstrated its feasibility and usability. We believe this methodology to be widely implementable. Its application in psychotherapy may provide greater insights in investigations into important aspects of therapeutic interaction (e.g. verbal/non-verbal communication between clinicians and patients in psychiatric contexts, quality of psychotherapeutic relationships, etc.), and may be exploitable in clinical psychotherapeutic settings to better understand mechanisms of change in therapy.

Therapeutic alliance is associated with treatment outcomes across a diverse range of intervention models and clinical populations. Likewise, the perceived quality of working alliance in adult psychotherapy has been a consistent predictor of engagement, while research with children is limited. This study draws from a sample of 180 children who were involved in a study of the Coping Power preventive intervention for elementary students with aggressive behavior. Approximately 1,080 individual intervention sessions were coded using the Therapeutic Process Observational Coding System (TPOCS-A). Results suggest that bonding, in the early sessions of Coping Power, predicted a reduction in teacher-reported externalizing behaviors. In addition, data analyses revealed that bonding interacted with children’s gender in the latter portions of Coping Power. Higher bonding for girls in later sessions predicted lower teacher-rated externalizing behaviors at 1-year follow-up. Finally, initial levels of aggression moderated the relationship between bond and parent-rated externalizing behaviors during the early and late sessions of Coping Power, with stronger effects for moderately aggressive children. Task orientation, a component of therapeutic alliance, in late sessions had counter-intuitive effect on parent-rated externalizing behaviors, possibly due to therapists’ efforts to impose more structure with children who were not progressing well.

15 Background: Discordance in prognostic understanding between caregivers of adults with cancer and the patient’s oncologist is common. However, the relationship between caregiver-oncologist discordance and caregiver bereavement outcomes is unknown. We evaluated the associations of caregiver-oncologist discordance in beliefs about the patient’s curability and life expectancy with caregiver-reported therapeutic alliance and anxiety. Methods: This is a secondary analysis of a multicenter study that assessed the effect of a communication intervention among patients with advanced cancer and their caregivers. Prior to intervention exposure, caregivers and oncologists were asked about their belief in the patient’s chances for cure and living ≥2 years: 100%, about 90%, about 75%, about 50/50, about 25%, about 10%, and 0%. Discordance was defined as a difference by 2 response levels on each prognostic understanding item. Outcomes at 7 months after patient death included caregiver-reported therapeutic alliance [modified 5-item Human Connection (THC) scale] and anxiety (Generalized Anxiety Disorder-7). We used multivariable linear regression models to assess the independent associations of discordance with therapeutic alliance and anxiety. Results: We included 97 caregivers (mean age 63, range 22-83). Approximately 40% of caregiver-oncologist dyads had discordant beliefs about curability (caregivers were more optimistic in 100% of dyads) and 63% had discordant beliefs about life expectancy (caregivers were more optimistic in 94% of dyads). On multivariate analysis, discordance in beliefs about prognostic estimates was associated with lower THC score (b = -6.94, SE 3.17, p = 0.03). Discordance in beliefs about curability was associated with lower anxiety levels (b = -1.79, SE 0.90, p = 0.05). Conclusions: Caregiver-oncologist discordance may decrease caregiver-reported therapeutic alliance and anxiety, both of which may shape how caregivers interact with the healthcare system. A better understanding the role of caregivers’ prognostic understanding will guide interventions to improve caregiver-oncologist therapeutic alliance and caregiver anxiety. Clinical trial information: NCT01485627.

In art therapy, art-making plays an important role in the therapeutic relationship. To better understand the triangular relationship between the art therapist, the client and the artwork, this study investigated the association between the therapeutic alliance and reactions to artistic experiences with art materials in an art therapy simulation. The simulation consisted of a series of 6–8 sessions in which art therapy students were divided into teams composed of a permanent observer (art therapist) and creator (client). The client's role was to self-explore through art- making, and the art therapist's role was to accompany the client. Thirty-four students, all women, who played the art therapist role, and 37 students (one male) who played the client participated in the study. Of these participants, there were 24 pairs where both participants filled out all the questionnaires. A short version of the Working Alliance Inventory (WAI) was completed by the clients and the art therapists on the second session (T1) and on the penultimate session (T2). The clients also completed the Art-Based Intervention Questionnaire (ABI) at T2. Significant positive correlations were found between indices of the WAI for the art therapist and the client and the clients' reactions to the artistic experience with art materials on the ABI. The evaluation of the emotional bond between the art therapist and the client at the start of the simulation significantly predicted the client's reactions to the artistic experience with art materials at the end of the simulation and explained 45.4% of the variance for this variable. These findings highlight factors related to the development and influence of the therapeutic alliance, as well as the role of the artistic experience in art therapy and lay the groundwork for further research.

The purpose of the article is to identify the features of the artistic space of a choreographic performance based on the vocabulary of ballroom dance. Methodology. A systematic method was applied, which helped to consider the choreographic performance as an integral artistic system; a philosophical and aesthetic method - for considering and comprehending a choreographic performance in the context of a new stage concept of the 21st century; the typological method used to identify and analyze the components of the artistic space; the method of interpretive analysis of the ways of creating the artistic space of the choreographic performance and the hermeneutic-phenomenological method, which helped to study the possibilities of understanding the meanings of the text of the choreographic performance and methods of their formation. Scientific novelty. The significance of the auditory and visual non-plastic elements of a choreographic performance (scenography, musical arrangement, and dramatic basis) in the context of the development of a holistic image of stage work is considered. The preconditions for the harmonious combination of movement, sound, light, and color are analyzed, and their importance in the process of creating a single artistic image of a choreographic performance is revealed. It has been established that in accordance with the basic principles of interaction and mutual influence of these components, as well as the specifics of their relationship with choreographic vocabulary, a process of qualitative transformation of figurative characteristics occurs while maintaining their inherent aesthetic nature. Conclusions. Choreographic art specifically transforms the features of the artistic space – the artistically reflected and transformed space of reality, endowed with aesthetic meaning and content. Despite the originality of each specific choreographic performance, created on the basis of the vocabulary of ballroom dance, the specifics of the staging solution, etc., a stage choreographic work is an integral artistic phenomenon. The study revealed that the radical transformation of the story sources of stage ballroom dance does not imply violation of their figurative and poetic concept. But the use of specific techniques and methods by choreographers-producers contributes to the creation of a multi-layered artistic space of the performance, the organic interaction of many subtexts, the emergence of innovative semantic perspectives, the generation of semantic potential, and the production of new meanings. It is stated that the multi-layered artistic space of a choreographic performance is due to the specifics of their plot basis, artistic principles, and the individual vision of the director.
