Oesophageal foreign body presents as a medical emergency and requires immediate evaluation and treatment. We are reporting a rare case of sewing needle in esophagus migrating into the prevertebral space at thoracic inlet level. A 13-year-old mentally retarded female child was brought in emergency at midnight with complaint of accidental ingestion of sewing needle with the thread. Patient was posted for rigid esophagoscopy under general anaesthesia, no needle could be visualised and only thread of the sewing needle was removed. So patient was taken up for neck exploration along with gastroenterologists. Further careful dissection confirmed the needle in the pre vertebral space, which was removed successfully with artery forceps. Patient was given IV antibiotics for 10 days. Patient had an uneventful recovery and was discharged after 10 days. Pointed metallic slender foreign bodies can perforate and migrate very fast in the neck or chest and can lead to morbidity and mortality. Multidisciplinary approach offers a great advantage in surgical planning and proper patient management.

Gastrointestinal obstruction is a life-threatening condition that is usually caused by accidental ingestion of inedible objects, through play or food indiscretions. Ingested foreign objects generate the mechanical obstruction of the gastrointestinal segment, characterized by a relatively typical set of symptoms, depending on the location. The aim of this study was the clinical and imaging description of some cases of gastrointestinal obstruction in dogs and cats, caused by swallowing textile foreign bodies. The study included 12 dogs and 8 cats of different breeds and ages, belonging to both sexes, which showed clinical signs suggestive from a clinical-anamnestic point of view for gastrointestinal obstruction. These patients had symptoms with acute or chronic progressive evolution of the gastrointestinal tract, including restlessness, deviation, vomiting (single or repeated episodes), abdominal tenderness, constipation and tenesmus. The diagnosis was confirmed by radiographic examination, simple and with contrast medium, and ultrasound, completed in some situations with laparotomy. Ingested textile foreign bodies were represented by socks/stockings (5 dogs and 3 cats), underwear (3 dogs), rope (1 dog and 3 cats), carpet fragments (2 dog and 1 cat), toy fragments (1 dog and 1 cat), and they were located in the stomach (7/12; 58.33%, respectively 4/8; 50%), in the gastro-duodenal segment (2/12; 16.67%, respectively 3/8; 37.5%), in the jejunum (3/12; 25%, respectively 1/8; 12.5%) and in the colon (1/12; 8.33%, respectively 1/8; 12.5%). Gastrointestinal obstruction in pets, caused by accidental ingestion of textile foreign bodies, is a medical emergency and imaging examinations are the main diagnostic tool in the establishment of a subsequent therapy with maximum efficiency.

Background: Foreign bodies ingestion is a emergency in otology, knowledge of people about foreign bodies ingestion is not enough. Objective: To study the clinical characteristics, paraclinical characteristics and results of treatment from foreign bodies ingestion in Hue Central Hospital and Hue University Hospital. Methods and patiens: A cross descriptive and prospective study over the period from 6/2014 to 5/2016, total are 137 patients come to be diagnosised and treatmented. Results: the average age is 35 years old. Gender: male (51.8%) and women (48.2%). Adults (84.7%) having more than children (15.3%). Age group from 16-30 years is highest (32.8%). There are 95.7% of organic foreign bodies, 4.3% are inorganic foreign bodies. There are 90.5% of patients on diagnosis and treatment in stages less inflammation, arthritis 8.0% in the period and 1.5% in the period complications. Foreign body in the throat problems (73.7%), esophageal foreign bodies (26.3%). Pick up directly foreign bodies 54%, indirectly by the mirror 11.7% and endoscopy 8%, rigid esophagoscopy is 17.5%, flexible esophagoscopy is 7.3%, cervicotomy is 1.5%. Conclusion: Practing direction with in the oropharynx foreign body, using the larynx mirror or endoscopy with in the laryngopharynx for the esophagus foreign bodies, rigid esophagoscopy is better. SummaryPainful orthopedic conditions associated with extreme tall stature and leg length discrepancy (LLD) include back pain and adopting bad posture. After failure of conservative treatment options, blocking of the growth plates (epiphysiodesis) around the knee emerged as gold standard in patients with tall stature and LLD in the growing skeleton. Surgical planning includes growth prediction and evaluation of bone age. Since growth prediction is associated with a certain potential error, adequate planning and timing of epiphysiodesis are the key for success of the treatment. LLD corrections up to 5 cm can be achieved, and predicted extreme tall stature can be limited. Percutaneous epiphysiodesis techniques are minimally invasive, safe and efficient methods with low complication rates. In general, a multidisciplinary approach should be pursued when treating children and adolescents with tall stature.

Introduction: Cellular angiofibroma is a rare benign mesenchymal tumor, occurring almost exclusively in the superficial soft tissues of the genital region. Some of these lesions presents as large, hypervascularized masses. There are no reports in the literature to date on the challenges of resection of hypervascularized vulvar masses, or of hypervascularized cellular angiofibroma.Materials and methods: We describe a case of a cellular angiofibroma of the vulva and the surgical approach with tumoral vessel embolization and posterior resection.Case: A 47-year-old women was referred to vulvar pathology section of Italian Hospital of Buenos Aires for evaluation of left vulvar mass. A pelvic angiography was performed 5 days prior to surgery. It showed a hypervascularized formation at the level of the left vulva at the expense mainly of branches of the left internal pudendal artery. A complete resection was performed afterwards at our institution. The histopathological diagnosis was compatible with cellular angiofibroma.Conclusion: We consider that surgical planning is essential and has to be performed with a multidisciplinary approach. Angiographic embolization should be taken into account as a valid resource when planning a surgery with a high risk of blood loss in order to decrease morbidity.

Background: Chronic diseases such as stroke most frequently occur on old people while literature on prognosis risk factors on elderly is rare particularly Sub-Sahara Africa. The aim of the study is to determine prognosis risk factors of stroke on elderly patients. Method: It was a prospective cohort study carried out during 6 months that included elderly patients over 70 years old admitted in two tertiary medical emergency departments. Results: 56 patients with mean age 75.2 ± 5.17 showed moderate NIHSS score (59%) without gender predominance. 75% of patients with a severe score were in the 70-79 years old group. Functional dependency (93%) increased with the NIHSS score severity and 50% of patients with a moderate NIHSS score showed normal nutritional status unlike patients with a severe NIHSS score and depression for most of the cases. Mortality was frequent for patients with hemorrhagic stroke with swallowing disorder and a severe NIHSS score. Conclusion: Stroke on very elderly patients appears moderate with mortality chances increasing according to the severity of the NIHSS score, swallowing disorders that require a multidisciplinary approach in a neurovascular unit.

Pituitary apoplexy is a rare clinical syndrome due to ischemic or haemorrhagic necrosis of the pituitary gland which complicates 2–12% of pituitary tumours, especially nonfunctioning adenomas. In many cases, it results in severe neurological, ophthalmological, and endocrinological consequences and may require prompt surgical decompression. Pituitary apoplexy represents a rare medical emergency that necessitates a multidisciplinary approach. Modalities of treatment and times of intervention are still largely debated. Therefore, the management of patients with pituitary apoplexy is often empirically individualized and clinical outcome is inevitably related to the multidisciplinary team’s skills and experience. This review aims to highlight the importance of a multidisciplinary approach in the management of pituitary apoplexy and to discuss modalities of presentation, treatment, and times of intervention.

Chronic aortoiliac occlusive disease most often affects the common iliac arteries and distal aorta but can progress all the way to the renal arteries, occluding the inferior mesenteric artery. A compensatory collateral network typically develops to preserve lower body perfusion. Inadvertent compression or ligation of such collaterals during surgery can have catastrophic consequences. In this article, we present the case of a 63-year-old patient with aortoiliac occlusive disease, requiring surgery for an adenocarcinoma of the rectosigmoid junction. A CT angiography was performed in order to map out the collateral pathways that had developed and Doppler ultrasound was used to mark their positions. The surgical procedure was adapted to his specific anatomy. A successful anterior resection was performed, and the patient made an uneventful recovery. In cases of aortoiliac obliteration, the existence of collaterals must be kept in mind and investigated with a multidisciplinary approach before any surgery is considered.

Among peritoneal dialysis patients, peritoneal dialysis-related peritonitis is a well-known complication, but it can also be non-peritoneal dialysis-related (e.g. ruptured appendix). Ileal perforation by foreign bodies such as blister pill packs can be seen in the emergency room. Differentiating this from peritoneal dialysis-related peritonitis can be difficult, since they can have both identical presentations. Computed tomography can be of value in detecting abscess, thickening of the digestive wall or adhesions, and exclude other causes of intra-abdominal sepsis. Because of the aging population, ingestion of foreign body can be expected to rise. We therefore recommend that blister pill packs should not be divided into single-dose pieces, and we invite elderly patient’s entourage to check medication administration. Furthermore, manufacturing efforts are highly recommended to improve blister pill pack’s design, to avoid accidental ingestion. We report the case of an elderly peritoneal dialysis patient who presented with peritonitis due to ileal perforation because of blister pill pack ingestion.

We present the case of a 20 year old female patient who presented following ingestion of multiple button magnets. She remained clinically well however serial abdominal radiographs demonstrated the magnets were not passing through the gastrointestinal tract and a CT was therefore performed for further assessment and to aid surgical planning. Artefact from the magnets made interpretation of the CT challenging. The use of a Metal Artefact Reduction (MAR) algorithm however enabled accurate localisation of the magnets thus guiding subsequent surgical intervention. Whilst MAR algorithms are usually used in the assessment of iatrogenic metallic devices (e.g., joint prostheses), this case demonstrates an example of their potential wider use.
