The Role of sFas in the Immune Escape Mechanism and Progression of Cancer Larynx

This work assess serum levels of soluble Fas form (sFas) in patients with different stages of laryngeal squamous cell carcinoma(LSCC) to investigate its prognostic significance. We correlate its levels with the morphological changes of peripheral blood cells via buffy coat examinatin. The study population included 70 patients clinically diagnosed and pathohistologically confirmed LSCC in addition to 20 healthy controls. According to TNM classification 33(47.1%) patients were in stage I (group I) and 24(34.3%) in stage II (group II), 13(18.5%) in stage III (group III). The results revealed that the mean serum level of sFas (pg/ml) in the control group was 51.2, in group I was 66.33, in group II was 81.33 and in group III was 112.45. Statistical analysis of the mean of sFas by ELISA test in the patients' groups in comparison to the control revealed a significant increase of both group II and III in comparison to the control group (P < 0.01) but there was no significant difference (P > 0.05) between group I in comparison to either the control or group II. There was a significant difference (P < 0.05) between group I in comparison to group III. LM examination revealed massive extent of the apoptotic cells in group III when compared to both group I and group II. EM examination of the buffy coat revealed apoptotic changes, mainly in the peripheral blood mononuclear cells (PBMNCs), represented by surface membrane ruffles and blebs with clumped nuclear chromatin and vacuolated cytoplasm. In conclusion, this study may help us to better understand one of the escape mechanisms in cancer larynx. This mechanism is represented by the significant increase in both the serum level of sFas and the morphological apoptotic changes that detected in PBMNCs. Soluble Fas may contribute to the progression of laryngeal cancer. It can be used as an attractive target for anticancer therapy and may be considered as a marker of disease progression and poor prognosis in laryngeal cancer.

This study was conducted at 45 students of SMAN 1 Cerme with aged between 15-16 years old. Students were  divided into 3 groups. the type of research used quantitative with quasi-experimental approach. Instruments of this study were test of leg strength and leg power using Leg Dynamometer and  Jump MD. The research design used matching only design. Data was anlyzed using ANOVA. The results of the  research was found that training program side hop over  barrier with dumbell and hexagon with  dumbbell significantly increase at leg strength and leg power (Sig. 0.000 <α = 0.05). Group I, Group II and Group III had significant difference (Sig. 0.000 α = 0.005). Percentage increase in leg strength group 1 of 1,80% and leg  power of 15,49%. Percentage increase in leg strength group II of  1,68% and a power of 14,27% and whereas in the control group increase in leg strength was 1.67% power at 12.32%.It can be concluded that There was an increase in leg strength and leg power in each group after the training was given. In addition, there were differences among the three groups effects seen improved in leg strength and leg power through ANOVA, where the training side hop over barrier with dumbbell and hexagon with dumbell had increased in leg strength and leg   power. But side hop over barrier with dumbbell had more significant increased than hexagon with dumbbell. Key words: Side hop over with barrier dumbell,  hexagon with dumbell, leg strength and leg power.

Functional TLR4 expression has been linked to HCC development. TLR4 may serve an important role in HCC development by promoting the malignant transformation of epithelial cells and tumor growth. The consequences might be dependent on the complex signaling networks triggered by TLR4 activation and the tumor microenvironment. The study included 90 consecutive subjects classified into 3 group their age from 40 to 70 years old. Group (I): HCC patients on top of chronic HCV infection. they were 45 patients 30 male and 15 females, their age ranged from 45 to 55 who were subdivided into 3 subgroups according to Barcelona clinic liver cancer (BCLC): Group (Ia): included 8 HCC patients in early stage. (stage A). Group (Ib): included 12 HCC patients in intermediate stage (stage B). Group (Ic): included 25 HCC patients in advanced stage. (stage C). Group (II): 30 Cirrhotic patients with chronic HCV, 21 male and 9 females, their age ranged from 50 to 60. This group was subdivided into 2 subgroups according to Child–Pugh score Group (IIa): included 8 Child–Pugh A. Group (IIb): included 22 Child–Pugh B and C. Group (III): controlled group included 15 normal subjects. 10 male and 5 females, their age ranged from 45 to 60. They were selected to match patients’ groups in demographic and socioeconomic standards. In our study where 15 persons are control showed lower level in TLR4 with mean 1.0±0.2, however 30 patients with HCV and other 45 patients with HCC showed higher level in TLR with mean 2.27±0.6 and 4.2±1.06 respectively. In our study there is statistically significant difference in serum TLR4 level between group (Ia) (2.25±0.5) and other subgroups which shows more increase in serum level of TLR4 in Group IB (3.2-1.06) than Group IA. Also shows more increase in serum level of TLR4 in Group IC (4.0±2.0) than Group IA and IB In our study HCC group showed higher level of LPS with mean 4.5±1.26 however lower in HCV group with mean 2.9-1.0 and least in control group with mean 1.1±0.4 In our study there is statistically significant difference in serum LPS level between group (IA) with mean 3.0±0.5 and other subgroups which shows more increase in serum level of LPS in Group IB with mean 4.4-1.0 than Group IA. Also shows more increase in serum level of LPS in Group IC with mean 4.0±1.76 than Group IA and IB In our study there is statistically significant difference in serum LPS level between group (IIB) and group (IIA) which shows more increase in serum level of TLR4 in Group IIB with mean 2.7±1.1 than Group IIA with mean 2.20±0.2 In our study there is statistically insignificant difference of the mean value ± SD of sex as regard to LPS and TLR expression (t = 1.2, p = 0.22). (t = 0.16, p = 0.87) respectively.In our study there is statistically significant positive correlation between ALT, AST, Platelets, alpha fetoprotein and LPS as regard to TLR4 expression in group II more in IIB,C than IA . but insignificant of the mean value ± SD of other parameters. In our study there is statistically significant difference of the mean value ± SD of ALT, AST, Platelets, alpha fetoprotein and TLR4 as regard to LPS expression in group I more in IB, C than IA. but insignificant of the mean value ± SD of other parameters.In our study there is statistically significant difference of the mean value ± SD of ALT, AST, Platelets and TLR4 as regard to LPS expression in group II more in IIB than IIA. but insignificant of the mean value ± SD of other parameters. Conclusion: TLR4 and LPS measurement should be carried for all patient with HCV Who are at risk for HCC with close monitoring. Conduct a study on a Gut microbiota as therapeutic targets for HCC.

ABSTRACT Aim The purpose of this study was to evaluate the fracture strength of roots instrumented with the self-adjusting file (SAF; ReDent-Nova, Ra'anana, Israel) and the Reciproc reciprocating file and that were and were not obturated using the warm vertical lateral compaction technique. Materials and methods In total, 75 mandibular premolar teeth were sectioned at or below the cementoenamel junction to obtain roots 13 mm in length. The roots were balanced with respect to buccolingual and mesiodistal diameters and weight. They were distributed into four experimental groups and one control group (n = 15): No instrumentation (group I), instrumentation with SAF files but no obturation (group II), instrumentation with SAF files and obturated with warm vertical lateral compaction (group III), instrumentation with Reciproc File but no obturation (group IV), and instrumentation with Reciproc File and obturated with warm vertical lateral compaction (group V). AH Plus sealer (Dentsply DeTrey, Konstanz, Germany) was used along with gutta-percha points. One week later, a vertical load was applied to the specimen's canal until fracture occurred. Data were statistically analyzed using one-way analysis of variance (p = 0.05). Results The mean fracture load was 312.83 N for group I, 297.35 N for group II, 359.15 N for group III, 231.51 N for group IV, and 275.81 N for group V. Conclusion The fracture resistances exhibited a statistically significant difference between all the groups. Teeth instrumented by SAF exhibited a better fracture resistance. The purpose of this study was to assess the sternal osteoblastic activity and perfusion in the early period after a coronary artery bypass graft (CABG) using single-photon emission computed tomography (SPECT) and three-phasic dynamic scintigraphy (3PDS) with 99mTc methylene diphosphonate (MDP). Methods: The study group consisted of 57 male patients that were 57.3 ± 6.6 years of age. Thirty-six of them were randomized into two groups: in group I (n = 18), the internal thoracic artery (ITA) was skeletonized, and in group II (n = 18), the ITA was pedicled. All the patients in groups I and II underwent an off-pump CABG using 1.7 ± 0.3 grafts, including one anastomosis of the ITA to the left anterior descending coronary artery. The control group III (n = 21) consisted of patients that underwent mitral valve repair surgery, in whom the sternotomy without the ITA harvesting was performed. The 3PDS and SPECT of the thorax with 99mTc MDP were performed 2 weeks after surgery. Results: The sternal phosphates uptake in group I was approximately twice as high as in group II and approximately 1.5 times higher than in group III (p < 0.05). The MDP uptake asymmetry after the ITA skeletonization was the same as in the group with both intact ITAs. In contrast, after the pedicled ITA harvesting, the osteoblastic activity of the ipsilateral side of the sternum was lower than in the contralateral one. There was no statistically significant difference in scintillation count in the xiphoid process between groups I and II (p > 0.05); however, we observed a significant difference in the manubrium and body (p < 0.05). Conclusion: The main factor that improved the sternal perfusion after a CABG was the preservation of branches supplying the sternum using the skeletonization technique of ITA harvesting.

Aim: Pakistan has 6.9 million people with diabetes mellitus (DM) that will be doubled by 2025. A study was designed to determine serum levels of IL-6 in type 2 diabetes mellitus (T2DM) patients.Methods: It was a cross-sectional case-control study of 212 subjects. Group-I included 30 subjects without DM, Group-II had 30 T2DM without retinopathy and Group-III had 152 T2DM with retinopathy. IL-6 was determined by ELISA technique. Data was analysed using SPSS 17.0.Results: More females were in Group-II (83%) and Group-III (66%) compared to Group-I (30%). Higher age was in Group-II (49yrs) and Group-III (50yrs) compared to Group-I (34yrs). Mean duration of disease (in years) was more in Group-III (10.51) than Group-II (7.76). Highest mean level of IL-6 was in Group-II, followed by Group-I and Group-III. On comparison, gender, age, duration of disease and the level of IL-6, there was a significant difference while there was no significant difference between percentages of HbA1c. The logistic regression model suggested low levels of IL-6 in patients of diabetic retinopathy was an independent predictor of retinopathy in patients with T2DM.Conclusion: Serum level of IL-6 was low in patients of diabetic retinopathopathy as compared to patients with T2DM without retinopathy.Bangladesh Journal of Medical Science Vol.16(4) 2017 p.525-529

Disinfection of dental impressions is a compulsory procedure performed in order to break the chain of infection. Disinfectants used for this purpose are divided into two large groups. These are ready-to-use disinfectants applied directly over the surface of the impression in the form of a spray and concentrates, which are used to prepare a solution in which the impressions are immersed for a specific period of time. This study aims to evaluate the influence of disinfectants on spatial dimensions of alginate mass impressions. Ninety cubic impressions were prepared using metal matrixes with external dimensions of 28 x 28 mm. Each impression was measured in two planes. The impressions were divided into three equinumerous sets of 30: Group I was disinfected with Zeta 7 Spray (n = 30), Group II was disinfected with Zeta 7 Solution (n = 30), and the control Group III (n = 30). The dimensions of the samples were measured in two planes after 24 (T1) and 120 (T2) hours. The obtained data was analysed statistically. The dimensions of the samples after 24 and 120 hours in the first group were significantly different from those of the control group and the differences were statistically significant (p <0.05). There was no statistically significant difference between the second and the control group after 24 and 120 hours. Ready-to-use disinfectants influence the dimensional stability of alginate-mass impressions. The time between taking the impression and preparation of the cast should be considered while choosing the disinfecting agent.

Background: Aim: To assess the effect of exercise on body composition in obese and overweight. Methods: A total of seventy- two overweight subjects of either gender were enrolled for the study. Two groups were prepared. One group was experimental group (n=36) and the second group was control group (n=36). Parameter such as age, height and weight was recorded. On the basis of variables body mass Index and body fat percentage of every subject was determined. The experiment group were put on aerobic exercises spread over duration of four weeks. Results: The mean height in group I was 163.7 cm and 165.4 cm in group II. Weight was 65.2 kg and 63.5 kgs in group II, BMI (Kg/m2) was 29.4 and 29.1, body fat was 29.3% and 29.6% in group I and II respectively. A significant difference was observed (P< 0.05). The mean pre- test BMI was 29.6 and 38.5 and post- test value was 26.4 and 38.9 in group I and II respectively. A significant difference was observed (P< 0.05). Conclusions: Regular physical activity appears to confer a health benefit to the people. There was a positive relationship between aerobic exercises and overweight women in order to reduce the value of fat in the body.

Demineralization is a process of partial or full tooth mineral loss which caused by acidic environment, for example the side effect of extracoronal bleaching treatment. Demineralization increases enamel surface roughness which leads plaque accumulation. Whey extract and calcium phosphopeptide-amorphous calcium phophate (CPP-ACP) contain calcium and phosphate that can stop the demineralization through remineralization process. This study aimed to determine the differences of enamel surface roughness after whey extract and CPP-ACP application post- extracoronal bleaching. Experimental laboratory with pre- and post-test control group design was performed on 24 first maxillary premolars which devided into 3 groups. On group I, specimens were immersed in artificial saliva. Specimens in group II were immersed in whey extract meanwhile specimens were immersed in CPP-ACP on group III. Whey extract and CPP-ACP immersions were conducted 10 minutes every 12 hours for 15 days. The enamel surface roughness test was performed twice, after extracoronal bleaching treatment and after 15 days remineralization agent application. This study result indicated significant differences between group I and group II and between group I and group III (p<0.05), but there was no significant difference between group II and group III (p>0.05). This study showed whey extract decreased more enamel surface roughness than CPP-ACP but the difference was not significant statistically. So, whey extract and CPP-ACP showed similar remineralization potential.

Background: The aim of this study was to evaluate the clinical effects of deproteinization of the hypomineralized enamel and different cavity designs on the performance of the composite resin restorations(CRRs) placed into the cavities of MIH (molar incisor hypomineralization)-affected molars. Study design: 95 MIH-affected permanent first molars (PFMs) and 31 caries but not MIH-affected PFMs (126 teeth in total) were included in the study. The MIH-affected molars were divided into three groups. In Group I, all hypomineralized tissue was removed until healthy enamel was reached. In Group II, carious and cheesy hypomineralized tissue was removed until a reasonable resistance was detected in the hypomineralized tissue. In Group III, cavities designed as Group II, differently from this group deproteinization of the left hypomineralized tissue was performed prior to the placement of CRRs. Group IV served as the control group consisting of unaffected carious PFMs. Restorations were evaluated according to modified USPHS criteria for 24 months. Results: The retention rates were 93.7% for Group I, 80.7% for Group II, 93.5% for Group III and 100% for Group IV. The success rate for the restorations in Group II proved significantly lower (p&lt;0.05) than that of the other three groups. No significant difference in success rates was observed between Group I, Group III and Group IV (p&gt;0.05) at the end of 24 months. Conclusions: Failure of the restorations was predominant in the group that the hypomineralized tissue was left surrounding the cavities. Deproteinization of the hypomineralized enamel was found to enhance the retention rates of CRRs.

Objective. To determine whether short term supplemantation at two levels (1 or 2 g/kg) of clinoptilolite (CLNP) in the colostrum of dairy calves had any effect on total weight gain (TWG) and mean daily gain of weight (mdwg). Materials and methods. A total of 24 clinically healthy calves without diarrhea were assigned in a subset of three major groups of calves were studied: group I (n=8) received colostrum with clnp at the rate of 1/kg started immediately following calving at 12 and 24 hours, for a total of 2 doses. Group II consisted of calves (n=8) receiving colostrum with clnp at the rate of 2/kg within the same interval as the first experimental group. The last group III, which served as control, received solely colostrum. Results. Group had significant effect on weight (kg) of Holstein calves [twg (mean±SE)- control: 12.66±0.349, group I: 14.73±0.414, group II: 14.19±0.468, p<0.01; mdwg (mean±SE)-control: 0.338±0.0155, group I: 0.396±0.0189, group II: 0.397±0.0196, p<0.05]. There was a significant difference observed for twg (p<0.01) and mdwg (p<0.05) among control group and both clnp treatment groups. Conclusions. The results showed that the addition of both levels of clinoptilolite administration via colostrum appeared to enhance twg and mdwg in newborn calves without having any observable adverse effect.
