The first physiological response to effective sexual stimulation, produced by a source of physical or mental stimulation, is the erection of the penis. Erection usually occurs within 3-8 seconds of the onset of arousal. When sexual tension and erection reach a certain level in the presence of the partner, the need for interference appears. The intensity of the erection may increase or decrease until it disappears, whether the arousal is prolonged or not. The complicated anatomical apparatus is regulated by a nervous mechanism, just as complex, being dependent to a remarkable extent on psychic influences. By the strong interweaving of these two components - psychological and functional - the sexual function is subject to changes. Erectile dysfunction, when it occurs, is obvious because, although there may be libido, the lack of an erection makes it impossible to perform sexual intercourse. Erectile dysfunction should not be confused with isolated or occasional failures to obtain or maintain a penile erection. They do not constitute a condition or disease that justifies medical attention and should be perceived as absolutely normal. The vast majority of men face such an episode at some point in their lives. One can speak of erectile dysfunction in the case of a recurrent or persistent inability to obtain an erection or to maintain it long enough to complete sexual intercourse, which lasts at least three months. It is especially important because in a normal activity, without erection, intercourse, ejaculation and orgasm cannot take place. (Pathologically, premature ejaculation can occur, without an erection, in the form of pollution!)

The article presents the results of the treatment of 60 patients with premature ejaculation in combination with erectile dysfunction. Patients were divided into three groups, in which patients were prescribed non-drug treatment, or sildenafil 50 mg on demand, or sildenafil 50 mg on demand in combination with melatonin 3 mg at night. Duration of treatment was 6 months. The psychoemotional status, quality of life, quality and duration of intercourse, erectile function and chrono-rhythms of patients were assessed. The results of the study showed that treatment with sildenafil in combination with melatonin was the most clinically effective, which manifested in prolongation of sexual intercourse, improvement of the quality of erection and normalization of the chronobiological status of patients.

Priapism is an uncommon urological emergency that can lead to permanent impotence if prompt presentation and medical intervention is not performed. It is a breakdown of the usual physiological mechanisms controlling penile tumescence and detumescence, leading to a prolonged penile erection (>4 hours) that is unrelated to sexual stimulation. Currently, there are three accepted subtypes: ischaemic, non-ischaemic, and stuttering priapism, which is also known as recurrent ischemic priapism. The aim of treatment is the immediate resolution of the painful erection and the preservation of cavernosal smooth muscle function in order to prevent cavernosal fibrosis, which can lead to penile shortening and permanent erectile dysfunction.

Abstract: Erectile dysfunction is the disability of a male to maintain consistant or repeated penile erections sufficiently for successful sexual intercourse. Many efforts have been done to handle this condition, so far. The psychosocial therapy and several medications inter alia: testosterone, yohimbine, trazadone, direct intracavernosus injection of alprostadil, penile implantation, and  sildenafil citrate, are not always suitable for the patients’ needs, especiallly those who suffer from cardiovascular diseases. Therefore, a new erectogenic agent has been introduced, the sublingual apomorphine. This apomorphine has a dopamine-like molecule that acts on the dopamine receptors in the paraventricular nucleus (PVN) of the central nervous system to increase the penile erection due to sexual stimulation (erotic imagination, audiovisual, or tactile). The sublingual apomorphine has been proved effective in overcoming the erectile dysfunction, especially the mild and moderate forms. Keywords: penile erection, sublingual apomorphin.     Abstrak: Telah banyak upaya dilakukan untuk mengatasi disfungsi ereksi yang merupakan ketidakmampuan seorang laki-laki secara konsisten atau berulang untuk mencapai dan mempertahankan ereksi penis yang cukup untuk melakukan hubungan seksual. Terapi psikososial dan pengobatan yang pernah diajukan seperti pemberian hormon testosteron, yohimbin, trazadon, injeksi langsung alprostadil intrakavernosa, implantasi penis serta sildenafil sitrat tidaklah selalu sesuai dengan kebutuhan penderita, khususnya penderita disfungsi ereksi dengan penyakit kardiovaskuler. Para ahli berusaha melakukan terobosan baru salah satunya adalah apomorfin sublingual yang merupakan agen erektogenik baru. Apomorfin adalah suatu molekul mirip dopamin yang bekerja pada reseptor dopamin pada paraventricular nucleus (PVN) di sistem saraf pusat untuk meningkatkan rangsangan erektil saat stimulasi seksual (imaginasi erotik, audiovisual dan perabaan) terjadi. Apomorfin sublingual terbukti efektif untuk mengatasi disfungsi ereksi, terutama disfungsi ereksi ringan dan sedang. Kata kunci: ereksi penis, apomorfin sublingual.

Epilepsy is a chronic neurological disorder that is characterized by episodes of seizure. Sexual dysfunction has been reported in patients with seizure, which mostly manifests as erectile dysfunction and premature ejaculation in men. In this study, we report the case of a 65-year-old Chinese man with frequent spermatorrhea. Electroencephalography suggested local epilepsy in the left temporal lobe. After treatment with anti-epilepsy drugs, the symptoms disappeared and did not recur. To the best of our knowledge, this is the first reported case of epilepsy-induced spermatorrhea. The symptoms of spermatorrhea are probably a rare manifestation of seizure. When repetitive stereotyped symptoms occur, seizure should be considered, and tentative anti-epileptic treatment may be a good option.

It has been estimated that currently over 150 million men worldwide suffer from erectile dysfunction (ED) and by 2025, the figure will increase beyond 322 millions. ED is the inability to achieve, and/or maintain penile erection sufficient for satisfactory sexual intercourse, and was previously regarded as the part of aging. It is associated with certain diseases and life style habits with a cause-effect relationship, including diabetes mellitus, hypertension, dyslipidemia, and cigarette smoking. Internationally, most of the men with ED fail to pursue treatment due to the complex nature of sexuality, taboos, cultural restrictions, and acceptance of ED as a normal sequel of aging. In this review, we discussed the physiology, diagnosis, and risk factors associated with ED and current treatment strategies for ED.Keywords: Erectile dysfunction, Diabetes, Penile erection, Phosphodiesterase inhibitors.

Objective: Prevention may improve the quality of life and sexual and reproductive health. To improve prevention require a comprehensive research approach that examines the frequency and risk factors for urologic conditions. In June 2016 the Italian Urologic Society coordinated a preventive initiative : the 1st Week of Male Urologic Prevention ”#Controllati”. Material and methods: During the 1st Week of Male Urologic Prevention “#Controllati”, men aged 18 years or more were invited to attend participating urologic centers for a free of charge visit for counseling about urologic or andrologic conditions. Each participating man underwent a physical examination. Further he was asked about his a medical history and about his urologic symptoms, sexual activity and possible related problems. Results: Data were collected in 81 centers: 2380 men answered the questionnaire. A total of 1226 subjects participating in the study reported one or more urinary symptom [51.5% (IC 95% 48.9%-54.5%)]. The risk of any urinary symptoms increased with age: in comparison with men aged < = 30 years or less the risk of any urinary symptoms was 2.31, 2.92, 5.12, 7.82 and 17.02 respectively in the class age 31-40, 41-50, 51-60, 61-70 and > = 71. Overweight/obese men were at increased risk of any urinary symptoms [OR1.35 (95% CI 1.12-1.64)]. 27.2% (IC 95% overall 25.2% -29.3%) of the subjects had at least a sexual disorder (erectile dysfunction, premature ejaculation, hypoactive sexual desire). The erectile dysfunction and hypoactive sexual desire increased with age, but premature ejaculation tended to be higher among younger aged men aged 40 years or more. Current any urinary symptoms [OR 1.85 (CI 1.40-2.43)], hypertension [OR 1.66 (95% CI 1.21-2.26) and diabetes (OR 2.37 (95% CI 1.45-3.88)] increased the risk of erectile dysfunction. Conclusions: This large survey gives a picture of the burden of the more frequent urologic conditions offering useful information in order to focus preventive campaign.

Abstract: Normal sexual function is a bio-psycho-social process which is significantly related to the quality of life (QOL). Any disturbance in one of the components can be a disaster in sexual life.  Erectile dysfunction is a disability to get an erection or to maintain the erection well enough for a sexual intercourse, which persists or recurs at least three months conse-cutively due to either psychogenic or organic disturbances, or both. Some psychogenic dis-turbances especially depression, anxiety, and relational distress, play some important roles in ED. Besides that, aging, low testoteron level, physical diseases, and some certain medicines have to be included. Key words: erection, erectile dysfunction, sexual stimulus, depression.     Abstract: Fungsi seksual yang normal merupakan suatu proses biopsikososial yang berkaitan langsung dengan kualitas hidup. Adanya gangguan pada salah satu komponen dapat menjadi malapetaka bagi kehidupan seksual. Disfungsi ereksi dapat dedefinisikan sebagai suatu keti-dakmampuan untuk ereksi atau mempertahankan ereksi yang cukup untuk melakukan hu-bungan seksual yang memuaskan, yang menetap atau berulang paling tidak selama tiga bulan berturut-turut. Gangguan psikogenik khususnya sindroma depresi, ankietas, dan distres relasional berperan penting dalam hal terjadinya disfungsi ereksi. Disamping itu usia lanjut, rendahnya kadar testoteron, penyakit fisik, dan beberapa jenis obat perlu diperhitungkan. Kata kunci: fisiologi ereksi, disfungsi ereksi, stimulus seksual, depresi.

The purpose of this study was to determine the prevalence and factors associated with of Premature Ejaculation &Erectile Dysfunctionamong Malaysian men. A cross-sectional study was conducted in urban and rural areas using standardized and validated self-administered questionnaires. A total of 319 samples were recruited for PE screening. The overall prevalence of PE was 31.7% with 20.7% and 40.8% for urban and rural area respectively. Depression, anxiety and frequent masturbation were significantly associated with PE. While For ED, infrequent sexual intercourse was the sole factor significantly associated with ED.PE and ED were highly prevalent in both urban and rural areas; with several significant preventable associated factors.
