In Ayurveda, the disease sinusitis can be co-related with “Dushta Pratishyaya” which arises as a complication of the disease “Pratishyaya”. A detailed description of Pratishyaya has been mentioned in Sushruta Samhita (Su. Ut 24/4). While explaining the complication of Pratishyaya, Dushta Pratishyaya has been mentioned. The features of the disease Dushta Pratishyaya, in general, is similar to that of Chronic Sinusitis in modern science. In modern medicine, management of Sinusitis includes Antibiotics, analgesics, Nasal steroids, Nasal Irrigation, Surgery etc. Even then there is no complete cure for Chronic Sinusitis through modern management. Hence to find out a solution through Ayurveda, Agnikumara Ras has been selected for the present study. Agni kumara Ras has been selected from the text Basavarajeeyam. It consists of five ingredients- Shuddha Vatsanabha, Maricha, Kushta, Mustha and Vacha. Keywords: Dushta Pratishyaya, Agnikumara Ras, Clinical evaluation

Difficulty in breathing or shortness of breath may be simply termed as Shwasa (Asthma), As per Ayurveda, Shwasa is mainly caused by the Vata and Kapha Doshas. Shwasa is broadly classified into five types in Maha Shwasa (Dyspnoea major), Urdhawa Shwasa (Expiratory Dyspnoea), Chinna Shwasa (Chyne-stroke respiration), Kshudra Shwasa (Dyspnoea minor), Tamaka Shwasa (Bronchial Asthma). In modern science Tamaka Shwasa can be correlated with Asthma, Asthma which is a chronic inflammatory disease of airway. In modern medicine there is no cure for Asthma, symptoms can typically be improved. In Ayurveda, Asthma can be effectively and safely manage the condition without inducing any drug dependency where Pachakarma procedures and use of internal medication detoxifies the body, provides nutrition and increases the elasticity of lung tissue it also develops natural immunity of the body thus decreasing episodic recurrence of the disease.

As the search for effective treatment for Covid-19 intensifies, traditional medicine systems are receiving increasing attention from researchers as well as the public. While scientific rigour is non-negotiable, there remain fundamental issues to be addressed when bringing evidence from traditional systems. Here we examine some of these issues pertaining to Ayurveda and the underlying philosophical underpinnings, and suggest potential ways to move forward. We find an ability to emerge from the cage of “biomedicalism” and its foundational reductionism essential for appropriate research in Ayurveda. We caution against pursuing research in Ayurveda by just mimicking modern medicine and highlight the need for appropriate use of modern science tools and methods to understand Ayurveda and explore its potential for healthcare. We emphasise the need and potential for transdisciplinary research in Ayurveda. A balance between evidence-based medicine and evidence-informed healthcare is required.

AbstractObjective:To investigate the impact of nasal irrigation with isotonic or hypertonic sodium chloride solution on mucociliary clearance time in patients with allergic rhinitis, acute sinusitis and chronic sinusitis.Patients and methods:Mucociliary clearance time was measured using the saccharine clearance test on 132 adults before and after 10 days' application of intranasal isotonic or hypertonic saline. Patient numbers were as follows: controls, 45; allergic rhinitis, 21; acute sinusitis, 24; and chronic sinusitis, 42. The results before and after irrigation were compared using the Wilcoxon t-test.Results:Before application of saline solutions, mucociliary clearance times in the three patient treatment groups were found to be significantly delayed, compared with the control group. Irrigation with hypertonic saline restored impaired mucociliary clearance in chronic sinusitis patients (p < 0.05), while isotonic saline improved mucociliary clearance times significantly in allergic rhinitis and acute sinusitis patients (p < 0.05).Conclusion:Nasal irrigation with isotonic or hypertonic saline can improve mucociliary clearance time in various nasal pathologies. However, these solutions should be selectively prescribed rather than used based on anecdotal evidence. Further studies should be conducted to develop a protocol for standardised use of saline solution irrigation in various nasal pathologies.

Functional endoscopic sinus surgery (FESS) for chronic sinusitis has become popular as a substitution for radical operations. We selected 24 patients with diffuse polypoid rhinosinopathy who had been admitted for FESS and followed up for at least 6 months. The recovery of 39 maxillary sinuses was inspected with endoscopy through the enlarged middle meatal antrostomies. We found that of 8 sinuses recovered, 19 still had diffuse polypoid change, and another 12 had focal polypoid sinuses. We conclude that for diffuse polypoid rhinosinopathy, management of maxillary polypoid mucosa is complicated, and recovery of polypoid mucosa is dependent not only on the patency of the sinus ostia, but also on other factors such as alterations of mucosal, glandular, and epithelial changes by chronic stimuli.

Raktapradar in Ayurveda is characterized by excessive or prolonged menstruation with or without intermenstrual bleeding, which is one of the most common bleeding disorders in women. Excessive bleeding from uterus either at the time of menses or in intermenstrual time is considered as Asrigdara or Raktapradar in Ayurveda. Normal menstrual bleeding including ovulation or more specifically the organized sequence of endocrine signals that characterizes the ovulatory cycle, menses regularities, predictability & consistency. It is most basic concept that control the endometrial cycle, the volume & the duration of menstrual flow. Cyclic regular menstrual bleeding which is excessive in amount & duration considered as Menorrhagia. Raktapradar can be correlated with menorrhagia. As per modern science, menorrhagia is defined as cyclic regular bleeding which is excessive in amount (>80ml) or duration (>7 days) or both. It is considered as one of the commonest leading gynecological problem. In modern medicine haemostatic, analgesic and hormonal therapies are advised for Menorrhagia, which includes hormonal therapy, antiprostaglandins & antifibrinolytic agents. These have not proven their definitive efficacy in spite of high costs; their side effects have led to hormonal imbalances hence it is need of time to have an integrated and comprehensive therapeutic intervention in Ayurveda to prevent recurrence& would overcome the modern medicine limitations. Many herbal & herbo-mineral preparations, Shodhan & Shaman Chikits as per Rugnabal are mentioned in Ayurveda to cure Raktapradar and related symptoms which can be used as per Anubandha Dosha and Lakshana.

AbstractObjective:Post-irradiation rhinosinusitis is one of the most common untoward side effects in patients with nasopharyngeal carcinoma. This study aimed to evaluate the effect of fluticasone propionate aqueous nasal spray on post-irradiation rhinosinusitis.Methods:Nasopharyngeal carcinoma patients who had undergone radiotherapy and subsequently developed chronic rhinosinusitis were randomised to receive either fluticasone propionate aqueous nasal spray 200 µg plus nasal irrigation or a single nasal irrigation, for six months. A questionnaire, nasal endoscopy and computed tomography were used to evaluate rhinosinusitis severity, at the beginning of treatment, and at three and six months after treatment.Results:The group who received fluticasone propionate aqueous nasal spray combined with irrigation had fewer nasal complaints (overall symptoms, blocked nose and headache were reduced), a better quality of life and less severe endoscopic findings than those who only received nasal irrigation at three and six months after treatment.Conclusion:Nasal steroids are a safe and effective therapy for patients with post-irradiation rhinosinusitis.

Infertility due to anovulatory cycles over recent years, has emerged as one of the most common health issues that many young women have been facing. Sedentary lifestyles with minimum physical activity, rising stress level and irregular sleep pattern are few of the reasons, according to many clinicians and health experts. PCOD is a common Endocrine disturbance affecting females of age group between 15-30 years. The disorder accounts for delayed periods, Infertility, obesity hirsutism and acne. According to modern science exact cause of disease is unknown. In Ayurveda all diseases of female reproductive system are classified under Yonivyapada, according to Ashtanga Hridayam 20 diseases of Vagina arises because of faulty food habits. PCOD is a kapha predominant disorder; kapha gets aggravated by consuming more sleshmika and sneha containing food items and by lethargic lifestyle. The line of treatment in modern medicine is Harmonal therapy. In Ayurveda the line of treatment is according to dosha predominance, here it is kaphahara, Strotoshodhaka and Anulomana. So, in the present study – Classical Virechana, Pushpadhanwa rasa, KanchanaraGugglu, Triphalvati, are selected. The result is outstanding, and symptoms subsided. Further clinical trials can be conducted to prove the efficacy of the drugs statistically.
