Fistula in ano is an abnormal communication between anal canal or rectum and the perianal skin which mostly follows previous ano-rectal abscess. Chief complaints include intermittent discharge, history of pre-vious pain, swelling and recurrent abscess that ruptured spontaneously or was drained surgically. Various surgical and para-surgical methods are available now a days to treat this disease. In the vast literature of Ayurveda same symptoms are described as Bhagandara. At first it appears as Pidika around Guda and when it bursts out, it is called as Bhagandara1. Acharya Sushruta has counted this disease in Ashta Ma-hagada suggesting its difficulty to treat2. There are many options available to treat fistula in ano ranging from seton application, fistulotomy, fistulectomy, fibrin glues, LIFT technique, advancement flaps etc. Acharyas has used Agni karma, Kshara karma, Shastra karma and Aushadha karma in Bhagndara. But in this case Kshara Sootra application was done to treat Bhagandara. Application of Kshara Sootra not only lessen rate of recurrence to a significant level but also free from almost all major post-operative complica-tion.

A 3 days old child presented with chief complaints of bilious vomiting and abdominal distention since few hours duration. Prenatal history revealed mother to be polyhydraminos and pregnancy induced hypertensive was on treatment with labetolol. Baby was a term, emergency lscs delivery. On examination baby was active, alert and with fair hydration. On per abdominal examination abdomen was distended and dilated bowel loops were seen. On per rectal wash pale jelly stools were expelled. Baby was taken up for emergency laprotomy and found to be having illeal atresia for which resection and anastamosis is done. Post operatively baby was doing well with satisfactory weight gain. Intestinal atresias are the major cause of intestinal obstruction in cases of neonates. They may be illeal or duodenal atresia. It is hypothesized to be occurring due intrauterine vascular assault and failure of recanalization. Babies present with vomiting and abdominal distention with mother having a positive history of polyhydraminos. On examination there will be abdominal distention with dilated bowel loops. X-rays would show dilated bowel loops and ultrasound shows decreased peristalisis in the bowel loops. Emergency laprotomy and surgical resection is the treatment of choice.

Abstract Acquired benign tracheoesophageal fistula (TOF) is a rare medical condition that usually results from trauma, foreign bodies or granulomatous infections. This is an unusual presentation of a male patient with a history of laryngectomy who has had over a period of several years inappropriately and vigorously used valve cleaning brushes to clean tracheal secretions, which has led to the formation of a TOF. Due to the patient’s obsessive habit, we could not manage him using conventional surgical methods. Instead, we opted for the placement of a salivary bypass tube, which yielded good results and recovery. To the best of our knowledge, no other case of similar aetiology has been published. We would like to highlight the importance of appropriate patient selection and education prior to performing a tracheoesophageal puncture to avoid developing life-threatening complications as demonstrated in our case report.

AbstractBackgroundAn astroblastoma is a rare primary glial tumour occurring preferentially in young adults. It is characterised by a perivascular arrangement of tumour cells forming perivascular pseudorosettes mimicking ependymomas. The histogenesis of astroblastoma is unclear.Case descriptionWe present the history of a 13-year-old girl with chief complaints of headache associated with vomiting, blurring of vision on the left eye and a history of diplopia on the right eye. She underwent left parietal parasagittal craniotomy and near-total excision of tumour. She was planned for postoperative radiotherapy 5,940 cGy in 28 fractions along with concurrent temozolamide100 mg. She had no neurological deficit or complaints during her last visit.ConclusionAstroblastomas are a distinct clinic pathologic entity, with well-described radiologic, pathologic and cytogenetic features. Its recurrence is high, and efforts must be made to elucidate the role and usefulness of radiotherapy and chemotherapy in these tumours.

Vestibular Neuronitis is a neuronal condition characterized by spontaneous and unilateral vestibular loss suddenly with preserved hearing and no signs of brain stem dysfunction. According to large population studies, it is the sixth cause of vertigo with an incidence of 8%. Here is the case report of VN which was observed in a super speciality hospital. A 42 years old male patient was admitted in a private super speciality hospital with the chief complaints of history of vomiting 4 episodes, neck pain, mild headache and gaint imbalance since 5 days. Complete analysis of diagnostic reports was done and appropriate treatment given. This case emphasizes the importance of periodic evaluation and reporting of vestibular neuritis cases accumulating clinical knowledge. In this particular type of cases it is difficult to identify the etiology and pathophysiology, hence it is vitally important to conduct such studies.

Abstract Aim The purpose of this article is to report a rare case of primary tuberculosis of the tongue. Background Tuberculosis is an infectious, chronic granulomatous disease that can involve almost any organ in the body, but primary lesions are usually confined to the lungs. Oral lesions are an infrequent occurrence in tuberculosis, and tuberculosis of the oral cavity is often a consequence of active pulmonary tuberculosis. Although primary tuberculosis in the oral cavity has been documented, it is a rare occurrence. Report A 42-year-old male patient presented with chief complaints of severe pain and ulceration on the ventrolateral surface of the left side of the tongue, measuring about 1.25×1.50 cm with surrounding erythema and induration of one-month duration. The ulcer was initially painless but became painful later with increased severity over time. The sore tongue caused difficulty in eating, drinking, swallowing, and even talking. The patient also complained of malaise for some duration, but there was no history of fever, cough, weight loss, and his bladder-bowel habits were normal. An excisional biopsy differentiated the lesion from squamous cell carcinoma and confirmed the diagnosis. Summary Even though primary tuberculosis in the oral cavity is a rare finding, it must be included in the differential diagnosis of mucosal lesions. This case provides a clinical example of the importance of this inclusion for the well being of the patient and for the community due to the communicable nature of the disease. Osseous endometrium is a rare clinical entity with the presence of bone in the endometrium. Almost all cases in literature have been reported as presenting with secondary infertility often having a prior history of a 1st or 2nd trimester abortion. This interesting case was 38 years age, presented with menorrhagia for few months only, had her last child birth 12 years ago by caeasarean delivery, her menstrual problem subsided with successful hysteroscopic removal of the bony chips. Although rare, the diagnosis is suspected on ultrasonography and can be managed easily with hysteroscopy which can visualize and simultaneously remove the bony spicules.

Introduction Rhinolith is a calcic deposition formed by mineralization of salts in an impacted nasal foreign body. It is a benign condition that can be troublesome owing to its size and extent of impact. Case Report A 35-year-old man with a history of right nasal obstruction with occasional foul-smelling discharge and right eye pain since ten years was diagnosed with rhinolith. During its removal, it was three times bigger than what could be visualized in nasal endoscopy. Conclusion Proper history, examination, and a high suspicion can clinch the diagnosis in almost all cases. A rigid diagnostic nasendoscopy is an important tool in the diagnosis. The treatment of choice is endoscopic removal under local or general anaesthesia.

AbstractCataract surgery is the most frequently performed ophthalmic surgery worldwide. This year approximately 32 million surgeries will be performed. The journey to modern, quick, and safe cataract surgery has been quite long. This review covers topics from ancient couching to the most modern phacoemulsification and femtosecond laser-assisted cataract surgery. The gain in quality of life is the largest with cataract surgery compared to other implant surgeries (e.g. knee, hip replacement, etc.). Ophthalmology has made huge advancements in recent decades. New microsurgical tools and diagnostic equipment have been developed, together with new surgical methods and foldable intraocular lenses, made from biocompatible material. From monofocal lenses, through aspheric, toric, and multifocal lenses, to multifocal toric lenses, today almost all kinds of refractive error can be compensated, including presbyopia. Teamwork, precise preoperative assessment, and fine surgical technique should also be emphasized in order to achieve the best and most predictable postoperative results for both patient and surgeon.

Background: A ventricular septal defect is a defect of the ventricular septal wall where the abnormal opening is present in between the two ventricles. Eisenmenger syndrome is a condition it may develop due to unrepaired heart defect and complication of unrepaired heart defect who is born congenitally. The congenital heart defects lead to Eisenmenger syndrome it developed due to abnormal blood circulate throughout the heart and lungs. Case Report: The 23-year-old female was admitted to AVBRH with the chief complaints of a history of amenorrhoea since 8 months, complaints of breathlessness during exertion since the 7th month of pregnancy and she was referred to AVBRH from Amravati on date 27/1/2021. At the age of 14 years, she had a history of cyanosis at the time of fever and had recurrent episodes of fever for 2-3 yrs, lasting for 2-3 weeks. She is primigravida and had undergone LSCS on date 3/2/2021. She delivered a male child on date 3/2/2021 of birth weight 1.9kg at 12:15 pm. Discussion: Patient with pulmonary hypertension and Eisenmenger syndrome where the surgery is contraindicated because in Eisenmenger syndrome surgery is life-threatening. Only medical management is available to treat Eisenmenger syndrome. Conclusion: Ventricular septal defect in adult females where the pregnancy is contraindicated. Epidural anesthesia should be preferred while the patient undergoes the lower segment cesarean section. This case report concludes that in this condition emergency LSCS is the first choice of treatment and oxygen therapy.

Exogenous ochronosis is an infrequent skin disorder characterised by bluish-black or grayish brown pigmentation on dermis. It is the most common condition caused due to long term application of Hydroquinone skin preparations for melasma, skin brightening, cholasma, acne induced pigmentation etc. This report refers to a case of 39-year-old female patient who presented to the hospital with chief complaints of progressive formation of dark lesions over face, neck since one and half-year. She had history of usage of hydroquinone (4%) cream for skin brightening for a period of three months. Based on clinical findings and history she was diagnosed to have acquired exogenous ochronosis and was treated with microdermabrasion, cosmelan peel, yellow peel, glutathione tablets, topical sunscreen and kojic acid cream. Patient noted 50% improvement in her condition after 4 months of treatment. It is believed that application of high concentrations of hydroquinone for a prolonged period causes exogenous ochronosis. This case report suggests that exogenous ochronosis can occur after three months of application.
