The article is devoted to a rare disease of the perianal region — Buschke-Levenshtein tumor. A clinical case and a literature review on the etiology, pathogenesis and experience of treating such tumors are presented. Evidence based sources show that currently there is no generally accepted tactics of managing patients with this pathology. In this regard, each case of treatment of such patients is of not only scientific but also practical interest. In this case, the tactics of staged treatment with the use of high-temperature dissectors and immunocorrection was chosen, which made it possible to achieve a good clinical result without the formation of extensive and long-term non-healing wound defects in the perianal region.

Malakoplakia is a rare chronic granulomatous inflammatory condition, commonly presenting in immunocompromised patients. It most commonly occurs in urinary tract. The cutaneous form is less frequent. Here, we report a case of cutaneous malakoplakia involving perianal skin in an immunocompetent 61-year old female diagnosed on histopathology utilizing special stains. To the best of authors’ knowledge, this is the 8case report of cutaneaous malakoplakia of perianal skin in the English literature and 3in the Indian literature.

Arsha is a gift of modern diets and busy lifestyles & many people are suffering from some sort of Anorectal disorder. It may be simple constipation to complex carcinoma, in which prominent disorder is Arsha. Derangement of Jatharagni leading to vitiation of Tridosha, mainly vata dosha these vitiated Doshas get localized in Gudavali and Pradhana dhamani which further vitiates Twak, Mamsa, and Meda Dhatus leads to development of Arsha.  The perianal skin is most pain sensitive, due to rich nerve endings. Hence even a mild form of disorder can produce great discomfort to the patient. On the basis of observation and statistics we got positive response for this parameters Malabaddhata 23 (71.88%), Abhishyandiahar 23 (71.88%), Guruahar 27 (84.38%), Diwaswap 18 (56.25%), Adhyashana 27 (84.38%), Prushtayana 29 (90.63%), Vegavidharana 32 (100%), Shavathu 24 (75%), Mamsashotha 19 (59.38%), Siragranthi 26 (81.25%), Amlika 21 (65.62%), Paridaha 18 (56.25%), Atop 28 (81.25%), Antrakujan 22 (68.76%), Gudaparikartan 32 (100%). In histopathology report we found fibromuscular tissue, stratified squamous epithelium, dilated and congested blood vessels which is the moolsthana of mamsavaha strotas. That means mamsavaha strotas dusti is co related with bahya arsha. This shows that there is significant correlation of origin of Bahya Arsha from Mamsavaha strotas dushti. Resumen El estreñimiento crónico es un problema frecuente en ancianos por ello se recurre al uso de laxantes para su resolución. En nuestra imagen presentamos una de las complicaciones poco frecuentes que pueden surgir tras el uso de enemas rectales que presentan fósforo en su composición y su tratamiento. Se inició tratamiento conservador local, presentando mala evolución, por lo que se decidió desbridamiento quirúrgico y colostomía. El postoperatorio evolucionó favorablemente sin presentar complicaciones. Se ha descrito en la literatura la necrosis directa tras la aplicación de enemas en pacientes con deshidratación o insuficiencia renal.

Perianal Paget disease (PPD) is a rare neoplastic condition defined by the presence of atypical Paget cells in the perianal skin, the aetiology of which remains largely unknown. It can be divided in primary forms, arising as an intraepithelial disease or manifestation of an underlying skin adenocarcinoma or secondary forms resulting from epidermotropic spread or metastasis of a concealed carcinoma. Indeed, because of its rarity, clear options regarding the treatment of these patients are yet to be clarified. A high level of suspicion is needed whenever dealing with any unhealed perianal skin lesions and, therefore, the need for close long-term follow-up must be highlighted. Herein, two cases of PPD, one primary and another secondary, treated at the same institution, are presented in an attempt to document the involved complexity and to bring further insight into the understanding of this entity.
