The clinical picture of lobular panniculitis associated with damage to the pancreas can vary widely, accompanied by damage to the joints and internal organs, which complicates the diagnosis of the disease. A clinical observation of a torpid course of lobular panniculitis with total damage to organs and systems is presented, which clearly illustrates the difficulties of differential diagnosis and therapy that arise in real therapeutic practice.

The objective of our study was to interpret and discuss atypical multiple myeloma case. The article describes the case of clinical observation of a patient K, in which manifestations of chronic kidney disease and circulatory failure prevailed in clinical picture of the disease. The authors recommended an X-ray examination of skull and pelvic bones as a screening method suitable for elderly people with symptoms of chronic renal insufficiency and chronic bone and muscle pain resistant to treatment.

In the article there is considered the clinical observation with a rare mechanism of human infection with rabies virus from a home decorative dog through the salivation of the mucous membranes and throat while caring for a dying animal. The diagnosis of rabies in the dead dog was established retrospectively after its exhumation and special studies. There is analyzed the dynamics of the clinical picture of rabies in human for the purpose of differential diagnosis and substantiation of clinical diagnosis. There is presented a brief review of the literature.

Cases of thallium poisonings are regularly reported in media. In the present work, the world experience in diagnostics and treatment of victims with thallium poisoning has been compiled and summarized, search for clinical guidelines for the diagnostics, treatment and rehabilitation of patients has been carried out. The toxicokinetics and toxicodynamics, as well as pathophysiological mechanisms of thallium influence on the body are described in detail. Toxic and lethal concentrations of poison in biological media, as well as its tropicity to various tissues and body systems were determined. The clinical picture depending on the timing of poisoning and the dose of poison is described in detail. Difficulties of the differential diagnosis are defined. The most interesting cases of criminal and household poisonings with various terms of establishment of the diagnosis, doses of poisoning and outcomes are presented.

A clinical case of observation of a 3-year-old child with a newly diagnosed optic nerve coloboma and multiple malformations is considered. The clinical picture and diagnostic methods necessary for the diagnosis are reflected. On the example of this child, the differential diagnosis with Charge syndrome is considered. After reviewing the literature on breath holding spells, it seemed desirable to determine whether any electroencephalographic abnormality is revealed by recordings during sleep in such cases. One hundred twenty-nine children between the ages of 7 months and 11½ years with breath holding spells were examined electroencephalographically while awake and during sleep. Seventy-seven of these children were boys and 52 were girls. All these children but one had normal electroencephalograms. One tracing taken during a breath holding spell is reproduced which shows slow wave but no seizure activity. The clinical diagnosis is discussed and it is concluded that the electroencephalogram in combination with the history and clinical observation is of value in the differential diagnosis between breath holding spells and convulsive disorders.

The peculiarities of the clinical picture and diagnosis of uncoronarogenic lesions of the myocardium in a period of 5 years are studied. The differential diagnosis of cardiomyopathies and myocardial dystrophies is performed, emphasizing the informativity of contrast and transesophageal echocardiography. Registration of electrocardiographic and echocardiographic data allows to choose among patients the persons demanding the expanding cardiologic examination that provides early diagnosis of latent forms of cardiomyopathies.
