Background: COVID-19 has emerged as a global health pandemic emergency with a massive effect on public health globally. In addition to its effects on respiratory tract, COVID-19 disease had been linked to many gastrointestinal symptoms. Rarely, it has been associated with intestinal ischemia due to the resultant hypercoagulable state. Case Summary: We report a case of a young lady previously healthy, who presented with severe abdominal pain associated with tenesmus and mucoïdbloody diarrhea, diagnosed to have ischemic colitis post covid19 infection. One of the port of entry of coronavirus 2 (SARS-CoV-2), to cause infection, is via an angiotensin-converting enzyme2 (ACE2). View that these ACE 2 receptors are highly expressed at the level of gastrointestinal tract, a variety of symptoms will occur. Recently, coagulopathy due to COVID-19 has emerged as a major component of the disease. The resultant ischemia has been reported to be associated with a hypercoagulable state. Lately, few cases were described worldwide of intestinal ischemia due to COVID-19 infection, mainly occuring in patients presenting with severe respiratory illness. Our patient did not complain of severe COVID-19 infection in the context of dyspnea or respiratory distress and did not have any features of shock requiring a vasopressor therapy. Treatment for ischemic colitis mainly involves supportive care with bowel rest. Conclusion: New datas concerning covid 19 infection are emerging. Ischemic colitis is a new aspect of manifestation of the infection, which can take place without necessarily being linked to a severe infection with covid 19 as in our case.

Following the 2009 H1N1 pandemic and ongoing sporadic avian-to-human transmission of H5N1 viruses, an emphasis has been placed on better understanding the determinants and pathogenesis of severe influenza infections. Much of the current literature has focused on viral genetics and its impact on host immunity as well as novel risk factors for severe infection (particularly within the H1N1 pandemic). An understanding of the host genetic determinants of susceptibility and severe respiratory illness, however, is currently lacking. By better defining the role of genetic variability in influenza infection and identifying key polymorphisms that impair the host immune response or correlate with protection, we will be able to better identify at-risk populations and new targets for therapeutic interventions and vaccines. This paper will summarize known immunogenetic factors associated with susceptibility or severity of both pH1N1 and H5N1 infections and will also identify genetic pathways and polymorphisms of high relevance for future study.

Ischemic colitis represents the most frequent form of intestinal ischemia occurring when there is an acute impairment or chronic reduction in the colonic blood supply, resulting in mucosal ulceration, inflammation, hemorrhage and ischemic necrosis of variable severity. The clinical presentation is variable and nonspecific, so it is often misdiagnosed. The most common etiology is hypoperfusion, almost always associated with generalized atherosclerotic disease. The severity ranges from localized and transient ischemia to transmural necrosis of the bowel wall, becoming a surgical emergency, with significant associated morbidity and mortality. The diagnosis is based on clinical, laboratory suspicion and radiological, endoscopic and histopathological findings. Among the radiological tests, enhanced-CT is the diagnostic investigation of choice. It allows us to make the diagnosis in an appropriate clinical setting, and to define the entity of the ischemia. MR may be adopted in the follow-up in patients with iodine allergy or renal dysfunctions, or younger patients who should avoid radiological exposure. In the majority of cases, supportive therapy is the only required treatment. In this article we review the pathophysiology and the imaging findings of ischemic colitis.

Testing for the SARS-CoV-2 infection is critical for tracking the spread of the virus and controlling the transmission dynamics. In the early phase of the pandemic in Italy, the decentralized healthcare system allowed regions to adopt different testing strategies. The objective of this paper is to assess the impact of the extensive testing of symptomatic individuals and their contacts on the number of hospitalizations against a more stringent testing strategy limited to suspected cases with severe respiratory illness and an epidemiological link to a COVID-19 case. A Poisson regression modelling approach was adopted. In the first model developed, the cumulative daily number of positive cases and a temporal trend were considered as explanatory variables. In the second, the cumulative daily number of swabs was further added. The explanatory variable, given by the number of swabs over time, explained most of the observed differences in the number of hospitalizations between the two strategies. The percentage of the expected error dropped from 70% of the first, simpler model to 15%. Increasing testing to detect and isolate infected individuals in the early phase of an outbreak improves the capability to reduce the spread of serious infections, lessening the burden of hospitals.

The SARS-CoV-2 virus was first detected in Wuhan, China in December 2019 and was known to produce acute severe respiratory illness in humans which rapidly spread almost throughout the world within a few months. This human coronavirus has seven strains and they commonly produce illness in the nervous system, respiratory system and hepato- intestinal systems. This present review is an attempt to illustrate recent reports pertaining to the management of SARS-CoV-2. Further, it also highlights the diagnosis and clinical management of COVID-19. Various search engines like Scopus, Pubmed and WHO databases were accessed and literature on current advances about COVID-19 including structural features, replication, possible pathogenic, symptoms, diagnosis, prognosis, methods of prevention and possible therapeutic agents used for treatment of patients was reviewed. Current studies indicate that COVID-19 is very infectious with droplet transmission potential. The key modalities to prevent the infection is by keeping social distancing, respiratory/hand hygiene, detection of infection and subsequent quarantine of the infected persons. Presently, either no vaccine for prevention or specific treatments available, however, COVID-19 patients may be managed by using some repositioned drugs and symptomatic treatment.

A 62-year-old white female with a history of early-stage triple-negative breast cancer on a combination of carboplatin and paclitaxel in the adjuvant setting presented with lower gastrointestinal bleeding. She tolerated 4 cycles of dose-dense adriamycin/cyclophosphamide with no major symptoms. After 6 cycles of weekly paclitaxel in combination with carboplatin every 3 weeks, she presented with diarrhea and lower gastrointestinal bleeding. Colonosopic examination showed erythema and inflammation in the splenic flexure, descending colon, and sigmoid colon consistent with ischemic colitis. Pathology favored the same diagnosis. She was treated conservatively with intravenous fluids and bowel rest. Chemotherapy was held for 2 weeks and resumed after recovery without carboplatin. She was able to tolerate the remaining 6 cycles of paclitaxel with no recurrence of her symptoms.

Chapter 6 explores the narratives of people who, due to vulnerabilities associated with their health status, including severe respiratory illness and HIV-positive serostatus, and because of coincident pregnancy, had to respond to the pandemic to protect themselves and unborn children. This chapter, therefore, addresses the importance of biography for understanding the social impact of pandemics. It shows how pandemics as historical events intersect with biographies and, from the point of view of individuals, cannot be meaningfully separated. This temporal intersectionality of pandemics and lived experiences is particularly well illuminated by a narrative approach. A feature of this chapter, too, is a focus on invisibility, that is, the ways in which being at risk was invisible to the “healthy” majority.
