Abstract Introduction A chalazion is a common non-effective granuloma of the meibomian glands of eyelids. They are commonly found on the tarsal conjunctival surface and the eyelid margins. It may be ignored by the patients until it reaches a considerable size. Recently the use of intralesional corticosteroids has shown promising results. We in the present study tried to evaluate the outcomes of intralesional triamcinolone acetonide injection in the management of Chalazion. Methods The current study was carried on Patients with chalazion attending Ophthalmology OPD of Rajiv Gandhi Institute of Medical Sciences, [RIMS], Adilabad. Successive patients with small multiple marginal chalazia were included. Patients were selected based on the amenability of treatment with intralesional triamcinolone acetonide injections. It may be ignored by the patients until it reaches a considerable size. Recently the use of intralesional corticosteroids has shown promising results. We in the present study tried to evaluate the outcomes of intralesional triamcinolone acetonide injection in the management of Chalazion. Results Group I with very small-sized chalazia< 5 mm out of n=22 included in Group I resolution after one week following treatment was found in 72.72%. Resolution following repeated injection after one week was found in 18.18%. In group II (chalazia size 5-7 mm) out of n=28 cases, 46.24% resolved after first injection and 39.28% cases resolved after the second injection and no resolution was found in 14.28% cases after one month. Conclusion Patients with small chalazia who are not amenable to incision and curettage intralesional triamcinolone acetonide injection appear to be a better option. Intralesional triamcinolone acetonide injections were found to be effective in resolving acute and sub-acute chalazia of soft to firm consistency irrespective of their duration.

Objectives: To determine the outcome of newborns, admitted with different stages of HIE in Neonatal Unit, Services Hospital, Lahore. Study design: Descriptive Study. Place and duration of study: Neonatology Section, Pediatrics Department, Services Institute of Medical Sciences (SIMS) / Services Hospital, Lahore. Over a period of Six months, from 1st August 2005 to 31st January 2006. Material & methods: A total of 170 newborns having Apgar score less than 7 were included in the study. Babies with any congenital anomaly were excluded. Results: 80% were males. 17.64% had weight less than 1.5 Kg, 37.06% weight between 1.5 Kg to 2.5 Kg and 45.29% had weight more than 2.5 Kg. 90.5% of our babies were delivered in Services Hospital, Lahore. 77.64% babies were full term. Babies were divided into three groups; Group-I had Apgar score 0-3, Group-II between 4 & 5 and Group-III had Apgar score 6-7. In Group-I, 65% babies died, whereas 100% had any one of the complications, in Group-II, there were 22% deaths and no baby died from Group-III. Conclusions: Despite advanced technology, a high number of babies have HIE and its complications. It is better to prevent babies from HIE, because once HIE occur, you cannot save babies completely from its complications.

Purpose To evaluate the effect of intravitreal triamcinolone acetonide on visual acuity and macular thickness using optical coherence tomography (OCT) in macular edema associated with various retinal vascular disorders. Methods This prospective nonrandomized clinical interventional study included 81 eyes (76 patients) comprised of Group I, 57 eyes (51 patients) with diabetic macular edema; Group II, 10 eyes (10 patients) with branch retinal vein occlusion; and Group III, 13 eyes (13 patients) with central retinal vein occlusion. All eyes received an intravitreal injection of 4 mg triamcinolone acetonide (with the solvent) in the operation theater under sterile conditions. Results Mean preinjection central macular thickness was 531.84±132 μm in Group I, 458.4±149 μm in Group II, and 750.81±148 μm in Group III. All groups showed a statistically significant decrease in mean central macular thickness at 1 month (300.7±119 μM in Group I, 218.2±99 μm in Group II, and 210.5 ±56 μm in Group III) and 3 months (253.19±109 μm in Group I, 187±47 μm in Group II, and 182±50 μm in Group III) after injection (p<0.05). Mean follow-up was 22±2.4 weeks. Mean visual acuity increased in all three groups (preoperative visual acuity in Group I, 1.2±0.4 logMAR units; Group II, 1.24±0.5 logMAR units; Group III, 1.1 ±0.4 logMAR units; 1 month postinjection in Group I, 0.88±0.3 logMAR units; Group II, 0.67±0.3 logMAR units; Group III, 0.86±0.4 logMAR units; 3 months postinjection in Group I, 0.84±0.4 logMAR units; Group II, 0.59±0.3 logMAR units; Group III, 0.82±0.5 logMAR units) (p<0.05). Forty-one eyes completed 6 months and 20 eyes completed 9 months follow-up. Twelve of 20 (41%) eyes in Group I, 2/6 (33%) eyes in Group II, 3/6 (50%) eyes in Group III, and 8/15 (53%) eyes in Group I, 1/3 (33%) eyes in Group II, and 2/2 (100%) eyes in Group III developed recurrence of macular edema with worsening of visual acuity at 6 and 9 months, respectively. Thirty-three (40.7%) eyes developed IOP elevation (at least one reading > 24 mmHg). One eye developed infective endophthalmitis. Conclusions Intravitreal injection of triamcinolone acetonide may be considered as an effective treatment for reducing macular thickening due to diffuse diabetic macular edema, venous occlusion associated macular edema, and may result in increase in visual acuity at least in the short term. Further follow-up and analysis is required to demonstrate its long-term efficacy.

Background: Arteriovenous fistula (AVF) is the primary mode of achieving vascular access for hemodialysis in chronic renal failure (CRF). Because of high complication rates like thrombosis, maintenance of the fistula is a major challenge. Although antiplatelets and anticoagulants are emerging rapidly for improving the outcome of AVF but fear of bleeding, hematoma, subsequent compression of AV fistula and blockade restrict their use in many dialysis centers. Methods: Seventy five patients for whom AVF was created for hemodialysis access from Feb 2005-April 2009 in Manipal Teaching Hospital, Pokhara, Nepal were included in a retrospective study. Analysis of results was done in two different age and sex matched groups; Group I had patients where no anticoagulants or antiplatelets were used and Group II had patients in which antiplatelets/anticoagulants were used. Results: There were 27 patients in group I and 48 patients in group II. Both the groups were followed up till 2 years post operatively to check for the patency of the fistula. AV fistula was repeated in 16 cases altogether. In group II there were 5 (10%) cases of repeat fistula while in group I it was repeated in 11 (40%) cases. Conclusion: Judicious use of antiplatelet/anticoagulant agents in cases of AVF for hemodialysis access can be beneficial in preventing the chances of occlusion of AVF and thus helps in its long term patency. <p class="abstract"><strong>Background:</strong> Alopecia areata (AA) is a chronic inflammatory disease characterized by nonscarring hair loss involving any hair-bearing surface of the body. The current therapy for AA is not curative but rather aimed at controlling or limiting the pathogenic process. Corticosteroids are the most popular drugs used, however localized atrophy is a common complication, particularly with triamcinolone. Promoting hair growth by application of autologous blood extracted platelet rich plasma (PRP) is a simple, yet effective procedure. The aim of the study was to evaluate and compare the efficacy of intralesional triamcinolone acetonide (5 mg/ml) with intralesional PRP in the treatment of two different scalp AA patches of same individual.</p><p class="abstract"><strong>Methods:</strong> A total of 30 patients having ≥2 patches of AA were randomized and given two separate modalities of treatment in two different patches and divided in Group I, treated with intralesional triamcinolone acetonide (5 mg/ml) and Group II, treated with PRP. Interval time between every follow-up was 3 weeks and four such follow-ups were done. Hair regrowth was calculated according to Mac Donald Hull and Norris grading system at every follow-up and was compared with the initial grading.<strong></strong></p><p class="abstract"><strong>Results:</strong> Group I patches, treated with triamcinolone acetonide have the significantly higher mean grading score than the group II patches, treated with PRP at each follow up.</p><p class="abstract"><strong>Conclusions:</strong> Intralesional triamcinolone acetonide is more efficacious than intralesional PRP in the treatment of scalp alopecia areata with no recurrences and very few adverse effects like atrophy and hypopigmentation.</p>

Objective Obesity is a common disorder in our country. Most widely used method to gauge obesity is Body Mass Index (BMI) which is equal to weight in kilogram divided by height in meter square. The objective of our study was to find the levels of liver enzymes ALT, AST & GGT in two groups (overweight individuals and obese individuals). Methods: A total no of 156 individuals were selected for the study and they were categorized into three groups on the basis of BMI- Group I Normal, BMI 18.5 to 24.9 (n=72), Group II Overweight, BMI 25.0 to 29.9 (n=39) and Group III Obese, BMI >30 (n=45). Serum ALT, AST & GGT were estimated by Accurex AC 112 plus semi auto analyser and values were given in U/L. Results: Values of ALT, AST & GGT were with in the normal references range in all the three groups. Conclusion: No significant relation was found between ALT, AST & GGT in normal, overweight and obese individuals. The aim: To increase the effectiveness of treatment of patients with onychomycosis with contraindications to administration of systemic antifungal therapy. Materials and methods: The study involved 63 patients with onychomycosis of the toes aged 18 to 72 years under supervision at the clinic of the Institute of Dermatology and Venereology of the National Academy of Medical Sciences of Ukraine, who were divided into groups: Group I (30 patients) received naftifine hydrochloride solution and cream; Group II (33 patients) additionally used nail kit mycosan and cytoflavin 2 tablets 2 times a day for 1-1.5 months.The study implied microscopic and molecular genetic examination of nail plates before treatment and starting from the 4th week every 2 weeks. The study included patients with onychomycosis with positive results of both microscopic and PCR studies. Mycological recovery was noted in case of coincidence of results of these studies. Results: Mycological recovery in patients of Group II began in the sixth week in 6.3% and occurred in 100% of cases at the 24th week of treatment, depending on the severity and clinical form. The overall effectiveness of therapy at the 48th week reached 56.7% in Group I and 72.7% in Group II. Conclusions: The developed method of treatment, which includes the use of naftifine hydrochloride solution and cream, nail kit micozan and cytoflavin, allows to achieve clinical and mycological recovery in 72.7% of patients and is a priority in the treatment of patients with onychomycosis with comorbid conditions.

Background: Caesarean section is the most common method of delivery in the present conditions. The present study conducted to evaluate the accuracy of transvaginal ultrasound in lower uterine segment caesarean scar integrity and histomorphometric analysis of scar tissue.Methods: The study was conducted in the department of OBG, SUT academy of medical sciences, Vattapara, Trivandrum, Kerala. Based on the inclusion and exclusion criteria a total of 144 patients were included in the study. All patients explained study procedure and informed consent was obtained. They were subjected to transvaginal ultrasound examination after that small piece of scar tissue was taken and used for histomorphic analysis. The data was analysed by SPSS (16.0) version used.Results: In group-I 31.11% showed 40 and above fibrosis and group-II 85.0% had fibrosis 40 and above. 8 in group-I showed 0-19% of fibrosis and 17 in group-II. 0-19% whereas vicryl had a range of 0-40% and above and the rate of fibrosis compared to transvaginal sonography not showed significant difference.Conclusions: Histomorphological analysis of scar tissue gave accurate results and it was similar to transvaginal ultrasonography results.

<p class="abstract"><strong>Background:</strong> Acute rhinosinusitis is an acute viral or bacterial in which there is inflammation of the mucosa of the nose and paranasal sinuses. The aim of study was to compare the efficacy of oral amoxicillin-clavulanate with levofloxacin in the management of acute maxillary sinusitis.</p><p class="abstract"><strong>Methods:</strong> The present prospective study was conducted in the Department of ENT, Rajendra Institute of Medical Sciences, Ranchi during a period of 8 months i.e. from July 2016 to June 2017. In Group I patients 1 gm of amox­icillin-clavulanate was given two times a day and in Group II, 500 mg of Levo­floxicin was given once a day for a period of 10 days. Patient’s complete demographic details were recorded including name, age and gender. Xylometazoline nasal spray and steam inhalations were given to all the patients.  </p><p class="abstract"><strong>Results:</strong> The mean age of the subjects was 36.43±6.43 years. In both the groups, majority of the subjects were between 36-45 years of age. There were 34.7% (52) in Group I and 38.7% (n=58) in Group II who belonged to this age group. Least number of subjects was those aged more than 48 years. There were 88% subjects (n=132) in group I and 86% (n=129) subjects in Group II in whom complete resolution of symptoms was seen.</p><p><strong>Conclusions:</strong> From the above study we can conclude that both amoxicillin–clavulanic acid and levofloxacin are equally efficacious in managing cases of acute sinusitis. In this study there was no difference in the rate and duration of resolution amongst both the groups.</p>

background: Postoperative sore throat (POST) is defined as pain or discomfort in the throat following general anesthesia. Throat packs are used by many surgical subspecialties for different benifits, however they may increase the incidence of POST. Many interventions can be used to decrease incidence of POST. Triamcinolone acetonide (TA) is a moderately potent topical corticosteroid preparation. In this study, we hypothesized that soaking the throat pack with TA may decrease POST. Methods and Material: This prospective interventional comparative study was performed on 54 patients planned for Functional Endoscopic Sinus Surgeries (FEES) surgery. After endotracheal intubation, a standard length of oro-pharyngeal pack was placed, then patients were randomly allocated into: Group I: Oro-pharyngeal packs were soaked with 15 mg Triamcinolone acetonide 0.1% and Group II: packs were soaked with the same volume of lubricating gel (K-Y gel®). The patients were postoperatively asked about: sore throat, dysphagia, hoarseness of voice and nausea and vomiting. Results: Thirty minutes and 24 hours after extubation, Group I patients showed lower but statistically insignificant sore throat scores. Two to six hours after extubation, Group I showed a statistically significant reduction in sore throat scores. Six patients suffered dysphagia in group I compared with 8 patients in group II. Hoarseness of voice occurred in 1 patient in group I and 3 patients in group II. No patient complained of nausea or vomiting. Conclusión: Soaking oropharyngeal pack with triamcinolone acetonide in orabase gel was able to decrease POST in FESS patients.

C57BL male mice (Jackson Lab., Bar Harbor, Maine) weighing about 18 gms were randomly divided into three groups: group I was fed sweetened liquid alcohol diet (modified Schenkl) in which 36% of the calories were derived from alcohol; group II was maintained on a similar diet but alcohol was isocalorically substituted by sucrose; group III was fed regular mouse chow ad lib for five months. Liver and heart tissues were fixed in 2.5% cacodylate buffered glutaraldehyde, post-fixed in 2% osmium tetroxide and embedded in Epon-araldite.
