AbstractGouty Arthritis is a disease associated with an inborn errors of purine matabolism. Gout Arthritis occurs due to a buildup of uric acid that exceeds the normal limit in the joints. This can cause pain in gouty arthritis sufferers so giving warm compresses is necessary for gouty arthritis sufferers. This study was a descriptive case study aiming to describe the application of warm compresses in reducing pain scale in patirnts with Gouty Arthritis. A client assessment form, observation sheets, pain scales, WWZ (Warm Water Zak), and uric acid measurement kid were the instuments being used to collect data. The findings showed that there was a decrease in the pain scale in patiens with Gouty Arthritis. In case I, there was a decrease of pain scale from 5 to 2; meanwhile, in case II the pain scale decreased from 6 to 3. The conclusion from this study was that the application of warm compress therapy with WWZ (Warm Water Zak) could reduce the pain scale. Thus, patients of Gouty Arthritis are suggested to apply warm compress therapy with WWZ (Warm Water Zak) to reduce the pain because of the disease.Keywords: Warm compress, pain, gouty arthritis AbstrakGout Arthritis merupakan hasil metabolisme atau pecahan purin dari makanan yang tinggi purin. Gout Arthritis terjadi karena penumpukan asam urat yang melebihi batas normal dipersendian. Hal ini dapat menyebabkan nyeri pada penderita gout arthritis sehingga pemberian kompres hangat sangat diperlukan untuk penderita gout arthritis. Studi kasus ini bertujuan untuk menggambarkan penerapan tindakan kompres hangat dalam menurunkan skala nyeri pada penderita Gout Arthtitis. Metode studi kasus ini menggunakan studi kasus deskriptif. Instrumen studi kasus ini dengan menggunakan format pengkajian klien, lembar observasi, skala nyeri, WWZ (Warm Water Zak) dan alat ukur untuk mengukur kadar asam urat. Hasil studi kasus ini menunjukkan ada penurunan skala nyeri pada penderita Gout Arthritis pada kasus I terjadi penurunan skala nyeri awal 5 turun menjadi 2 sedangkan pada kasus II skala nyeri awal 6 turun menjadi 3. Simpulan dari studi kasus ini bahwa terapi kompres hangat dengan WWZ (Warm Water Zak) dapat menurunkan skala nyeri. Saran bagi klien yang mengalami Gout Arthritis agar dapat mengontrol dengan menggunakan terapi kompres hangat menggunakan WWZ (Warm Water Zak) secara mandiri.Kata kunci: Kompres hangat, nyeri, gout arthritis

AbstractGout is a disease characterized by high levels of uric acid in the blood that normally is 2-7 mg/dl in men and 2-6 mg/dl in women. Consequently, uric acid was build-up in the joints and the other organs. High level of uric acid related to consuming high-purines foods. The signs and symptoms were joints sore, painful and inflammation. The purpose of this case study was to applied warm water compress to reduce pain in elderly gout patients. The cash study was used a descriptive case study method. The study subjects were two elderly patients who have uric acid above >6 mg/dl for women and >7 mg/dl for men, pain scale <4, and complaints of joint pain in the legs. The study was conducted in Wonokerto Kulon Village, Wonokerto District, Pekalongan Regency. The results showed a that uric acid levels was decreased and pain was reduced in both clients. As the conclusion, warm water compress is applicable to reduce pain among gout patients. Accordingly, warm water compress can be an alternativ non-pharmacological therapy to reduce pain in gout patients.Keywords: Gout, Elderly, Warm Water Compress AbstrakAsam urat adalah penyakit kadar asam urat yang tinggi di dalam darah melebihi batas normal menyebabkan penumpukan asam urat di dalam persendian dan organ tubuh lainnya penumpukan yang disebabkan karena terlalu banyak mengkonsumsi makanan tinggi purin dan membuat sendi sakit, nyeri dan meradang dengan nilai normal pada laki-laki 2-7 mg/dl dan pada wanita 2-6 mg/dl. Tujuan dari studi kasus ini adalah untuk mengaplikasikan tindakan kompres air hangat untuk menurunkan nyeri penderita asam urat pada lansia. Rancangan Karya Tulis Ilmiah ini menggunakan metode studi kasus deskriptif dengan subyek dua klien asam urat yang mengalami asam urat diatas >6 mg/dl untuk wanita dan >7 mg/dl untuk laki-laki dengan skala nyeri <4 dengan keluhan nyeri sendi pada kaki di desa Wonokerto Kulon Kecamatan Wonokerto Kabupaten Pekalongan. Hasil studi ini menunjukan adanya penurunan asam urat dan menyatakan nyeri berkurang pada kedua klien. Simpulan studi kasus ini menunjukan bahwa Terapi Kompres Air Hangat mampu menurunkan nyeri pada penderita asam urat. Untuk itu perawat diharapkan dapat memberikan tindakan terapi nonfarmakologis untuk menurunkan nyeri yang berupa Terapi Kompres Air Hangat pada penderita asam urat.Kata kunci:Asam Urat, Lansia, Kompres Air Hangat

AbstrakLansia yang mengalami gangguan kualitas tidur perlu mendapatkan perhatian dengan strategi dan teknik-teknik khusus dalam menyelesaikan masalah. Implementasi hydrotherapy air hangat menjadi salah satu pilihan yang tepat. Tujuan penelitian untuk mendapatkan gambaran tentang asuhan keperawatan pada lansia Ny. R dengan hipertensi dalam penerapan implementasi hydrotherapy air hangat dalam meningkatkan kualitas tidur di Desa Balinggi Kecamatan Balinggi Kabupaten Parigi Moutong. Desain penelitian studi kasus deskriptif, responden sebangak 1 orang, pengumpulan data dengan cara wawancara, observasi dan pemeriksaan fisik, instrumen yang digunakan yaitu format pengkajian PSQI dan benda berupa termometer laboratorium. Analisa data yang digunakan yaitu reduksi data, penyajian dan kesimpulan. Adapun intervensi yang dilakukan yaitu implementasi hydrotherapy rendam kaki air hangat. Kesimpulan evaluasi keperawatan setelah dilakukan tindakan keperawatan selama 3 kali pertemuan dalam 1 minggu didapatkan hasil kualitas tidur klien meningkat. Diharapkan kiranya dari hasil studi kasus asuhan keperawatan penerapan implementasi hydrotherapy air hangat ini dapat diterapkan dalam kegiatan praktek Home Care pada lansia binaan Akper BK Palu.Kata kunci : Hipertensi, Hydrotherapy Air Hangat, Kualitas Tidur, Lansia.ABSTRACKElderly with sleep quality disturbance needs to get attention with specific strategies and techniques in solving the problems. The implementation of warm water hydrotherapy is one of the right choices. The research objective was to get an overview about nursing care of an elderly namely Mrs. R who has hypertension in the implementation of warm water hydrotherapy to improve sleep quality in Gitgit Sari Hamlet, RT 000 / RW 001, Balinggi Village, Balinggi Sub- District, ParigiMoutong District.  Descriptive case study research design, respondent was 1 person, data was collected by interview, observation and physical examination, the instrument used is the PSQI assessment format and laboratory thermometer. Data analysis used is data reduction, presentation and conclusion. The intervention carried out was the implementation of warm watersoaking feet hydrotherapy. The conclusion of the nursing evaluation after conducting nursing action for 3 meetings a week showed that the client's sleep quality improved. AbstractFever is a condition where body temperature exceeds normal limit, caused by an increase in the temperature-regulating center in the hypothalamus. Warm compress is an easy way to lower body temperature. This type of compress involves a clean cloth soaked in warm water, which is applied to the skin. The purpose of this case study is to evaluate a warm compress method in reducing fever. Two preschool-aged children with fever were treated by warm compresses. The results of this case study showed that the first and second patients experienced a decrease in body temperature after warm compresses. Body temperature for the first patient was 39ºC and dropped to normal (37ºC). meanwhile, the second patient temperature was 38,7ºC dropped to 36,5ºC. The conclusion of this study is warm compress can reduce body temperature in children with fever. Nurses or patients may be albe to apply warm compresses as a first ald in reducing fever.Keywords: Preschool Age Children, Fever, Warm Compress AbstrakDemam adalah kondisi suhu tubuh melebihi batas normal yang diakibatkan oleh meningkatnya pusat pengatur suhu di hipotalamus. Salah satu tindakan untuk menurunkan suhu tubuh anak yaitu dengan kompres hangat. Kompres adalah pemeliharaan suhu tubuh dengan menggunakan cairan atau alat yang dapat menimbulkan hangat atau dingin pada bagian tubuh. Tujuan dilakukan penerapan ini untuk mengambarkan penerapan kompres hangat terhadap penurunan suhu tubuh anak demam usia prasekolah. Metode yang digunakan dalam studi kasus ini adalah penelitian deskriptif dengan subyek dua pasien anak usia prasekolah yang mengalami demam. Hasil dari studi kasus ini menunjukan bahwa klien pertama dan kedua mengalami penurunan suhu tubuh setelah dilakukan kompres hangat, pada kasus 1 suhu awal 39ºC setelah dilakukan kompres hangat suhu menjadi 37,1ºC, sedangkan pada kasus 2 dengan suhu awal 38,7ºC setelah dilakukan kompres hangat suhu menjadi 36,5ºC. Kesimpulannya adalah pemberian kompres hangat dapat menurunkan suhu tubuh pada anak dengan demam. Saran bagi perawat dan orangtua untuk dapat menerapkan kompres hangat sebagai alternatif menurunkan suhu tubuh anak dengan demam.Kata kunci: Anak Usia Prasekolah, Demam, Kompres Hangat

Gout Arthritis is one of the most common inflammatory joint disease that is characterized by a buildup of monosodium urate crystals in or around the joints, causing swollen pain in the joints. Management of this pain is done by using warm water compress therapy using ginger aiming to reduce pain in the joints.The goal is to analyze the results of the implementation of community nursing care by giving warm water compresses using ginger to the elderly to meet pain needs. The method in writing Scientific Papers is a case study taken in RT 09 Margabatin Village, East Lampung, with 10 respondents. Results intervention carried out for 3x 60 minutes by giving warm water compresses using ginger is obtainedthat the average pain scale before applying warm water compresses using ginger is 4 (moderate pain), after a warm water compress is done using ginger is 2 (mild pain). So it can be concluded that there is an effect of giving warm.

Gout or Gouty Arthritis is a degenerative disease. Gouty arthritis is a clinical syndrome that is caused by deposition of purine crystals in the tissues, due to levels of uric acid (hyperuricemia) in the extracellular fluid that passes through saturation. The etiology of gout arthritis included age, sex, history of medication, obesity, consumption of purine and alcohol. Aging is an important risk factor for men and women. This study uses an analytical-descriptive approach with cross-sectional design. This study was conducted to obtain a relationship between nutritional status and uric acid levels in the community in the area of ​​Al-Hidayah Islamic Boarding School, Ngawi. This research was conducted in Majasem Village, Kendal District, Ngawi Regency, starting from 7 June - 31 August 2018. The results showed that there was a relationship between nutritional status and uric acid levels in the community in the area of ​​Al-Hidayah Islamic Boarding School, Ngawi (p=0,008). Suggestions for further research are bivariate analysis based on gender, knowledge and attitude to response to diet so that other factors that influence can be seen clearly

Acute gouty arthritis represents an inflammatory response to microcrystals of monosodium urate that precipitate in joint tissues from supersaturated body fluids or are shed from preexisting articular deposits [1]. Gout is a metabolic disease characterized by recurrent episodes of arthritis associated with the presence of monosodium urate crystals in the tissue or synovial fluid during the attack.These forms of crystal-induced arthritis usually affect peripheral joints, including knee, ankle, wrist, and metacarpophalangeal and metatarsophalangeal joints. All of them may be associated with other inflammatory, endocrine diseases [2]. The present study was done to highlight the relationship between increased levels of uric acid and specific laboratory tests in order to possible forecast development of further disease in patients with gouty arthrithis.The present study was done on 34 patients hospitalized in Felix Hospital of Rehabilitation in 2015-2016, with age between 44 and 74, having the main diagnosis of gouty arthritis.We studied the following laboratory tests:urea and other related analysis, like uric acid, creatinine, cholesterol, glutamate pyruvate transaminase and glutamate oxalate transaminase.

Gouty Arthritis has now become a common disease condition which we deal in Ayurveda, but a proper treatment protocol is not followed in many cases. The case reported here was as a result of improper diet and lack of exercise which resulted in an increase serum uric acid level and joint inflammation. The treatment was given at IPD level diagnosing it as Gambhira Vatarakta with valid Chikitsa Siddhanta. This case report provides us a guidelines that even a chronic gouty arthritis with a very high serum uric acid can be treated as per Vataraktha Chikitsa Siddhanta in Ayurveda.

This study explored understandings shared by fifth-grade gifted students as they read the book Restart, which explores bullying. Students read, created representations, and discussed the text. Grounded by Langer’s stances of envisionment, this descriptive case study analyzed student representations and conversations. Each of the stances was represented with most responses being represented in Stances 1 (getting a sense of the text), 2 (interpreting text), and 4 (analyzing the text). In addition, most students viewed bullies and their behavior as being in a fixed state, which was tied to the perceived power a bully held. The results from this study have implications for teachers who work with gifted and talented students, counselors who work with students in mental health and resilience programs, and the collaboration of these school personnel.

Sam-Myo-Whan (SMW) has been used in Korean and Chinese traditional medicine to help treat gout, by reducing swelling and inflammation and relieving pain. This study compared the effects of SMW extracted by using different solvents, water (SMWW) and 30% EtOH (SMWE), in the treatment of gouty arthritis. To this end, we analyzed the main components of SMWW and SMWE, using high-performance liquid chromatography (HPLC). Anti-hyperuricemic activity was evaluated by measuring serum uric acid levels in hyperuricemic rats. The effects of SMWW and SMWE on swelling, pain, and inflammation in gouty arthritis were investigated by measuring affected limb swelling and weight-bearing, as well as by enzyme-linked immunosorbent assays, to assess the levels of proinflammatory cytokines and myeloperoxidase (MPO). In potassium oxonate (PO)-induced hyperuricemic rats, SMWW and SMWE both significantly decreased serum uric acid to similar levels. In monosodium urate (MSU)-induced gouty arthritis mice, SMWE more efficiently decreased paw swelling and attenuated joint pain compare to SMWW. Moreover, SMWE and SMWW suppressed the level of inflammation by downregulating proinflammatory cytokines (interleukin-1β, tumor necrosis factor-α, and interleukin-6) and MPO activity. HPLC analysis further revealed that berberine represented one of the major active ingredients demonstrating the greatest change in concentration between SMWW and SMWE. Our data demonstrate that SMWE retains a more effective therapeutic concentration compared to SMWW, in a mouse model of gouty arthritis.
