ABSTRACT Aim The aim of the survey conducted in dental laboratories was to evaluate the current trend in fabricating a full ceramic restoration for discolored anterior teeth through a questionnaire and personal interaction with the dental technician. Materials and Methods The survey questionnaire consisted of 20 questions aimed toward the current trend in fabricating a full ceramic restoration for discolored anterior teeth. This questionnaire was validated by a panel of senior prosthodontists and technicians and was sent to 32 dental technicians in Mumbai and Navi Mumbai. Results and conclusion The results of the study showed that majority of the dental technicians preferred taking the stump shade themselves by visiting the clinic or inviting the patient to the laboratory. Majority of the dental technicians preferred using lithium disilicate material and its medium opacity ingots to fabricate restorations by the layering technique for masking discolored anterior teeth. Majority of the technicians believed that their dentists did not use try-in paste prior to luting of the restoration. Clinical significance This survey would help to improve the rapport between the clinicians and dental laboratories and optimize the esthetics of the restorations fabricated for anterior discolored teeth. ABSTRACT Aim To evaluate the interference of the intracanal medication Calen® (SSWhite, São Paulo, Brazil) on the filling of simulated lateral canals. Materials and methods Twenty human anterior teeth were used. Before the endodontic filling procedures the access of cavity was made, and after this root canals were made in all the teeth to simulate the presence of lateral canals. After preparation, the teeth were randomly divided into two groups (n=10). In group I, the root canal system was filled directly after chemicalmechanical preparation; in group II, endodontic treatment was performed in multiple sessions, and after preparation the calcium hydroxide-based intracanal medication Calen® was inserted. After the period of 7 days, the root canals were vigorously irrigated and then they were filled. Next, the teeth were radiographed to verify the quality of the filling. Results The results demonstrated that the teeth treated in a single session, without calcium hydroxide medication, presented 47 canals out of 60 with radiographic evidence of filling, whereas the teeth in which intracanal medication was used, only 07 presented a radiographic image compatible with filling (p < 0.05). Conclusion The use of the calcium hydroxide-based medication Calen made it difficult to obtain a hermetic filling of the root canal system. Clinical significance The clinical significance of this work basing on the fact that once the dentist knowing that property obliteration of calcium hydroxide can be taken care when they are used in the presence of lateral canals. Introdução: A aparência estética é uma das preocupações do ser humano, o que leva a uma busca constante por uma estética que expresse harmonia, naturalidade e expressividade. Sendo assim, os procedimentos restauradores têm contribuído positivamente para a resolução dos desafios odontológicos estéticos. Objetivo e relato de caso: Relatar um caso clínico com associação de clareamento dental, confecção de pino intrarradicular e coroa indireta em dissilicato de lítio em que foi devolvida ao paciente a estética desejada. Pacientede 23 anos de idade fraturou o incisivo central superior direito na infância; insatisfeito com a instabilidade de cor do dente, buscou novas alternativas de tratamento. Conclusão: A associação de diferentes tratamentos na Odontologia estética pode apresentar resultados satisfatórios para o restabelecimento estético e funcional de dentes anteriores.Introduction: The aesthetic appearance is one of the human being concerns, leading to constant search for an aesthetic that expresses harmony, naturalness and expressiveness. So, the restorative procedures have contributed positively to overcome the aesthetic dentistry challenges. Objective and case report: Report a clinical case in association to dental whitening, making of intraradicular pin and indirect crown of lithium disilicate when it was returned to the patient the desired aesthetic. A 23 years old patient fractured the right central incisor in the childhood, dissatisfied with the tooth color instability, he looked for new treatment alternatives. Conclusion: The association of different treatments in the aesthetic dentistry might present satisfactory results to the aesthetic and functional reestablishment of anterior teeth.

The aesthetic treatment for anterior teeth requires a series of clinical and technical evaluations to obtain a predictable result, following a well-defined operating sequence. Today, the clinical–technical team can use different digital tools in the different steps of the workflow. A preventive assessment, the knowledge of limits, and the possibilities of surgical and prosthetic procedures allow to use these devices. Sharing goals of the treatment with the patient according to their expectations and needs is the key point of the treatment plan. Setting a defined treatment plan avoids invasive procedures. In this clinical case report, a 27-year-old patient affected by Class II div 2 malocclusion with deep bite was treated with a full digital workflow. Previewing the aesthetics was through dedicated software, which shows operators and patients the objectives of the therapy and guides the dental technician in the first phase of the work. The use of digital flows in the prosthetic phases reduces the working time. In this case, it shows the impact of a digital workflow on peri-prosthetic therapy for the aesthetic rehabilitation of the upper central incisors in a young adult.

Aim. Using the method of questionnaire survey of dentists to assess their awareness of the current problems of modern practical therapeutic dentistry.Materials and methods. The analysis of the results of a survey- questionnaire of 376 practical dentists, who is practicing a therapeutic dentistry (dentists with legal certificates in fields: ’’therapeutic dentistry” or "general practice dentistry"). Questioning of 238 dentists (63.3%) was carried out in person, questioning of 138 dentists (36.7%) was carried out remotely using the stomweb.com network resource.Results. Significant differences and contradictions in approaches to the implementation of key dental procedures were revealed: aesthetic restoration of anterior teeth, restoration of the contact surfaces of posterior teeth, adhesive preparation, therapeutic tactics for deep caries and reversible forms of pulpitis, approaches to the diagnosis and treatment of cervical non-carious dental defects, preparation tactics cavities, the use of intracanal pins in the restoration of endodontically treated teeth, attitudes towards teeth whitening, fluoride-containing toothpastes and electric toothbrushes, etc.Conclusions. The data obtained indicate the need for targeted scientific research, further trainings on a hans-on courses, addressing several studied issues, based on the principles of evidence-based medicine, as well as the expediency of reforming the education system for dental professionals in order to limit the dissemination of information that does not have scientific justification and/or evidence base.

Management of severe worn dentition in patients with bruxism is challenging as a result of the loss of tooth structure and occlusal vertical dimension, temporomandibular implications, tooth hypersensitivity, and masticatory or aesthetic impairment. This case describes the 4-year follow-up clinical evaluation of a full mouth tooth-supported rehabilitation made on a 66-year-old man with bruxism and tooth wear, with aesthetic complaints and compromised masticatory function. The prosthetic treatment was planned with a digital smile design and a mock-up technique for an aesthetic and minimally invasive approach using lithium disilicate pressed and layered veneers on anterior teeth, posterior CAD/CAM lithium disilicate overlays with facial coverage, and CAD/CAM monolithic zirconia crowns with facial feldspathic ceramic on maxillary and mandibular canines and first molars in order to ensure the occlusal stability at the increased occlusal vertical dimension. After 4 years of function, no complications were registered. The choice of an appropriate material for the rehabilitation of these patients is essential to improve treatment prognosis and should be guided by mechanical and aesthetical properties. The use of four-point occlusal stabilization with CAD/CAM high strength monolithic zirconia crowns combined with ceramic veneers and overlays appears to be a reliable treatment option that enhances aesthetics and minimizes the occurrence of ceramic fractures, ensuring the treatment prognosis and predictability.

Abstract Objectives Primary aim of this study was to evaluate survival rate of lithium disilicate veneers in upper and lower anterior teeth. Secondary aims were to evaluate changing in proportions of teeth before and after restorations and to assess mean thickness of the veneers. Materials and Methods Seventy-nine upper and lower lithium disilicate veneers were made in 13 patients with worn teeth. Mean follow-up was 3 years. To perform anterior definitive rehabilitations, malocclusions and loss of vertical dimension were treated by full mouth rehabilitations to obtain proper occlusal conditions. Veneers were made of lithium disilicate core and fluorapatite-based ceramic stratification. Survival rate was calculated by Kaplan–Meier analysis. Changing in teeth proportion before and after restorations was analyzed by a paired t-test. Descriptive statistics of thickness values were also performed. Results One case of detachment was observed with a 98.7% survival rate. Teeth’s proportions were preserved although the first upper right incisor and canine changed in dimension. Conclusions Lithium disilicate veneers in esthetical rehabilitations of worn teeth proved to be an effective way of treatment in a medium follow-up of 3 years. Proportions seemed to be maintained with a minimum dental removal.

ABSTRACT Aim The aim of this study was to design and introduce a retainer that overcomes the common shortcomings seen in other retainers. Materials and methods Hard thermoplastic sheet of 0.5 mm thickness is vacuum or pressure-molded onto the patient cast. Lingual portion of the retainer is trimmed according to the contours of the anterior teeth. Contact points between the maxillary and mandibular anterior teeth are marked on the retainer and reduced. Punch cut holes are placed on the retainer for the exit of flash and air bubbles while fixation. The retainer is bonded onto the lingual surface of the anterior teeth using composite. Results A 1-month review of the retainer showed no patient discomfort, occlusal interference, or bond failure. The aim of the article was found to have been achieved. Conclusion Initial evaluation has shown positive findings. Long-term clinical findings will determine the overall success of this new retainer. Clinical significance As compared with other retainers, thermoplastic retainer has shown reduced tendency to debond from occlusal forces, decreased patient discomfort, and occlusal interference. ABSTRACT Background Avulsion or exarticulation accounts for up to 16% of all traumatic injuries. It is most common in age group of 7 to 14 years. Following the loss of anterior teeth, the replacement is necessary to maintain esthetics, phonetics and avoid arch length discrepancy. Following replantation, postoperative care and management is done in several ways. Case report This article reports replantation and use of laser for endodontic sterilization and healing in a 12-year-old boy. Conclusion It is suggested that replantation of an avulsed tooth as soon as possible using appropriate splint and regular followup will show good prognosis. Clinical significance Laser offers itself as a good alternative to Ca(OH)2 for root canal sterilization and improved periapical healing. Rehabilitation of worn dentition is a challenging task because several predisposing factors may be included in the pathogenesis. The treatment options and process should be carefully considered based on the clinical condition and patients’ requirements. This case report represents the esthetic rehabilitation of a 68-year-old patient who exhibited severely worn teeth caused by attrition and erosion. After clinical evaluation, 3 mm increase in occlusal vertical dimension was planned with partial lithium-disilicate ceramic restorations as laminate veneers for anterior teeth and vonlays (onlays with veneer extentions) for posterior teeth in maxilla. The adaptation of the patient to the increased occlusal vertical dimension was tested for 1 month by provisional direct composite restorations. As the patient did not show any discomfort, definitive restorations were completed. The patient was satisfied with the esthetics and functionality of her new dentition.
