Physiological effects of a technology-based music-making program in skilled nursing residents

This study evaluated the effectiveness of a technology-based music-making intervention (the Beamz) in elevating heart rate and producing a relaxation response in skilled nursing home residents. Using a quasi-experimental counterbalanced design, results indicated that this brief intervention produced a statistically significant elevation in heart rate and systolic blood pressure when played at a fast tempo. Heart rate was also significantly elevated when slow tempo music was used. In addition, participants reported a statistically lower level of relaxation following the slow tempo session and expressed that they found this new activity intervention enjoyable. Implications of findings and future research possibilities using technology-based music-making interventions in recreation therapy practice are discussed.

ABSTRACTObjective:This study aimed to evaluate whether using the interRAI Palliative Care instrument (the interRAI PC) in nursing homes is associated with reduced needs and symptoms in residents nearing the end of their lives.Method:A quasi-experimental pretest–posttest study using the Palliative care Outcome Scale (POS) was conducted to compare the needs and symptoms of residents nearing the end of their lives in the control and intervention nursing homes. Care professionals at the intervention nursing homes filled out the interRAI PC over the course of a year for all residents aged 65 years and older who were nearing the end of their lives. This intervention was not implemented in the control nursing homes.Results:At baseline, POS scores in the intervention nursing homes were lower (more favorable) than in the control nursing homes on the items “pain”, “other symptoms”, “family anxiety”, and the total POS score. Posttest POS scores for “wasted time” were higher (less favorable) than pretest scores in the intervention nursing homes. In the intervention nursing homes where care professionals did not have prior experience with the interRAI Long-Term Care Facilities (LTCF) assessment instrument (n = 8/15), total POS scores were lower (more favorable) at posttest.Significance of results:One year after introducing the interRAI PC, no reduction in residents' needs and symptoms were detected in the intervention nursing homes. However, reductions in needs and symptoms were found in the subgroup of intervention nursing homes without prior experience with the interRAI LTCF instrument. This may suggest that the use of an interRAI instrument other than the interRAI PC specifically can improve care. Future research should aim at replicating this research with a long-term design in order to evaluate the effect of integrating the use of the interRAI PC in the day-to-day practices at nursing homes.

Passivity is a major debilitating problem for nursing home residents. Reports show that anywhere from 24 to 90 percent of residents living in nursing homes exhibit passive behaviors, apathy, and functional decline due to inactivity. The purpose of this study was to examine the effects that animal-assisted therapy (AAT) has on the passivity, engagement, and mood of rural nursing home residents. Using a quasi-experimental design, researchers assigned 10 rural nursing home residents to AAT and 10 rural nursing home residents to traditional recreation therapy interventions. Each group received five, one- hour sessions during a span of six weeks. AAT was shown to significantly decrease passive behaviors and significantly improve mood. The traditional recreation therapy control group did show improvements but none were significant.

During the pregnancy, fetal electrocardiogram (FECG) is deployed to analyze fetal heart rate (FHR) of the fetus to indicate the growth and health of the fetus to determine any abnormalities and prevent diseases. The fetal electrocardiogram monitoring can be carried out either invasively by placing the electrodes on the scalp of the fetus, involving the skin penetration and the risk of infection, or non-invasively by recording the fetal heart rate signal from the mother’s abdomen through a placement of electrodes deploying portable, wearable devices. Non-invasive fetal electrocardiogram (NIFECG) is an evolving technology in fetal surveillance because of the comfort to the pregnant women and being achieved remotely, specifically in the unprecedented circumstances such as pandemic or COVID-19. Textiles have been at the heart of human technological progress for thousands of years, with textile developments closely tied to key inventions that have shaped societies. The relatively recent invention of smart textiles is set to push boundaries again and has already opened the potential for garments relevant to medicine, and health monitoring. This paper aims to discuss the different technologies and methods used in non-invasive fetal electrocardiogram (NIFECG) monitoring as well as the potential and future research directions of NIFECG in the smart textiles area.

Abstract In two studies, we tested the relationship between children’s label-learning experience and label-learning ability within diverse superordinate categories with complex perceptual organization (animals, clothing, foods). Using both quasi-experimental and experimental designs, we examined 18- and 24-month-old children’s ability to generalize labels for novel members of superordinate categories as a product of their previous experience in learning labels for members of those categories. As predicted, children properly generalized more labels for members of the categories within which they had more label-learning experience than for members of the categories within which they had less label-learning experience. Results are consistent with the idea that children develop category-specific label-learning biases through their experience in learning labels for category members; they carry implications for multiple accounts of vocabulary acquisition and identify directions for future research.

Forest Bathing, where individuals use mindfulness to engage with nature, has been reported to increase heart rate variability and benefit wellbeing. To date, most Forest Bathing studies have been conducted in Asia. Accordingly, this paper reports the first pragmatic controlled trial of Forest Bathing in the United Kingdom, comparing Forest Bathing with a control comprising an established wellbeing intervention also known to increase heart rate variability called Compassionate Mind Training. Sixty-one university staff and students (50 females, 11 males) were allocated to (i) Forest Bathing, (ii) Compassionate Mind Training or (iii) Forest Bathing combined with Compassionate Mind Training. Wellbeing and heart rate variability were measured at baseline, post-intervention and three-months follow-up. There were improvements in positive emotions, mood disturbance, rumination, nature connection and compassion and 57% of participants showed an increase in heart rate variability. There were no significant differences between conditions, showing that Forest Bathing had equivalence with an established wellbeing intervention. The findings will help healthcare providers and policy makers to understand the effects of Forest Bathing and implement it as a feasible social prescription to improve wellbeing. Future research needs to involve clinical populations and to assess the effects of Forest Bathing in a fully powered randomised controlled trial.

ECTemp™ is a heart rate (HR)-based core temperature (CT) estimation algorithm mainly used as a real-time thermal-work strain indicator in military populations. ECTemp™ may also be valuable for resting CT estimation, which is critical for circadian rhythm research. This investigation developed and incorporated a sigmoid equation into ECTemp™ to better estimate resting CT. HR and CT data were collected over two calorimeter test trials from 16 volunteers (age, 23 ± 3 yrs; height, 1.72 ± 0.07 m; body mass, 68.5 ± 8.1 kg) during periods of sleep and inactivity. Half of the test trials were combined with ECTemp™’s original development dataset to train the new sigmoid model while the other was used for model validation. Models were compared by their estimation accuracy and precision. While both models produced accurate CT estimates, the sigmoid model had a smaller bias (−0.04 ± 0.26°C vs. −0.19 ± 0.29°C) and root mean square error (RMSE; 0.26°C vs. 0.35°C). ECTemp™ is a validated HR-based resting CT estimation algorithm. The new sigmoid equation corrects lower CT estimates while producing nearly identical estimates to the original quadratic equation at higher CT. The demonstrated accuracy of ECTemp™ encourages future research to explore the algorithm’s potential as a non-invasive means of tracking CT circadian rhythms.

This article discusses a longitudinal study of change in disruptive behaviors among nursing home residents treated with neuroleptics compared with those not treated with neuroleptics. Observations were made of 201 participants on admission to and after 1 year in eight skilled nursing facilities. Nine disruptive behaviors were measured using the Psychogeriatric Dependency Rating Scale with nursing assistants. Neuroleptic use was documented from medication records. Odds ratios are reported for the association of behavior at baseline and use of neuroleptics on nine problem behaviors. For those who received neuroleptics during the year, there was greater change in both developing and resolving disruptive behaviors than for those not receiving neuroleptics. For both groups, restless or pacing behavior and belligerent behavior manifested by refusing instructions changed the most, both in developing and in apparently resolving. Our results show that change in disruptive behaviors occurs among nursing home residents regardless of neuroleptic use, but it occurs more frequently among those who receive neuroleptic medication. Knowledge of which disruptive behaviors are most likely to resolve or develop is important in training nursing home staff to cope with the behaviors as well as in planning interventions that may modify such behaviors.

In light of contemporary pedagogical methods, the flipped classroom has been recognised as an effective pedagogy in English as a Foreign Language (EFL). This study employed a quasi-experimental one-group research design to investigate the impact of flipped learning on Omani EFL learners’ grammar and to examine students’ perceptions on the flipped classroom. An intact group of students (n=28) enrolled at the foundation programme in Arab Open University-Oman was randomly selected. Seven videos on English grammar were developed and shared with the students prior to the class. A varaiety of activities were conducted in the class following task-based language teaching. Students met for 8 lessons over the period of 8 weeks. Pre-test, post-test and semi-structured interviews were used in the study. The findings indicated that flipped learning had a positive impact on students’ understanding and usage of English grammar. Students’ perceptions on the flipped approach were positive. The study also provided pedagogical insights for the flipped classroom and recommendations for future research.

Scholarship on prison music-making projects and programmes to date has largely overlooked the perspectives of prison music facilitators, who form an integral part of many prison music activities. The aim of the study, which was exploratory in nature, was to contribute to a better understanding overall of the relationship between music and imprisonment by focusing on the perspectives of prison music practitioners. Drawing from data collected in four Norwegian prisons through ethnographic research, data was analysed thematically with four key themes emerging: interpersonal communication and emotional connection; social responsibility; prison system and environment, and (in)difference and exclusion. The findings highlight the fact that the range of prison music activities offered in many Norwegian prisons affects music facilitators deeply in a number of ways, and support existing studies that find that prison music practices can contribute to creating a community of caring individuals both inside and outside prisons. Notably, the emergence of the (in)difference and exclusion theme demonstrates a more critical and nuanced view of prison music facilitators’ experiences as going beyond simplistic, romantic notions of music’s function in social transformation. Concerns raised for those who appear to be excluded or differentiated from music-making opportunities in prison – in particular foreign nationals and women – suggest that (even) in the Norwegian context, music in prisons remains a “reward” rather than a fundamental “right.” This study marks a step towards a richer and more critical understanding of prison musicking and aims to inform future research, practice, and the processes involved in the possibilities for offering music in prisons.
