Background: The present study makes part of the project ‘Systematic Review of Crystallization Processes Applied for Medical Purposes (SyRCrysMed)’. SyRCrysMed is planned to lead to a publication of three review articles: (i) on crystallization of blood and its derivatives (serum, plasma) for diagnostic purposes, (ii) on crystallization of other biological fluids for diagnostic purposes, and (iii) on crystallization applied to homeopathy (both, basic and clinical research). Medical crystallization is a wide, however fragmented and little known field of science. It embraces different crystallization methodologies and applications. The commune scope of most of the crystallization-based methods is to access a more complex (or different) kind of information on the diagnosed/analyzed subject than it is possible by the use of conventional methods. The underlying thought to this analysis possibility provided by crystallization is that crystallization is an extremely sensitive process and is able to visualize not only the material dimension of the sample (e.g. composition), but also immaterial forces (e.g. force-like effects, sample’s vitality). This sensitivity of crystallization encouraged some researchers (both in the past and nowadays) to apply this process also in homeopathic research. Here we present a first summary of the crystallization-based methodologies with focus on these applied in homeopathy. Materials and methods: 177 articles, books, and book chapters on medical crystallization have been collected from scientific databases, university libraries, and the library at the Goetheanum in Dornach/Switzerland. The collected literature was divided into experimental studies and other literature. All methodologies described in the experimental studies were summarized in accordance to following criteria: type of the biological fluid, dilution degree of the fluid in the crystallizing solution, presence and type of reagent in the crystallizing solution, amount of the crystallizing solution per pattern, type of diagnosed disorder, and sensitivity and specificity of the method. Results: The different diagnostic methodologies concerned mainly the crystallization of blood, plasma, serum and saliva, however also tears, urine, cerebrospinal fluid, vaginal mucosa, and sweat were used. The concentration of the sample in the crystallizing solution ranged from 100% to trace amounts. In case of diluted samples additions of salts, amino acids, neurotransmitters were used. The volume of the crystallization solution per pattern ranged from 3 μl to 6ml. The evaporation took place on a glass surface in forms of droplets, smears, or on round dishes (10cm diameter). The methodologies were applied for the diagnoses of cancer (different types and stages, including pre-symptomatic diagnosis), other diseases (e.g. diabetes, hepatitis B&C, multiple sclerosis, tuberculosis, cystic fibrosis, Sjogren’s syndrome, gastro-intestinal disorders, colon polyps, senile dementia), and physiological stages (fertility days and pregnancy in women, differences before and after physical activity). Finally, the mean sensitivity of the methods amounted to 82.1% (from 40.6% to 100%) and the mean specificity to 84.9% (from 62.3% to 96.7%). In overall the experimental methodologies can be divided into following groups: - Evaporation of droplets of an undiluted biological fluid, - Evaporation of droplets of a diluted biological fluid with addition of salts, - Evaporation of droplets of a diluted biological fluid with addition of salts and amino acids, - Evaporation of droplets containing salts, amino acids and neurotransmitters, - Evaporation of droplets of a watery solution of ashes of a biological fluid (spagyric crystallization), - Evaporation of films/smears of an undiluted biological fluid (ferning tests), - Evaporation of larger amounts of crystallizing solution of a strongly diluted biological fluid with addition of copper chloride placed on dishes (Pfeiffer’s crystallization), Within the analyzed literature there were 10 works on crystallization applied for homeopathy. They concerned three different crystallization approaches, all characterized by a low concentration of the analyzed sample in the crystallization solution (from 1% to trace amounts), or even its absence. These methodologies, besides blood, concerned also plant models and crystallization of the homeopathic preparations by themselves (without a biological sample). In these approaches the crystallization took place with or without the addition of a reagent (salt). Summary & Conclusions: Our preliminary results indicate that crystallization based methods might constitute in future valid, non-invasive, and cost-saving tools enabling, inter alia, pre-symptomatic cancer diagnosis. The studies on crystallization based methods applied to homeopathic research point at a great potential of these methodologies for both basic research and possibly also clinical applications and screening tests. The implementation of quality assurance schemes for the assessment of PDO food authenticity is an issue involving manufacturers, traders, retailers and consumers. In this respect, reliable analytical methods are needed to integrate paper-trailing information. The feasibility of distinguishing the Italian Fontina PDO cheese from the generic Fontal cheese was preliminarily evaluated on a set of commercial samples by measuring selected parameters (pH, alkaline phosphatase activity, content of copper, volatiles, extent of proteolysis) related to the different manufacturing processes. The relative profile of free amino acids proved to be a promising tool. A new set of 41 samples of Fontina PDO cheese was collected at representative dairies within the recognized production area and analyzed for free amino acids. A chemometric model of Fontina PDO cheese was built based on the mean content and standard deviation of 15 free amino acids. On this basis, all of the PDO samples were correctly identified, whereas all of the Fontal cheeses were recognized as different cheeses.

AbstractA numerical study is presented that deals with the flow in the mold of a continuous slab caster under the influence of a DC magnetic field (electromagnetic brakes (EMBrs)). The arrangement and geometry investigated here is based on a series of previous experimental studies carried out at the mini-LIMMCAST facility at the Helmholtz-Zentrum Dresden-Rossendorf (HZDR). The magnetic field models a ruler-type EMBr and is installed in the region of the ports of the submerged entry nozzle (SEN). The current article considers magnet field strengths up to 441 mT, corresponding to a Hartmann number of about 600, and takes the electrical conductivity of the solidified shell into account. The numerical model of the turbulent flow under the applied magnetic field is implemented using the open-source CFD package OpenFOAM®. Our numerical results reveal that a growing magnitude of the applied magnetic field may cause a reversal of the flow direction at the meniscus surface, which is related the formation of a “multiroll” flow pattern in the mold. This phenomenon can be explained as a classical magnetohydrodynamics (MHD) effect: (1) the closure of the induced electric current results not primarily in a braking Lorentz force inside the jet but in an acceleration in regions of previously weak velocities, which initiates the formation of an opposite vortex (OV) close to the mean jet; (2) this vortex develops in size at the expense of the main vortex until it reaches the meniscus surface, where it becomes clearly visible. We also show that an acceleration of the meniscus flow must be expected when the applied magnetic field is smaller than a critical value. This acceleration is due to the transfer of kinetic energy from smaller turbulent structures into the mean flow. A further increase in the EMBr intensity leads to the expected damping of the mean flow and, consequently, to a reduction in the size of the upper roll. These investigations show that the Lorentz force cannot be reduced to a simple damping effect; depending on the field strength, its action is found to be topologically complex.

This paper treats the motion of a particle on a vibratory feeder whose track has directional characteristic in repulsive motion, for examples, obliquely bristled track, obliquely sliced track and so on. Under some assumptions, the practical equation for predicting the mean conveying velocity is shown and the relations between conveying condition and the mean conveying velocity are clarified theoretically. These relations are shown in various diagrams. Referring these diagrams, the optimum conveying conditions are discussed also. The theoretical results show that the mean conveying velocity is considerably larger than that of the ordinary feeder. The theoretical results are confirmed by experimental studies.

Endopeptidases containing supplements may digest gluten and reduce the impact on celiac and gluten-sensitive subjects who inadvertently consume gluten. We investigated the relative rate of disappearance of coeliac relevant epitopes in extracts of nine commercial supplements, using two competitive enzyme-linked immunosorbent assays (ELISAs)—Ridascreen (detects QQPFP, QQQFP, LQPFP, and QLPFP) and Gluten-Tec (detects Glia-α20 and PFRPQQPYPQ). All epitopes are destroyed by cleavage after P and Q amino acids. Rates at pH 3.5 and pH 7.0 were measured. These experiments were designed to measure relative rates of epitope digestion not to mimic in vivo digestion. The supplements were: 1 GluteGuard, 2 GlutenBlock, 3 GliadinX, 4 GlutnGo, 5 GlutenRescue, 6 Eat E-Z Gluten+, 7 Glutenease, 8 Glutezyme, and 9 Gluten Digest. The mean initial rate and half-lives of epitope digestion were deduced and extrapolated to rates at the recommended dose of one supplement in a fasting stomach volume. At pH 7, supplement 1 was the fastest acting of the supplements, with Ridascreen ELISA, more than twice as fast as the next fastest supplements, 5, 6, 7, and 8. Supplements 2, 3, and 4 showed little activity at pH 7.0. Supplement 1 was also the fastest acting at pH 7 with Gluten-Tec ELISA, more than three times the rate for supplements 2 and 3, with supplements 4–9 showing minimal activity. At pH 3.5, supplement 1 acted more than five times as fast as the next fastest supplements, 2 and 3, when measured by Ridascreen, but supplements 2 and 3 were over two times faster than supplement 1 when measured by Gluten-Tec. Supplements 4–9 demonstrated minimal activity at pH 3.5 with either ELISA. Supplement 1 most rapidly digested the key immuno-reactive gluten epitopes identified by the R5 antibody in the Codex-approved competitive Ridascreen ELISA method and associated with the pathology of celiac disease.

Thrombocytopenia is a common occurrence (20%) in sick neonates, but the causes have not been well studied. In this report we demonstrate that thrombocytopenia in the neonate is characterized by increased platelet destruction as shown by shortened homologous 111In-oxine-labeled platelet life spans. Thirty-one prospectively studied thrombocytopenic neonates were investigated by measuring the 111In-labeled platelet life span, platelet-associated IgG (PAIgG), and coagulation screening tests. In every infant, the thrombocytopenia was shown to have a destructive component since the mean platelet life span was significantly shortened to 65 +/- 6 (mean +/- SEM) hours with a range of one to 128 hours compared with adult values (212 +/- 8; range, 140 to 260; gamma function analysis). The platelet survival was directly related to the lowest platelet count and inversely related to both the highest mean platelet volume and duration of the thrombocytopenia. In 22 infants the percent recovery of the radiolabeled platelets was less than 50%, which suggested that increased sequestration also contributed to the thrombocytopenia. Infants with laboratory evidence of disseminated intravascular coagulation (n = 8) or immune platelet destruction evidenced by elevated levels of PAIgG (n = 13) had even shorter platelet survivals and a more severe thrombocytopenia compared with the ten infants in whom an underlying cause for the thrombocytopenia was not apparent. Full-body scintigraphic images obtained in 11 infants showed an increased uptake in the spleen and liver, with a spleen-to- liver ratio of 3:1. This study indicates that thrombocytopenia in sick neonates is primarily destructive, with a subgroup having evidence of increased platelet sequestration.

BACKGROUND Globally, the uptake of health screening is suboptimal, especially in men and those of younger age. In view of the increasing internet access and mobile phone ownership, ScreenMen, a mobile Web app, was developed to improve health screening uptake in men. OBJECTIVE This study aimed to evaluate the utility and usability of ScreenMen. METHODS This study used both qualitative and quantitative methods. Healthy men working in a banking institution were recruited to participate in this study. They were purposively sampled according to job position, age, education level, and screening status. Men were asked to use ScreenMen independently while the screen activities were being recorded. Once completed, retrospective think aloud with playback was conducted with men to obtain their feedback. They were asked to answer the System Usability Scale (SUS). Intention to undergo screening pre- and postintervention was also measured. Qualitative data were analyzed using a framework approach followed by thematic analysis. For quantitative data, the mean SUS score was calculated and change in intention to screening was analyzed using McNemar test. RESULTS In total, 24 men participated in this study. On the basis of the qualitative data, men found ScreenMen useful as they could learn more about their health risks and screening. They found ScreenMen convenient to use, which might trigger men to undergo screening. In terms of usability, men thought that ScreenMen was user-friendly and easy to understand. The key revision done on utility was the addition of a reminder function, whereas for usability, the revisions done were in terms of attracting and gaining users’ trust, improving learnability, and making ScreenMen usable to all types of users. To attract men to use it, ScreenMen was introduced to users in terms of improving health instead of going for screening. Another important revision made was emphasizing the screening tests the users do not need, instead of just informing them about the screening tests they need. A Quick Assessment Mode was also added for users with limited attention span. The quantitative data showed that 8 out of 23 men (35%) planned to attend screening earlier than intended after using the ScreenMen. Furthermore, 4 out of 12 (33%) men who were in the precontemplation stage changed to either contemplation or preparation stage after using ScreenMen with P=.13. In terms of usability, the mean SUS score of 76.4 (SD 7.72) indicated that ScreenMen had good usability. CONCLUSIONS This study showed that ScreenMen was acceptable to men in terms of its utility and usability. The preliminary data suggested that ScreenMen might increase men’s intention to undergo screening. This paper also presented key lessons learned from the beta testing, which is useful for public health experts and researchers when developing a user-centered mobile Web app.

Background: Brain edema is a common phenomenon after traumatic brain injury (TBI) resulting in increased intracranial pressure and subsequent neurological deterioration. Experimental studies have proven that brain edema is biphasic (cytotoxic followed by vasogenic). Till date, all studies, including the corticosteroid randomization after significant head injury (HI) trial, have used high-dose steroids in the acute period during which the edema is essentially cytotoxic in nature. No clinical data exist pertaining to delayed cerebral edema (vasogenic) and steroids. Methods: Patients who had received steroids for delayed cerebral edema after TBI were retrospectively analyzed over a 2-year period. Steroid dose, timing of steroid prescription, time to improvement of symptoms, and complications were noted. Results: There were six males and three females. Mean age was 41.1 years. There were no severe HI cases. All subjects had cerebral contusions on imaging. Dexamethasone was the preferred steroid starting with 12 mg/day and tapered in 5–7 days. The mean interval to steroid administration after trauma was 7 days. The mean duration of steroid prescription was 6.3 days. All patients had complete symptomatic improvement. The mean time to symptom resolution was 3.8 days. No patients experienced any complications pertinent to steroid usage. Conclusion: This is the first study to document efficacy of steroids for delayed cerebral edema after TBI, at least in mild/moderate head injuries. The timing of steroid usage and dose of steroids is key aspects that might determine its efficacy in TBI which was the drawbacks of the previous studies. Future prospective trials with the above factors in consideration may confirm/refute above findings.

An important issue in the treatment of vaginitis is the amount of time the drug remains on the vaginal mucosa. If the contact time is too short, the drug cannot work effectively to ensure the correct pH in the vaginal environment. This study evaluated formulations of globules containing sodium alginate, lactic acid and chitosan with different pH and rheological properties. The experimental studies revealed that it is possible to produce a preparation with optimal pharmaceutical and application properties. The use of an appropriate ratio of lactic acid to chitosan in the complex and the appropriate concentration of sodium alginate produces a preparation with excellent properties to coat the surface of the vaginal mucosa.

SUMMARYExperimental studies on the survival of Fasciola hepatica miracidia show no evidence that miracidial mortality varies with the pH of the medium, at least in the range 6·0–8·0. On the other hand, miracidial mortality is shown to vary with both the temperature of the medium and the age of the larvae. The mean expected life-span of the miracidium decreases from about 35 h at 6°C to about 6° h at 25° C. The Gompertz survival function provides a good description of the miracidial survivorship curves over the range of temperatures used, and we describe, a maximum likelihood method of estimating the mean values of the parameters of this function, together with their approximate 95% confidence limits.
