To characterize the elderly with cognitive déficit, and family caregivers who live in the urban communit,y and to identify the caregiver burden. METHODS: An observational, cross-sectional study, conducted between January and July 2009. Seventy elderly (aged 65 and older) were assessed for cognitive deficit using the Mini Mental State Examination (MMSE); their 70 caregivers were evaluated by the Zarit Burden Scale (ZBS) and the Self Reporting Questionnaire (SRQ). The data were entered and validated by double entry in the MS Excel Program, and were analyzed using SPSS software, version 15.0. RESULTS: Of the caregivers, 47.1% were children. We obtained a mean for the ZBS of 30.3 (± 17.3) and 6.2 (± 3.2) for the SRQ, p <0.05, correlating lower scores of the MMSE with high scores on ZBS and SRQ. CONCLUSIONS: Cognitive deficit of the elderly was a predictive factor for burden and emotional distress of caregivers. Knowing the reduction of cognitive capacity related to caregiver burden is essential for preserving caregiver health.

Abstract Introduction The prevalence of various diseases in the elderly has increased the use of drugs, which is coupled with age-related physiological changes and places the elderly at risk for multiple side effects. The present study aimed to determine awareness, attitudes and practices of elderly towards self-medication. Materials and Methods The sample included 300 participants over 60 years under any of the health centers in urban and rural areas of Abadeh city, who were selected based on stratified random sampling method. In order to collect data, the questionnaire including demographic information, awareness, attitude and performance of the elderly and the checklist of drug intractable consumption. Results Based on the results, 76% of the elderly reported drug consumption. Regarding lifestyle, the average score of the elderly’s awareness for the elderly who lived with and without families were 78.6±14.2 and 69.2±13.6, respectively. In other words, a significant difference was observed between these two groups. In addition, there was a significant between the elderly’s performance score with and without family members about the self-medication (p=0.001). Conclusion Considering the increase in self-medication among the elderly, adherence to prescription criteria for drug among this age group is recommended. Finally, designing and implementing various educational programs in health education center is necessary for the elderly and their families for the purpose of improving awareness and practice.

AbstractObjectiveTo estimate the prevalence of malnutrition among free-living elderly in a rural population of south India.DesignCross-sectional study. Nutritional status was assessed using the Mini Nutritional Assessment (MNA) questionnaire, which is an eighteen-item nutritional screening instrument used in the elderly.SettingKaniyambadi block, a rural development block in the state of Tamil Nadu, south India.SubjectsCommunity-dwelling elderly (aged 60 years and above).ResultsAs evaluated by the MNA, 14 % of the 227 subjects were malnourished and 49 % were at risk of malnourishment. No significant difference was found between men and women. The majority of the elderly were living with their children, had no income and consumed three meals per day. Older age (P < 0·001), decreased food intake (P < 0·001) and consuming fewer meals (P < 0·001) were independently associated with lower MNA scores.ConclusionsMore than 60 % of the subjects had low MNA scores (<23·5) indicating that deficient protein–energy intake is common among rural elderly of south India and requires more attention.

The age of patients is a controversial issue in admission to intensive care unit (ICU). The aim of this study was to compare severity and nursing workload of elderly patients with 60-69, 70-79, and e"80 years of age and to identify predictors of nursing workload in elderly patients. A cross sectional study was performed with a sample of 71 elderly patients admitted to three ICU in the city of Sao Paulo, Brazil from October to November 2004. Data were prospectively collected using Nursing Activities Score (NAS) and Simplified Acute Physiology Score II (SAPS II). There was no significant difference in nursing workload among the elderly patients age subgroups (p=0.84). Multiple regression analysis indicated that the independent risk factors of high nursing workload were severity, age e"70 years, and to be a surgical ICU patient. Age as an isolated factor should not be discriminative for elderly patients admission to ICU.

Abstract Background Peripheral vestibular vertigo is a common cause of vertigo especially in the elderly. The neutrophil to lymphocyte ratio (NLR) is a rapid and cost-effective inflammatory marker that has been assessed previously in peripheral vestibular disorders. However, its relation to the severity of peripheral vertigo has not been previously investigated. The aim of this study is to assess the levels of NLR in peripheral vestibular vertigo of various severity categories. This was a cross-sectional study at the Wahidin Sudirohusodo Hospital and Hasanuddin University Teaching Hospital in Makassar. The NLR was measured among subjects with periphel vestibular vertigo with severity categorised using the Dizziness Handicap Inventory (DHI) questionnaire. Results A total of 39 participants with peripheral vestibular vertigo were recruited. A statistically significant difference was found (p = 0.002, Kruskal–Wallis ANOVA) between the mean NLR for the mild, moderate, and severe DHI categories were 2.47 ± 1.66, 2.64 ± 0.96, and 5.15 ± 2.59 respectively. Conclusion A significant difference in the NLR was found between the three different vertigo severity groups, wherein NLR values rise with increase in vertigo severity. This warrants further exploration on the role of inflammatory biomarkers in vertigo pathophysiology and clinical assessment.

Background: Some variables, such as age, gender, regional and dialectical differences influence speech tempo. Men and younger individuals speak faster than women and the elderly. Therefore, these variations should be considered when assessing speaking rate. Objectives: Since different accents influence speaking rates and there is no previous study investigated speech tempo with respect to regional and accent differences in Iran, and given that the elderly are more prone to problems influencing speaking rate, the present study was done to compare speech tempo and speaking rate in two different accents, namely Tehrani and Semnani, and to investigate some related factors. Methods: This cross-sectional study was performed on 200 elderly women selected via convenience sampling method. Speech tempo, speaking rate, verbal fluency, and cognition scores were compared using an independent-samples t-test. Pearson’s correlation coefficient test was used to assess correlations between speaking rate and level of education, Montreal cognitive assessment (MoCA), and verbal fluency scores. Results: No significant difference was found in speech tempo between the studied accents (P = 0.13). Speaking rate was significantly slower in the Tehrani accent than the Semnani one (P = 0.04). The Tehrani elderly obtained significantly less scores in verbal fluency and MoCA ((P ≤ 0.001) and (P = 0.04), respectively. In both groups, speaking rate had a significant correlation with verbal fluency and MoCA scores but not with level of education. Conclusions: Although, our results showed no difference in speech tempo between the studied accents, the Tehrani elderly unexpectedly spoke more slowly meaning that they paused more while speaking. There was a relationship between faster speaking, better verbal fluency, and cognitive performance.

Objective: To study the age related radiological finding in pulmonary tuberculosis. Study design: A cross sectional study. Place and duration of study: From January 2009 to December 2009 pulmonary department of Bahawal Victoria Hospital Bahawal Pur. Patients and method: The cases diagnosed as pulmonary tuberculosis of either gender above age of 12 years were included in the study. Patients suffering from extra pulmonary tuberculosis, treatment failure, relapse, drug resistant tuberculosis and HIV sero-positive patients were excluded from the study. Patients were divided into groups according to the age. Group 1 consist of patients having age ≥ 50years while group 2 consist of patients having <50 years. Data was recorded on the Proforma and was analyzed statistically on SPSS 11. Results: this study consists of 106 patients and divided into two groups. It has been found that apical zone of lung involvement was more common in patients younger than 50 years while involvement of lower zone was more common in patients with age ≥50years. No significant difference was found regarding the involvement of middle zone, multiple zones and the type of lesions as the p-value was >0.05. Conclusions: the elderly patients with pulmonary tuberculosis have predominant involvement of lower zones. So, lower zone involvement of radiological lesions should be evaluated for pulmonary tuberculosis to start the treatment earlier and to minimize the risk of missing the diagnosis.

Anxiety, stress, and depression are the three most common negative emotional constructs found in the elderly. Evidences available worldwide about how psychological problem could be resulted in mental disorder, and there is significant difference in the context of living in the nursing home and in community setting. This study aimed to determine the best predictor of anxiety, stress, and depression in elderly living in the nursing home, useful for future modification and intervention development. This cross-sectional study involved 145 elderly in a private nursing home in Surabaya, Indonesia. HARS, SPST-20, and GDS were used in data collection. Linear regression and one way ANOVA tests were used in data analysis (α<0.05). Results showed that mostly in old individuals, mild anxiety and stress, and undepressed state were found. Sensory problems and concentration difficulties were the best predictor of anxiety and stress respectively, which were accounted for 61.2% and 65.6% variances of anxiety and stress in nursing home residents respectively. Spirits, life energy, happiness, and feeling wonderful to be alive could not predict depression significantly. Feeling inferior to others is the best predictor of depression, which was accounted for 25.9% variance of depression in this population. Low self-esteem leads to depression in nursing home residents.
