ABSTRACT Objectives: to describe home care practices performed by family members for maintaining the life of children with gastrostomy. Methods: qualitative research using the Sensitive Creative method, with the Criatividade e Sensibilidade Corpo Saber [Corpo Saber Creativity and Sensitivity] dynamics. The participation included ten family caregivers of children with gastrostomy. The outpatient clinic of a federal hospital in Rio de Janeiro was the setting. Lexical analysis was used through the IRaMuTeQ® software. Results: Theme 1, entitled “The care for maintaining life performed by family members of children with gastrostomy at home”, was analyzed, comprising three classes: “The gastrostomy tube home care routine”; “The care with the gastrostomy/stoma”; and “Food and medication home care routine of children with gastrostomy”. Final Considerations: family caregivers used strategies to maintain the device and acquired new knowledge in this field, and in nursing competence, regarding stoma care, food administration, medication, and syringes.

BackgroundCritical care specialty deals with the complex needs of critically ill patients. Nurses who provide critical care are expected to possess the appropriate knowledge and skills required for the care of critically ill patients. The aim of this study was to assess the effect of an educational programme on the competence of critical care nurses at two tertiary hospitals in Lilongwe and Blantyre, Malawi.MethodsA quantitative pre- and post-test design was applied. The training programme was delivered to nurses (n = 41) who worked in intensive care and adult high dependency units at two tertiary hospitals. The effect of the training was assessed through participants’ self-assessment of competence on the Intensive and Critical Care Nursing Competence Scale and a list of 10 additional competencies before and after the training.ResultsThe participants’ scores on the Intensive and Critical Care Nursing Competence Scale before the training, M = 608.2, SD = 59.6 increased significantly after the training, M = 684.7, SD = 29.7, p <.0001 (two-tailed). Similarly, there was a significant increase in the participants’ scores on the additional competencies after the training, p <.0001 (two-tailed). ConclusionThe programme could be used for upskilling nurses in critical care settings in Malawi and other developing countries with a similar context.

Abstract Objective The number of older patients with dementia hospitalised in acute care hospitals increased and these patients underwent physical restrictions leading to a degeneration of essential mental and physical function. The dementia nursing competence e-learning program with audio-visual materials has been developed in acute care hospitals. Methods An application form that explained the research was distributed to 1,944 registered nurses from seven hospitals, and 110 people applied. Nurses used an e-learning program for a month in May and practiced applying the knowledge learned from programs June through November 2020. The nurses completed a questionnaire survey at four periods: first (before program/baseline), second (after program), third (three months later), and fourth (six months later). In the second, third, and fourth periods, ‘Technical knowledge of the dementia nursing’ and ‘dementia nursing intervention’ were significantly improved as compared with the first. In the fourth period, ‘confidence of reduce of physical restriction’ showed significant improvement compared to the first. In ‘Ethical sensitivity scale of nurses’, the first of the four sub-scales significantly increased as compared with the first period. In ‘Self-assessment Scale of Nursing Practice for Elderly Patients with Cognitive Impairment with the Aim of Person-centred Care in Acute Care Hospitals’, the fourth sub-scale showed significant improvement compared to the first. In the ‘Personhood’ sub- scale of the Japanese version of Approaches to Dementia Questionnaire, the fourth period showed a significant increase compared to the first. Conclusion The results suggest that this program was effective and led to reduced physical restrictions in nursing practice.

Evaluating nursing competence is challenging and among many instruments the Nurse Competence Scale is one of the most used. This integrated literature review aimed to describe how ontological and contextual nursing competence becomes evident in evaluations done with it and the value of using it for professional development in nursing. The starting point was a former systematic review and additional searches were carried out using electronic databases with keywords and Boolean operators. The search followed the PRISMA search strategy and the articles were appraised against the JBI Critical Appraisal Checklist. Thirty-four original research articles published between January 2004 and April 2020 were included. The data were displayed and analyzed descriptively. Results showed that the Nurse Competence Scale covers both ontological and contextual competence, that the competence profiles vary in different cultures, cohorts and contexts, but that it is suitable for evaluating and following up competence development in nursing.

In response to the cancellation of in-person objective structured clinical examinations (OSCEs) prompted by confinement due to the COVID-19 pandemic, we designed a solution to adapt our traditional OSCEs to this new reality in nursing education. We implemented an innovative teaching proposal based on high-fidelity virtual OSCEs with standardized patients. The purposes of our study were to describe this innovative teaching proposal and compare nursing competence acquisition in final year nursing students through virtual and in-person OSCE modalities. The study included 234 undergraduate students: 123 students were assessed through high-fidelity virtual OSCEs during May 2020, whereas 111 students were assessed through in-person OSCEs during May 2019. The structure of OSCEs, including its stations, clinical simulated scenarios, and checklists, was the same in both OSCE modalities. The effect size of the differences among the competence categories of checklists, including their total scores, was small. Regarding our virtual OSCEs was similarly successful to in-person OSCEs, this online format was found to be useful, feasible, and cost-saving when in-person OSCE was not possible. Therefore, high-fidelity virtual OSCEs with standardized patients could be considered as another choice of OSCE not only in the current COVID-19 pandemic but could also be extended to normal situations, even post-pandemic.

Bencana yang sering dan mengancam banyak orang di dunia, menuntut kebutuhan untuk mempersiapkan perawat bencana. Perawat merupakan salah satu tenaga medis berkontribusi sangat besar dalam penanganan dan perawatan pada korban bencana. Mahasiswa keperawatan perlu dipersiapkan untuk memenuhi kebutuhan tersebut. Sehingga, mahasiswa keperawatan harus memiliki persepsi yang baik terhadap kompetensi dasar dalam memberikan respon kegawatan ketika terjadi bencana. Penulis bermaksud melakukan studi terintegrasi  dengan tujuan meninjau kompetensi yang diperlukan sebagai calon tenaga kesehatan dalam merespon kegawatdaruratan bencana. Metode yang digunakan adalah literature review dengan menggunakan kata kunci disaster nursing competence dan nursing student. Artikel didapatkan dari database PubMed, Sciencedirect, CINAHL, Sagebud, Garuda, dan grey literature. Kriteria inklusi artikel berbahasa Inggris dan Indonesia tahun terbit 2010-2020, selanjutnya dianalisis dengan pendekatan deskriptif. Kompetensi dalam meningkatkan respon keperawatan dalam bagi mahasiswa keperawatan diantaranya perlu diseminasi peningkatan pengetahuan dan keterampilan, dan sikap keperawatan bencana. Kompetensi inti yang harus dimiliki oleh mahasiswa keperawatan diantaranya sikap, penilaian, penyediaan, manajemen, dan pengembangan profesional keperawatan bencana.Kompetensi bagi mahasiswa keperawatan berupa pengetahuan, keterampilan, dan sikap mampu meningkatkan respon gawat darurat terhadap bencana.
