Interlocking box jejunostomy: a new technique for enteral feeding

A new jejunostomy tube placement technique is described and compared to traditional methods. The interlocking box technique was compared to a simple purse-string with jejunopexy and an inverting serosal tunnel (Witzel technique) with jejunopexy. Procedures performed in fresh canine cadavers were fluid pressure tested following tube removal. Mean+/-standard deviation (SD) intraluminal pressure necessary to induce leakage at the jejunopexy site of the interlocking box group (87.63+/-40.56 cm H2O) was significantly greater (p less than 0.001) than the simple purse-string (43.17+/-31.69 cm H2O) and serosal tunnel (46.33+/-23.60 cm H2O) groups. Significant differences were not identified between the latter groups. The interlocking box technique resisted leakage following acute removal better than conventional techniques and should be tested clinically.

Salivary Bypass Tube is an important tool to prevent or treat some complications of laryngeal and hypopharyngeal surgery and its placement may prove difficult. In this article, we propose a new technique to simplify its management by using an Oral/Nasal Tracheal Tube Cuffed-Reinforced that allowed us to reduce operating times, complications related to prolonged general anesthesia, and the traumas on the tissues incurred during the forced positioning of the device with standard techniques.

A new percutaneous insertion technique for esophageal feeding tubes in cats is presented. The technique has been successfully applied in 12 feline patients. The placement technique is relatively simple, takes approximately five minutes to perform, and requires a scalpel blade, a curved hemostat, and an applicator for the insertion of the feeding tube. In contrast to other esophageal tube placement techniques, the tube is inserted into the definitive aboral position in a one-step procedure. Because of its shoehorn shape, the applicator allows the tube to be inserted into the esophagus safely and precisely. Placement of the tube in the midcervical area does not interfere with the function of the pharynx and avoids having the animal irritated by the presence of the tube. The chosen diameter of the tube is large enough to permit feeding of diluted, blended, commercial canned food. For the patients of this study, feeding was started after recovery from anesthesia, and tubes were removed without complications once the animals had started to eat voluntarily.

AbstractA connectionist model for decision support was constructed out of several back-propagation modules. Manifestations serve as input to the model; they may be real-valued, and the confidence in their measurement may be specified. The model produces as its output the posterior probability of disease. The model was trained on 1,000 cases taken from a simulated underlying population with three conditionally independent manifestations. The first manifestation had a linear relationship between value and posterior probability of disease, the second had a stepped relationship, and the third was normally distributed. An independent test set of 30,000 cases showed that the model was better able to estimate the posterior probability of disease (the standard deviation of residuals was 0.046, with a 95% confidence interval of 0.046-0.047) than a model constructed using logistic regression (with a standard deviation of residuals of 0.062, with a 95% confidence interval of 0.062-0.063). The model fitted the normal and stepped manifestations better than the linear one. It accommodated intermediate levels of confidence well.

Auto white balance (AWB) is an important function of digital camera. The purpose of white balance is to adjust the image to make it look like taken under standard light conditions. We present a new technique to detect the reference white point of image in this paper. This technique detects the white point of image by using dynamic threshold method, thus making it more flexible and more applicable compared to other algorithms. We test 50 images which were taken under different light sources, and find that this algorithm is better than or comparable to other algorithms both in subjective and objective aspects. At the same time, this algorithm has low complexity, and it can be easily applied to hardware implementation.

Abstrak: Penelitian ini bertujuan untuk mengetahui apakah pemahaman konsep matematika siswa yang belajar dengan menggunakan model pembelajaran kooperatif tipe jigsaw lebih baik daripada pemahaman konsep matematika siswa yang belajar dengan menggunakan model pembelajaran konvensional di Kelas IX MTs Negeri 1 Kota Palu. Hipotesis penelitian ini adalah pemahaman konsep matematika siswa yang belajar dengan menggunakan model pembelajaran kooperatif tipe jigsaw lebih baik daripada pemahaman konsep matematika siswa yang belajar dengan menggunakan pembelajaran konvensional. Jenis penelitian ini adalah eksperimen semu. Desain penelitian ini adalah pre-post test control design. Populasi penelitian ini adalah seluruh siswa kelas IX MTs Negeri 1 Kota Palu dengan teknik pengambilan sampel cluster random sampling. Nilai rata-rata siswa kelas eksperimen 47 dan standar deviasi 17,04 sedangkan nilai rata-rata siswa kelas kontrol 40 dan standar deviasi 13,40. Data yang diperoleh berdistribusi normal dan homogen, maka pengujian hipotesis menggunakan teknis statistik parametris yaitu uji t. Hasil pengujian hipotesis diperoleh bahwa nilai ????tabel = 1,67 dan ????hitung = 2,96 sehingga H1 diterima dan H0 ditolak. Hal ini menunjukkan bahwa pemahaman konsep matematika siswa yang belajar dengan menggunakan model pembelajaran kooperatif tipe jigsaw lebih baik daripada pemahaman konsep matematika siswa yang belajar dengan menggunakan model pembelajaran konvensional di kelas IX MTsN 1 Kota Palu. The type of research is a quasi experiment. Design of research is pre-post control design. The population of this research is all students of class IX  MTs Negeri 1 Kota Palu with samples taken by cluster random sampling technique. The average score of the experiment class students is 47 and standard deviation is 17.04 and the average score of the control class students is 40 and standard deviation is 13.40. The data is normal distribution and homogen, hypothesis test using t test. Results of hypothesis test obtained value ????table = 1.67 and ????count = 2.96 it means H1 is accepted and H0 is rejected. This indicated that mathematics concept understanding students who learn by using jigsaw cooperative learning model have better than mathematics concept understanding students who learn by using conventional learning model in class IX MTsN 1 Kota Palu. Abstract We consider the median solution of the Newsvendor Problem. Some properties of such a solution are shown through a theoretical analysis and a numerical experiment. Sometimes, though not often, median solution may be better than solutions maximizing expected profit, or maximizing minimum possible, over distribution with the same average and standard deviation, expected profit, according to some criteria. We discuss the practical suitability of the objective function set and the solution derived, for the Newsvendor Problem, and other such random optimization problems.

This research describes the differences of student’s learning with the application of cooperative learning type make a match in the electrolyte and non-electrolyte at the X grade student’s of the Madrasah Aliya DDI Lonja. This research used quasi experimental with non randomized pretest-posttest control group design. This research was conducted with two groups: XA as the experimental (n=21) XB as the control group (n=18). The data findings means score of experimental group (X1) is 70,5 with the standard deviation 9,6 and the control group (X2) is 62.0 with the standard deviation is 6. Based on the results, the researcher concludes cooperative learning type make a match has better than control class that used conventional learning model.

Objective. To compare transpyloric feeding tube placement using a pH-assisted placement technique versus a standard placement technique in pediatric patients requiring enteral nutrition. Methods. Critically ill children younger than 4 years were prospectively and randomly assigned to either a pH-assisted or a standard feeding tube placement group. Identical pH-assisted feeding tubes were used in both groups; however, feeding tubes in the standard group were not attached to a portable pH meter. Successful transpyloric placement was confirmed by radiography before beginning feedings. If placement was not successful, a second placement attempt was made after metoclopramide administration. Information regarding tube placement success, number of radiographs, time to initiaction of feedings, and daily caloric intake was collected. A cost comparison between the two groups was performed. Results. Thirty-four patients were enrolled in the pH-assisted group, and 34 were enrolled in the standard feeding tube group. Ninety-seven percent of patients in the pH-assisted group had successful placement after the first attempt, compared with 53% of patients in the standard group. The average time to successful placement of pH-assisted feeding tubes was 6 minutes. All patients in the pH-assisted group had successful placement after the second attempt, compared with 78% of patients in the standard group. A pH of greater than 5.6 accurately predicted transpyloric placement in 97% (33 of 34) of individuals in the pH-assisted group. Children in the pH-assisted group required significantly fewer radiographs than those in the standard group. Hospital costs were $114 per patient in the pH-assisted group and $135 per patient in the standard group. Conclusions. Our findings indicate that bedside transpyloric placement of pH-assisted feeding tubes can be accomplished rapidly and with a high success rate. This method is associated with decreased radiation exposure and economic savings when compared with a standard placement technique.
