As dental implant treatment has become a part of mainstream dental therapy, it is imperative to implement dental implant maintenance guidelines to achieve the long-term success of implant prostheses. Earlier, the success of a dental implant was mainly focused on the surgical phase to achieve good primary stability, with time, this belief has taken a major paradigm shift towards implementing and ensuring a periodic recall and following a maintenance phase for dental implants to achieve long-term success. As the dental team strives to attain and maintain the long-term success of implant prostheses, the patient should also recognize that their contribution towards the success of implant prostheses is also equally indispensable. This chapter highlights the importance of maintaining oral hygiene in implant rehabilitated patients and enumerates the implant maintenance protocol to be followed along with the different in-home and in-office procedures which can be implemented to achieve long-term success of the implant and peri-implant structures.

This article provides a narrative review of the use of dental implants in patients with periodontitis. Using clinical examples where possible, consideration is given to the survival and success of implants, peri-implantitis, comparison of periodontally compromised teeth to implants and to treatment planning to help achieve favourable outcomes. The challenges associated with restoring an edentulous arch or partially dentate dentition with implants where significant alveolar atrophy has occurred can be considerable. Compromised outcomes may be commonplace. Dental implant treatment is more likely to be successful for those patients who attain and maintain excellent plaque control. Professional support should focus on managing underlying periodontitis prior to commencing implant therapy and providing long term, regular supportive periodontal care upon completion of treatment.

The major risk of dental implant treatment in the mandible is represented by the mandibular canal. Precise location of the mandibular canal is essential for oral rehabilitation with dental implants. The aim of this study was to analyze the topography of the mandibular canal in order to increase the long-term performance of oral rehabilitation with dental implants in „poor areas“. The topography of the mandibular canal was statistically studied on 11 human mandibles. The results obtained show similarities but also differences with data reported in other specialized references.

Adequate bone remodeling may be a primary parameter for long-term successful complication-free dental implant treatment. A 1.8-mm osseous thickness around dental implants is thought to be the minimum thickness for adequate vasculature for osteocyte nutrition and function. A dental implant does not provide progenitor cells or angiogenic or osteogenic factors. Thus, the surrounding bone may need to have a 1.8-mm thickness to accommodate the vasculature necessary for nutrients for appropriate remodeling. Additionally, the 1.8-mm dimension may provide for mechanical load resistance. There is no evidence to illustrate the physiologic need for the 1.8-mm dimension. This dimension requirement is based on clinical outcome observations. Basic science research for bone survival around dental implants is needed.

Background/Aim. Flapless implant surgery has become very important issue during recent years, mostly thanks to computerization of dentistry and software planning of dental implants placements. The aim of this study was to compare flap and flapless surgical techniques for implant placement through radiographic and radiofrequency analyses. Methods. The experiment was made in five domestic pigs. Nine weeks following domestic pigs teeth extraction, implants were placed, on the right side using surgical technique flap, and flapless on the left side. Digital dental Xrays were applied to determine primary dental implant stability quality (ISQ). At certain intervals, not later than three months, the experimental animals were sacrificed, and just before it, control X-rays were applied to measure dental implants stability. Results. Radiographic analysis showed that peri-implant bone resorption in the first 4 weeks following placement implants with flap and flapless surgical techniques was negligible. After the 3 months, mean value of peri-implant bone resorption of the implants placed using flap technique was 1.86 mm, and of those placed using flapless technique was 1.13 mm. In relation to the primary dental implant stability in the first and second week there was an expected decrease in ISQ values, but it was less expressed in the dental implants placed using the flapless technique. In the third week the ISQ values were increased in the dental implants placed by using both techniques, but the increase in flapless implant placement was higher (7.4 ISQ) than in flap implant placement (1.5 ISQ). The upward trend continued in a 4- week period, and after 3 months the dental implant stability values in the implants placed using flap technique were higher than the primary stability for 7.1 ISQ, and in the implants placed using flapless technique were higher comparing to the primary stability for 10.1 ISQ units. Conclusion. Based on the results of radiographic and resonance frequency analyses it can be concluded that the flapless technique in surgical implants placemat, leads to better results.

Dental implant treatment is a highly predictable therapy, but when potentially lethal symptoms or complications occur, dentists must remove the implant fixture. Recently, reports on antiresorptive agent-related osteonecrosis of the jaw have increased in the field of dental implants, although the relationship between dental implant treatment and antiresorptive agents remains unclear. Here, we report a case of antiresorptive agent-related osteonecrosis of the jaw that developed after dental implant removal. A 67-year-old Japanese woman with a medical history of osteoporosis and 7 years of oral bisphosphonate treatment was referred to our hospital with a chief complaint of painful right mandibular bone exposure. A family dentist removed the dental implants from the right mandible using a trephine drill without flap elevation in August 2016. However, the healing was impaired; she was referred to our hospital 3 months after the procedure. We performed a sequestrectomy of the mandible under general anesthesia. In conclusion, this patient's course has two important implications: First, the removal of dental implants from patients who are prescribed oral bisphosphonates for long durations can cause antiresorptive agent-related osteonecrosis of the jaw. Second, meticulous procedures are required to prevent and treat the development of antiresorptive agent-related osteonecrosis of the jaw after dental implant removal.

AbstractSmall diameter (mini) dental implants have become more popular in recent years as alternatives to classical implant treatment in clinical cases with critical bony situations. However, an in-depth scientific analysis of the mechanical and biomechanical effects of small diameter implants has not yet been published. The aim of the present study was to investigate experimentally different commercial mini implants by measuring their displacements under immediate loading. Twelve commercially available mini implants were measured. Implants were inserted into porcine mandibular segments and loaded by means of a predefined displacement of 0.5 mm of the loading system. The implants were loaded at an angle of 30° to the implant long axis using the self-developed biomechanical hexapod measurement system. Implant displacements were registered. The experimental results were compared to the numerical ones from a previous study. Measured implant displacements were within the range of 39–194 μm. A large variation in the displacements was obtained among the different implant systems due to the different designs and thread profiles. Comparing experimental and numerical results, the displacements that were obtained numerically were within the range of 79–347 μm. The different commercial mini implants showed acceptable primary stability and could be loaded immediately after their insertion.

This study is to investigate the effect of bisphosphonates on the osseointegration of dental implants in a rabbit model. Twenty female New Zealand White rabbits were equally assigned into control and experiment groups which received saline or zoledronic acid treatment 4 weeks prior to surgery. Titanium dental implant was placed on the calvarial bone. Zoledronic acid or saline treatment continued after surgery for 4 weeks (short-term subgroup) or 8 weeks (long-term subgroup) until sacrifice. Three different fluorochrome labeling solutions were administrated for assessing bone growth rates. Samples of the calvarial bone and mandible were subjected to microcomputed tomography (micro-CT), confocal microscope, and histology analysis. Zoledronic acid treatment significantly reduced bone growth rates in the calvarial bone, but had no significant influence in bone mineral density and trabecular microarchitecture. Significantly lower bone-to-implant contact ratios were found in zoledronic acid-treated animals compared to controls at week 4 but not at week 8. Oncologic dose zoledronic acid suppresses the bone growth rates of the calvarial bone; ZA may have an adverse effect on osseointegration of dental implant in short term, but this effect tends to diminish in long term.

Dental implants are made of medical titanium and perfectly fit into human bone tissue; the dental implant can last a lifetime. Not beeing living organic structures, there are no nerve endings, neither at the implant level nor at the artificial crown level. The use of quality implants by an experienced implantologist, assisted by advanced technology, transforms the treatment with dental implants into the medical-surgical act with the highest success rate among dental and even medical treatments.Through the complete replacement of the tooth, including the root, can artificially reproduce the function of the natural tooth, with a strong and stable base. The implant crown, made of aesthetic materials (porcelain, zirconium) and anchored to it by means of the prosthetic abutment, will be surrounded by a healthy and aesthetic gum. Especially if the prosthetic abutment (the connecting element between the implant itself and the artificial crown) will be made of zirconium - natural light will cross ceramic layers, similar to enamel and dentine, offering a white of envy and glitter to the smile. Around the porcelain crowns (whole ceramics or zirconia ceramics) the gingiva will conform healthily, without the slightest sign of inflammation. These elements, the white of the teeth and the pink of the gums define the concept of dental aesthetics. The main problems that diabetic patients may encounter, are gingival inflammation and periodontal disease, dental mobility and tooth loss. When a dental implant is influenced by the type of diabetes, its failure rate is higher in patients with type 1 diabetes than in patients with type 2 diabetes. The study included a number of 56 patients, who presented themselves for performing an implant. Of these, 7 patients did not perform an implant. Diabetes mellitus defines a chronic metabolic disorder, which may have multiple etiopathogenesis, characterized by changes in carbohydrate, lipid and protein metabolism. Stability of the implant in the bone, immediately after implantation is crucial for the success of the treatment; this immediate stability is called primary stability and is purely mechanical in nature. Background: Implant stability is a mandatory factor for dental implant (DI) osseointegration and long-term success. The aim of this study was to evaluate the effect of implant length, diameter, and recipient jaw on the pre- and post-functional loading stability. Materials and methods: This study included 17 healthy patients with an age range of 24-61 years. Twenty-two DI were inserted into healed extraction sockets to replace missing tooth/ teeth in premolar and molar regions in upper and lower jaws. Implant stability was measured for each implant and was recorded as implant stability quotient (ISQ) immediately (ISQ0), and at 8 (ISQ8) and 12 (ISQ12) weeks postoperatively, as well as post-functional loading (ISQPFL). The pattern of implant stability changes throughout the study period and its correlation with the recipient jaw and the DI dimensions were evaluated. Results: There was a significant difference in ISQ values throughout the study. DI stability in the maxilla was significantly higher than that in mandible for the ISQ0, with no significant effect for the rest time points. The effect of implant diameter was significant with DI of 4.1mm diameter being more stable. While for the length, there was no significant difference regarding its effect on ISQ values through-out the study period. Conclusions: DI inserted in the maxilla demonstrated better primary stability with no effect of recipient jaw on secondary stability and after functional loading, also DI with wider diameter had better stability throughout the study whereas DI length showed no significant effect on stability
