There has been an increased interest in Housing First services in Sweden over the past few years. The model was first developed in New York by the organization Pathways to Housing. The growing interest in Housing First as a response to ending long-term homelessness is seen not only in the US and Canada, but also in Europe. One reason for this is the mass of evidence showing high housing retention rates with Housing First services as compared with traditional services. This article aims to analyse the drivers and barriers that hinder or facilitate the scaling up of Housing First pilots. The research question is: Is it possible to incorporate lessons learned from the Housing First pilot into the existing system of homelessness services? In other words: Is it possible to put new wine into old bottles? This is an ongoing research project on Housing First services in Sweden, with the specific focus on the Housing First pilot in the city of Helsingborg. The empirical material consists of in-depth interviews and focus group interviews with project managers, support workers and other stakeholders. The main results show that leadership is essential and a key driver for the scaling-up process. Another driver is the importance of repeating the vision and goals of the idea to keep the story alive. A third key driver is that the staff has identified the Housing First approach as a relative advantage and believes in the idea. The main barrier to the scaling-up process lies in the structures that maintain and surround the social housing programme.

Human rights education (HRE) is an emerging practice across formal and informal educational sectors worldwide. However, most literature and theory on HRE emphasize the importance of imparting knowledge about human rights. In this paper, we argue that increasing tolerance among students is a vital but understudied aspect of HRE. This paper is based on the results of a mixed methods longitudinal study conducted in three classrooms across two New York City public high schools. Our methods include a pre-/post- survey, classroom observations, and semi-structured individual and group interviews. The findings indicate that merely teaching about human rights issues is necessary but not sufficient to shift deeply embedded attitudes that contribute to the transformative nature of the human rights framework. We present tolerance as a necessary precursor to positive social change and sustainable human rights implementation.

Abstract Objective New York City was the epicenter of the outbreak of the 2020 COVID-19 pandemic in the United States. As a large, quaternary care medical center, NYU Langone Medical Center was one of many New York medical centers that experienced an unprecedented influx of patients during this time. Clinical leadership effectively identified, oriented, and rapidly deployed a “COVID Army”, consisting of non-hospitalist physicians, to meet the needs of this patient influx. We share feedback from our providers on our processes and offer specific recommendations for systems experiencing a similar influx in the current and future pandemics. Methods In order to assess the experiences and perceived readiness of these physicians (n=183), we distributed a 32-item survey between March and June of 2020. Thematic analyses and response rates were examined in order to develop results. Results Responses highlighted varying experiences and attitudes of our front-line physicians during an emerging pandemic. Thematic analyses revealed a series of lessons learned, including the need to: (1) provide orientations, (2) clarify roles/ workflow, (3) balance team workload, (4) keep teams updated on evolving policies, (5) make team members feel valued, and (6) ensure they have necessary tools available. Conclusions Lessons from our deployment and assessment are scalable at other institutions.

Aims and objectives: This study analyzes the proclitic and enclitic positions of Spanish clitic se (e.g., ella se quería ir / ella quería irse ‘she wanted to go’) across two generations of Spanish speakers in New York City. In an effort to contribute to ongoing research aimed at better understanding Spanish in the US, the following questions are addressed. In syntactic environments that permit variation, does placement of Spanish se differ between the two generations? From the internal variables identified for this study (nonfinite verb type, finite verb, tense of finite verb, grammatical person, use of se, grammatical mood of finite verb, negation), which ones have a statistically significant effect on placement? From the external variables identified for this study (national origin, region, areal origins, sex, age, years in US, socioeconomic class, education, English skill, Spanish skill, general Spanish use), which ones have a statistically significant effect on placement? Design and data: This study is carried out within a variationist-sociolinguistic framework and the sample consists of 50 participants, 25 from the first generation (G1) and 25 from the second (G2). Analysis: Bivariate chi-square tests are performed in order to determine what internal and external variables constrain placement of the dependent variable (clitic se placement). Findings: Generation has a statistically significant effect on placement ( p = .016), wherein proclisis is more frequent amongst the G2 participants. These results corroborate previous research showing an overall preference for proclisis in both monolingual and bilingual/heritage speakers. Further, chi-square tests pinpoint five conditioning effects for G1 (nonfinite verb type, use of se, finite verb, years in US, and English skill), but only two for G2 (use of se and English skill). Originality and implications: The present study is the first to discover strong correlations between the proclitic position and the numerous internal and external variables quantitatively assessed. Future research is thus warranted.

Abstract Cognitive and physical activity are important to maintaining daily functioning in older adults. While bidirectional associations between cognitive and physical activity magnify with increasing age, elucidating shared benefits has been difficult as few interventions explicitly train on cognition and mobility simultaneously. We conducted focus groups among 14 older adults residing in an independent-living center who participated in an interactive video game study called Bandit the Dolphin, where participants simultaneously incorporated cognitive exercise and physical activity while navigating within a complex spatial environment to help Bandit jump, eat fish, and stun sharks. Using ‘sneaky exercise’ tactics, participants utilized upper extremities in conjunction with slight lower extremity movement to move Bandit within a 3-D oceanic environment. We conducted 3 semi-structured focus groups and analyzed the data using the “Sort and Sift, Think and Shift” method to assess general likes and dislikes as well as the primary motivators, barriers, and reasons for remaining in the study. Participants enjoyed the immersive nature, challenge, and “fun factor” of the game. Primary motivators for joining were generativity/helping others, self-improvement, from peer referrals, and because the study looked interesting. Key barriers reported in the study were exhaustion from standing, learning how to play in 3-D space, and frustration from lack of level advancement. Reasons for retention were due to the game being fun, a sense of duty, and fulfilling commitments. This information will guide ongoing research efforts to design interactive video game interventions that are enjoyable for older adults and maintain high retention rates.

Abstract Objective: Candida auris infections continue to occur across the United States and abroad, and healthcare facilities that care for vulnerable populations must improve their readiness to respond to this emerging organism. We aimed to identify and better understand challenges faced and lessons learned by those healthcare facilities who have experienced C. auris cases and outbreaks to better prepare those who have yet to experience or respond to this pathogen. Design: Semi-structured qualitative interviews. Setting: Health departments, long-term care facilities, acute-care hospitals, and healthcare organizations in New York, Illinois, and California. Participants: Infectious disease physicians and nurses, clinical and environmental services, hospital leadership, hospital epidemiology, infection preventionists, emergency management, and laboratory scientists who had experiences either preparing for or responding to C. auris cases or outbreaks. Methods: In total, 25 interviews were conducted with 84 participants. Interviews were coded using NVivo qualitative coding software by 2 separate researchers. Emergent themes were then iteratively discussed among the research team. Results: Key themes included surveillance and laboratory capacity, inter- and intrafacility communication, infection prevention and control, environmental cleaning and disinfection, clinical management of cases, and media concerns and stigma. Conclusions: Many of the operational challenges noted in this research are not unique to C. auris, and the ways in which we address future outbreaks should be informed by previous experiences and lessons learned, including the recent outbreaks of C. auris in the United States.

Abstract Background The Infectious Diseases Society of America has identified the potential use of SARS-CoV-2 genomic load for prognostication purposes as a key research question. Methods We designed a retrospective cohort study that included adult patients with COVID-19 pneumonia who had at least 2 positive nasopharyngeal tests at least 24 hours apart to study the correlation between the change in the genomic load of SARS-CoV-2 in nasopharyngeal samples, as reflected by the Cycle threshold (Ct) value of the real-time Polymerase Chain Reaction (PCR) assay, with change in clinical status. The Sequential Organ Failure Assessment (SOFA) score was used as a surrogate for patients’ clinical status. A linear mixed-effects regression analysis was performed. Results Among 457 patients who presented to the emergency department between 3/31/2020- 4/10/2020, we identified 42 patients who met the inclusion criteria. The median initial SOFA score was 2 (IQR 2–3). 20 out of 42 patients had a lower SOFA score on their subsequent tests. We identified a statistically significant inverse correlation between the change in SOFA score and change in the Ct value with a decrease in SOFA score by 0.05 (SE 0.02; p < 0.05) for an increase in Ct values by 1. This correlation was independent of the duration of symptoms. Before repeat testing can be recommended routinely in clinical practice as a predictor of disease outcomes, prospective studies with a standardized interval between repeat tests should confirm our findings. Reporting on the development of an ongoing qualitative research project with clients of midwifery care in New Brunswick, Canada, this article details the ways that methodology is complexly interwoven with political praxis. Working through the development of this project, this article models one way to enact politically engaged feminist research at each stage of the research process, from developing the research question, through research design, data collection, analysis, and theory generation. In the process, three core principles of feminist research methodologies are extended: co-construction of knowledge, researcher reflexivity, and reciprocal relationships in research. This research is caught up in and responds to a fraught political context where supports for reproductive healthcare are limited, and midwifery, abortion, and gender-affirming care are all framed as “fringe” services that exceed the austerity budget of the province. Participants engaged in this study with a clear understanding of this political terrain and approached interviews as an opportunity to share their experiences, and to advocate for the continuation and expansion of midwifery and related services in the province. Through the research process, it has become evident that midwifery must be understood as part of the struggle toward reproductive justice in this province. These reflections will direct further stages of the project, including ongoing research and dissemination.

The island-nation of Dominica, 790 km2 in area, is located approximately 1126.5 km Southeast of Florida, USA. In the mid-1990s, Dominica embarked on developing the 183.5 km Waitukubuli National Trail (WNT), the first and only national trail model in the Eastern Caribbean. The 14-segment WNT, officially opened in May 2013, passes through some of the island’s most diverse landscapes. WNT was designed to link as many rural communities as possible and promote ecotourism and community socioeconomic development. Rehabilitation work continues on the WNT and the rest of Dominica, following the devastation caused by Hurricane Maria of 2017. This article, based primarily on a limited questionnaire survey, selected interviews, and literature review, explored the research question ‘What were the challenges faced by a small-island nation in the development of a National Trail? Five specific research objectives focused on the conceptualization, implementation challenges, partnerships fostered, national and sub-regional significance, and future outlook of the Trail were considered. Questionnaire respondents identified the island’s topography and negotiations with private landowners as significant trail construction challenges. A 100% of survey respondents anticipate that reliable funding to support timely maintenance would likely be a critical future issue. Some respondents identified marketing, public engagement, and visitor safety as potential issues. Face-to-face interviews found strong support for the WNT. The interviewees were confident that Dominica would realize sustainable socioeconomic benefits from the Trail. The article concludes with lessons learned and proposes strategies and research themes to ensure the Trail’s sustainability.
