We present 19 ray lengthenings in 14 patients done with a small external fixator. In six cases the thumb was lengthened and in the other 13 cases, other digital rays. The most frequent reason for lengthening was an amputation sustained in a work accident. All the lengthenings were done by an osteotomy and subsequent gradual distraction with a small external fixator. The mean lengthening achieved was 20 mm (range, 0–32). An iliac-crest graft was needed in nine cases, corrective osteotomy in five cases and a deepening of the web in the six cases of thumb lengthening. In five cases an additional technique was needed to achieve bony consolidation. We have analysed the functional results and the ability to perform activities of daily living and resume employment. Although most of the patients had multiples injuries, the results have been very favourable, achieving a very high level of patient satisfaction.

SummaryTwenty-four dogs (27 limbs) were evaluated after surgery for correction of forelimb angular limb deformities. Partial ulnar ostectomies or definitive corrective osteotomies were performed depending upon the age of the dog. According to owner assessment nine of fourteen limbs were considered functionally good, or excellent, after partial ulnar ostectomies. Younger dogs appeared to have better functional results after dynamic correction with the mean age at surgery of dogs with good to excellent results being 6.5 months contrasted to the mean age at surgery of dogs with fair to poor results being 9.75 months. Ten of fourteen limbs were considered functionally good or excellent after definitive corrective osteotomy. One dog had definitive osteotomy after partial ulnar ostectomy in order to further correct a residual angular deformity. However, 58% of the limbs with radiographic follow-up had signs of degenerative joint disease (DJD). There were not significant differences between neither degree of angulation remaining after surgery and the functional result nor the degree of angulation remaining after surgery and the development of DJD. A prospective study is warranted to more objectively assess the efficacy of surgical correction of angular limb deformities in dogs.Twenty-four dogs were evaluated after surgery for correction of forelimb angular limb deformities. The results are described.

This study determined the mediating effect of symptoms of anxiety on the relationship between appraisal and teaching competence of teachers in elementary and secondary schools of San Isidro North and South Districts. The descriptive-correlational research design was utilized in this study employing stratified random sampling with 300 sampled teachers. Data collection was made through administration of survey questionnaires. Three sets of adapted survey questionnaires were used and were subjected for content validity and reliability test. The analysis of data was done using the Mean, Pearson-r, Multiple Regression Analysis and Medgraph using Sobel z-test. The results revealed that the level of appraisal and teaching competence of teachers were very high and the level of symptoms of anxiety was high. A positive significant relationship between appraisal and teaching competence, appraisal and symptoms of anxiety, and symptoms of anxiety and teaching competence was proven in the study. Further, the findings showed that symptoms of anxiety played a partial mediation, thus, it cannot fully account on the influence of appraisal towards teaching competence. Although teachers manifested a high level of symptoms of anxiety, still, they showed a very high level of appraisal and a very high level of teaching competence. Moreover, a high level of symptoms of anxiety did not hinder teachers capability to be very highly competent in teaching.

SUMMARY The concentration of progesterone receptors in rat uterine cytosol and nuclei was measured during the oestrous cycle and pregnancy. The method used allowed the measurement of the total concentration of binding sites (unbound and hormone-bound). During the oestrous cycle, the concentration of receptors in the cytosol peaked at pro-oestrus, low concentrations were observed at oestrus and metoestrus, and an increase was seen at dioestrus. In the nuclei, maximum concentrations occurred at pro-oestrus and dioestrus. The number of receptors in the cytosol was very low during the first half of pregnancy, but the concentration increased progressively after day 15 to attain a very high level (about 26 000 binding sites/cell) on day 22. In the nuclei, the concentration of receptors was low at the beginning of pregnancy. On day 5 (day of implantation) there was a slight increase, which corresponded to a decrease in the number of cytosolic receptors and a small peak in the level of progesterone in the plasma. Maximum concentrations were attained during a 'plateau' period between days 9 and 15. Thereafter, there was a decrease in the concentration of nuclear receptors and on day 22, the mean value was very low; in some animals, probably on the verge of parturition, no receptors were detectable in the nuclei.

Aim. Over the last few years an increasing number of people have started training out with kettlebells (KBs) all over the world. Unfortunately, there is very little scientific data about the effect of training with KBs, and essentially no research has been done on the various kettlebell sports which are developing dynamically. The main aim of this paper was to analyse the intensity of physical effort during the 10 minute Snatch Test (10 min ST) at the HardStyle Kettlebell Polish Championship (HSKPC). Our research also included an analysis of the contestants’ body composition on the competition day. Basic procedures. Ten male contestants and five female contestants were examined. Physical effort intensity was measured using Polar Team2 Pro. Body composition was evaluated with the Tanita SC-330 immediately before the start of the competitions. The mean values of the analyzed parameters in the study contestants were compared with the best male and female contestants of the Championship. Results. The heart rate during intensive exertion for the 10 min ST was consistently at a high level (80-89% HRmax) or very high level (90-100% HRmax). Such a high HR was maintained for 98% of the exercise performance, and the mean relative energy expenditure were 14.4 METs among the women and 15.5 METs among the men. An analysis of body composition showed that participants of the HSKPC were characterized by an athletic build. Conclusions. Even though the contestants lift relatively heavy loads during the 10 min ST (men 24 or 28 kg, women 16 kg) the effort can be described as an endurance test. Considering all the competitions taking place during the HSKPC and the results of the participants it can be stated that these competitions are unusually demanding and require versatile physical fitness, as well as strength, endurance and coordination. Key words: kettlebell, snatch test, physical effort, Hardstyle

Aims The treatment of tibial aplasia is controversial. Amputation represents the gold standard with good functional results, but is frequently refused by the families. In these patients, treatment with reconstructive limb salvage can be considered. Due to the complexity of the deformity, this remains challenging and should be staged. The present study evaluated the role of femoro-pedal distraction using a circular external fixator in reconstructive treatment of tibial aplasia. The purpose of femoro-pedal distraction is to realign the limb and achieve soft tissue lengthening to allow subsequent reconstructive surgery. Methods This was a retrospective study involving ten patients (12 limbs) with tibial aplasia, who underwent staged reconstruction. During the first operation a circular hexapod external fixator was applied and femoro-pedal distraction was undertaken over several months. Subsequent surgery included reconstruction of the knee joint and alignment of the foot. Results The mean follow-up was 7.1 years (2 to 10). The mean age of the patients at the time of the application of the fixator was 2.3 years (1.1 to 5.0). The mean time under distraction was 139.7 days (81.0 to 177.0). A mean fibular distalization of 38.7 mm (14.0 to 67.0) was achieved. Pin infections occurred in four limbs (33.3%) and osteitis in one. A femoral fracture occurred in one patient. Premature removal of the frame was not required in any patient. Sufficient realignment of the leg as well as soft tissue lengthening was achieved in all patients, allowing subsequent reconstruction. All patients were able to mobilize fully weight bearing after reconstruction. Functional outcome was limited in all limbs, and five patients (50.0%) required additional reconstructive operations. Conclusion Regarding the functional results in the treatment of tibial aplasia, amputation remains superior to limb salvage. The latter procedure should only be performed in patients whose parents refuse amputation. Femoro-pedal distraction efficiently prepares the limb by realigning the leg and soft tissue lengthening. Minor complications are frequent, but usually do not hinder the continuation of distraction. Even though a fully weight-bearing limb is achieved, the functional outcome of reconstructive treatment remains limited. Recurrent deformities frequently occur and may require further operations. SummaryNormal human platelets have been separated according to density on continuous Percoll gradients and the platelet distribution divided into five fractions containing approximately equal numbers of platelets. The mean volumes and protein contents of the platelets in each fraction were found to correlate positively with density while the protein concentration did not differ significantly between the fractions. Four mitochondrial enzymes (monoamine oxidase, glutamate dehydrogenase, cytochrome oxidase and NADP-dependent isocitrate dehydrogenase) were assayed and their activities per unit volume were found to increase in a very similar monotonie fashion with platelet density. When MAO and GDH were assayed on the same set of density fractions the correlation between the two activities was very high (r = 0.94–1.00, p <0.001) and a similar close correlation was found between MAO and ICDH. The results support the hypothesis that high density platelets either have a higher concentration of mitochondria or have larger mitochondria than low density platelets.
