Abstract Our objective was to describe, for the first time in an English-speaking Caribbean country, the contribution of ultra-processed foods (UPFs) to nutrients linked to non-communicable disease. Using a cross-sectional study design, dietary data were collected from two non-consecutive 24-h dietary recalls. Recorded food items were then classified according to their degree of processing by the NOVA system. The present study took place in Barbados (2012–13). A representative population-based sample of 364 adult Barbadians (161 males and 203 females) aged 25–64 years participated in the study. UPFs represented 40⋅5 % (838 kcal/d; 95 % CI 791, 885) of mean energy intake. Sugar-sweetened beverages made the largest contribution to energy within the UPF category. Younger persons (25–44 years) consumed a significantly higher proportion of calories from UPF (NOVA group 4) compared with older persons (45–64 years). The mean energy shares of UPF ranged from 22⋅0 to 58⋅9 % for those in the lowest tertile to highest tertile. Within each tertile, the energy contribution was significantly higher in the younger age group (25–44 years) compared with the older (45–64 years). One-quarter of persons consume ≥50 % of their daily calories from UPF, this being significantly higher in younger persons. The ultra-processed diet fraction contained about six times the mean of free sugars and about 0⋅8 times the dietary fibre of the non-ultra-processed fraction (NOVA groups 1–3). Targeted interventions to decrease the consumption of UPF especially in younger persons is thus of high priority to improve the diet quality of Barbadians.

Abstract Background Depression is an ever more common chronic non communicable disease and its control constitutes a growing public health concern given its links with a number of co-morbidities, including diabetes mellitus. The study aimed to estimate the prevalence of depression at a population level across groups of different glycaemic status, whilst establishing its socioeconomic phenotypic characteristics.Methods A nationally representative cross-sectional study was conducted in Malta between 2014 and 2016. Participants were categorized into different sub-populations according to their glycaemic status. Depression prevalence rates and socio-economic characteristics for each sub-population were established. Multiple regression analysis was performed to identify links with depression.Results Depression was prevalent in 17.15% (CI 95%: 16.01 – 18.36) with a female predominance. The normoglycaemic sub-population had the highest depression rates. However, persons with known diabetes had a higher probability of having a history of depression (OR:2.36 CI 95%:1.12 – 4.96), as well as with being of the female gender, having lower educational status, having a history of smoking tobacco and having established cardiovascular disease.Conclusions Depression was highly prevalent among the normoglycaemic population especially as age progress. Physicians in primary care should implement a depression screening tool as part of their routine health check-ups, with special attention to those with cardiovascular co-morbidities and any signs of psycho-socio-economic burden.

Background: Non communicable diseases are responsible for a high proportion of deaths and disabilities. The major risk factors attributed to this are smoking, heavy alcohol drinking, inadequate fruit, and vegetable consumptions, and physical inactivity. While there are data available to understand the prevalence of non-communicable risk factors among men and women, there is no data among transgender community. Hence this study was conducted to assess the prevalence of NCD risk factors among the male-to-female transgender community.Methods: This cross sectional study was conducted among 54 transwomen who had been registered with two social welfare organizations for transgender community in Chennai. The risk factor was assessed using WHO STEPS instrument.Results: Among the transwomen, 16.7% were currently smoking tobacco, while 18.5% were using smokeless forms of tobacco. Alcohol consumption was observed in 44.4% of the population. 62.9% of them were not involved in any kind of exerting physical activity for recreation. 25.9% had undergone hormone supplementation for gender affirmation. Prevalence of obesity was 20.4% in the study population. The prevalence of hypertension, diabetes and hypercholesterolemia was 11.1%, 5.6% and 7.4% respectively.Conclusions: High Prevalence of certain risk factors such as smokeless tobacco usage, alcohol consumption and hormone supplementation were seen among the male-to-female transgender community compared to the general population. Targeted interventions must be made so that specific attention is given to the transgender community in various NCD programs by the government. They must also be supported and accepted by every member of this society.

Abstract Background The addition of low-calorie sweeteners (LCS) has been increasingly used by industry but their metabolic effects are controversial. Objectives: to estimate the consumption of foods and beverages with LCS among adolescents, adults and the elderly and to verify which food categories most contribute to this consumption. Methods Cross-sectional study. We used data from the Campinas Health Survey (ISACamp 2014-15) and the Food Consumption and Nutritional Status Survey (ISACamp-Nutri 2015-16) of 2570 individuals from 10 years old. We estimated the prevalence of LCS consumption and described the food categories that most contributed to this consumption. Using regression analysis, we verified which population groups had the highest consumption of products with LCS. Results More than 40% of adolescents, adults and the elderly consumed at least one product with LCS, mainly coming from sweetened beverages, tabletop sweeteners, and yogurt and other dairy beverages. The consumption of LCS was evenly found across adolescents and elderly from all socioeconomic strata, and whether elderly participants presented overweight and/or diabetes. Among adults, greater prevalence of foods and beverages with LCS consumption was found in those with higher education but did not vary by the presence of overweight and/or diabetes. Conclusions We found similar consumption of foods and beverages with LCS in most socioeconomic strata, and in participants with and without overweight or diabetes. Key messages The results can contribute to the development of public policies on clearer information of LCS addition in food labels.

Abstract Background Dog ownership (DO) has been associated with higher levels of self-reported walking and physical activity. However, compared to device-based measures, self-reported measures of physical activity may suffer from bias due to recall and social desirability. They are also incapable of quantifying light-intensity physical activity (LPA) and step volume, both of which may have important health benefits, especially for older adults. In this study, we investigated the association of DO with accelerometer-measured physical activity of different intensities and daily steps in 70-year-old individuals. Methods This was a population-based cross-sectional study including 1406 participants aged 70 years [54.1% female] who participated in a health survey in Umeå, Sweden between February 2017–November 2019. All participants self-reported DO [yes/no]. Daily averages of LPA, moderate-to-vigorous-intensity physical activity (MVPA), and steps per day [steps/d] were measured for 1 week using hip-mounted Actigraph GT3X+ accelerometers. Associations were investigated using linear- and logistic regression models, adjusted for sociodemographic and health-related factors, date of examination, and accelerometer wear time. Results The prevalence of DO was 14.1% [N = 199]. After adjustment for all covariates, DO was associated with 19.2 more minutes/d of LPA [95% CI, 8.8–29.6], 11.4 more minutes/d of MVPA [95% CI, 8.0–14.9] and 1738 more steps/d [95% CI, 1326–2149]. DO was also associated with twice the odds of meeting the physical activity recommendations [OR, 2.07, 95% CI, 1.48–2.90]. Exploratory interaction analyses showed that the association between DO and steps/d was stronger [Pinteraction = 0.030] in female [β = 2165, 95% CI, 1585–2744] than in male [β =1255, 95% CI, 664–1845], with a similar trend for MVPA [Pinteraction = 0.082]. Conclusions In this study of community-dwelling 70-year-old individuals, DO was associated with higher levels of daily LPA, MVPA, and steps. With the limitation of the observational design of the study, these findings add knowledge regarding the beneficial role that DO may play for promoting physical activity in the older population. In turn, these findings could support the development and evaluation of targeted interventions seeking to promote dog-friendly environments and facilitate dog walking in the community.

AbstractObjectiveFe is an essential element for erythropoiesis and Hb synthesis. High Hb levels affect the blood’s viscosity and are associated with cardiovascular dysfunction. The aim of the present study was to examine relationships of Hb and cardiometabolic abnormalities with the risk of alanine aminotransferase (ALT) elevation in adolescents.DesignA population-based, cross-sectional study.SettingNational Nutrition and Health Survey in Taiwan (2010–2011, adolescents).SubjectsHealthy adolescents aged 13–18 years.ResultsIn total, 1941 adolescents (963 boys and 978 girls) were entered in the study. The mean age was 15·3 (sd 0·1) years (boys, 15·3 (sd 0·1) years; girls, 15·2 (sd 0·1) years). ALT tertile cut-off points for boys were 11 and 16 U/l, and for girls were 9 and 12 U/l. Girls without dyslipidaemia and presenting in the highest quartile (Q1) of Hb (>13·6 g/dl) were 1·89 and 3·76 times more likely to have raised serum ALT (9 and >12 U/l, respectively) than the reference (lowest quartile of Hb (Q1), <12·4 g/dl). Moreover, for those girls with dyslipidaemia, serum ALT seemed to increase with an increase in Hb levels. Specifically, girls with dyslipidaemia and Hb levels of 12·4, 13·1 and 13·6 g/dl were, respectively, 2·86, 3·53 and 5·64 times more likely to have elevated serum ALT levels (>12 U/l) than the reference (Q1 of Hb, <12·4 g/dl). The only effect found in boys was for those who had dyslipidaemia and presenting in Q4 of Hb (>15·4 g/dl), who were 7·40 times more likely to have elevated serum ALT of >16 U/l than the reference (Q1 of Hb, <14·1 g/dl).ConclusionsOur findings suggest that an increased Hb level is a predictor of elevated serum ALT in adolescent girls with dyslipidaemia. Our study also highlights the importance of further research to establish cut-off points for Hb and its utility in diagnosing and preventing the onset of dyslipidaemia in adolescents.

AimsTo determine a normative of tumbling E optotype and its feasibility for visual acuity (VA) assessment in children aged 3-4 years.MethodsA cross-sectional study of 1756 children who were invited to participate in a comprehensive non-invasive eye exam. Uncorrected monocular VA with crowded tumbling E with a comprehensive ophthalmological examination were assessed. Testability rates of the whole population and VA of the healthy children for different age subgroups, gender, school type and the order of testing in which the ophthalmological examination was performed were evaluated.ResultsThe overall testability rate was 95% (92% and 98% for children aged 3 and 4 years, respectively). The mean VA of the first-day assessment (first-VA) and best-VA over 2 days’ assessments was 0.14 logMAR (95% CI 0.14 to 0.15) (decimal=0.72, 95% CI 0.71 to 0.73) and 0.13 logMAR (95% CI 0.13 to 0.14) (decimal=0.74, 95% CI 0.73 to 0.74). Analysis with age showed differences between groups in first-VA (F(3,1146)=10.0; p<0.001; η2=0.026) and best-VA (F(3,1155)=8.8; p<0.001; η2=0.022). Our normative was very highly correlated with previous reported HOTV-Amblyopia-Treatment-Study (HOTV-ATS) (first-VA, r=0.97; best-VA, r=0.99), with 0.8 to 0.7 lines consistent overestimation for HOTV-ATS as described in literature. Overall false-positive referral was 1.3%, being specially low regarding anisometropias of ≥2 logMAR lines (0.17%). Interocular difference ≥1 line VA logMAR was not associated with age (p=0.195).ConclusionsThis is the first normative for European Caucasian children with single crowded tumbling E in healthy eyes and the largest study comparing 3 and 4 years old testability. Testability rates are higher than found in literature with other optotypes, especially in children aged 3 years, where we found 5%–11% better testability rates.

Abstract Despite some advantages over traditional methods, Web-based studies elicit concerns about generalizability. To address this issue, we compared dietary intakes between an electronic (e-) cohort study and a nationally representative survey. We studied 49,443 French volunteers aged 18–74 years recruited during 2009–2010 in the NutriNet-Santé Study, a general population-based e-cohort study. The Etude Nationale Nutrition Santé (ENNS; 2006–2007), a cross-sectional study with a nationally representative sample of 2,754 French adults aged 18–74 years, served as the reference data set. Reported dietary intakes from three 24-hour dietary records were weighted and compared between the two studies via Student t tests for mean location, using a &gt;5% cutoff for establishing practically meaningful differences. We observed similar intakes as regards carbohydrates, total lipids, protein, and total energy. However, intakes of fruit and vegetables, fiber, vitamins B6, B9, C, D, and E, iron, and magnesium were higher in the e-cohort than in the ENNS, while intakes of alcohol and nonalcoholic beverages were lower in the e-cohort. Significant sex-specific differences were observed regarding vitamins A and B12, zinc, and potassium. True intake differences, mode effects, and volunteer bias might each contribute to explaining the findings. In the future, repeated use of the same tool in large e-cohorts with heterogeneous dietary exposures could serve research purposes and supplement group-level monitoring of dietary trends.

Pleural effusion is present when there is >15ml of fluid is accumulated in the pleural space. It can be divided into two types; exudative and transudative pleural effusion. Tuberculosis and parapneumonic effusion are the common cause of exudative pleural effusion whereas heart failure accounts for most of the cases of transudative pleural effusion. This study was a hospital based cross sectional study performed at Nepal Medical College during the period of January 2016-December 2016. A total of 50 patients who fulfilled the inclusion criteria were enrolled. Pleural effusion was confirmed by clinical examination and radiology. After confirmation of pleural effusion, pleural fluid was aspirated and was analysed for protein, LDH, cholesterol. The Heffner criteria was compared with Light criteria to classify exudative or transudative pleural effusion. Among 50 patients, 30 were male and 20 were female. The mean age of patient was 45.4±21.85 years. The sensitivity and specificity of using Light criteria to detect the two type of pleural effusion was 100% and 90.9%, whereas using Heffner criteria was 94.87%, 100% respectively(P<0.01). There are variety of causes for development of pleural effusion and no one criteria is definite to differentiate between exudative or transudative effusion. In this study Light criteria was more sensitive whereas Heffner criteria was more specific to classify exudative pleural effusion. Hence a combination of criteria might be useful in case where there is difficulty to identify the cause of pleural effusion.
