Previous employment histories and quality of life in older ages: sequence analyses using SHARELIFE

ABSTRACTThis article summarizes previous employment histories and studies associations between types of histories and quality of life in older ages. Retrospective information from the Survey of Health, Ageing and Retirement in Europe (SHARE) was used and the occupational situation for each age between 30 and 65 of 4,808 men and 4,907 women aged 65 or older in Europe was considered. Similar histories were regrouped using sequence analyses, and multi-level modelling was applied to study associations with quality of life. To avoid reverse causality, individuals with poor health prior to or during their working life were excluded. Men's employment histories were dominated by long periods of paid employment that ended in retirement (‘regular’ histories). Women's histories were more diverse and also involved domestic work, either preceding regular careers (‘mixed’ histories) or dominating working life (‘home-maker’ histories). The highest quality of life was found among women with mixed histories and among men with regular histories and late retirement. In contrast, retirement between 55 and 60 (but not earlier) and regular histories ending in unemployment or domestic work (for men only) were related to lower quality of life, as well as home-maker histories in the case of women. Findings remain significant after controlling for social position, partnership and parental history, as well as income in older ages. Results point to the importance of continuous employment for health and wellbeing, not only during the working life, but also after labour market exit.

Objective: To investigate variations in quality of life at older ages, we take a life course perspective to analyse long-term effects of physical working conditions upon quality of life after retirement. In doing so, we study to what extent these associations are explained by individuals’ health at older ages. Method: We use administrative data and self-administered questionnaire responses from the French GAZEL cohort. Quality of life was assessed with CASP-19 in 2009 and related to three types of physical working conditions during previous working life: (1) ergonomic strain, (2) physical danger and (3) exposures to chemicals. Health was assessed in 2007 with the SF-36 Health Survey. Multiple regressions were calculated in retired men only, controlling for important confounders including social position. Results: In contrast to men, few women were exposed to strenuous and dangerous working conditions in this cohort and were not included in subsequent analyses. Negative effects on retired men's quality of life were found for the physical occupational exposures of ergonomic strain and physical danger, but not for chemical exposures. Effects were attenuated after the introduction of physical and mental health to the models, indicating an indirect effect of physical working conditions upon quality of life via health. Conclusion: Adverse physical working conditions have long-term consequences for health and quality of life at older ages. Improvements to physical working conditions may improve individuals’ quality of life over the long term.

Objetivo: avaliar a qualidade de vida no trabalho da equipe de enfermagem do centro cirúrgico. Metodologia: estudo descritivo, de corte transversal e com abordagem quantitativa, desenvolvido com 70 profissionais de enfermagem do centro cirúrgico em um hospital de referência em Teresina (PI), com a aplicação dos questionários sociodemográfico e Quality Working Life Questionnaire-bref. A análise foi realizada com o SPSS versão 21.0, utilizando a correlação de Pearson e os testes alfa de Cronbach, Kolmogorov-Smirnov, Qui-quadrado e regressão linear múltipla, com intervalo de confiança de 95%. Resultados: a maioria dos participantes (62,9%) tiveram menor impacto na qualidade de vida no trabalho, entretanto a renda familiar e a formação acadêmica foram bastante significativas. Conclusão: apesar do trabalho no centro cirúrgico apresentar baixo impacto na qualidade de vida, o domínio psicológico demonstrou impacto considerável, destacando que a preocupação com a saúde do trabalhador e a valorização da equipe de enfermagem ainda precisam avançar.Descritores: Qualidade de vida; Enfermagem de centro cirúrgico; Saúde do trabalhador.QUALITY OF LIFE IN THE WORK OF THE SURGI CAL CENTER NURSING TEAMObjective: to evaluate the quality of life in the work of the nursing staff of the surgical center. El análisis se realizó con el programa SPSS versión 21.0 mediante la prueba de correlación de Pearson y alfa de Cronbach, prueba de Kolmogorov-Smirnov, regresión lineal chi-cuadrado y múltiple con un intervalo de confianza del 95%. Resultados: La mayoría de los participantes (62,9%) tenían un menor impacto en la calidad de la vida laboral, aunque el ingreso familiar y la formación académica eran bastante significativa. Conclusión: Aunque el trabajo en la sala de operaciones tiene un bajo impacto en la calidad de vida, el dominio psicológico mostró un impacto considerable, teniendo en cuenta que la preocupación por la salud de los trabajadores y la apreciación del equipo de enfermería todavía tienen que seguir adelante.Descriptores: Calidad de vida; Enfermería de quirófano; Salud laboral.

Ketamine therapy with culturally attuned trauma-informed psychotherapy in a collaborative cross-cultural partnership may provide a critical step in the operationalization and optimization of treatment effectiveness in diverse populations and may provide a foundation for an improved quality of life for Indigenous people. Decolonizing Indigenous health and wellbeing is long overdue, requiring an equal partnership between government and Indigenous communities, built upon an aboriginal culture holistic foundation of balance of mind, body, social and spiritual realms, and within the context of historical and lived experiences of colonialism. Culturally attuned trauma-informed psychotherapy paired with ketamine—a fast-acting antidepressant that typically takes effect within 4 hours, even in cases of acute suicidality—may be uniquely qualified to integrate into an Indigenous based health system, since ketamine’s therapeutic effects engage multiple neuropsychological, physiological, biological, and behavioral systems damaged by intergenerational complex developmental trauma. Ketamine holds the potential to serve as a core treatment modality around which culturally engaged treatment approaches might be organized since its brief alteration of normal waking consciousness is already a familiar and intrinsic element of healing culture in many Indigenous societies. There is great need and desire in Indigenous communities for respectful and sacred partnership in fostering more effective mental health outcomes and improved quality of life.

BACKGROUND: Accumulating evidence suggests that cancer survivors are able to return to work. However, little is known about their work situation 5 years after diagnosis. OBJECTIVE: To explore fluctuations in employment status and its association with quality of life 2, 3, and 5 years after cancer diagnosis of 65 cancer survivors employed at diagnosis. METHODS: In association with a randomised controlled trial (RCT), questionnaires were administrated to eligible cancer survivors at diagnosis, 2, 3, and 5 years thereafter comprising of validated questionnaires related to work (i.e. Work Ability Index (WAI), cancer, and quality of life (QOL) (i.e. SF-36, VAS QOL). The RCT studied a hospital-based work support intervention in female breast and gynaecological cancer survivors who were treated with curative intent and had paid work at diagnosis. Descriptive statistics and longitudinal multi-level analysis were employed. RESULTS: Sixty-five of the 102 eligible cancer survivors participated, who were primarily diagnosed with breast cancer (63%). Two and 5 years after cancer diagnosis respectively 63 (97%) and 48 (81%) participants were employed. Reasons for not being employed after 5 years included receiving unemployment benefits (7%), voluntary unemployment (3%), receiving disability benefits (3%), and early retirement (3%). Longitudinal multi-level analysis showed that employed cancer survivors reported in general statistically significant better quality of life outcomes at 5 years follow-up compared to those not being employed. CONCLUSIONS: We found high employment rates and few fluctuations in employment status. The steepest decline in employment rate occurs after the first two years of diagnosis. Employed participants reported better quality of life outcomes. Survivorship care should therefore focus on the population at risk possibly within the first two years after diagnosis.

When we examine the quality of life for each individual, several specific dimensions need to be considered. Health conditions and quality of family relationships are critical. When the individual in question is married, his/her marital quality is important. When he/she has a job, subjective assessment of that job is important. Social relationships regarding friends, neighbors, colleagues, relatives, etc. may be critical also. Although the quality of life is more than the sum of qualities of these specific areas of one's life, it is still determined to a great extent by the qualities of these sub-areas, including marital and family relationships (Proulxet al., 2007).

Background: Vision plays an important role in proper growth of a child, as poor vision can lead to reduction in the learning potential of a child. Various causes can bring about poor vision in children and proper evaluation at an early stage can help improve the quality of life. The objective of the study was to assess the prevalence of ocular morbidity in school children. Methods: A cross-sectional study was conducted in 6 schools, which included Government and private schools around the field practice areas of a medical college. Eye examination was done which included testing for visual acuity, external examination and anterior segment examination with a pen torch and posterior segment examination with a direct ophthalmoscope. Statistical analysis: Data was analysed using SPSS. Results: Of the 402 children, 51.5% of the children were males and 48.5% were females. All the children were aged between 10 and 13 years. The most common eye symptom the students presented with was headache on reading for long durations (24.6%). The overall prevalence of ocular morbidity was found to be 20.4%. Refractive errors were the most common cause of ocular morbidity (14.2%). Parental history of wearing spectacles had an association with ptosis in children (p<0.026) and faulty near vision (p<0.022). Conclusions: Ocular morbidity is high in school going children. Since most of the causes of ocular morbidity are preventable or treatable, regular screening and early diagnosis and treatment can help improve the quality of life of children.

Quality of life is one of the most important characteristics that reflects a comprehensive description of the level and living conditions. Nowadays quality of working life is an integral feature of “Quality of Life”. Quality of working life identifies working conditions and labor management. The quality of working life allows us to determine the working conditions, its organization from the perspective of the optimal realization of the abilities of employees.The article analyzes the domestic approaches to the definition and evaluation of the quality of working life. The conducted research allowed the authors to draw the following conclusions: the quality of working life is characterized by a multitude of indicators, and the required condition for studying the quality of working life is the development of techniques that allow the evaluation of the studied category.The goal is to evaluate the quality of working life in Russia.Methods. A number of indicators are presented to estimate the quality of working life of employees in the Russian Federation. According to the authors, those indicators are employment, labor safety, work safety, salary, and competence level and workforce productivity. The analysis that is based on data from Federal State Statistics Service has been done in view of gender factors and federal district differences. According to this analysis, the general part of workforce has jobs with harmful or dangerous working conditions. Extractive and process industries are considered the most hazardous.The article describes the method of integrated assessment of working life quality, based on the study of social and labor sphere of the Federal districts of Russia. Based on this author’s method, the integral indicators of the quality of working life of the population of the Russian Federation in the context of Federal districts are calculated and analyzed and the rating of each of them is determined.Results. The article notes that the quality of working life reflects not only the working conditions and its payment, but also the relations in the working collective, the motivation of the personnel to work. The most important motivational factors and characteristics of the socio-psychological climate of the enterprise are determined. The set of indicators, proposed by the authors and the methods of assessing the quality of the working life of the population can be useful to local governments for determining the rating of the quality of working life of officers in municipalities, and thus for making various managerial decisions.

Quality of working life has been differentiated from the broader concept of quality of life. To some degree, this may be overly simplistic, as Elizur and Shye,(1990) concluded that quality of work performance is affected by quality of life as well as quality of working life. However, the specific attention to work-related aspects of quality of life is valid. Whilst quality of life has been more widely studied, quality of working life remains relatively unexplored and unexplained. A review of the literature reveals relatively little on quality of working life. Where quality of working life has been explored, writers differ in their views on its core constituents. Various authors and researchers have proposed models of quality of working life which include a wide range of factors. This paper concentrates on the role of job satisfaction in quality of work life.
