Association of Adult Socioeconomic Status with Body Mass Index: A Within- and Between-Twin Study

Abstract Adult socioeconomic status (SES) has been consistently associated with body mass index (BMI), but it is unclear whether it is linked to BMI independently of childhood SES or other potentially confounding factors. Twin studies can address this issue by implicitly controlling for childhood SES and unmeasured confounders. This co-twin control study used cross-sectional data from Twins Research Australia’s Health and Lifestyle Questionnaire (N = 1918 twin pairs). We investigated whether adult SES, as measured by both the Index of Relative Socioeconomic Disadvantage (IRSD) and the Australian Socioeconomic Index 2006 (AUSEI06), was associated with BMI after controlling for factors shared by twins within a pair. The primary analysis was a linear mixed-effects model that estimated effects both within and between pairs. Between pairs, a 10-unit increase in AUSEI06 was associated with a 0.29 kg/m2 decrease in BMI (95% CI [−.42, −.17], p < .001), and a 1-decile increase in IRSD was associated with a 0.26 kg/m2 decrease in BMI (95% CI [−.35, −.17], p < .001). No association was observed within pairs. In conclusion, higher adult SES was associated with lower BMI between pairs, but no association was observed within pairs. Thus, the link between adult SES and BMI may be due to confounding factors common to twins within a pair.

Abstract Background The aim of the present study was to examine the influence of body mass index category, as well as of demographic and socioeconomic factors on the participation in organized physical activity (OPA) of schoolchildren attending primary and secondary school in Greece. Furthermore, to compare the difference between the two levels. Methods This is a cross-sectional study conducted on a representative elementary and secondary school cohort, derived using stratification and probability proportional to size (PPS) methodology. The final sample included 18,264 subjects, aged 6 to 15 years. Parents of all students and students of secondary schools fulfilled validated questionnaires evaluating socioeconomic status, and participation to OPA. International Obesity Task Force (IOTF) cut offs were used to classify the children. Univariate and multivariate logistic models examined factors associated with OPA. All analyses were stratified by school level. Results Sport participation was not reported in 37.7 and 54.4% of primary and secondary schoolchildren respectively. Having BMI within normal range, being male, having parents participating in an organized activity and a high socio-economic status seem to encourage participation in OPA in both school levels. Conclusions Children with normal BMI are more likely to participate in OPA. Parents as role model as well as higher socioeconomic status of the family emerge as important influencers. Participation in OPA declines as students enter secondary school. Interventions focusing on increasing physical education and activity into school daily program should be tailored to the specific needs of different weight categories and can possibly eliminate the impact of SES inequalities.

Aims: This study investigates associations between the combined physical activity environment and obesity and explores any sub-group effects by individual-level socioeconomic status. Methods: In a large cross-sectional cohort ( n = 22,889) from the Yorkshire Health Study, body mass index was calculated using self-reported height and weight and obesity was defined as a body mass index ≥ 30. The physical activity environment was split into ‘unfavourable physical activity’, ‘moderately favourable physical activity’ and ‘favourable physical activity’ environments. This was based on the count of parks and physical activity facilities within a 2 km radial buffer centred on home addresses. A favourable physical activity environment was defined as having ≥1 physical activity facility and ≥1 park, unfavourable as having no physical activity facility and park and any other combinations defined as moderately favourable. Logistic regression (odds ratios) identified associations with obesity. Results: Relative to ‘unfavourable physical activity environments’, individuals within favourable physical activity environments were less likely to be obese (odds ratio = 0.90; 95% confidence interval = 0.82–0.97), and there was no effect for moderately favourable environment. Furthermore, once stratified by education level, this relationship was only present for those of higher education. Conclusion: Our findings provide novel UK evidence and is one of the first papers internationally that highlights the importance of considering the interplay of individual-level socioeconomic factors when investigating associations between the physical activity environment and obesity.

Abstract Background Evidence regarding the association between demographics, socioeconomic status and metabolic syndrome is limited. We aim to investigate whether demographics and socioeconomic status are correlated with metabolic syndrome using data from National Health and Nutrition Examination Survey through 2013/2014 to 2015/2016.Methods A total of 4313 selected participants were included in this cross-sectional study. The independent variables were demographics (age, gender,and race/ethnicity) and socioeconomic status (the ratio of family income to poverty). The dependent variable was metabolic syndrome. The covariates included data release cycle, education level, marital status, dietary data, health insurance, average alcoholic drinks, current smoking, sedentary activity hours, physical activity minutes, and body mass index. Logistic regression analysis was used to evaluate the association between demographics, socioeconomic status and metabolic syndrome.Results In fully-adjusted models, we found that age was positively associated with metabolic syndrome (OR:1.05, 95%CI:1.04-1.05); compared with the male group, female was positively associated with metabolic syndrome in participants with body mass index under 25 kg/m 2 (OR:1.84, 95%CI:1.07-3.18) whereas it was negatively associated with metabolic syndrome in those with body mass index equal to or greater than 30 kg/m 2 (OR:0.62, 95%CI:0.48-0.81); compared with Mexican American, non-Hispanic Asian and other race/ethnicity were positively associated with metabolic syndrome in participants with body mass index under 25 kg/m 2 (OR: 7.30, 95%CI: 1.50-35.62 and OR: 6.09, 95%CI: 1.25-29.74, respectively) while non-Hispanic Black was negatively associated with metabolic syndrome in participants with body mass index equal to or greater than 25 kg/m 2 and less than 30 kg/m 2 (OR:0.58, 95%CI:0.36-0.92); there was no significant association between the ratio of family income to poverty and metabolic syndrome.Conclusions Among the population of nationally representative non-pregnant American adults, there is a correlation between demographics and metabolic syndrome whereas no correlation between socioeconomic status and metabolic syndrome after multivariates adjustment. Healthcare interventions targeting those with metabolic syndrome including older individuals, obese males, along with females, non-Hispanic Asian and other race/ethnicity with BMI under 25 kg/m 2 are required to address these disparities.

Aims While international variations in the prevalence of hypertension are well described, less is known about intra-national disparities and their determinants. We wanted to describe the variations in hypertension prevalence within France and to determine how much lifestyle and socioeconomic factors contributed to explain these regional variations. Methods Participants (62,247 French adults aged 18 to 69 years) were recruited in the 16 centres of the CONSTANCES study between 2012 and 2015. Hypertension was defined as blood pressure higher than 140/90 mmHg and/or taking antihypertensive medications. The contribution of lifestyle and socioeconomic factors to hypertension prevalence variations among centres was examined using sequential hierarchical logistic models. Results Hypertension prevalence was 37.3% (95% confidence interval (CI) = 36.6–38.0) in men and 23.2% (95% CI = 22.7–23.8) in women. Hypertension prevalence rates varied by almost two-fold among centres (1.9 in men, 1.6 in women) with the highest prevalence in the north and the east of France. Body mass index was strongly associated with hypertension in women (odds ratio (OR)1-unit increase = 1.11 (95% CI = 1.11–1.12)) and was the highest contributor to between-centre variations (27% in women), followed by socioeconomic characteristics (e.g. ORhigh versus low education = 0.85 (95% CI = 0.83–0.87), contributing to 14% of variations in women). Together, family history of hypertension, body mass index, education, occupation and residential area socioeconomic level explained about 30% and 40% of between-centre variations in men and women, respectively. Conclusion Hypertension prevalence greatly varies among French regions and this is partly explained by known lifestyle and socioeconomic factors. Nevertheless, these variations and all the hypertension determinants have not been fully deciphered yet.

Introduction: Praying represents a fundamental activity of daily living in Muslim community. Muslims need to adopt several postures that require deep flexion of the knee and hip to perform this daily obligation. This is a preliminary report of the study on hip and knee range of motion conducted to obtain normative passive and functional range of flexion data during Muslim prayers in both weight-bearing joints of normal Muslim adults. Methods: A cohort of Malay men aged between 20 to 30 years was recruited in this cross-sectional study. Passive range of motion and flexion angles of the joints in various postures during prayer were measured using a standard goniometer. The difference against existing normative range of motion database available and the influence of various factors were analysed. Factors analysed include body mass index and other anthropometric measurements. Results: Sixty participants recruited for the preliminary phase of this study. The mean (SD) of passive hip and knee flexions were higher compared to other existing normative range of motion database. A unit increase in body mass index will have 0.782 units lower of knee range of motion. The hip and knee flexion arc were from 74.1° to 119.0° and from 3.3° to 119.7° respectively. Conclusion: The range of motion involved for prayers was more for the knee but less for the hip as compared to the measured passive range of motion. Body mass index has a significant linear negative relationship with the passive range of motion of the knee, but not the hip.

Background: Overweight and obesity as well as underweight in adolescence are risk factors of chronic diseases that occur in adulthood. The objective of this research was to assess the Body Mass Index of in-school adolescents and its relationship to socioeconomic status of their parents.Methods: This is a cross-sectional study of 454 adolescents aged 10-19 years selected from five secondary schools using multistage sampling technique. A semi-structured interviewer questionnaire was used to collect relevant information from the adolescents and their anthropometric measurement to determine their Body Mass Index. Their Body Mass Index was categorized into obese, overweight, normal and underweight using the International Obesity Task Force reference. Analysis was done using IBM Statistical Package for Social Sciences, version 20.1. Results: The prevalence of obesity, overweight, and underweight among the adolescents was 7.7%, 1.1% and 32.8% respectively. None of the students in the public schools was obese while 2.1% of the adolescents were obese in the private schools. Adolescents in public schools had a higher percentage of underweight (65.4%) compared to those in private schools (34.5%) and this was found statistically significant (p<0.001). There was an overall higher prevalence of overweight and obesity among the females than found among the males. This was also the case for adolescents from higher socio-economic parents. These differences were found to be statistically significant (p<0.001). Conclusion: The Body Mass Index status of the in-school adolescents studied was found to increase with increased socioeconomic status of their parents. This underscores the need for sensitization of in-school adolescents to see the need of knowing their nutritional status and ways to avoid the consequences of abnormal Body Mass Index.

Abstract Purpose: This study aims todelineate the association between household economic inequalities, along with other confounders, and anemia among ever married women in Bangladesh.Methods: A cross-sectional population sample comprising of 5920 ever-married women aged between 15–49 years were selected for this Analysis were performed through the application of proportional odds models.Results: The respondents from low and medium socioeconomic status (SES) households vs. richest households were 1.90 (95% CI, 1.65-2.17; p<.01) and 1.41 (95 % CI, 1.12-1.78; p<.01) times more likely to suffer from anemia. The likelihood of being anemic reduces for the lowand medium SES groups vs. high SES group when sociodemographic variables are added (OR,1.69, 95% CI, 1.43-1.99 and OR, 1.35, 95% CI, .07-1.70;p<0.01 respectively). After adding the variables of diabetes and maternity status, the association betweenhaving anemia belongingto low and medium SES vs. high SESbecomes weaker (OR,1.36, 95% CI,0.85-2.15 and OR,1.15, 95% CI, 0.6-2.19; p>.05 respectively). Moreover, the strength of the association between anemia and the subjects with pregnant and lactating vs. the subjectswithout these conditionswas reduced after factoring body mass index (BMI).Conclusions: BMI, pregnancy, and diabetes mellitus were observed to be significantly associated with anemia and the strength of the association was low with anemia and socioeconomic inequality.

Objective: The main objective of this study was to determine association between dental caries and body mass index (BMI), and dental caries and frequency of sugar consumption freshmen undergraduate student in Nigeria. Methods: This was a cross-sectional study involving 1,164 students between 15 to 23 years of age. The independent variables for this study included age, sex, socioeconomic status, BMI, frequency of consuming refined sugars between meals and action taken after consumption of refined sugars. Descriptive analysis was used for all the quantitative variables while inferential analysis using logistic regression was used to determine predictors of dental caries. Results: A total of 1,164 students participated in the study, comprising 61.9% females and 38.1% males. The mean age of the participants was 18.13 (±4.99) and mean BMI was 21.68 (±7.93).  The prevalence of dental caries was 10.6% and the mean Decay Missing and Filled Teeth (DMFT) was 0.32 (±1.05). Age and socioeconomic status were the significant predictors of dental caries (p=0.02; p=0.04 respectively). However, there was increase odds of having dental caries in students with normal BMI ((AOR: 1.19; P=0.62), students that consumed refined sugar 3-4 between meals (AOR: 1.27; P=0.28) and students that did nothing after consumption of refined sugar (AOR: 1.43; P=0.51). Conclusion: The prevalence of dental caries in this study population was low, older students from middle socioeconomic status had significant caries prevalence and there were increase odds of dental caries in students with normal BMI, and in those that consumed refined sugars 3-4 times between meals.

Background: The aim of the present study was to examine the effects of socioeconomic status, nutrition and physical activity lifestyle habits and perceptions on Body Mass Index (BMI) in children aged 12–15 years in Greece. Furthermore, to compare the difference between the two sexes. Methods: This is a cross-sectional study conducted on a representative secondary school cohort that included 5144 subjects, aged 12 to 15 years. Students and their parents filled in validated questionnaires evaluating socioeconomic status, nutrition and physical activity. International Obesity Task Force cut offs were used to classify the children. Factor analysis of mixed data and partial proportional ordered logistic models were used to analyze ΒΜΙ distributions. All analyses were stratified by gender. Results: Boys were 2.9 (95%CI: 2.592–3.328) times more likely to be overweight/obese than girls. Partial proportional ordinal models indicate significant associations between nutritional and physical habits and perceptions variables but also significant gender differences in socio-demographic, nutritional risk factors as well as physical activity habits and perceptions. Conclusions: A clear understanding of the factors that contribute to the sex differences in nutrition and physical activity habits and perceptions may guide intervention efforts.
