Aim: To understand the experience of informal caregivers in the care of the elderly victims of fall and proximal fracture of the femur and surgery. Method: This is a qualitative study carried out in a teaching hospital, using Bardin's content analysis as a methodological reference, and Orem's General Theory of Nursing as a theoretical reference. Results: Of the 14 informal caregivers, the majority were female and daughters of the patients. From the speeches of the participants, the thematic categories emerged: The day of the fall, a traumatic event; agony and tension by waiting for the bed and surgery; fear, frustration, and lack of preparedness of the informal caregiver; changes in the life, deprivation and overload of the caregiver; resilient care. Conclusion: Informal caregivers should be trained from the prevention of the event to the transition from hospital care to home rehabilitation, rescuing the role of the nurse within the multidisciplinary team, and with the support by the General Theory of Nursing.

Abstract Objective: To characterize long-stay institutions for the elderly in respect to physical-structural and organizational aspects. Method: This descriptive, cross-sectional, quantitative research was carried out in four long-stay institutions for the elderly in Salvador - Bahia, Brazil, between September and November 2013. Data were collected from the technical officers of the institutions using a questionnaire based on National Health Surveillance Agency standards. Results: This study highlights a partial compliance with current regulations both in respect to the physical-structural and organizational aspects. The elderly are exposed to an environment that is sometimes unhealthy with risk factors for health problems. Conclusion and implications for practice: There is need for adjustments in the institutions to comply with current legislation. This study shows the importance of a multidisciplinary team to provide comprehensive care of institutionalized elderly, with emphasis on the role of nurses in the qualification of institutions and valorization of the elderly.

Abstract. Prospective Memory (PM; i.e., the ability to remember to perform planned tasks) represents a key proxy of healthy aging, as it relates to older adults’ everyday functioning, autonomy, and personal well-being. The current review illustrates how PM performance develops across the lifespan and how multiple cognitive and non-cognitive factors influence this trajectory. Further, a new, integrative framework is presented, detailing how those processes interplay in retrieving and executing delayed intentions. Specifically, while most previous models have focused on memory processes, the present model focuses on the role of executive functioning in PM and its development across the lifespan. Finally, a practical outlook is presented, suggesting how the current knowledge can be applied in geriatrics and geropsychology to promote healthy aging by maintaining prospective abilities in the elderly.

For many with heart failure, including the elderly and those with a preserved ejection fraction, both risk stratification and treatment are challenging. For these large populations and others there is increasing recognition of the role of cardiac fibrosis in the pathophysiology of heart failure. Galectin-3 is a novel biomarker of fibrosis and cardiac remodelling that represents an intriguing link between inflammation and fibrosis. In this article we review the biology of galectin-3, recent clinical research and its application in the management of heart failure patients.

Heart failure (HF) is a burden for the healthcare system. The incidence will increase significantly due to the aging of population, which is associated with multiple comorbidities. Anemia and iron deficiency are common in patients with HF, their etiology being often multifactorial. The screening for anemia and iron deficiency is recommended as soon as possible. There are often no targeted investigations, therefore a significant proportion of cases are underdiagnosed. The management of patients may focus on identifying and correcting the cause. Anemia can occur due to nutritional deficiencies, inflammation, renal failure, bone marrow dysfunction, neurohormonal activity, treatment and hemodilution. The appropriate therapy for the patients with anemia and HF will contribute to the improvement of life quality. The only recommended iron product is ferric carboxymalose administered by intravenous infusion. Under the appropriate treatment, the patients showed an increase in effort tolerance, with an improvement in symptomatology and a lower number of hospitalization days. The management of these cases is handled by a multidisciplinary team consisting of a general practitioner, a cardiologist and other specialists if the patient has other comorbidities. The role of the general practitioner is essential, as he can perform proper screening, prevention and management, developed by a multidisciplinary team, in order to reduce the cardiac morbidity and mortality.
