Clinicians’ emotional responses to suicidal patients are important factors in the assessment of imminent suicide risk and in treatment outcome. This chapter provides a comprehensive review of the role that emotional responses to suicidal patients play in clinical work and provides a framework for factoring them into the imminent risk assessment. The chapter discusses emotion differentiation and mindfulness, followed by a description of common psychological responses to suicidal individuals—“countertransference love” and “countertransference hate.” Common psychological defenses, including reaction formation, repression, turning against self, projection, denial, and rationalization, are described. This chapter also provides a framework for a self-assessment of one’s emotional response to a suicidal patient. The chapter concludes with case examples describing emotional responses and psychological defenses elicited when working with imminently suicidal individuals.

Psychotherapy with suicidal patients is inherently challenging. Psychodynamic psychotherapy focuses attention on the patient's internal experience through the creation of a therapeutic space for an open-ended exploration of thoughts, fears, and fantasies as they emerge through interactive dialogue with an empathic therapist. The Boston Suicide Study Group (M.S., M.J.G., E.R., B.H.), has developed an integrative psychodynamic approach to psychotherapy with suicidal patients based on the authors’ extensive clinical work with suicidal patients (over 100 years combined). It is fundamentally psychodynamic in nature, with an emphasis on the therapeutic alliance, unconscious and implicit relational processes, and the power of the therapeutic relationship to facilitate change in a long-term exploratory treatment. It is also integrative, however, drawing extensively on ideas and techniques described in Dialectical Behavioral Therapy (DBT), Mentalization Based Treatment (MBT), Cognitive-Behavioral Therapy (CBT), as well on developmental and social psychology research. This is not meant to be a comprehensive review of psychodynamic treatment of suicidal patients, but rather a description of an integrative approach that synthesizes clinical experience and relevant theoretical contributions from the literature that support the authors’ reasoning. There are ten key aspects of this integrative psychodynamic treatment: 1. Approach to the patient in crisis; 2, instilling hope; 3. a focus on the patient's internal affective experience; 4. attention to conscious and unconscious beliefs and fantasies; 5. improving affect tolerance; 6. development of narrative identity and modification of "relational scripts"; 7. facilitation of the emergence of the patient's genuine capacities; 8. improving a sense of continuity and coherence; 9 attention to the therapeutic alliance; 10. attention to countertransference. The elements of treatment are overlapping and not meant to be sequential, but each is discussed separately as an essential aspect of the psychotherapeutic work. This integrative psychodynamic approach is a useful method for suicide prevention as it helps to instill hope, provides relational contact and engages the suicidal patient in a process that leads to positive internal change. The benefits of the psychotherapy go beyond crisis intervention, and include the potential for improved affect tolerance, more fulfilling relational experiences, emergence of previously warded off experience of genuine capacities, and a positive change in narrative identity.

Countertransference is an inescapable component of all psychotherapy. Intense countertransference reactions often occur during the treatment of suicidal patients. Lack of awareness of countertransference reactions of malice and aversion may be suicide-inviting. On the other hand, awareness of these reactions may enhance the treatment by alerting therapists that the suicidal patient is fomenting issues he is unable or unwilling to articulate. Problems that arise in the course of psychotherapy of suicidal patients may be understood in relation to the therapist’s countertransference reactions. In this chapter, we review case examples of countertransference manifestations and their effects on the treatment of the suicidal patient.

Background: Although intervention with suicidal patients is one of the hardest tasks in clinical practice, little is known about health professionals’ perceptions about the difficulties of working with suicidal patients. Aims: The aims of this study were to: (1) describe the difficulties of professionals facing a suicidal patient; (2) analyze the differences in difficulties according to the sociodemographic and professional characteristics of the health professionals; and (3) identify the health professionals’ perceived skills and thoughts on the need for training in suicide. Method: A self-report questionnaire developed for this purpose was filled out by 196 health professionals. Exploratory principal components analyses were used. Results: Four factors were found: technical difficulties; emotional difficulties; relational and communicational difficulties; and family-approaching and logistic difficulties. Differences were found between professionals who had or did not have training in suicide, between professional groups, and between the number of patient suicide attempts. Sixty percent of the participants reported a personal need for training and 85% thought it was fundamental to implement training plans targeted at health professionals. Conclusion: Specific training is fundamental. Experiential and active methodologies should be used and technical, relational, and emotional questions must be included in the training syllabus.

Most studies of emotional responses have used unimodal stimuli (e.g., pictures or sounds) or congruent bimodal stimuli (e.g., video clips with sound), but little is known about the emotional response to incongruent bimodal stimuli. The aim of the present study was to evaluate the effect of congruence between auditory and visual bimodal stimuli on heart rate and self-reported measures of emotional dimension, valence and arousal. Subjects listened to pleasant, neutral, and unpleasant sounds, accompanied by videos with and without content congruence, and heart rate was recorded. Dimensions of valence and arousal of each bimodal stimulus were then self-reported. The results showed that heart rate depends of the valence of the sounds but not of the congruence of the bimodal stimuli. The valence and arousal scores changed depending on the congruence of the bimodal stimuli. These results suggest that the congruence of bimodal stimuli affects the subjective perception of emotion.

Patients’ emotional responses to the hospital environment can be considered as important as medical technology and equipment. Therefore, this study investigated their experiences to determine whether the pattern using hospital identity (HI) elements, a widely used design method for patient clothing in university hospitals, can affect their emotional response and contribute to healing. It aimed to identify whether controlling the motif characteristics, arrangement, and spacing in this pattern design, and the direction between motifs, could be a method to design patient clothing for healing. To investigate patients’ emotional response and suggestions for patient clothing design, an interview-based qualitative approach was used. In-depth interviews were conducted with 12 patients discharged from Kyung Hee University Hospital Medical Center (KHUMC), Seoul. The interview questions consisted of two parts. One part featured questions about participants’ emotional responses to the medical environment and their latest patient clothing experience, and the other featured questions about their emotional response to, and suggestions for, the healing expression of pattern design using HI. The results confirmed that the motif characteristics, arrangement, and spacing, and the direction between motifs, influenced patients’ positive emotions and contributed to the healing effect. Therefore, when the HI elements of a medical institution are applied in the design of patient clothing with the characteristics of a healing design, patients perceive this as providing stability and comfort. The design of patient clothing becomes a medium that not only builds the brand image of medical institutions, but also enhances the quality of medical services centered on patient healing.

AbstractJuslin & Västfjäll (J&V) propose a theoretical framework of how music may evoke an emotional response. This commentary presents results from a pilot study that employed young children as participants, and measured musically induced emotions through facial expressions. Preliminary findings support certain aspects of the proposed theoretical framework. The implications of these findings on future research employing the proposed framework are discussed.

Patients with narcissistic personality disorder are among the most difficult to treat in therapy, especially for their strong resistance to treatment and several difficulties in establishing a therapeutic relationship characterized by intimacy, safety, and trust. In particular, therapists’ emotional responses to these patients can be particularly intense and frustrating, as reported in the clinical literature but rarely investigated empirically.The aims of this preliminary study were: 1) to examine the associations between patients’ narcissistic personality disorder and distinct therapists’ countertransference patterns; and 2) to verify whether these clinicians’ emotional reactions were influenced by their theoretical orientation (as well as gender and age).A national sample of psychiatrists and clinical psychologists (N = 250) completed the Therapist Response Questionnaire (TRQ) to identify patterns of therapist emotional response, and the Shedler-Westen Assessment Procedure-200 (SWAP-200) to assess personality disorder and level of psychological functioning in a patient currently in their care and with whom they had worked for a minimum of eight sessions and a maximum of 6 months (one session per week).From the complete therapist sample, we identified a subgroup (N = 35) of patients with narcissistic personality disorder, and we found that this personality pathology was positively associated with criticized/mistreated and disengaged countertransference, but negatively associated with positive therapist response. Moreover, our results confirmed that the relationship between patients’ narcissistic personality disorder and therapists’ emotional responses was not dependent on clinicians’ theoretical approach (as well as their age and gender).These findings are consistent with clinical observations, as well as some empirical contributions, and have meaningful implications for clinical practice of patients suffering from this challenging pathology.

Four composite case examples are presented and discussed as they relate to emotional expression, significant moments in the therapeutic process, and communication using a variety of modalities in music therapy with adults diagnosed with intellectual and developmental disabilities. Building on therapeutic awareness through discussing musical elements, body movement and posture, countertransference, and interactional patterns, the implications of deep emotional connection and processing are approached using primarily nonverbal methods. Composite vignettes from the author’s clinical work demonstrate awareness of these factors in the moment as they impacted the session, therapeutic relationship, and other professionals’ understanding of music therapy in this population. Implications for emotional processing in clinical practice are presented as they relate to the concepts presented in this paper.

James Joyce’s Portrait of the Artist as a Young Man has fascinated readers for more than a century and there are layers of psychological meaning to be found throughout the novel. The novel is the perfect vehicle to discuss the relationship between form language and emotion as Joyce deliberately manipulated the emotional response of the reader through innovations in form and language, departing dramatically from previous literary traditions. This paper attempts to take a fresh look at the novel from a psychological perspective and seeks to examine underlying conditioning processes at work in the narrative – particularly the concept of associative learning. Understanding emotional responses to different stimuli is the bedrock of psychological investigation and 100 years after the date of its publication, Portrait of an Artist presents remarkably fresh insights into the human experience of emotion. Despite its age, Portrait of the Artist contains many contemporary psychological insights.

Emotions are now widely accepted as important elements of qualitative research, in studies of individuals and communities. However, collective emotional status—what a community feels about a given situation or proposition—can be challenging to assess. In this study, we examined the validity and acceptability of a new mixed methods survey tool, primarily qualitative, to address this challenge—the Assessment of Collective Emotional Status (ACES). The tool begins with an adjustable set of questions about emotion, to draw respondents’ attention to their own feelings. These are followed by an emotional self-assessment, in which respondents select and prioritize five emotional responses to a standardized stimulus, drawing from a new taxonomy of emotion words. In this study, the stimulus was a proposition that gun violence should be approached as a public health problem. We tested the tool in an international survey of public health professionals, mostly in the US and Europe (n=160). Qualitative and quantitative data were collected on knowledge and importance of emotion, adequacy of the taxonomy, emotional responses to the stimulus, and use of the tool. Scores were high for knowledge and importance of emotion. Perceived adequacy of the taxonomy was also high, especially with Black and Hispanic respondents, signifying good construct validity. The total weighted frequency of emotions in response to the stimulus was highest for encouraged (92.2%), open (78.8%), hope (77.2%) and interest (77.2%). Qualitative data yielded six themes—on knowledge of emotion, the taxonomy, responses to the stimulus, and use of the tool, which many respondents found easy and interesting to use. This study demonstrates the prima facie validity and acceptability of the ACES with an educated adult population.
