Abstract Severe acute respiratory syndrome coronavirus 2 is associated with severe disease in patients with hematologic malignancy. We report a series of patients with underlying hematologic malignancy and coronavirus disease of 2019 with discrepancy between radiographic findings and molecular testing. Initial chest x-ray findings should raise suspicion in immunosuppressed patients with typical clinical presentation even with negative initial testing.

Objectives: To document the pattern of chest radiographic findings in coronavirus disease 2019 (COVID-19) patients with moderate to severe disease. Design: Retrospective cross-sectional study. Setting: The study site was Komfo Anokye Teaching Hospital (KATH) located at Bantama in Kumasi, the capital town of the Ashanti Region. It is the second largest hospital in the country and the major referral site for Ashanti region and the northern part of the country. The hospital hosts a highly infectious isolation unit (HIIU) for COVID19 patients with moderate to severe infections and receives referred cases from the region and within the hospital. Participants: The study involved 27 patients admitted to the HIIU at KATH with COVID-19 infection who underwent chest X-ray as part of their investigations. Results: The study enrolled 12 males and 15 females. The common comorbidities were hypertension and diabetes. Chest x-ray findings in most of the patients (81.5%) revealed ground-glass opacities while a few of them (18.5%) had lung consolidations without ground-glass opacities. For those with ground-glass opacities, eight (29.6%) had superimposed consolidation. All the participants had positive chest x-ray findings. Conclusion: The chest x-ray findings in the 27 COVID-19 positive patients with moderate to severe disease on admission at the KATH HIIU enrolled in this study showed significant pulmonary abnormalities. The predominant pulmonary abnormalities were bilateral peripheral ground-glass opacities with the lower lung zones being mostly affected.

Despite many advances in the diagnosis, screening, and rapid treatment of tuberculosis, it is still a public health concern in the world. Due to the importance of this issue in diagnosis and reduction of transmission of infection and treatment of the disease especially where this study is conducted due to the high prevalence of tuberculosis, this study was done to determine The relationship between sputum smear positivity grade and chest X-ray findings in pulmonary tuberculosis patients in a hospital in southeast of Iran. This cross-sectional study was performed on all patients with pulmonary TB referencing the health centers in Zabol city, southeast of Iran from 1 January 2015 to 30 December 2020. Sputum smear and radiographic findings of the chest X-ray were evaluated. Data was collected using a form of information and finally analyzed by SPSS 22. Out of 101 patients examined in the present study, 71 were women and 30 were men. The mean age of the patients was 62.68 ± 13.61 years. The frequency of opacity in patients with grades 1, 2, and 3 was 71.4, 78.5, and 76.5%, respectively. Frequency of cavitation in patients with Grade 1, 2 and 3 was 11.5%, 28.5% and 52.9% respectively (P value 0.001). The frequency of reticulonodular presentations in patients with grade 1, 2, and 3 was 24.2, 7.1, and 0%, respectively.  In general, the results of this study showed that, with increasing grading of smears (1+, 2+, and 3+), the frequency of cavitation presentation increased significantly and the frequency of reticulonodular presentations decreased significantly. The findings of the present study can help physicians better diagnose TB.

ABSTRACT Objectives: To assess the association of diagnostic predictors available in the emergency department (ED) with the outcome diagnosis of severe acute respiratory syndrome (SARS). Methods: This retrospective cohort study describes all patients from the Amoy Garden complex who presented to an ED SARS screening clinic during a 2-month outbreak. Clinical and diagnostic predictors were recorded, along with ED diagnoses. Final diagnoses were established independently based on diagnostic tests performed after the ED visit. Associations of key predictors with the final diagnosis of SARS were described. Results: Of 821 patients, 205 had confirmed SARS, 35 undetermined SARS and 581 non-SARS. Multivariable logistic regression showed that the strongest predictors of SARS were abnormal chest x-ray (odds ratio [OR] = 17.4), subjective fever (OR = 9.7), temperature >38°C (OR = 6.4), myalgias (OR = 5.5), chills and rigors (OR = 4.0) and contact exposure (OR = 2.6). In a subset of 176 patients who had a complete blood cell count performed, the strongest predictors were temperature ≥38ºC (OR = 15.5), lymphocyte count <1000 (OR = 9.3) and abnormal chest x-ray (OR = 5.7). Diarrhea was a powerful negative predictor (OR = 0.03) of SARS. Conclusions: Two components of the World Health Organization case definition — fever and contact exposure — are helpful for ED decision-making, but respiratory symptoms do not discriminate well between SARS and non-SARS. Emergency physicians should consider the presence of diarrhea, chest x-ray findings, the absolute lymphocyte count and the platelet count as significant modifiers of disease likelihood. Prospective validation of these findings in other clinical settings is desirable.

COVID-19 is an exceptionally infectious disease caused by severe acute respiratory syndrome. The illness has spread itself worldwide rapidly and can lead to death only in a few days. In this context, investigating fast ways of detection that help physicians in the decision-making process is essential to help in the task of saving lives. This work investigates fourteen convolutional neural network architectures using transfer learning. We used a database composed of 2,928 x-ray images divided into three classes: Normal, COVID-19, and Viral Pneumonia. Results showed that DenseNet169 presented the best results regarding classification reaching a mean accuracy of 94%, a precision of 97.6%, a recall of 95.6%, and an F1-score of 96,1%, approximately.

Background. The emergence of Human immunodeficiency virus (HIV), led to the rise in the incidence and prevalence of tuberculosis (TB) worldwide. However, the trend is being reversed recently due to the widespread use of effective Anti-Retrovirals. Immunosuppression induced by HIV infection modified the clinical presentation of TB, resulting in atypical signs and symptoms, and a more frequent extrapulmonary presentation. This study was undertaken to determine the pattern of presentation and prevalence of TB in HIV seropositive patients seen in Benin City, Nigeria, from January to April, 2007. Method. The study was done using 330 HIV positive patients (123 males and 207 females). A designed questionnaire was used as a diagnostic instrument. Results. The prevalence of TB HIV was found to be 33.9%. It was found to be commoner in females, commonest in the age group 30–39 years. Pulmonary TB was the commonest type of TB found (78.6%). This was followed by TB adenitis (12.5%). The incidence of extrapulmonary TB was 21.4%. There was a high incidence of atypical chest X-ray features and high frequency of negative sputum smears. Conclusions. The overall prevalence rate of TB in HIV (33.9%), and the extrapulmonary presentation of TB are high. Some investigation results were found to be atypical in those with both infections. Physicians should be aware of this pattern of presentation and the atypical findings on investigation for early diagnosis and treatment.

Abstract Background: Coronavirus disease (COVID-19) is rapidly spreading worldwide. Although 10%–20% of patients with COVID-19 have severe symptoms, little is known about the risk factors related to the aggravation of COVID-19 symptoms from asymptomatic or mild to severe disease states. Methods: This retrospective study included 211 patients who were asymptomatic or with mild presentations of COVID-19. We evaluated the differences in in demographic and clinical data between the cured (discharged to home) and transferred (aggravated to severe-stage COVID-19) groups. Results: A multivariate logistic analysis showed that body temperature, chills, initial chest X-ray findings, and the presence of diabetes were significantly associated with predicting the progression to severe stage of COVID-19 (p < 0.05). The odds ratio of transfer in patients with COVID-19 increased by 12.7-fold for abnormal findings such as haziness or consolidation in initial chest X-ray, 6.32-fold for initial symptom of chills, and 64.1-fold for diabetes.Conclusions: Even if patients are asymptomatic or have mild symptoms, clinicians should closely observe patients with COVID-19 presenting with chills, body temperature >37.5°C, findings of pneumonia in chest X-ray, or diabetes.

A chest X-ray (CXR) is routinely performed after percutaneous dilatational tracheostomy (PDT). The purpose of this study was to evaluate the diagnostic yield of routine CXR following PDT and its impact on patient management and to identify predictors of post-PDT CXR changes. Two-hundred-and-thirty-nine patients who underwent PDT in a 21-bed intensive care unit were included prospectively in the study. The following data were collected: patient demographics, APACHE III scores, pre-PDT FiO2 and PEEP, PDT technique, perioperative complications and the use of bronchoscopic guidance. We compared post-PDT CXR with the last pre-PDT CXR. We documented any post-PDT new radiographic findings including atelectasis, pneumothorax, pneumomediastinum, surgical emphysema, pulmonary infiltrates or tracheostomy tube malposition. We also recorded management modifications based on post-PDT radiographic changes, including increased PEEP, chest physiotherapy, therapeutic bronchoscopy or chest tube insertion. Atelectasis was the only new finding detected on post-PDT CXRs of 24 (10%) patients. The new radiographic findings resulted in a total of 14 modifications of management in 10 (4%) patients including increased PEEP in six, chest physiotherapy in six and bronchoscopy in two patients. Trauma and pre-PDT PEEP >5 cmH2O were independent predictors of post-PDT CXR changes. Routine CXR following PDT has a low diagnostic yield, detecting mainly atelectasis and leading to a change in the management in only a minority of patients. Routine CXR after apparently uncomplicated PDT performed by an experienced operator may not be necessary and selective use may improve its diagnostic yield. Further studies are required to validate the safety of selective versus routine post-PDT CXR.

Zygomycosis occurs primarily in immunosuppressed patients and those with diabetes mellitus. Incidence of zygomycosis has increased among transplant recipients, patients with hematologic malignancy and Covid-19 associated pulmonary complications. Soft tissue zygomycosis may be localized, extend to deep underlying tissues, or may be disseminated. The most common clinical presentation is induration of the skin with surrounding erythema, rapidly progressing to necrosis.Histological examination and culture of soft tissue are important for the diagnosis of cutaneous zygomycosis. Treatment consists of surgical excision or debridement, reversal of predisposing factors and administration of antifungal agents (amphotericin B formulations, Posaconazole/or Isavuconazole).

Abstract CASE A 49-year old Hispanic male diagnosed with chronic phase CML in 2008 was referred for a SCT for progressive TKI-refractory disease. In May, 2014, he received a haploidentical SCT following treatment with melphalan 140mg/m2 and fludarabine 40mg/m2. GVHD prophylaxis was provided by post-transplant cyclophosphamide 50mg/kg IV on days 3 and 4, tacrolimus 0.015 mg/kg IV and MMF 15mg/kg PO tid. Bacterial, fungal and viral prophylaxis included levofloxacin, voriconazole and valacyclovir respectively. On transplant day 11, he developed asymptomatic neutropenic fever and was started on IV vancomycin and cefepime. The fevers abated by day 14, but he developed acute dyspnea and hypoxia requiring intermittent BiPAP treatment. A CAT scan showed new bilateral lung patchy ground-glass opacities [Figure 1] compared to the baseline chest X-ray. Bronchoscopy could not be performed due to the tenuous respiratory status. MMF was held and methylprednisolone (MP) 1gm/kg IV daily was started for concerns of pulmonary toxicity. His symptoms improved and the chest x-ray normalized within a few days. MMF was resumed at same dose and MP dose was reduced to 0.5 mg/kg/day; within four days there was a rapid recurrence of the pulmonary symptoms and re-emergence of diffuse airspace opacities. MMF was held again and MP was increased to the original dose, resulting in resolution of his symptoms and the radiographic findings within days. During this period, he was afebrile, infectious work-up (including comprehensive viral, bacterial and fungal testing) was negative and no RBC antibodies were detected. Table 1 summarizes his disease course. DISCUSSION To the best of our knowledge, this is the first reported case of potential MMF-related pulmonary toxicity after SCT, although similar associations have been reported in a few cases after renal and cardiac transplants.(1-4) The causal relationship of MMF-induced toxicity in our case is suggested by the temporal association, exclusion of other plausible causes, and temporal re-emergence when challenged again, and subsequent resolution of symptoms and x-ray findings with discontinuation of MMF. The underlying mechanism is unknown, although genetic polymorphisms in UGT, an enzyme involved in its metabolism, may explain unexpected toxicities – a phenomenon known to enhance toxicities of various drugs metabolized by this enzyme. (5) CONCLUSION MMF should be considered as a potential etiology of post-transplant acute pulmonary toxicity in patients undergoing hematopoietic transplantation. REFERENCES: 1.
